2 김태훈 외: CT상 오진된 급성 충수염의 후향적 원인 분석 포는 21세-78세(평균 44세)였다. 각 병원 1명씩 총 2명의 복 부 영상의학 전문의가 총 94명 환자의 CT를 임상 정보를 모 르는 상태에서 급성충수염의 가능성을 염두에 두고 다음의 항 목들을 중심으로 후향적으로 분석하였다; 1) 비정상 충수의 유 무, 2) 충수주위 지방의 염증 소견, 3) 맹장주위 액체, 4) 맹 장주위 기체, 5) 충수결석, 6) 맹장 벽의 비후, 7) 소장의 확 장, 8) 맹장주위 지방의 양. 비정상 충수는 급성 충수염의 소 견 즉 주로 확장된(지름>7 mm) 그리고 비후된 충수를 의미 하며, 충수주위 지방의 염증 소견은 염증성 작은 결절 및 선 상음영의 증가를 의미하며, 맹장 벽의 비후유무는 맹장과 연 결된 정상 상행결장벽과 비교하여 결정하였다. 맹장주위 지방 의 양은 경도, 중등도, 풍부한 정도의 3단계로 나누었는데, 경 도는 맹장주위 1/3 이하를 지방이 감싸는 경우, 중등도는 1/32/3인 경우이고 풍부한 정도는 그 이상이다. 급성충수염의 CT 상 진단 기준은 비정상적인 충수의 소견, 혹은 충수결석과 충 수주위 염증 소견이 동반된 경우로 하였다(16). 통계적 유의 성은 SPSS 프로그램을 이용하여 Chi square test 와 Fisher s exact test를 이용하여 확인하였다. 나선식 CT 검사는 57명에서 GE Prospeed(GE Medical Systems, Milwaukee, WI, U. S. A.)를, 21명에서 GE HISpeed Advantage(GE medical systems, Milwaukee, WI, U.S.A.)를, 그리고 16명에서 LightSpeed Ultra(GE Medical Systems, Milwaukee, WI, U. S. A.)를 사용하였다. 경구 조영 제는 사용하지 않았으며, 150 ml의 비이온성 정맥 조영제 (Ultravist 300, Schering, Germany)를 자동주입기(Medrad, Pittsburgh, PA, U.S.A.)로 초당 3 ml의 속도로 주입 약 60 초 후에 촬영을 시작하였다. 절편두께 5-7 mm, 테이블 이동 속도 초당 5-7 mm, 피치 , 재구성간격 5-7 mm로 횡 격막 상연에서 골반강 하연까지 촬영하였다. 결 과 Fig. 1. Mcburney 등(16)은 이 경우 신속한 진단과 초기 수술을 강 조하였으며, 이 당시 이들이 사용한 진단방법은 주로 환자의 병력청취와 신체검사였다. 그러나 최근 초음파 기계 및 나선 식 CT의 발달로 진단방법의 많은 변화가 일어났다(16). CT는 급성 충수염을 정확히 진단할 수 있는 검사로, 병의 진행 정도, 천공의 유무, 체형 그리고 충수의 해부학적 위치에 관계없이 높은 민감도와 특이도를 보이는 것으로 알려졌다 (16). Weltman 등(14)은 절편두께 5 mm 이하 나선식 CT가 10 mm로 촬영한 나선식 CT에 비해 비정상 충수(94% vs 69%), 충수결석(38% vs 19%), 충수주위 염증(98% vs 75%) 등의 발견율이 높다고 보고했다. 정맥 조영제 주입 후 CT는 비정상 충수의 발견율을 높이는데, 특히 경증의 충수염, 복강 내 지방이 적은 환자 그리고 천공성 충수염의 경우 진단에 많 은 도움을 준다(3, 4). 특히 천공성 충수염 시 남아있는 부서 진 충수의 발견에 도움을 줄 수 있다(16) (Fig. 5). 경구용 조 Fig. 5. CT scan with contrast media in a 72-year-old woman shows a abnormal appendix tip (long arrow), extraluminal air (short arrow), cecal wall thickening (curved arrow) and surrounding inflammatory changes, allowing confident diagnosis of perforated appendicitis. 영제 복용 후 CT도 급성 충수염의 가양성율을 낮추는 것으로 알려졌다(5). 이와 같이 충수염의 진단시 CT의 우수한 역할 에 대한 연구 및 논문이 많이 보고되었지만, 반대로 CT상 급 성충수염으로 진단하지 못한 경우에 대한 관심은 상대적으로 대단히 적다(15). 급성 충수염의 가장 흔한 오진 원인 중의 하나는 복강 내 적 은 양의 지방과 관련이 있다(8, 13). 복강 내 지방은 충수의 대조도를 높이는 자연적인 조영제라 할 수 있는데, 약간의 염 증조차도 쉽게 구분할 수 있다(15). 결국, 마른 환자에서 적 은 양의 복강 내 지방으로 CT상 충수를 정확히 구별하기 힘 들 수 있다. Malone 등(8)은 급성 충수염의 진단을 내리지 못 한 총 10예의 오진 중 8예에서 마른 환자였다. Lane과 Mindelzum (18)은 총 5예의 오진 중 3예가 복강 내 지방이 적은 환자였다. 본 연구에서는 복강전체 내의 지방의 양보다 맹장 주위 지방의 양으로 평가했는데 오진군에서 경도(Fig. 2, 3) 및 풍부한 지방은 각각 54%와 8%였고, 대조군에서는 각 각 21%와 44%로 대조군에서 통계적으로 의미 있는 (p=0.001) 풍부한 맹장 주위 지방이 있었으며, 맹장 주위를 감싸는 지방의 양이 많으면 많을수록 비정상 충수의 발견율이 높아지는 것을 알 수 있었다. 영상의학과 의사는 CT에서 급성충수염의 진단 시 일반적으 로 여러 소견을 종합하여 내린다(15). 즉 충수의 팽창 유무, 충수주위 염증 소견, 맹장 벽의 국소적 비후 그리고 충수결석 등의 소견을 종합적으로 보고 판단하며, 이러한 소견이 모두 보이면 명확한 진단을 내릴 수 있다. 본 연구에서 오진군과 대 조군을 비교하면 통계적으로 유의하게 오진군에서 이러한 CT 소견 즉 팽창된 충수, 충수주위 지방의 염증, 충수결석 그리고 풍부한 맹장주위 지방의 양 등의 소견들이 적은 것을 알 수 있 으며, 반대로 소장 확장의 충수염 이외의 다른 질환을 암시하 는 소견은 더 높은 비율을 보여, 이러한 차이가 오진군에서 수 93

Materials and Methods: We reviewed the pathology reports of the patients with surgically proven appendicitis from two hospitals during a 3-year period. Thirty-seven such cases with a misdiagnosis were identified and they served as our misdiagnosed group (17 females and 20 males, mean age: 58 years, age range 1568 years). These were cases that were misdiagnosed on preoperative abdominal CT. All 57 patients in the control group (30 females and 27 males, mean age: 44 years, age range: 2178 years) had undergone laparotomy for acute appendicitis and they had been correctly diagnosed preoperatively on CT. Two abdominal radiologists evaluated the following items from all 94 CT examinations: 1) an abnormal appendix, 2) periappendiceal fat inflammation, 3) pericecal extraluminal fluid, 4) pericecal extraluminal air, 5) appendicolith, 6) cecal wall thickening, 7) small bowel dilatation, and 8) the pericecal fat content. Statistical analysis was performed using a Chisquared test and Fisher s exact test. Results: Any abnormal appendix was not visualized, even retrospectively, in 27 (73%) of the 37 patients from the misdiagnosed group, whereas it was not visualized in 13 (23%) of the 57 patients in the control group (p=0.001). Of the patients who had been misdiagnosed, inflammation of the pericecal fat was observed in 21 patients (57%) as compared to 50 (88%) patients in the control group (p=0.001). Pericecal fluid and air were noted in 15 (41%) and 9 (24%) patients, respectively, in the misdiagnosed group and in 19 (33%) and 14 (25%) patients, respectively, in the control group, (p=0.477 and p=0.901, respectively). Appendicolith was found in 3 (8%) misdiagnosed subjects and in 10 (18%) of the controls (p=0.001). Focal cecal wall thickening was noted in 14 (38%) misdiagnosed patients and in 28 (49%) control patients (p=0.28). Small bowel dilatation was noted in 5 (14%) and 4 (7%) patients in the misdiagnosed and control groups, respectively (p=0.477). The numbers of patients showing abundant pericecal fat were 3 (8%) in the misdiagnosed group and 25 (44%) in the control group (p=0.001). Conclusion: The main causes of missed diagnosis of acute appendicitis on CT are: non-visualization of an abnormal appendix and a lower index of suspicion by the radiologist. Knowledge of the aforementioned factors may help to avoid the misdiagnosis of appendicitis and so improve the diagnostic accuracy.
