1 DOI: /jkstro 전이성 뇌종양 환자에서 원발 종양 가중치에 따른 생존율 분석 인하대학교 의과대학 인하대학교병원 방사선종양학교실 곽희근ㆍ김우철ㆍ김헌정ㆍ박정훈ㆍ송창훈 목 적: 전이성 뇌종양 환자에서 전뇌조사를 시행받은 160명의 환자를 대상으로 원발 종양 가중치에 따른 생존율을 후향적으로 분석해보고자 하였다. 대상 및 방법: 2002년부터 2008년 사이에 인하대병원에서 전이성 뇌종양으로 진단받은 암환자들 중 전뇌 조사 방 사선 치료 30 Gy를 받은 160명의 환자를 대상으로 후향적으로 분석하였다. 원발성 종양이 유방암인 경우는 20명, 폐암인 경우는 103명이었다. 160명의 환자 중 척수 연수막 전이(leptomeningeal seeding) 환자를 제외한, 142명의 환자를 대상으로 예후 인자 및 Recursive Partitioning Aanalysis (RPA) 분류에 따른 생존율, 중앙생존기간과, RPA 분 류에 원발 종양의 종류에 따라 가중치를 둔 새로운 Weighted Partitioning Analysis (WPA) 분류에 따른 생존율과 중 앙생존기간을 분석하였고 RPA분류와 비교 분석하였다. 결 과: RPA분류에 의한 중앙생존기간은 분류 I (8명), 분류 II (76명), 분류 III (58명)가 각각 20.0개월, 10.0개월, 3.0개월이었으며(p=0.003), WPA 분류의 경우에는 분류 I (3명), 분류 II (9명), 분류 III (70명), 분류 IV (60명)가 각 각 36개월, 23.7개월, 10.9개월, 8.6개월로(p=) RPA 분류보다 더 우위성을 보였다. 결 론: 새로운 예후 지표인 WPA 분류가 기존의 RPA 분류보다 전이성 뇌종양 환자에서 치료 방침을 결정함에 있어 도움을 줄 것으로 생각된다. 핵심용어: 뇌전이, 전뇌 조사, 예후 지표, 중앙생존기간 서 전이성 뇌종양은 영상의학적 진단과 항암치료와 방사선 치료의 효율성이 증대되어 생존기간이 길어지면서 현재 증가 추세에 있다. 한편 방사선종양학 임상의가 전이성 뇌 종양 환자를 어떻게 치료를 하는가가 환자의 여명과 삶의 질 측면에서 매우 중요하다고 할 수 있다. 방사선치료만을 놓고 보더라도 전뇌조사, 감마나이프 혹은 사이버 나이프 를 이용한 방사선 수술 방법 등 치료 방법이 다양하고 이 를 어떻게 조합하는가에 따라 환자의 생존율에 어떻게 영 향을 미치는 지에 대해 다양한 연구가 진행되어 왔다. 1) 하지만 아직 전이성 뇌종양 환자에게 최적의 치료법이 무엇인지는 논란의 여지가 많고, 이런 이유로 최선의 치료 법을 선택할 수 있게 해주는 예후지표(prognostic index)의 이 논문은 2009년 6월 3일 접수하여 2009년 9월 3일 채택되었음. 책임저자: 김우철, 인하대학교병원 방사선종양학과 Tel: 032) , Fax: 032) hkgwak@inha.com, cancer@inha.ac.kr 본 연구는 인하대학교 교내 연구비 지원으로 수행되었음. 론 필요성이 대두되었다. 기존의 Recursive Partitioning Analysis (RPA) 분류 2) 가 있지만 환자의 치료법을 선택하기에는 잘 맞지 않는 부분들이 많고 Score Index for Radiosurgery (SIR) 분류 3) 는 치료 전 치료법을 선택하기 위한 예후 지표 여야 하는데 특정 치료를 대상으로 한 예견 인자(predictive factor)이므로 치료법을 선택하기 위한 예후 지표로서 적합 하지 않다고 할 수 있다. 4) 이에 저자들은 인하대학교 병원에서 전이성 뇌종양으로 진단 받은 환자들을 전뇌 조사하면서 얻은 경험을 바탕으 로, 방사선치료 방법을 선택하게 할 수 있는, RPA 분류에 원발 종양의 종류에 따라 가중치를 둔 Weigted Partitioning Analysis (WPA) 분류법을 제안하고자 본 연구를 실시하였다. 대상 및 방법 2002년 1월부터 2008년 1월까지 인하대학교병원 방사선 종양학과에서 전이성 뇌종양으로 진단받고 전뇌조사 방사 선치료를 받은 160명의 환자를 대상으로 하였다. 방사선치 료는 4 MV의 광자선을 이용하여 일회 3 Gy씩 주 5회, 총

2 곽희근 외 4인: 전이성 뇌종양 환자에서 원발 종양 가중치에 따른 생존율 분석 30 Gy를 전뇌에 조사하였다. 원발 종양이 폐암인 경우가 103명(64.4%)이었고, 유방암 의 경우가 20명(12.5%)이었으며, 직장암 7명, 대장암 5명, 위암 6명, 원발병소미확인전이암 5명, 자궁경부암2명, 자궁 내막암 2명, 신장암 2명, 간암 2명, 식도암 1명, 전립선암 1 명, 방광암 1명, 갑상선암 1명, 원발성 중복암 1명, 후두암 1명으로 폐암과 유방암을 제외한 나머지 원발 종양 환자는 함께 묶어 비교 분석하였다(Table 1). 160명의 환자 중 모두 두개척수조사(craniospinal irradiation)를 시행하지 않았던 척수 연수막 전이(leptomeningeal seeding) 환자 18명을 제외한 142명에 대해 환자의 예후와 관련된 인자들의 단변량, 다변량 분석을 하였고 RPA분류, WPA 분류에 따른 생존율과 중앙생존기간을 통계적 유의 성과 함께 비교 분석해보았다. WPA 분류는 다음과 같이 정의하였다. RPA 분류 I, II, III를 1, 2, 3으로 생각하고 여기에 원발 종양이 유방암일 경우 1, 폐암일 경우 2, 기타 암일 경우 3의 값을 각각 곱 하여 값을 구한다. I (1) (RPA 분류) II (2) 폐암 (2) (원발 종양) III (3) 기타 (3) Table 1. Univariate and Multivariate Analyses of Prognostic Factor Influence on Survival Variables 결 대상으로 한 160명의 중앙생존기간은 9개월이었고, 1년 생존율은 39%, 2년 생존율은 23%였다. 이중 척수 연수막 전이 환자 18명과 구제 치료를 받은 23명을 제외한 119명 의 중앙생존기간은 8개월이었고, 1년 생존율은 33%, 2년 생존율은 17%였고, 척수 연수막 전이환자 18명만을 제외 한 142명의 중앙생존기간은 9개월이었고, 1년 생존율은 41%, 2년 생존율은 23%였다. 18명의 척수 연수막 전이 환 자를 제외한 142명의 환자에 대해서 예후 인자 분석을 하 였다. 단변량 분석에서는 65세 이상의 나이, 70 이상의 Karnofsky 전신수행능력상태, 원발 종양의 조절 여부, 뇌외 전이 여부, 원발 종양의 종류, RPA분류, WPA분류, 구제치료여 부 등이 환자의 예후에 영향을 미치는 인자로 통계적으로 유의하게 나왔다. 다변량 분석에서는 Karnofsky 전신수행능력상태(p=), RPA분류(p=0.003), WPA분류(p=)가 통계적으로 유의 Age (<65 yr) KPS* ( 70) Systemic disease controlled Extracranial metastasis. of lesion ( 3, 2, 1) Primary tumor oncotype (1, 2, 3) Sex RPA WPA Salvage treatment Univariate p < < Survival Multivariate p *karnofsky performance status, recursive partitioning analysis, weighted partitioning analysis 과

RPA분류에 따른 중앙생존기간과 2년 생존율은 RPA 분 류 I (8명), 분류 II (76명), 분류 III (58명)이 각각 20.0개월 (50%), 10.0개월(24.2%), 3.0개월(0%)이었으며(Fig. 1), WPA 분류의 경우에는 분류 I (3명), 분류 II (9명), 분류 III (70 명), 분류 IV (60명)이 각각 36개월(100%), 23.7개월(45.7%), 10.9개월(14.5%), 8.6개월(0%)로(Fig. 2) RPA 분류보다 통계 적 유의성은 비슷하나 환자의 중앙생존기간과 생존율을 더 잘 설명하는 것으로 우위성을 보였다(Table 3). 고안 및 결론 전이성 뇌종양의 치료법에는 수술, 전뇌조사, 방사선수 술법 등 다양하다. 기존의 보고에 의하면, 여러 개의 병변 인 경우에는 전뇌 조사가 효과적이며, 5 8) 표재성 병변이 하나인 경우, 신경 외과적 수술이 효과적이며 9 13) 수술 후 전뇌조사를 한 경우 생존율의 향상을 보고한 바 있다. 14,15) 최근 방사선치료 기술의 발달로 이전 감마나이프 외에 사 이버 나이프 등 방사선 수술 요법이 다양해짐에 따라 전이 성 뇌종양의 치료법 또한 다양하게 되었다. 방사선수술이 기존의 신경 외과적 수술을 대체할 수 있 는가에 대해서는 다음 저자들에 의해 보고 된 바 있다. Schoggl 등 16) 에 의하면 방사선수술, 전뇌조사 치료군(67명, 중앙 생존 기간: 12개월)과 신경외과적 수술, 전뇌조사 치 료군(66명, 중앙 생존 기간: 9개월) 사이에 통계적으로 유 의한 생존율의 차이를(p=0.55) 보이지 않아 3 cm 이상의 큰 병변을 제외한 한 개의 전이성 뇌종양은 신경 외과적 수술보다는 방사선 수술을 추천하였고, O Neill 등 17) 에 의 하면 방사선 수술 치료군(23명, 중앙 생존 기간: 13개월)과 신경 외과적 수술 치료군(74명, 중앙 생존 기간: 16개월) 사이에 통계적으로 유의한 생존율의 차이는 없었다(p= 0.15)고 보고하였다. 방사선수술과 전뇌조사를 조합할 경우에 전뇌 조사의 역할에 대해서는 매우 많은 논란의 여지가 있다. 기존의 보고들에 의하면 방사선 수술 후 예방적 목적의 전뇌 조사 를 시행하지 않을 경우 국소 재발의 위험이 높아지는 것으 로 보고하고 있고, 18 22) 국소 재발 시 신경 인지학적 장애 를 동반한다고 보고하고 있다 ) 하지만 Sneed 등 20) 에 의하면 방사선수술 후 예방적 목적의 전뇌조사를 받은 환 자들의 재발 시 구제 치료로서 추가적인 전뇌조사를 받은 환자들(중앙 국소 제어 기간: 18.1개월)과 방사선 수술만 받은 환자들의 재발 시 구제 치료로서 전뇌조사를 받은 환 자들(중앙 국소 제어 기간: 19.8개월) 사이에는 통계적으로 유의한 차이가 없었다고 보고하고 있다(p=0.31)

4 곽희근 외 4인: 전이성 뇌종양 환자에서 원발 종양 가중치에 따른 생존율 분석 Sneed 등 20) 의 위와 같은 보고와, Aoyama 등 23) 의 방사선 수술과 전뇌조사후의 Mini-Mental State Examination (MMSE) 를 이용한 방사선 치료 후 신경 인지학적 부작용의 보고와 최근 암 치료법의 효율성 증대로 암환자들의 예후가 좋아 지면서 전뇌조사 이후 보통 12개월 후에 치매와 같은 방사 선 치료의 만성 부작용을 26,27) 고려하여 보면 전뇌조사는 방사선수술 후 재발하였을 때에만 구제 치료로서 시행하 는 것이 합당하다고 하겠다. 이에 저자들은 RPA분류에 원발 종양에 따른 가중치를 둔, 새로운 예후 지표인 WPA 분류를 해보았으며, 예후가 가장 좋은 WPA 분류 I인 경우는 원발 종양이 유방암이고 RPA 분류 I인 경우로 본 연구에서는 환자수가 적지만 통 계적으로 의미 있는 높은 생존율을 보였기 때문에 전뇌 조 사의 만성 부작용, 환자의 여명과 신경 인지 기능의 부작 용을 고려해 볼 때 첫 치료로 절대적으로 방사선수술이 필 요한 환자 군으로, 재발 시에도 같은 이유로 다시 방사선 수술을 고려하는 것이 옳다고 본다. WPA 분류 II인 경우 는 원발 종양이 유방이면서 RPA 분류 II이거나, 폐암이면 서 RPA 분류 I인 환자 군으로 마찬가지로 방사선수술을 먼저 시행 후 재발하였을 경우 전뇌조사를 고려하는 것이 합당하다고 생각한다. WPA 분류 III인 경우에는 중앙생존 기간이 10.9개월로 환자의 여명이 짧아 전뇌조사가 첫 방 사선 치료로 적합하나, 환자의 전신수행능력 상태가 좋다 면 첫 치료로 방사선수술을 고려해 볼 수 있다고 생각한 다. 가장 예후가 나쁜 WPA 분류 IV인 경우는 원발 종양이 폐암이면서 RPA 분류 III이거나 원발 종양이 유방암과 폐 암을 제외한 다른 종양이면서 RPA 분류 II인 경우로 환자 의 여명을 고려할 때 대증 요법 치료나 전뇌조사를 고려하 는 것이 적합하다고 생각한다. 결론적으로, 환자수가 적어 일반적으로 적용하기에는 무 리가 있지만, RPA분류에 원발 종양에 따라 가중치를 준 WPA 분류가 환자의 생존율을 더 잘 설명하였으며, 전이성 뇌종양 환자에 있어서 치료 방침을 결정할 수 있는 예후지 표로서 의미가 있을 것으로 생각된다. 참 고 문 헌 1. Materials and Methods: A total of 160 metastatic brain cancer patients who were treated with whole brain radiotherapy of 30 Gy between 2002 and 2008 were retrospectively analyzed. The primary tumor oncotype of 20 patients was breast cancer, and that of 103 patients was lung cancer. Except for 18 patients with leptomeningeal seeding, a total of 142 patients were analyzed according to the prognostic factors and the Recursive Partitioning Analysis (RPA) class. Weighted Partitioning Analysis (WPA), with the weighting being done according to the primary tumor oncotype, was performed and the results were correlated with survival and then compared with the RPA Class. Results: The median survival of the patients in RPA Class I (8 patients) was 20.0 months, that for Class II (76 patients) was 10.0 months and that for Class III (58 patients) was 3.0 months (p<0.003). The median survival of patients in WPA Class I (3 patients) was 36 months, that for the patients in Class II (9 patients) was 23.7 months, that for the patients in Class III (70 patients) was 10.9 months and that for the patients in Class IV (60 patients) was 8.6 months (p<). The WPA Class might have more accuracy in assessing survival, and it may be superior to the RPA Class for assessing survival. Conclusion: A new prognostic index, the WPA Class, has more prognostic value than the RPA Class for the treatment of patients with metastatic brain cancer.
