These mutations comprise deletions in approximately 55~65% of patients, duplications in 5~10%, and point mutations or small insertion/deletions in the remainder. Unfortunately, current diagnostic assays for dystrophin do not accurately detect duplication mutations or female carriers. In this study we employed multiplex ligation-dependent probe amplification (MLPA) analysis to detect deletions or duplications of the dystrophin gene in patients with DMD/BMD, and in potential female carriers. Methods: A total of 41 subjects was recruited for this study, comprising 35 male DMD/BMD patients, 1 female patient with Turner syndrome, and 5 females with a family history of DMD/BMD. The MLPA method was employed to determine the copy number of each of the 79 exons of the dystrophin gene in the 41 subjects. Results: MLPA analysis for dystrophin was informative in 71.4% (25/35) of patients with DMD/BMD patients, identifying deletions in 60.0% (21/35) and duplications in 11.4% (4/35). MLPA analysis showed the presence of a deletion of the DMD gene in one female patient with Turner syndrome. Of the five female patients with a family history of DMD/BMD, this assay revealed exon deletion in one and duplications in one. Conclusions: The reported findings reveal that the MLPA method is a powerful tool for detecting duplications and female carriers, as well as DMD gene deletions. MLPA should be considered the method of choice for an initial genetic analysis of DMD/BMD patients. 2 Multiplex Ligation-dependent Probe Amplification (MLPA) 방법에 의한 디스트로핀 유전자 돌연변이 분자학적 진단의 유용성 DMD보다 증상이 경미하고, 상대적으로 늦은 나이에 발현되며, 진행 양상이 완만하다. 2 디스트로핀 유전자는 Xp21.2에 위치하며, 2.4 Mb 크기의 79 개의 엑손(exon)으로 구성되어 있는 매우 큰 유전자이다. 디스 트로핀 유전자는 디스트로핀 단백질을 생성하며, 이는 427 kda 크기의 근육속막 단백질(sarcolemmal protein)로 세포외 바 탕질(extracellular matrix)과 세포질 세포골격(cytoplasmic cytoskeleton) 사이의 결합에 관여한다. 디스트로핀 단백질이 소실 혹은 감소되면 근섬유막의 안정성에 장애를 일으키고, 근 섬유의 괴사 및 재생이 발생하여 근력저하, 근육위축, 보행장애 와 호흡장애 등이 일어난다. 3,4 디스트로핀 유전자의 변이는 결손(deletion)이 가장 흔하여 전체 환자의 55~65%에 달한다. 5,6 결손 부위가 mrna의 번역 해독틀(translational reading frame)을 이동시킬 경우(out of frame deletion)에는 전사가 조기에 종식하게 되어 디스트로핀 단백질이 형성되지 않게 되고, 증상이 심한 DMD형이 발현하 게 된다. 그러나 결손에도 불구하고 해독틀이 보존되는 경우(in frame deletion)에는 분자량이 작거나, 비정상적인 단백질이 생성되어 증상이 경미한 BMD형이 발현한다. 7 이 외에도 디스트 로핀 변이 중에서 중복(duplication)은 약 5~10% 8,9 에서 발생 하며, 나머지 20~30%는 점돌연변이(point mutation), 미세결 손(microdeletion), 미세삽입(microinsertion) 등의 작은 변이 로 이루어진다. 10 DMD/BMD의 진단에는 디스트로핀 단백질의 발현 유무와 정 도를 western blot이나 면역조직화학염색 등을 이용하여 직접 확인하는 방법과, 유전자검사를 이용하여 디스트로핀 변이를 확인하는 방법이 있으며, 최근에는 후자인 유전자검사법이 보편 화되어 비교적 용이하게 진단할 수 있게 되었다. 유전자검사법 에는 Multiplex PCR (mpcr), 정량(quantitative) Southern blotting, 정량실시간(quantitative real-time) PCR, 형광제 자리교잡법(Fluorescence in situ hybridization, FISH) 및 Multiplex amplifiable probe hybridization (MAPH) 등의 여 러 가지가 있다. 그 중에서도 가장 일반적으로 사용하는 다중 PCR법은 결손 이외의 중복 돌연변이 검출이나 여성 보인자에 대한 검사가 불가능하다는 한계점을 가지고 있다 또한 이 외에도 정량 Southern blotting, MAPH는 검사 과정에 많은 시 간과 노력이 필요하여 검사하기가 힘들고, 8,14 정량실시간 PCR 은 민감점(hot spot) 이외의 엑손에 대해서는 돌연변이의 검출 이 어렵다는 점, 15 FISH는 비용이 많이 들고, 중복의 검출이 불 가능하다는 단점이 있다. 16 이러한 검사법들의 단점을 보완한 것으로 Multiplex Ligation-dependent Probe Amplification (MLPA) 방법을 새롭게 사용하고 있으며, 검사 방법이 간단하면 서도 결손, 중복 및 여성 보인자 또한 진단이 가능하다. 17,18 본 연구는 디스트로핀 유전자검사법 중 MLPA 방법을 이용 하여 DMD/BMD 환자의 진단, 그 중에서도 기존 방법으로는 검 사가 어려웠던 중복 변이의 검출 및 여성 보인자 진단에 있어서 MLPA의 유용성을 함께 알아보고자 하였다. 대상과 방법 2007년 8월부터 2009년 5월까지 본원에 내원한 35명의 DMD/ BMD 남성과 6명의 여성을 대상으로 총 41명에서 디스트로핀 유전자검사인 MLPA를 하였으며, 이들 모두에게 본 연구에 대 한 설명을 하였고 유전자 동의서를 받았다. 35명의 남성 환자는 임상 경과, 근전도검사, 근육생검에서 근디스트로피의 소견을 보이고 면역조직화학염색에서 디스트로 핀 염색의 이상 소견을 보였던 DMD/BMD 환자를 대상으로 하 였으며, 6명의 여성은 한 명의 BMD/DMD증상이 있는 터너증후 군(XO) 환자 및 가족력이 있으며 잠재적으로 보인자 가능성이 있는 5명의 여성을 대상으로 하였고, 이들은 환자의 어머니 2명 과 환자의 누나 2명, 그리고 환자의 사촌 1명이었다. MLPA는 MLPA DMD test kit (MRC-Holland, Amsterdam, The Netherlands)를 이용하였다. 프로브 혼합물 034 (DMD 엑 손 1-10, 21-30, 41-50, 61-70), 035 (DMD 엑손 11-20, 31-40, 51-60, 71-79)와 교잡하여 결합시킨 후 PCR 증폭을 하였 으며, PCR 증폭 산물은 ABI PRISM 3100 Genetic Analyzer (Applied Biosystems)로 분석하였다. 19 결 과 MLPA를 이용한 41명 중 변이가 검출된 환자의 유전자 분석 결과는 Table 1과 같다. 남성 환자 중 71.4%(25/35)에서 변이 가 검출되었고, 이 중 60.0%(21/35)에서 결손, 11.4%(4/35)에 서 중복이 관찰되었다(Table 2). 6명의 여성 대상자 중에서 증 상이 있는 터너증후군 환자에서는 결손이 그리고 5명의 잠재적 인 보인자 중에서는 환자의 어머니 2명에서 엑손 42번부터 45 번까지의 결손 1명과 엑손 52, 53, 56~61번까지의 중복 1명이 발견되었다. 결손 환자 중에서 49, 50번 엑손의 결손이 각각 10명으로 가 장 흔했고, 다음으로 흔한 위치는 45,46,47,48번으로 각각 8명 에서 발견되었다. 결손이 확인된 21명 중 20명(95.2%)이 다빈 도 결손 부위로 알려진 엑손 1~20, 엑손 44~55 사이에서 결손이 확인되었으며, 3명에서 43, 44, 51번의 단일 엑손 결손이었다. Results of Dystrophin gene analysis by MLPA method in male patients with Duchenne/Becker muscular dystrophy MLPA analysis Number of patients (%) No mutation 10 (28.6%) Mutation 25 (71.4%) Deletion 21 (60.0%) Duplication 4 (11.4%) Total 35(100%) MLPA; Multiplex Ligation-dependent Probe Amplification, Del; deletion, Dup; duplication 고 찰 MLPA는 디스트로핀 유전자의 79개 모든 엑손에 대해 정량 적인 검사가 가능하며, 결손 외에도 중복 및 여성 보인자 진단 이 가능한 획기적인 방법이다. 본 연구에서 사용된 MLPA는 45개 이상의 대상 염기서열을 동시에 증폭하여 커다란 유전자의 재배열을 빠르게 검출할 수 있는 방법으로, 22~43개의 두 올리고핵산염기로 구성된 탐색 자(probe)를 환자의 DNA와 하룻밤 정도 교잡하면 탐색자가 연 결 효소에 의해 결합된다. 이 산물이 PCR을 통해 증폭되며, 각 PCR 산물의 상대적인 양은 원하는 염기의 중복된 수에 비례하 므로 DMD 유전자의 결손과 중복을 알 수 있게 된다. 각 엑손에 대응하는 그래프의 최고치는 표준물질에 대한 상대적인 이동거 리의 차이로 구분하며, 최고치의 차이로 결손과 중복 및 보인자 를 감별할 수 있다(Fig.) 20 본 연구에서는 BMD/DMD 환자 35명 중 25명(71.4%)에서 유 전자 돌연변이 검출률(mutation rate)을 보였으며, 대만의 60.7% 21, 덴마크에서의 61.4% 17 와 비교하여 약간 높았다. 디스트로핀 유전자 변이 중 가장 흔한 결손은 60.0%(21/35) 에서 확인되었고, 이는 전체 환자 중 55~65%로 발견되었던 이 전의 연구와 비슷하였다. 6,8 디스트로핀 유전자 결손은 일정 부 위에서 호발하는 것으로 알려져 있으며, 이러한 지역을 민감점 (hot spot)이라고 한다. 디스트로핀의 민감점은 5 끝부분에서 500 kb 이내에 위치한 최초 20개의 엑손부위와 첫 번째 엑손으 로부터 약 1200 kb에 위치한 엑손 44번과 55번 사이이고, 5,6 이 두 곳 중에서 1200 kb의 중간 부위에서 결손 빈도가 가장 높다 고 보고되고 있다. 8,12,22 본 연구에서도 디스트로핀 유전자 결손 이 있는 21명 중 다빈도 결손 부위로 알려진 두 곳에서 전체 결 손 중 각각 14.3%(3/21)와 81.0%(17/21)가 발견되었다. 이전 연구에 따르면 유럽인에서는 1~20번 엑손 사이에서 21%, 41~ 60번 엑손 사이에서는 76%의 결손이 확인되었으며, 터키인에 서는 전체 결손의 15%와 85%가 이 두 지역에 편중된 양상이었 고, 본 연구 결과와 일치하는 것을 알 수 있었다. 23 이처럼 결손이 일정 부위에서 호발하는 것을 이용한 방법이 바로 현재까지 가장 많이 이용되었던 다중 PCR법이다. 다중 PCR 법은 가장 흔히 결손되는 민감점에 속한 18개 엑손(3, 4, 6, 8, 12, 13, 17, 19, 43, 44, 45, 47, 48, 49, 50, 51, 52, 60)을 동시에 증폭하여 돌연변이를 검출하는 방법으로, 이환된 환자에 있어 서 전체 결손의 약 90~98%를 검출할 수 있다. 11,12 본 연구에서 MLPA를 사용하여 결손이 검출된 25명의 환자들은 모두 다중 PCR의 18개 엑손에 포함되는 부위에서 결손이 검출되었고, 이 에 속하지 않은 결손은 단 한 명도 없었으며, 결손 검출률 자체 에는 다중 PCR과 비교하여 볼 때 큰 효율은 없었다. 또한 4명(11.4%)에서 중복(duplication)이 발생하였으며, 이 는 이전의 연구 결과인 5~10% 8,9 와 비슷하였다. 중복의 발생 위 치도 이전의 연구 결과와 비슷하게, 8 4명 모두 두 곳의 결손 돌 연변이의 민감점과 같은 부위에서 중복이 발견되었다. 결손과 중복은 모두 감수분열(meiosis) I 시기에 주로 일어나게 되는 데, 두 개의 상동염색체가 짝을 이룬 후 나누어질 때 잘못된 결 합이 이루어져, 염색체가 정렬되지 않은 상태로 분열이 일어나 게 되면, 한쪽은 결손이 있는 염색체, 반대쪽은 중복이 있는 염 색체로 나뉘게 된다 그러므로 결실과 중복이 비슷한 위치 24

5 조한나 홍지만 이경아 최영철 이 확인된 경우에는 직접 염기서열 분석과 같은 확인 검사가 필 요하며 본 연구에서도 3명의 단일 엑손 결실을 나타낸 환자에 대해서 각 엑손에 대한 단독 PCR 방법으로 이에 대해 확인하였 다. 18 그리고 결손과 중복 이외의 약 20~30% 정도에서 발견되 는 점돌연변이, 미세결손, 미세삽입 등의 작은 변이는 확인할 수 없다는 한계를 가지고 있다. 결론적으로, MLPA법은 디스트로핀 유전자의 결실뿐 아니라 중복 돌연변이 및 여성 보인자 검출이 가능한 획기적인 방법이 다. 이 방법은 이전까지 널리 사용하던 다중 PCR법 이외 다른 검사법들의 단점을 보완한 것으로, 검사 과정이 비교적 간단하 면서도 디스트로핀 유전자를 이루는 79개 모든 엑손에 대한 정 량적인 검사가 가능한, 매우 민감하고 유용한 방법이다. 앞으로 DMD/BMD 환자와 가족의 진단과 상담에 큰 도움이 될 것으로 생각하며, 추후 앞에서 언급한 이 검사법의 한계점을 보완할 수 있는 새로운 검사법의 개발 또한 필요하겠다.
