Background : Epithelial-mesenchymal transition (EMT) is critical for morphogenesis during embryonic development and is also implicated in the conversion of early-stage tumors into invasive malignancies. Recently, Twist has been identified to play an important role in EMTmediated metastatic progression of several types of human cancer. The present study examined the expression of Twist and evaluated its clinicopathologic significance in urothelial carcinoma of upper urinary tract. Methods : Immunohistochemical staining for Twist expression was performed on 70 upper urinary tract urothelial carcinomas (UUT-UCs) using tissue microarray. Results : Immunohistochemical staining for Twist was positive in 31/70 cases (44.3%) of UUT-UCs. Twist expression was associated with high-grade and advanced-stage (ISUP grade, p<0.01; stage, p=0.045). The patients with Twist positive-tumors revealed lower disease free survival rate than those with Twist negative-tumors (p<0.01). The overall survival for patients with Twist positive-tumors was slightly worse than the patients with Twist negative-tumors, but the difference was not statistically significant (p=0.12). Conclusion : Our results suggest that Twist is a novel marker for advanced UUT-UC. Key Words : Carcinoma, transitional cell; Twist transcription factor; Disease free survival 요로상피세포암종(Urothelial carcinoma)은 요로에서 발생하 는 암종 중 가장 흔한 암종으로, 신우와 요관으로 국한하였을 경우 90% 이상을 차지한다. 1 신우와 요관에 발생하는 요로상피 세포암종은 근치절제술 이후에도 재발하거나 원격 전이되는 경 우가 많으며, 방광에서 발생하는 요로상피세포암종에 비해 좀 더 진행된 병기와 등급을 보인다. 2,3 수술 후 재발이나 원격 전이 가능성이 높은 환자를 대상으로 적절한 보조요법이 필요하므로 종양 등급이나 병기 외에 다른 예후 인자에 대한 연구가 진행되 고있다. 상피-중간엽 이행(Epithelial-mesenchymal transition, EMT) 은 상피세포가 극성(polarity)과 세포 간 접촉(cell-cell contact) 을 잃고 세포뼈대(cytoskeleton)가 재형성(remodeling)되는 과 정을 의미하며, 최근에 일부 종양의 전이 및 진행에 관여한다는 사실이 알려졌다. 4 상피세포 유착(Epithelial cell adhesion)과 세포뼈대 성분(cytoskeleton component)을 동시에 잃음으로써 세포는 EMT를 거쳐 중간엽 성분(mesenchymal component)을 발현하게 된다. 상피-중간엽 이행이 배아의 형태 발생 과정에서 중요한 역할을 한다는 사실이 1980년대에 처음으로 알려졌으며 최근 들어 암의 진행과도 밀접한 연관이 있다고 보고되었다. 4,5 Twist는 고도로 보전된 기초 나선-고리-나선 전사 인자(highly conserved basic helix-loop-helix transcription factor)이며 EMT를 조절하는 중요한 요소 가운데 하나다. 6 최근 다양한 암 에서 Twist의 발현이 암의 높은 병기 및 전이 가능성과 관련이 있음이 보고되고 있다. 5,7-11 예를 들면, 흑색종 환자를 대상으로 한 연구에서 Twist가 과발현된 그룹이 저발현된 그룹보다 생존 율이 낮다고 보고하였으며, 11 또 Twist 발현 양상이 골육종 환 324

2 상부 요로상피세포암종에서 Twist 발현의 의의 325 자에서 화학요법에 대한 반응을 예측하는 데 도움이 된다고 하 였다. 12 Zhang 등 13 은 Twist 발현이 방광암 진행과 관련이 있다 고 보고하였다. 그러나 Zhang 등 13 은 Twist 발현과 환자의 생 존율의 관련성을 언급하지 않았고 방광에 발생한 요로상피세포 암종에 국한된 보고를 했을 뿐이다. 이에 저자들은 요관 및 신우 에서 발생한 상부 요로상피세포암에서 Twist 발현과 암의 진행 및 환자의 생존율의 관련성을 알아보고자 하였다. 재료와 방법 연구 대상 1999년 1월부터 2005년 12월까지 서울대학교병원에서 콩팥요 관절제술(nephroureterectomy)이나 구역 요관절제술(segmental ureterectomy)을 받은 70명의 상부 요로상피세포암 환자를 대상으로 하였다. 진단 당시 환자의 연령은 43세부터 80세까지 분포하였으며 평균 64.2세였다. 환자의 남녀 성비는 2.9:1이었고, 평균 추적관찰 기간은 16.8개월이었다. 면역조직화학염색 Tissue microarray (TMA)는 대상 증례의 헤마톡실린-에오신 염색 슬라이드를 재검색하여 각 증례의 대표적인 병변과 정상 부위를 표시하고, 해당하는 파라핀 포매 조직에서 tissue array block 제작기구를 이용하여 2 mm 지름으로 각 증례당 병변 두 개의 core와 정상 하나의 core를 채취하여 제작하였다. 그런 다 음 10행 6열로 배열하여 TMA 파라핀 블록 여섯 개를 만들었 다. TMA 파라핀 블록을 5 m 두께의 절편으로 박절하여 슬 라이드에 부착한 후 크실렌에 5분간 3회 탈파라핀시켰다. 그런 다음 함수 과정을 거친 후 증류수로 세척하였다. 항원성 회복을 위해 TMA 슬라이드를 citrate 완충용액(10 mm, ph 6.0)에 담근 후 전자레인지를 이용하여 95 에서 15분간 끓였다. 비특 이 단백의 결합을 제거하기 위해서 normal goat serum에 실온 에서 10분간 작용시켰다. 일차 항체로는 Twist (Santa Cruz Biotechnology, Inc., Santa Cruz, CA, USA)를 사용했으며 1:100으로 희석한 후 실온에서 90분간 반응시킨 다음 biotin이 부착된 이차 항체(biotinylated anti-mouse immunoglobin antibody, Dako Co., Ltd., Kyoto, Japan)를 30분간 반응시켰다. 세척 완충액으로 3분씩 3회 씻어낸 후 streptavidin peroxidase detection system (Zymed, San Francisco, CA, USA)으로 실온에서 10분간 방치하여 biotin-avidin 특이 결합을 유도하였다. 다시 세척 완충액으로 씻어낸 후 diaminobenzidine으로 10분간 발색시켰다. 그런 다음 증류수로 세척하고 Meyer s hematoxylin으로 대조염색 후 봉 입하였다. 면역조직화학염색 결과 판독 종양세포 내의 염색 강도를 네 등급(0: 음성, 1: 약양성, 2: 양성, 3: 강양성)으로 나누고 전체 종양세포 중 양성을 보이는 종양세포의 비율을 다섯 등급(0: 0%, 1: 1-25%, 2: 26-50%, 3: 51-75%, 4: %)으로 나누어 염색 강도에 대해서 0, 1, 2, 3점, 염색 범위에 대해서 0, 1, 2, 3, 4점을 매겼다. 염색 범위는 종양세포의 밀도가 높은 부분을 4개 이상의 고배율 시야 ( 400배)에서 1,000개의 종양세포 중 양성인 세포의 수를 세어 백분율을 구하였다. 염색 강도와 범위의 점수를 곱한 후 그 수치 가 4점 이상인 것을 양성으로 판정하였다. 통계학적 분석 종양의 분화도, 임상 병기, 림프절 전이 유무에 따른 발현율의 차이를 Pearson s chi-square test를 사용하여 분석하였다. 무병 생존(disease free survival)은 수술을 받은 시점부터 최종 추적 관찰 또는 요로상피세포암이 진행한 증거를 발견한 시기까지의 기간으로 정의하였고, 총 생존(overall survival)은 수술을 받은 시점부터 최종 추적 관찰 또는 요로상피세포암으로 사망하기까 지의 기간으로 정의하였다. Twist 발현에 따른 환자들의 무병 생존율 혹은 총 생존율의 차이는 Kaplan-Meier test를 이용하여 분석했으며 log-rank test 로 유의성을 평가하였다. 무병 생존에 영향을 미치는 다양한 인 자(factor)들의 상대적인 기여도를 평가하기 위해 Cox regression model을 사용하였다. 통계 분석에 사용한 프로그램은 SPSS 12.0 (SPSS Inc., Chicago, IL, USA)이며, p값이 0.05 미만일 경우 통계적으로 유의하다고 판정하였다. 결 과 임상 및 병리학적 소견 전체 상부 요로상피세포암 환자 70명 중 원발 부위가 신우인 환자는 60명(85.7%), 요관인 환자는 10명(14.3%)이었다. T병 기에 따라 분류하면 Ta/Tis는 5예(7.1%), T1이 23예(32.9%)였 고, T2, T3, T4가 각각 10예(14.3%), 30예(42.9%), 2예(2.9%) 였다. 림프절 전이 여부에 따라 분류하면 림프절 전이가 없는 군 이 66예(94.3%), N1이 1예(1.4%), N2가 3예(4.3%)였다. 임상 병기에 따라 분류하면 병기 0a/0is가 5예(7.1%), 병기 1이 23예 (32.9%), 병기 2가 10예(14.3%), 병기 3이 24예(34.3%), 병기 4가 8예(11.4%)였다. ISUP 등급에 따라 분류하면 저등급은 39 예(55.7%), 고등급은 31예(44.3%)이었다.

Twist 단백은 ISUP 등급이 높을수록 발현이 증가하는 경향을 보였다 (p<0.01). 또한 Twist 단백은 임상 병기가 증가할수록 발현이 증가하였고(p=0.045) (Fig. 1), T병기가 증가할수록 발현이 증 가하는 양상을 보였으나 통계적으로 유의하지는 않았다(p< 0.01). Twist 단백 발현과 N병기, M병기는 통계적 관련성을 보 이지 않았는데 이는 림프절 전이나 원격 전이가 있는 증례 수가 적었기 때문으로 판단된다. 임상 병리학적 특징, Twist 발현과 무병 생존 기간의 연관성 전체 환자의 평균 추적 관찰 기간은 1개월에서 83개월이었으 며 평균 16.8개월이었다. 70명의 환자 중에서 7명(10%)이 재발 Table 2. Multivariate analysis of prognostic factor in 70 patients with upper urinary tract urothelial carcinoma Variables Relative hazard 또는 원격 전이하였고, 12명(17.1%)이 사망하였다. 여러 임상 병리학적 인자들과 무병 생존의 연관성을 분석한 결과 임상 병 기가 높을수록 무병 생존율이 감소하였고(p<0.01), ISUP 등급 이 높을수록 무병 생존율이 감소하였다(p=0.02). Twist 단백 양성인 환자군이 음성인 환자군보다 통계적으로도 유의하게 무병 생존율이 낮았다(p=0.01)(Fig. 2A). 총 생존율은 Twist 단백 양성인 환자군이 음성인 환자군보다 낮은 경향을 보였으나 통계 적으로 유의하지는 않았다(p=0.12)(Fig. 2B). Cox regression model을 사용한 다변수 분석에서는 T병기만이 무병 생존 기간 에 영향을 미치는 독립적인 예후 인자로서 통계적 유의성을 보 였다(p=0.02)(Table 2). 고 이번 상부 요로상피세포암 70예에 대한 연구에서 Twist 단백 발현의 증가가 높은 임상 병기 및 종양 미분화도와 관련이 있음 이 나타났다. 이는 이전의 Zhang 등 13 이 방광 요로상피세포암 에 대해 시행한 연구와 일치하는 결과였다. Zhang 등 13 의연구 에서는 Twist와 더불어 E-cadherin 면역염색을 시행하였고 Twist 발현이 증가할수록 E-cadherin의 발현이 감소한다고 보 고하였고, 이 결과는 Twist와 E-cadherin을 함께 염색함으로써 원발성 방광암의 침윤성을 조기에 발견하는 데 도움이 된다고 하였다. The overall survival for patients with Twist positivetumors was slightly worse than the patients with Twist negative-tumors (log-rank test, p=0.12) (B). 을 보이는 이유는 Twist가 직간접적으로 촉진자 부위(promoter region)의 E-box를 통해서 E-cadherin을 전사 단계에서 억제 하기 때문이다. 5 사람 세포에서 Twist가 하는 기능은 머리뼈붙음증(craniosynostosis)을 특징으로 하는 흔한 보통염색체 우성 증후군(autosomal dominant syndrome)인 Saethre-Chotzen 증후군에서 처 음으로 알려졌다. 14 이후 Twist는 뼈모세포 분화(osteoblastic differentiation)와 세포자멸사에서 중요한 조절인자(regulator) 로서 역할을 한다고 보고되었다 Twist의 종자계 돌연변이 (germ line mutation)는 뼈모세포에서 Twist 단백을 불활성화 시켜 조기에 머리뼈를 골화(premature cranial ossification)시 킨다. 16,18 최근 인간에게서 발생하는 다양한 암에서 Twist의 과 발현이 보고되었지만 암의 발생 및 진행에 Twist가 어떠한 영 향을 미치는지는 아직 확실히 밝혀지지 않았다. 5,7-11 그렇지만 Twist가 p53/arf, N-myc, Wnt, EMT 등 다양한 암관련 신호 경로(cancer-related signaling pathway)에서 조절인자 (regulator) 역할을 한다는 보고가 있다. 5,7,8,15,19-21 Zhang 등 13 의 연구에서와 마찬가지로 본 연구에서도 고등급 의 상부 요로상피세포암에서 Twist 발현이 증가하였고, 그 외 미 분화 고등급 전립선암종과 고등급의 신경아교종에서도 발현이 증가한다고 보고되었다. 8,10 유방상피세포에서 Wnt1 경로(pathway)를 통해서 Twist가 과발현되어 세포 분화를 억제할 수 있 다는 보고가 있고, 19 최근 연구에 따르면 Wnt 길항제(antagonist)의 불활성은 방광암에서 흔히 일어나는 현상으로 전이 가능 성을 예측하는 데 도움이 될 수 있다고 한다 본연구결과 와 연관시켜 볼 때, 요로상피암세포에서 Twist 과발현은 Wnt1 경로를 통해서 세포 분화를 억제하고 암의 진행을 증진시킨다고 예상할 수 있다. 23,24 Zhang 등 13 은 저등급에 비해 고등급 방광암 에서 Twist 발현이 증가하는 양상을 보였으므로 Twist 발현의 증가가 낮은 병기의 방광암이 높은 병기로 진행하리라는 것을 예견할 수 있는 척도가 될 수 있다고 주장하고 있다. 비록 논란이 있지만 EMT는 암종의 진행(carcinoma progression)에 중요한 단계로 제안되고 있으며, 25,26 최근 연구에서 는 Twist가 세포 분화를 억제하고 p53 종양 억제 인자 경로(p53 tumor suppressor pathway)를 방해함으로써 암종의 진행을 촉 진한다고 보고하고 있다. 27,28 사람에게서 발생한 암세포가 항암제 내성(chemoresistance)을 획득하는 과정에서 Twist가 하는 기 능이 보고되었다. Twist의 과발현은 미세관표적 항암제(microtube-targeting anticancer drug)에 대한 세포 저항성(cellular resistance)과 연관이 있다고 한다. 29 Dupont 등 30 의 연구에서는 Twist가 비활성화(downregulation)되면 DNA를 손상시키는 항암제인 etoposide에 대한 감수성이 증가된다고 한다. 이러한 결과들은 Twist가 약제 유도 세포자멸사(drug-induced apoptosis)에 대한 암세포의 감수성을 증가시키는 치료 표적(therapeutic target)이 될 수 있음을 시사한다. 본 연구에서는 Twist 발현이 증가할수록 무병 생존율이 낮아 지는 결과를 보였으며, 이는 Twist가 상부 요로상피세포암 수술 후 재발이나 원격 전이를 예상하는 데 도움이 되는 예후 인자가 될 수 있음을 시사한다. 총 생존율도 Twist 발현이 증가할수록 감소하는 경향을 보였지만 통계적으로 유의한 차이를 보이지는 않았는데, 이는 증례 수의 부족이 원인이었던 것으로 보인다. 따 라서 추후에 더 많은 증례를 대상으로 한 연구가 필요할 것이다.
