In this study, we evaluated the efficacy of PGD for Duchenne muscular dystrophy (DMD) cases by the fluorescent PCR with polymorphic linked markers and the conventional duplex-nested PCR methods. Methods: Biopsy of one or two blastomeres was done from the embryos fertilized by ICSI on the third day after fertilization. We performed two cases of PGD-DMD by the duplex-nested PCR for the causative mutation loci and the SRY gene on Y chromosome. The triplex fluorescent PCR for the mutation loci, the SRY gene and the polymorphic microsatellite marker on X chromosome was applied for two cases of PGD-DMD. Results: By the duplex-nested PCR, successful diagnosis rate was 95.5% (21/22), but we could not discriminate the female embryos whether normal or carrier in this X-linked recessive disease. However, the triplex fluorescent PCR method showed 100% (27/27) of successful diagnosis rate, and all female embryos (n=17) were distinguished normal (n=10) from carrier (n=7) embryos. Unaffected and normal embryos were transferred into mother's uterus after diagnosis. A healthy normal male was achieved after PGD with the duplex-nested PCR method and a twin, a male and a female, were delivered with triplex fluorescent PCR method. The normality of dystrophin gene was confirmed by amniocentesis and postnatal genetic analysis in all offsprings. Key Words: Preimplantation genetic diagnosis (PGD), Duchenne muscular dystrophy (DMD), Duplexnested PCR, Fluorescent PCR, Polymorphic marker 최근인간의체외수정및배아이식술분야에서, 유전적으로이상이있는환아를출산할확률이높은부부들에게정상아의출산기회를제공하기위해서착상전유전진단 (preimplantation genetic diagnosis, PGD) 방법이도입, 적용되고있다. 이러한착상전유전진단은기존의산전진단에비하여유전적으로비정상인배아의이식을배제하고정상적인배아만을선별적으로이식하여, 유전질환을갖는태아의임신과유산으로인한정신적, 육체적부담을감소시킬수있다. 현재의착상전유전진단의개념은 1968년 Edwards와 Gardner 1 에의해처음으로제시되었으며, 그후착상전유전진단의성공적인임상적용은 1990년 Handyside 등 2 에의해최초로보고되었다. 이들은 X-연관유전질환에서중합효소연쇄반응 (polymerase chain reaction, PCR) 방법을이용하여 Y 염색체의특정부위를증폭시켜성별을구분하는방법으로여성배아만을이식함으로써유전질환에이환되지않은여아의출산에성공하였다. 이후착상전유전진단은습관성유산등임신예후가좋지않은불임부부를대상으로한염색체수적이상 (aneuploidy) 검사와 cystic fibrosis 3 와 β-globin gene defect 4,5 같은여러단일유전자이상질환의위험이있는부부를대상으로전세계적으로널리시행되고있다. 이러한착상전유전진단은임신후에시행되는유전질환에대한산전진단 (prenatal diagnosis) 을대체할수있는가장이상적인방법으로생각되고있다. 본연구실에서도수년전부터착상전유전진단을시행하고있으며, 대부분염색체의수적이상이나구조적이상을대상으로형광직접보합법 (fluorescent in situ hybridization, FISH) 을이용하여시행한사례들을보고하였다. 6~9 또한, 최근 PCR 방법을이용하여근이영양증, ornithine transcarbamylase (OTC) deficiency, lactic acidosis 그리고 epidermolysis bullosa (EB) 등단일유전자이상질환에서착상전유전진단시행결과를보고한바있다. 10,11 근이영양증 (Duchenne muscular dystrophy, DMD) 은가장유병율이높은신경근질환으로써신생남아 3,500명당 1명의빈도로발생하며, 환아는점진적인근육의약화로결국 20세를전후하여사망하는것으로보고되고있다. 12 근이영양증은 X 염색체단완 (Xp21) 에존재하는 dystrophin 유전자의이상으로인해발생하는 X-연관열성유전질환이며, 13 dystrophin 유전자는 79개의 exons이약 2.5 Mb의 genomic DNA에걸쳐있는현재까지밝혀진유전자중가장큰것으로알려져있다. 14 근이영양증에대한착상전유전진단시기존에사용하였던성염색체에대한형광직접보합법이나 duplex-nested PCR 방법으로는여성배아의경우해당배아의유전형이정상인지보인자인지명확하게구분할수가없었다. 이에저자들은이러한문제점을해결하기위해 dystrophin 유전자와연관되어있는 polymorphic marker를동정하고, 이를착상전유전진단에적용하여배아의유전형을정확하게진단할수있는방법을시도하였다. 본연구에서는 duplex-nested PCR 방법과최근에새로확립한 polymorphic marker를이용한 triplex fluorescent PCR 방법을적용한근이영양증에대한착상전유전진단의효용성과임상결과를살펴보고자한다. 연구대상및방법 1. 연구대상연구대상은총 4 가계였으며, 이들은근이영양증을앓고있는아들이나친척을두고있었다. 그중 case 4를제외한나머지 3 cases는기존의 Chamberain's primer set나 Begg's primer set으로결실되어있는부위를확인할수있었다 (data not shown). 그러나 case 4의경우기존의 primer sets으로결실부위를확인할수없었기때문에환자의가족을대상으로여러가지 marker를이용한 linkage analysis를수행하여보인자인어머니와환아인아들의돌연변이 allele를확인할수있었다 (Figure 1)

Genomic DNA의분리와 single lymphocyte preparation 각부부와질환이있는아들또는친척들의혈액에서 AquaPure Genomic DNA kit (Bio-Rad Laboratories, Hercules, CA, USA) 을이용하여 genomic DNA 를추출한후사용전까지 -20 에보관하였으며, 이를이용하여각가계에서 dystrophin 유전자에대한돌연변이여부를재확인하였다. 착상전유전진단을시행하기전각 primer의 PCR 조건, amplification rate 그리고 allele drop-out (ADO) rate를조사하기위하여각환자의혈액으로부터 Ficoll-Paque density gradient separation (Ficoll-Paque TM PLUS, Amersham Biosciences AB, Uppsala, Sweden) 방법으로 lymphocytes를분리하였다. 배양접시에 Ca 2+ /Mg 2+ -free pho- sphate buffered saline (PBS) 로소적을만들고그소적에 lymphocytes 일부를넣어희석한후현미경하에서 micropipette을이용하여각각의 lymphocyte 를 5 µl의 lysis buffer (200 mm KOH, 50 mm DTT) 가들어있는각각의 PCR tube에넣었다. 이렇게준비된 single lymphocyte tube는사용전까지 -70 에보관하였다. 3. 난자의채취및배아의배양 Gonadotrophin-releasing hormone (GnRH) agonist, human menopausal gonadotrophins (hmg) 와 human follicular stimulating hormone (hfsh) 을이용하여과배란을유도하였다. 초음파로난자의크기를관찰하여최소한 2개이상의난포가 18 mm 이상, 17-β estradiol의농도가 500 pg/ml 되었을때 human cho

4 rionic gonadotrophins (hcg) 를 10,000 IU 주사하였다. hcg 주사후 34시간에질식초음파를이용하여난자를채취하였다. 채취된난자는 G-Fert 배양액 (Vitrolife Sweden AB, Kungsbacka, Sweden) 에넣어, 37, 5% CO 2, 95% 공기중배양기에서 3~4시간배양하였다. 배양후성숙된난자를대상으로세포질내정자주입술을시행하였으며, 16~18시간후에수정을확인하였다. 수정이확인된수정란은 48시간동안배양한후할구생검을실시하였다. 4. 할구생검 (blastomere biopsy) 배양 3일째 6~10 세포기의배아를대상으로할구생검을시행하였다. 생검과정을용이하게하기위하여먼저배아를 Ca 2+ / Mg 2+ 이들어있지않은배양액 (EB-10, Vitrolife Sweden AB, Kungsbacka, Sweden) 에 5분간처리하였다. Biopsy pipette 내에적당량의 acid Tyrode 용액 (ph 2.4) 을채우고 holding pipette으로배아를고정한후, acid Tyrode 용액으로투명대의일부를제거하였다. 투명대제거후뚜렷한 1개의핵을갖는할구 1~2개를생검하였다. 할구생검시에상태가양호한 6-세포기이상의배아에서는 2개의할구를생검하였다. 할구를생검한후배아는 3~4회수세한후배양액으로옮겨, 배아이식전까지배양하였다. 5. Single cell lysis 및 duplex -nested PCR analysis 준비된 single lymphocyte 또는생검할구는 alkaline lysis buffer 를이용하여용해시키고, neutralization buffer (900 mm Tris-HCl, ph 8.3, 300 mm KCl, 200 mm HCl) 를넣어중화시켰다. Duplex-nested PCR 방법에있어서첫번째 PCR의경우, 10 mm Tris-HCl (ph 8.3), 50 mm KCl, 1.5 mm MgCl 2, 각 0.2 mm dntps, 10 pmol primer pairs, 1 unit Taq DNA polymerase를혼합한후전체반응액이 30 µl가되게하였다. PCR은 DNA thermal cycler (ABI 2700, Applied Biosystems, Foster City, CA, USA) 에서수행하였으며, 96 에서 10분, 94 에서 40초 ( 처음 10 cycles 에서는 96 에서 40초 ), 각 primer의 annealing temperature에서 1분, 72 에서 1분의 cycle을 25회수행한후최종적으로 72 에서 10분간반응시켰다. 일차 PCR 반응이끝난후그산물의 1 µl를사용하여 nested PCR을진행하였다. Nested PCR의반응조건은일차 PCR과동일하게수행하였으며, 그산물은 2% agarose gel 전기영동법으로확인하였다. 또한, 착상전유전진단의오진원인중하나인오염여부를확인하기위하여 PCR 시 2개이상의 negative control tube를포함시켰으며 biopsy 당시에도배아를배양하였던배양액만을수획하여동일하게실험함으로써오염여부를확인하였다. 6. Fluorescent PCR and fragment analysis PCR 조건이서로다른 primer를사용하기때문에일차 PCR의경우는기존의 duplex-nested PCR 방법과동일한방법으로수행하였다. 일차 PCR의증폭산물 1 µl를사용하여 triplex fluorescent nested PCR 을진행하였으며 10 mm Tris-HCl (ph 8.3), 50 mm KCl, 1.5 mm MgCl 2, 각 0.2 mm dntp, 1 pmol FAM (HEX or NED) - labelled primers (Applied Biosystems, Foster City, CA, USA), 1 unit Taq DNA polymerase를혼합한후 20 µl가되게하였다. 반응조건은 94 에서 10분, 94 에서 40초, 각 primer의 annealing temperature에서 1분, 72 에서 1분의 cycle을 40회수행한후최종적으로 72 에서 10분간반응시켰다. 증폭산물은 ABI 3100 Avant automatic genetic analyzer (Applied Biosystems, Foster City, CA, USA) 를이용하여 capillary electrophoresis한후, GeneScan Analysis software version 3.7 (Applied Biosystems, Foster City, CA, USA) 과 GeneScan-ROX1000 Size Standard (Applied Biosystems, Foster City, CA, USA) 를이용하여 PCR fragment analysis를수행하였다. 결과 1. Preclinical single lymphocyte test 착상전유전진단을실제로임상에적용하기이전에환자와그가족들의 genomic DNA를이용하여유전검사결과를재확인하였으며, 그결과를토대로각각의 nested primer와 fluorescent primer를합성하였다. Genomic DNA가아닌 single cell level에서의 amplification rate와 ADO rate 그리고해당 primer의효율성을점검하기위하여 single lymphocyte

Duplex-nested PCR 방법을이용한착상전유전진단 1) 사례 1 Dystrophin exon 45가결실되어있는근이영양증아들이있는부부로서착상전유전진단을위해본원에내원하였다. 회수된난자 (n=8) 중에서 6개의성숙난자를대상으로세포질내정자주입술을수행하였다. 총 6개의배아로부터 1개씩의할구를생검하였으며그중 2개의배아에서는할구의핵이관찰되지않았지만이후의분석과정을동일하게진행하였다. 각각의할구를 dystrophin 유전자의결실부위인 exon 45와 Y 염색체특이적인 Sry를증폭시키는일반적인 duplex-nested PCR 방법을사용하여진단한결과예상과같이생검당시핵이관찰되지않았던 2개의할구에서는 PCR 산물을관찰할수없었으며, 나머지 4개의할구로부터는 2개의정상남성배아와 2개의여성배아를확인할수있었다. 그러나여성배아의경우보인자인지완전한정상유전형을가지고있는지는구분할수없었다 (Figure 2). 이중 2개의배아를이식하였으며, 그후 β-hcg 검사결과양성으로확인되었으며, 임신 17주후양수검사에서태아가염색체및 dystrophin 유전자에대해정상임을확인하였다. 최근임신 36주만에제왕절개를시행하여 2.8 kg의건강한남아를분만하였으며신생아로부터혈액을채취하여유전검사를시행한결과정상으로진단되었다 (Table 1). 2) 사례 2 근이영양증아들이있는부부로서아들의경우 dystrophin 유전자의 exons 6~17이결실되어있었으며부인은보인자로확인되었다. 이부부의경우과거에본원에서형광직접보합법을이용한성별검사방법으로착상전유전진단으로실시하여건강한여아를출산한바있으며, 다시아이를원하여내원을하였다. 첫번째 PCR-PGD cycle 에서는과거착상전유전진단시동결하였던 20개의동결란중 12개를융해하였으며, 12개모두생존하여 8개의배아로부터는각 1개씩, 4개의 6 세포기이상의배아로부터는 2개씩의할구를생검하여총 16개의할구를대상으로 dystrophin 유전자의결실부위인 exon 17과 Y 염색체특이적인 Sry를증폭하는 duplex-nested PCR 방법을사용하여착상전유전진단을수행하였다. 그결과 1개의배아에서는 PCR 산물을관찰할수없었으며, 나머지 11개의배아로부터는 2개의정상남성배아와 3개의돌연변이남성배아, 그리고 6개의여성배아를확인할수있었다. 이중정상적

Electropherogram A (top): the unaffected male partner; B: the carrier female partner; C the affected son; D and E: the carrier female embryos; F and G: the normal female embryos. 인 배아 3개를 이식하였으나 임신에는 성공하지 못 를 생검하였다. 기존의 duplex-nested PCR 방법과 하였다 (Figure 2). 그 후 두 번째 PCR-PGD cycle에 fluorescent PCR 방법을 병행하기 위해 dystrophin 서도 남은 동결란 8개를 융해하여 6개가 생존하여 exon 45 부위, Y 염색체 특이적인 Sry, 그리고 poly- 생검을 수행하였으며 이들 중 4개로부터는 2개씩의 morphic marker인 DYSI 부위를 동시에 증폭시키는 할구를 생검하여 총 10개를 대상으로 착상전 유전 triplex PCR을 수행하였다. Arrows and arrow heads indicate normal and mutant alleles, respectively. 2) 사례 4 성배아 4개, 그리고 보인자 여성배아 3개로 각각 본 사례의 경우 근이영양증 환자인 아들을 대상 진단되었으며 정상배아 4개를 이식하였으나 bioche- 으로 Chamberain's primer set와 Begg's primer set 등 mical pregnancy로 확인되었다 (Figure 4, Table 1). 총 18개의 primer로 근이영양증을 검사하였으나 결 고 실 부위가 확인되지 않은 사례로서, 연관분석을 통 찰 해 아들의 돌연변이 allele과 부인의 allele이 같음을 확인하였으며, dystrophin 유전자와 연관되어 있는 착상전 유전진단의 도입 초기에는 주로 형광직접 polymorphic marker를 이용해서 착상전 유전진단을 보합법을 이용한 염색체 구조적 이상이라든가 수적 실시하였다 (Figure 1). Polymorphic markers로는 3'- 이상에 대한 정보만을 이용하여 정상 혹은 보인자 CA와 STR44 marker를 사용하였으며 Y 염색체 특 의 가능성이 있는 배아의 이식을 시도하였었다. 형 이적인 Sry primer를 같이 증폭하는 triplex fluore- 광직접보합법 외에 PCR을 이용한 착상전 유전진단 scent PCR 방법을 수행하였다. 총 11개의 배아를 대 은 여러 가지 문제점으로 인하여 초기에는 그 결과 상으로 할구 생검을 시행하였으며, 2개의 배아에서 가 비효율적이거나 명확하지 않은 경우가 많았다. 는 1개씩의 할구, 나머지 9개의 배아로부터는 2개 그러나 이러한 문제점들을 해결하기 위하여 nested 씩의 할구를 생검하였다. 착상전 유전진단 결과 정 PCR이나 multiplex PCR 등 여러 가지 방법들이 고 상 남성배아 2개, 돌연변이 남성배아 2개, 정상 여 안되어 시행되었으며, 그 외에도 Taq polymerase의

9 linked polymorphic marker를이용한 fluorescent PCR 방법을착상전유전진단에적용하였다. 본연구에서의효용성를각방법별로분석해보면, duplex-nested PCR 방법을사용한사례 1과 2의경우총 22개배아중 21개의배아를진단하여 95.5% 의진단성공률을나타내었으며, linked polymorphic marker를 triplex fluorescent PCR 방법을사용한사례 3과 4의경우총 27개배아모두진단에성공하여 100% 의진단성공률을나타내어진단성공률에는큰차이를보이지않았다. 그러나 duplexnested PCR 방법을사용한사례들에서여성배아의경우정상과보인자여부가확실하게진단되지않아보인자를원하지않는부부에게는이식할수없었다. 따라서실제이식가능배아수는전체진단배아의 28.6% (6/21) 로나타났다. 반면에 linked polymorphic marker를사용하여돌연변이 allele의유전여부를정확하게확인할수있었던사례 3과 4의경우기존의 duplex-nested PCR 방법을사용하여 17 개의여성배아중 10개 (58.8%) 의배아를정상여성배아로정확히진단함으로써전체진단배아중 59.3% (16/27) 의이식가능배아수를확보할수있었으며성 (sex) 에관계없이정상으로진단된배아를이식할수있었다. 이와같은효율적인결과는일반적인 PCR과 agarose gel 전기영동보다더민감한 fluorescent PCR과모세관전기영동을이용한방법의적용으로가능하였으며또한, 각가계에유용한 linked polymorphic markers를이용하였기에가능하였다고생각된다. 결과적으로근이영양증에대한착상전유전진단을통해 duplex-nested PCR 방법을시행한 2 사례중 1 사례가임신에성공하였으며, fluorescent PCR 방법을사용한 2사례중 1 사례는쌍둥이임신에성공하였으며 1 사례는 biochemical pregnancy를나타내었다. 이들임신된사례의경우각각임신 17주에양수검사와분만후신생아의혈액을채취, 분석하여 dystrophin 유전자의정상여부를최종적으로확인하였다. 본논문의연구결과는근이영양증질환을대상으로국내에서최초로시도된 duplex-nested PCR 방법과 triplex fluorescent PCR 방법으로정상임신및출산에성공한착상전유전진단사례이다. 특히, polymorphic marker를이용한 fluorescent PCR 방법 의도입으로여성배아의정확한유전형을확인하여여성배아의일부 ( 이론상 50%) 를이식가능한정상배아로정확히진단함으로써이식가능한배아의숫자를증가시킬수있었다. 이러한유용한 polymorphic marker를이용한 fluorescent PCR은단일유전자이상에대한착상전유전진단의특이성과신뢰도를높일수있는효과적인방법으로생각된다. 참고문헌 1.
