The objective of this study was to evaluate the bioequivalence of two losartan formulations, Sarlortan 50 mg tablet (Chong Kun Dang, Pharmaceutical Corp. Seoul, Korea) as a test drug and Cozaar 50 mg tablet (MSD Korea, Co., Ltd., Seoul, Korea) as a reference drug. The bioavailability was evaluated based on the requirement of 20% deviation at a power of 80%. Methods: This study had a randomized, open-label, 2-period, crossover design. There was a 12-hour treatment period for each formulation, with a 1-week washout period between formulations. Each subject received one 50 mg tablet of the reference or test formulation of losartan. Blood samples were obtained during the 12-hour period after the dose in each treatment period. Concentrations of losartan in plasma were analyzed using a liquid chromatography system with tandem mass-spectrometric detection (LC/MS/MS). The primary pharmacokinetic parameters were C max (maximum concentration) and AUC t (area under the concentration-time curve from time 0 to the last sampling time). Results: A total number of 36 healthy malevolunteers participated in the study and 33 volunteers completed both treatment periods (age range, years; mean height, cm mean weight, 67.8 kg). The 90% CIs for the geometric mean ratios of the pharmacokinetic parameters (test:reference drug) were 0.96 ~ 1.07 for AUC t and 0.83 ~ 1.21 for C max, lying within the range of the 80% to 125% bioequivalence criterion. Conclusion: The obtained results indicated that pharmacokinetic exposure to Sarlortan tablet was bioequivalent to that of Cozaar tablet. 2 이윤정외 : 로살탄제제인살로탄 정의코자 정에대한생물학적동등성연구 있다 2-4,8-10). 이에반해안지오텐신 Ⅱ 수용체길항제 (angiotensin Ⅱ receptor antagonists; AⅡAs) 는 angiotensin Ⅱ의제1형안지오텐신수용체 (type 1 angiotensin II receptor; AT1) 에대한결합을선택적으로억제함으로써마른기침과같은부작용없이효과적으로혈압을조절하는것으로보고되고있다 2,7,9,10). 코자 정 (losartan potassium, 한국엠에스디제약 ) 은 1995년미국 FDA의승인을받은최초의 AⅡAs 계열경구투여약물로고혈압및고혈압을가진제2형당뇨병환자의신질환치료에쓰이고있다 1,9-12). Losartan 은경구투여시생체이용률이약 33% 이며, cytochrome P450 (CYP) 3A4와 2C9에의해경구투여된용량의약 14% 가활성대사체로전환된다 1,13). 코자 정경구투여후 losartan 과활성대사체는각각 1시간이내, 3-4시간이내에최고혈중농도 (C max ) 에도달하고, 말단소실반감기 (terminal half-life; t 1/2 ) 는각각약 2시간, 6-9 시간으로알려져있다 1,2,12). 본연구에서는국내에서새로발매하고자하는 losartan 제제인살로탄 정 ( 종근당 ) 과기존의코자 정 ( 한국엠에스디 ) 의생체이용률의동등성여부를검토하기위하여건강한한국인성인남성을대상으로생물학정동등성시험을실시하여두제제간약동학파라미터 (AUC, C max ) 의동등성을비교평가하였다. 정하고, α(1종오류 )=0.05, β(2종오류 )=0.2에서관찰된파라미터의예상평균치의차이가 20% 일때 2 2 교차시험설계에의한동등성검정에필요한최소한의피험자수는군당 14명으로산출되었으며, 20% 의탈락율을고려하여군당 18명, 총 36 명의건강한한국인남성지원자를대상으로하였다. 선정기준으로는 1) 건강진단시연령이만 19~40세인건강한성인남자, 2) 선천성또는만성질환이없고내과적인진찰결과, 병적증상또는소견이없는사람, 3) 담당의사가의약품의특성에따라설정실시한혈액병리검사, 혈액화학검사, 뇨검사등임상병리검사결과피험자로적합하다고판정된자, 4) 시험참가에앞서시험의목적, 내용, 시험약물의특성등에대하여충분히설명을듣고서면동의한자등이었다. 제외기준으로는 1) 시험개시전 1개월이내바르비탈류약물등의약물대사효소유도및억제약물의복용이나과도한음주를한자, 2) 시험개시전 10일이내시험에지장을줄우려가있는약물복용자, 3) 시험개시전 2개월이내에타임상시험에참여한자, 4) 담당의사가생물학적동등성시험에적합하지않다고판단한자등이었다. 본연구는세브란스병원의연구심의위원회 (Institutional Review Board; IRB) 의심의를거쳤으며헬싱키선언의정신에입각하여수행되었다. 본론연구대상및방법 1. 대상피험자본연구는 losartan 제제에대한기존에외국에서 25명을대상으로수행된생체이용률비교임상시험 14) 을근거로개체내변이를약 25% 라고가 2. 연구설계본연구는총 36명의피험자를두군 (A군, B군 ) 으로무작위배정하여 2군 2기교차설계로진행되었다. 제1기 A군에는대조약인한국엠에스디의코자 정 50 mg을, B군에는시험약인종근당 에서제조한살로탄 정 50 mg을 240 ml의물과함께단회경구투여하였다. 제1기투약이끝난후휴약기간인 7일뒤에제2기투약을실시하였으며, 78

Lee et al.: Bioequivalence study of two losartan formulations, sarlotan tablet and Cozaar tablet A군에는시험약을, B군에는대조약을제1기와동일한방법으로투여하였다. 피험자는시험 1주전부터음주및약물복용을일체금하였으며, 투약전일저녁부터 10시간이상공복한후투약일오전에공복상태에서대조약및시험약을복용하였다. 투약 4시간후모든피험자에게동일한식사를제공하였으며투약 10시간후에동일한저녁식사를제공하였다. 약동학적특성을평가하기위해혈액은투약전, 투약후 0.25, 0.5, 0.75, 1, 1.33, 1.67, 2, 2.5, 3, 4, 6, 8, 10, 12시간까지투약직전팔의정맥부위에설치한헤파린을채운카테터 (heparin-locked catheter) 를통하여매회 7 ml씩채취되었다. 혈액채취시간은생물학적동등성시험기준제18조 2 항에따라 15), losartan 제제의최고혈중농도도달시간 (time to maximum plasma concentration; T max ) 이약 1시간, t 1/2 이약 2시간임을고려하여, 1,2,12) T max 도달전적어도 2회이상, t 1/2 의 3배이상시점까지총 15시점을채혈하도록설정하였다. 채취된혈액은 3000 rpm에서 10분간원심분리하여혈장을분리한후분석시까지 -20 에서냉동보관하였다. 3. 혈장중 losartan 농도분석전처리된혈장시료는다음의 LC/MS/MS 조건에서정량하였다. 장치로는 Waters 사의 HPLC 를, 검출기로는 Micromass 사의 triple quadrupole mass spectrometers 를이용하여 MRM(multiple reaction monitoring) 으로표준물질 (losartan [M+H] > 207.2) 및내부표준물질 (valsartan [M+H] > 291.4) 을검출하였다. Column 은 CAPCELL PAK MG-C18(2.0 mm 35 mm, 5 μm, Shiseido, Japan) 를, 데이터처리장치로는 Micromass사의 MassLynx 4.1을사용하였다. 이동상으로는 (0.2% formic acid/acetonitrile - 45/55, v/v) 혼합용액을사용하고유속은 0.3 ml/min에서정량하였다. Losartan 표준품을 methanol 에녹여 free base로서 100 μg/ml 로만든후냉장보관시키고, 이용액을냉동보관하였던 blank 혈장으로희석하여 losartan 의혈중농도가각각 4, 10, 50, 100, 500, 1000, 1500, 2000 ng/ml 농도가되도록혈장시료를조제하였다. 각각의표준혈장 200 μl에 2% formic acid 100 μl를첨가한후, 내부표준용액 (valsartan, 1 μg/ml) 100 μl를넣고혼합하였다. 여기에추출용매 MTBE(methyl-tert-butyl ether) 1 ml를가하여 3분동안 vortex mixer 로혼합한후 rpm에서 3분간원심분리하였다. 유기용매층을취하여깨끗한튜브에옮긴후 45 에서증발, 건조시켰다. 이잔사를이동상 100 μl로재용해하여 5 μl를취하여 LC/MS/MS 에주입하여정량하였다. -20 이하에서보관하였던혈장시료는실온에방치하여녹인후 30초간진탕한다음이혈장의 200 μl를취하여튜브에옮기고여기에내부표준용액 (valsartan, 1 μg/ml) 100μL를가한후검약선작성방법과동일한방법으로전처리한후 LC/MS/MS 에주입하였다. 얻어진크로마토그램으로부터내부표준물질의피크면적에대한 losartan 의피크면적비를구하여미리작성한검량선으로부터혈장중 losartan 의농도를구하였다. 4. 약동학적특성및생물학적동등성통계분석시험약과대조약간의생물학적동등성평가를위한비교평가항목은생물학적동등성시험기준제 19조 15) 에준하여, 투약시간부터마지막혈중농도정량시간까지의혈중농도-시간곡선하면적 (AUC t ), 최고혈중농도 (C max ) 로하였으며, 최고혈중농도도 79

Demographic characteristics of study subjects (mean±sd [range]) Group A(RT) Group B(TR) Total P-value Number of subjects Age (yrs) Height (cm) Weight (kg) unpaired t-test 23±3 [19-28] 176.1±5.0 [ ] 68.2±7.5 [ ] 25±2 [22-29] 176.5±5.7 [ ] 67.4±6.7 [ ] 24± ± ± 달시간 (T max ), 말단소실반감기 (t 1/2 ), 평균체류시간 (mean residence time; MRT) 및전신청소율 (total body clearance; CL/F) 은참고파라미터로하였다. 이때약동학적파라미터들은 WinNonlin 5.2 (Pharsight, USA) 소프트웨어를이용하여비구획방법 (noncompartmental method) 으로구하였다. 이때 C max 와 T max 는관측된값을사용하였고, AUC t 는혈중농도-시간곡선그래프로부터사다리꼴방법 (trapezoidal rule) 으로계산되었다. t 1/2 는소실속도상수 (elimination rate constant; λ) 를구한후 ln2/λ 식을이용하여산출하였으며, MRT 는혈장농도에시간을곱한값에대한면적 (area under the moment curve; AUMC) 를 AUC로나누어산출하였다. 또한 CL/F는용량을 AUC로나누어산출하였다. 대조약과시험약의비교평가항목치를로그변환하여통계처리하였을때, 로그변환한평균치차의 90% 신뢰구간이 log 0.8에서 log 1.25 이내에있을때두제제는생물학적으로동등하다고판정하였으며, 순서효과 (sequence effect), 시기효과 (period effect), 제형효과 (formulation effect) 를평가하기위해 SAS 9.1 (Cary, NC, USA) 을사용하여유의수준 (α) 0.05에서일반선형모델 (General linear model) 을이용한분산분석 (ANOVA) 을시 행하였다. 결과총 36명의피험자가본연구에참여하였으며, 33명이종료하였다. 연구를종료한피험자의평균연령은 24세였고, 신장은 176.3±5.3 cm, 체중은 67.8±7.0 kg 이었다 (mean±sd). 연령을제외한, 신장, 체중은두군간에유의한차이가없었다 (Table 1). 두군간평균연령은통계적으로는유의한차이를보였으나그차이가근소하여 (Group A, 23세 ; Group B, 25세 ), 임상적으로큰의미가없을것으로사료되었다. 1. 약동학적평가결과두제제의시간경과에따른평균 losartan 혈장농도추이는 Figure 1에나타내었으며, 각약동학파라미터의평균치는 Table 2에나타내었다. AUC t 는대조약이 399.8±113.9 ng*hr/ml, 시험약이 407.7±119.6 ng*hr/ml 이었고, C max 는대조약이 246.0±129.7 ng/ml, 시험약이 249.5±129.7 ng/ ml이었다 (mean±sd). T max 는대조약은 1.0 hr [ ], 시험약은 1.0 hr [ ] 이었으며 (median 80

AUC t 의경우로그변환한대조약과시험약의평균의차의 90% 신뢰구간이 log 0.96-log 1.07, C max 의경우 log 0.83-log 1.21 로동등성기준인 log 0.8-log 1.25 이내의범위에들었다. 고찰생물학적동등성시험의목적은제제학적으로동등한용량의유효성분을가진약제를동일한투여경로로투여하였을때생체이용률에있어서동 등함을통계학적으로입증하기위함이다 15). 미국에서는 1977년에대체의약품 (generic drug) 의시판허가신청시생물학적동등성시험결과요구에관한법적근거 16) 를마련하였으며, 우리나라에서는 1989년 1월 1일이후허가받은신약에대한동일품목의의약품의시판승인과정에생물학적동등성시험의자료를요구하고있다 17. 본연구에서는 2008년 11월로신약물질특허기간이만료되는한국엠에스디의코자 정과종근당 의살로탄 정간의생체이용률을비교하였다. 본연구와동일한용량의 losartan 제제를투여한후문헌상에보고된약동학적파라미터값은 Table 4와같다 12-14,18-19). AUC의경우, 각연구에따라채혈종료시간이동일하지않아 AUC inf 를기준으로비교하였으며, 본연구로부터산출된값은 ng*ml/hr, 기존연구에서보고된파라미터값은 330.2~523.0 ng*ml/hr 범위로서본연구 82

C max 의경우, 본연구에서는 ng/ml, 문헌에보고된범위는 195.3~265.0 ng/ml 로서, 기존에보고된값과크게다르지않았다. 본연구로부터얻어진 T max, t 1/2 의경우도각각 1.0 hr, 2.1 hr 로 서, 각각문헌에서보고된값인 0.8~1.5 hr, 1.7 ~2.8 hr과크게다르지않았다. MRT와 CL/F의값은각각 2.5 hr, L/hr 로서문헌에서보고된값 3.3~3.5 hr, 114.0~116.0 L/hr과다소차이가있지만, 정확한비교를하기에는보고된예가 83

Subject-Profiles Plot 적었다. 본연구와문헌에서보고된 AUC t 와 C max 의변이계수 (coefficient of variance; CV) 는 Table 5와같다 12-14,18-20). 이때총변이는개체간변이 (interindividual variance) 와개체내변이 (intraindividual variance) 를모두포함하는개념으로개체간변이는각개체가가지는약의용출에서부터흡수, 분포, 대사, 소실에관여하는많은생리적요인의고유한특징으로인해발생하는변이를의미한다. 반면, 개체내변이는개체간변이를제외한모든변이를의미하는개념으로동일피험자내에서의제형효과, 시기효과, 채혈시각오차또는농도측정오차등을포함한다. 본연구에서총변이계수및개체내변이계수는로그변환한파라미터에대한 ANOVA 분석으로얻어진평균잔차제곱 (mean squared error; MSE) 을이용하여 CV(%)= squared(e MSE -1)*100의식으로부터산출하였다 21). 그결과, 산출된 AUC t 의총변이계수는 29%, 개체내변이계수는 13% 로, 기존문헌에서산출된총변이계수 13~38%, 개체내변이계수 10~15% 에서크게벗어나있지않았다. C max 의경우, 총변이계수 57% 는기존문헌에서산출된총변이계수 38~69% 의범위에서크게벗어나있지않았으나, 개체내변이는 47% 로기존문헌의값 22~29% 에비해상당히큰값을가지는것으로나타났다. 이 때본연구에서시험약과대조약간의 C max 비가 2 배이상되는 9명의피험자를제외한경우의개체내변이계수는 27% 로, 제외하지않은경우의 47% 에비해크게줄어듦을확인할수있었다. 이로부터본연구의개체내변이가문헌에비해크게나타난주요원인이일부피험자들의 1기와 2기간의 ( 즉제제간의 ) 농도편차때문인것을알수있었다. 본연구진행시채혈은연구계획서에명시된시간을준수하여수행되었으므로채혈시각에의한오차의발생가능성은적을것으로생각되며농도를측정하는과정에서관측자, 측정조건, 측정시간등에의해추가적으로발생한오차가본연구의개체내변이를크게한원인으로사료된다. 유의수준 (α=0.05) 에서분석한분산분석결과 AUC t, C max 에대한순서효과, 시기효과, 제형효과는각각통계적으로유의하지않았다 (Table 6). Figure 2는각군에대한피험자별약동학파라미터를제형에따라시각적으로비교한것으로, n (C max ) 는 n(auc t ) 에비해상대적으로큰개체간변이를나타내고, 순서효과, 시기효과, 및제형효과는뚜렷하지않음을보여주고있다. 이상을종합하면시험약인종근당 살로탄 정은대조약인코자 정과비교하여생물학적으로동등하다고사료되며실제환자에게적용시유효성측면에서유사한양상을나타낼것으로기대된다. 84
