Methods: The medical records of 76 patients, admitted between June 1 and November 30, 2013, were reviewed retrospectively. Patients were divided into 2 groups: the nutrition consultation group (n=17) and the no consultation group (n=59). Patients were also divided into 3 groups: the enteral nutrition (EN) group (n=8), the parenteral nutrition (PN) group (n=25), and the enteral and parenteral mixed nutrition (ENPN) group. Total delivered/required caloric ratio and serum albumin, serum total protein, hemoglobin and other biochemical variables were compared in each group. Results: Mean daily required and delivered caloric/protein amount were EN group 60.0%, PN group 64.6%, and ENPN group 86.9%. ENPN group showed statistically significant difference when compared with EN group, PN group (P=0.005). When the proportion of patients who were fed more than 75% of the daily required calories was calculated, EN, PN, and ENPN showed 37.5%, 25.0% and 81.8%, respectively. ENPN group were significantly more supplied (P=0.007). Although neither the nutrition consultation group nor the non-consultation group received more than 80% of the daily required calories, the nutrition consultation group received 73% of the daily required calories whereas the no consultation group only received 46% (P=0.007). The total delivered/required protein ratio was approximately 69% of the nutrition consultation group and approximately 42% of the no consultation group (P=0.006). Conclusion: The results of providing nutritional consultation to intensive care unit patients showed an increase in the nutrition support. Nutrition education, continuous monitoring and management for nutritional support by systemic administration of a nutritional support team should be considered in order to achieve effective clinical outcomes in critically ill surgical patients. 2 Effects of Nutrition Consultation 증 발생률 증가, 병원 감염에 대한 치료 반응 저하, 의료비용 상 승 및 삶의 질 저하, 사망률 증가 등의 결과를 초래한다. 5-8 중환자실에 입원한 환자들은 대사율이 증가하고, 영양학적 대 사기전의 문제로 일반 환자들에 비하여 영양불량의 위험이 더 크다. 9 중환자의 질병 중증도가 심할수록 대사율의 변화가 크고, 적절한 영양지원이 안 될 경우 합병증 발생의 빈도가 높아지는 것으로 보고된 바 있다. 10 외과 환자의 경우에는 수술로 인한 신 체 손상으로 대사반응이 변하게 되며, 복부 수술 또는 복부 외상 으로 입원하는 경우에는 경구 섭취가 제한되는 경우가 많다. 뿐 만 아니라 위장관 질환으로 인해 장기간의 만성 영양불량인 경 우가 많아서 외과 중환자에서의 영양지원의 중요성이 더욱 더 강조된다. 그러나 영양지원의 중요성에도 불구하고 여전히 중 환자에 대한 영양지원을 간과하여 적절한 영양 공급을 받지 못 하는 경우가 있다. 본 연구의 목적은 단일 대학 병원 외과계 중환자실의 외과 환 자의 영양지원 현황을 파악하고, 영양집중지원팀에 의뢰하여 영양협진이 적용된 군과 영양집중지원팀에 의뢰되지 않고 자체 적으로 영양 지원을 시행한 군(영양협진이 적용되지 않은 군) 간의 영양공급량의 차이를 분석하여 영양지원의 중요성을 확인 하고자 하는 것이다. 대상 및 방법 1. 연구대상 2013년 6월 1일부터 2013년 11월 30일까지 6개월간 외과계 중환자실에 입원한 외과 환자 중 총 76명을 대상으로 분석하였 다. 연구대상자는 18세 이상의 성인 환자로, 2일 이상 중환자실 에서 치료를 받은 환자를 선정하였으며, 18세 미만이거나 임산 부, 수유부, 외과계 중환자실에 2일 미만으로 입원했던 환자는 제외하였다. 본 연구는 신촌 세브란스병원 임상연구윤리심의위 원회 승인 하에 시행되었으며(IRB No ), 후향적 연구로 연구대상자에 대한 동의서는 면제되었다. 2. 연구 방법 대상자의 전자의무기록을 후향적으로 검토하여 아래와 같은 자료를 수집하였다. 1) 환자의 일반적 특성 대상 환자의 성별, 연령, 신장, 체중, 진단명, Acute Physiology and Chronic Health Evaluation II (APACHE II) 점수, 병원 및 중환자실 재원일수, 중환자실 입실 사유, 영양지원 시작일, 영양 지원 경로, 칼로리 및 단백질 공급량 및 목표량대비 공급량의 비 율(공급률)을 조사하였다. 2) 생화학적 검사 및 영양 지표 영양상태의 변화를 평가하기 위하여 중환자실 입원기간의 혈 청 알부민, 혈청 총단백, 헤모글로빈, hematocrit을 확인하였다. 중환자실 입실일부터 퇴실일까지 열량, 단백질의 권장량과 실 제 공급된 양을 기록하여 계산하였다. 3) 영양상태 평가 본 연구에서는 2012년 개발된 Severance nutrition screening index를 이용하여 영양검색을 실시하였다. 11 일일 칼로리 요구 량은 단위체중당 열량을 통한 계산(rule-of-thumb)법을 사용하 여 체중 1 kg 당 25 kcal로 계산하였다. 일일 칼로리 공급량은 금 식 기간을 제외하고, 영양지원이 이루어진 기간 동안 지원된 양 의 평균값으로 하였으며, 영양 공급률은 일일 칼로리 요구량 대 비 일일 칼로리 공급량의 비율(%)로 정의하였다. 4) 자료분석 방법 연속형 변수에 대해 정규분포를 따르는 경우는 Student s t-test, 정규분포를 따르지 않는 경우는 Mann-Whitney U test 로 비교하였으며, 범주형 변수는 카이제곱 검정을 시행하였다. 연속형 변수의 각 결과는 평균±표준편차 또는 중위수(사분위 간 범위)로 표시하였고, 범주형 변수는 빈도(%)로 표시하였다. 영양지원 방법에 따른 영양 요구량, 공급량, 공급률의 차이는 one-way ANOVA로 분석하였고, Turkey-Kramer multiple comparison test로 사후비교검정을 하였다. 자료의 분석은 IBM SPSS Statistics ver (IBM Co., Armonk, NY, USA) 을 이용하였고, P<0.05인 경우 통계적으로 유의하다고 판단하 였다. 결 과 1. 일반적 특성 연령은 64.3±13.1세였고, 성별은 남성이 43명(56.6%), 여성 이 33명(43.4%)이었다. 입원 시 키는 162.2±8.6 cm, 체중은 59.5±12.1 kg이었으며, APACHE II 점수는 17.5±8.4점이었 다. 중환자실 입실 기간 동안 사망한 환자는 6명(7.9%)이었고, 중환자실 입원 사유는 주요 수술 직후 관찰 및 집중치료 목적으 로 54명(71.1%), 외상 후 입원 15명(19.7%), 수술 후 감염성 합 병증 6명(7.9%), 수술 후 폐 합병증 1명(1.3%)으로 확인되었다 (Table 1). APACHE II = Acute Physiology and Chronic Health Evaluation II; ICU = intensive care unit. 영양섭취한 환자(oral provision, oral군=17명) 등 5개의 군으 로 나누었다. 그 결과 신장(P=0.005), 체중(P=0.007), 평균재원 기간(P=0.001), 중환자실 입원기간(P<0.001)에서 각 군 간에 유의한 차이를 보였다(Table 2). 2) 영양지원 경로에 따른 칼로리 및 단백질 공급률 5개의 군 중 EN, PN, ENPN, 세 군 45명을 대상으로 각 군에 대해 영양지원 현황을 분석하였다. 칼로리 공급률은 EN군 60.0%±27.7%, PN군 64.6%± 20.1%, ENPN군 86.9%±14.3% 였다. ENPN 군의 칼로리 공급률이 EN군과 PN군에 비해 통계 적으로 유의하게 높았다(P=0.005). 그러나 평균 지원율은 약 70%로 목표량에는 부족하게 영양지원이 이루어지고 있었다. 반 면 단백질 공급률은 세 군 간에 유의한 차이는 보이지 않았다 (Table 2). Table 3. 4 Effects of Nutrition Consultation 칼로리 공급률이 75% 이상인 환자는 EN군 3명(37.5%), PN 군 6명(25.0%), ENPN군 9명(81.8%)이었으며, 75% 미만인 환자는 EN군 5명(62.5%), PN군 18명(75.0%), ENPN군 2명 (18.2%)이었다. 경장영양과 정맥영양을 혼합하여 공급한 환자 들의 영양공급률이 단독으로 시행한 환자들의 영양공급률보다 유의하게 높았다(Table 3). 중환자실 재원기간 동안 영양공급을 시행하지 않은 환자를 제 Table 4. ICU = intensive care unit; PN = patients who were exclusively received parenteral nutrition; Oral = oral provision; EN = patients who were exclusively received enteral nutrition through a feeding tube. 외한 62명의 환자들의 영양지원의 시작 시기는 PN군 1.5±1.4 일, 경구섭취군 1.9±1.1일, EN군 3.5±3.1일 순으로 중환자실 입원 후 처음 영양지원의 방법으로 정맥 영양이 가장 빠르게 시 작되고, 경장영양은 가장 나중에 시작되었다(Table 4). 3. 영양협진 적용 유무에 따른 특성 및 영양상태 1) 영양협진 적용 유무에 따른 일반적 특성 성별, 연령, 입원 시 체중, 신장 등의 일반적 특성에서 두 군 간 에 유의한 차이가 없는 것으로 나타났다. 영양협진 적용 유무에 따라 평균 재원일과 intensive care unit (ICU) 입원기간은 영 양협진 적용군과 비적용군에서 각각 51.3일과 29.7일(P=0.024), 14.9일과 5.4일(P<0.001)로 영양협진군에서 유의하게 길게 나 타났다. 2) 영양협진 적용 유무에 따른 영양상태 변화 콜레스테롤, 헤모글로빈, hematocrit은 두 군 모두 중환자실 퇴실 시 통계적으로 유의하게 감소하였고, 혈청 총단백은 두 군 Table 5. 5 Hee Sook Hwang, et al. 모두 중환자실 퇴실 시 통계적으로 유의하게 증가하였다(Table 5). 칼로리 공급률은 영양협진 적용군에서 73.48%±25.77%, 영 양협진 비적용군에서는 45.63%±36.25%로 유의한 차이를 보 였다(P=0.007). 비록 두 군 모두 칼로리 공급률이 80% 미만으 로 영양 공급이 제대로 시행되지 않았으나, 영양협진이 적용된 군이 영양협진이 적용되지 않은 군에 비하여 1.5배 이상의 공급 률을 보였다. 단백질 공급률은 영양협진 적용군에서 69.02%±25.05%, 영 양협진 비적용군에서 42.26%±34.04%로 두 군 간의 유의한 차 이를 보였으나(P=0.006), 지원율이 80% 미만이었고, 영양협 진 적용군이 영양협진 비적용군에 비하여 1.5배 이상의 지원율 을 보였다(Table 5). 3) 영양협진 적용과 영양지원 경로에 따른 임상 결과 영양지원 경로에 따른 재원 기간과 중환자실 입원기간은 영양 협진 적용 군과 비적용 군 사이에 유의한 차이는 없었다(Table 6). 칼로리 공급률은 영양지원 경로에 따라 영양협진 적용군과 비적용군 사이에 유의한 차이를 보이지 않았으나, 단백질 공급 률은 PN군에서 영양협진을 했을 때 유의하게 높은 것을 확인할 수 있었다(P=0.049; Table 7). 고 찰 입원 첫 24~48 시간 이내에 시작하는 조기 경장영양은 감염 성 합병증을 예방하는 등의 긍정적인 임상결과를 나타낸다고 알려졌다. 12 따라서 입원 후 3일 안에 충분한 경구섭취가 어렵다 고 판단되면 초기에 충분한 영양 공급이 가능하도록 경장영양 을 시작하도록 권장하고 있으나, 이에 대한 순응도나 조기 영양지원 현황에 대한 국내 외과 중환자를 대상으로 한 연구는 거의 없는 실정이다. 이에 본 연구는 외과 중환자에서 영양지원 의 현황을 파악하고 영양지원의 중요성에 대한 근거를 제공하 여 체계적인 프로토콜을 정립하는 기초자료로 사용되도록 하는 데 그 의의가 있다. ENPN군의 칼로리 공급률이 EN군이나 PN군에 비해 통계적 으로 유의하게 높았지만(P=0.005), 공급률은 약 70%로 적절 하게 공급되지 않고 있음을 알 수 있었다. Moon 등 18 의 연구결 과에서도 중환자 247명 대상으로 칼로리 요구량 대비 공급량을 비교한 결과, 142명의 ENPN군의 칼로리 공급량이 76.7%로, EN군(54명) 50.1%, PN군(51명) 42.5%에 비하여 유의하게 높 았다고 보고하였으며, 평균 영양공급률이 약 64%에 그친 것은 의료진의 인식부족과 관련되어 있다고 지적하고 있다. 본 연구 에서도 이와 유사한 결과를 보였다. 일반적으로 영양 권장량의 Table 7. 6 Effects of Nutrition Consultation 75% 미만을 섭취하면 영양 결핍의 위험성이 있는 것으로 판단 한다. 본 연구에서도 영양지원 경로에 따라 칼로리 공급률의 차 이를 75% 기준으로 하였으며, 19 그 결과 경장영양과 정맥영양 을 혼합한 경우가 경장 또는 정맥영양을 단독으로 시행한 경우 에 비해 영양 공급량이 유의하게 높은 것을 알 수 있었다. 국내 의 연구에서도 경장영양과 정맥영양을 혼합한 경우에 31.7%의 환자에서 영양공급률이 80% 미만이었고, 단독 경로로 영양지 원을 실시한 97.1 %의 환자에서 80% 미만으로 공급되었다. 18 영양지원을 받은 62명의 환자에서 영양지원 시기는 정맥영양 이 1.5±1.4일, 경구 섭취가 1.9±1.1일, 경장영양이 3.5±3.1일 순으로 정맥 영양이 가장 빠르게 공급되고, 경장영양은 가장 나 중에 시작되었다. European Society of Parenteral and Enteral Nutrition (ESPEN) 지침에서 12 3일 안에 충분한 경구섭취가 어 려울 경우에 경장 영양지원을 하도록 권장하고 있는데 본 연구에 서는 ESPEN 지침보다는 다소 지연되어 경장 영양지원이 시행 되고 있었다. 환자가 장 기능에 문제가 없고, 혈역학적으로 안정 적인 상태가 되면 24~48 시간 이내에 경장영양을 시작하도록 권장하고 있으나, 12 본 연구의 대상 환자에서 주된 영양지원 방법 은 정맥영양임을 알 수 있었다. 이는 중환자라는 특성상 혈역학 적 불안정, 혹은 외과 중환자의 경우 위장관 수술을 받은 환자들 이 대부분이기 때문에 문합부 누출에 대한 위험성을 고려한 결과 라 생각해 볼 수 있다. 20 영양협진 적용 유무에 따른 평균 재원일과 ICU 입원일은 영양 협진 적용군과 영양협진 비적용군이 각각 51.3일과 29.7일 (P=0.021), 14.9일과 5.4일(P<0.001)로 영양협진이 적용된 군 에서 유의하게 길게 나타났다. 이러한 결과는 두 군 간의 APACHE II 점수가 vs (P=0.045)으로 영양협진 적용군의 중증도가 높은 것과 연관이 있을 것이라 생각되며, 실 제 중증 환자이거나 중환자실 입원기간이 긴 환자에서 우선적으 로 영양협진이 이루어졌을 가능성 또한 있을 것으로 생각된다. 영양협진이 적용된 군의 중환자실 입실 시와 퇴실 시의 체중 은 각각 62.8 kg, 62.0 kg이었고, 영양협진이 적용되지 않은 군 의 중환자실 입실 시와 퇴실 시의 체중은 각각 58.5 kg, 59.8 kg 으로 증가하였다. 중환자실이라는 특수한 환경을 고려할 때, 이 러한 변화를 단순히 영양상태의 변화로 간주하는 것은 논란의 여지가 있다. 실제로 중환자실에 입원하는 환자는 입원 전에 많 은 환자들이 적극적인 수액요법을 받기 때문에 중환자실 입실 시에 평소 체중이 아닐 수 있다는 것을 간과해서는 안 될 것이 다. 21 또한 대상 환자들의 입원기간에 차이가 있으므로 중환자 실에 입실한 후 일정 기간이 지난 시점에서 체중을 측정한다면 중환자실 재원 기간의 차이에 따른 체중 변화를 확인하는 데 도 움이 될 것으로 생각된다. 평가항목 중 콜레스테롤, 헤모글로빈, hematocrit은 두 군 모 두 중환자실 퇴실 시 통계적으로 유의하게 감소하였고, 혈청 총 단백은 두 군 모두 중환자실 퇴실 시 통계적으로 유의하게 증가 하였다. 중환자의 중증도 정도인 APACHE II 점수는 영양협진 이 적용된 군에서 21.24점이었고, 영양협진이 적용되지 않은 군 에서 16.36점으로 영양협진이 적용된 군에서 중증도가 높은 환 자들이 많은 것으로 나타났다. 이는 중환자의 중증도 정도인 APACHE II 점수가 유의하게 높은 점에도 불구하고 생화학적 검사 수치의 추이에는 차이가 없었다는 점에서 영양협진의 필 요성을 간접적으로 보여주고 있다고 생각한다. 본 연구에서 환자에게 필요한 일일 칼로리 및 단백질 요구량 과 실제로 환자에게 공급된 일일 칼로리 및 단백질 공급량과 공 급률이 영양협진 적용군에서 비적용군보다 유의하게 높았지만, 실제 공급량은 80% 미만으로 영양지원이 적절하게 이루어지지 않고 있었다. 그러나 영양협진 적용군이 비적용군보다 1.5배 정 도 더 많이 공급되고 있었다. 이러한 결과는 중환자 관리의 여러 요소들 중에서 영양지원의 중요성에 대한 관심이 부족함을 보 여주고 있다. 중환자의 영양불량은 매우 흔하여 43%~88%까 지도 보고되며, 이는 임상적 예후를 나쁘게 할 수 있다. 22,23 이러 한 보고들과 마찬가지로 본원의 칼로리 및 단백질 공급량 모두 현저히 낮은 수준이었다. 그러나 본 연구 결과에서 영양협진 적 용군이 영양협진 비적용군에 비하여 높은 영양공급률을 보였으 므로 영양지원에 대한 인식과 임상 적용 양식의 시작을 위한 프 로토콜 적용이 필요할 것이며, 영양상태를 개선하기 위해 적극 적으로 강조할 필요가 있겠다. 본 연구에는 몇 가지 한계점이 있다. 첫째, 단일 기관의 후향 적 연구이며, 환자 구성에서 상당한 이질성이 있다는 점이다. 둘 째, 적절한 통계분석을 하기에는 그 표본수가 충분하지 못하다 는 점이다. 셋째, 내장단백질의 일종인 프리알부민은 반감기가 2~3일로 알부민에 비해 짧아 단백질 결핍을 민감하게 표시하 나 본 연구에서는 입원 시 프리알부민 처방의 일반적으로 시행 되지 않아 적절한 영양 공급 지표를 사용하기 어려웠으며, 또한 면역기능을 측정하는 총 림프절수 검사 역시 누락이 많아 영양 지표의 분석이 충분히 이루어지지 않았다. 결 론 중환자실 영양지원 현황을 확인한 결과, 칼로리 및 단백질의 영양지원이 대부분의 중환자에서 일일 요구량보다 실제 영양 공급량은 충분하지 못했다. 또한, 경장 영양의 긍정적인 효과에 도 불구하고, 외과 중환자실 입실 후 실질적인 영양지원 방법은 정맥영양이었으며, 알려진 지침과 달리 조기에 시작됨을 알 수 Volume 7, Number 1, April

7 Hee Sook Hwang, et al. 있었다. 비록 연구 디자인의 한계로 인해 영양자문의 효과가 임 상 지표의 향상을 가져오는 지에 대해서는 보이지 못하였으나, 영양협진 적용군에서 열량과 단백 공급량은 증가시켰다. 결론적으로 중환자실에 입실 직후 적극적이고 영양집중지원 팀을 통한 올바른 영양집중지원이 이루어지도록 체계적인 영양 지원 프로토콜의 개선 및 관리가 필요할 것으로 생각된다.
