Approximately 20% AAD cases are due to Clostridium difficile. Over the last decade, the incidence of Clostridium difficile-associated disease (CDAD) has progressively increased, and now a significant clinical problem. Recent change in the epidemiology of CDAD and the emergence of an epidemic hypervilruent strain suggest the need for greater attention for infection control, early diagnosis, and more effective treatment modality. However, since most cases of CDAD are both iatrogenic and nosocomial, careful selection of antibiotics, combined with proper hand hygiene and precaution by medical staffs are required. (Korean J Gastroenterol 2009;54:5-12) Key Words: Antibiotics-associated diarrhea (AAD); C. difficile; Clostridium difficile-associated disease (CDAD) 서론항생제사용이늘어남에따라항생제연관설사의빈도가증가하고있고병원환경이다양하게바뀌면서이전까지알려진질병양상과다르게나타나는경우가많아지고있다. 입원환자에서항생제를사용하면약 5-25% 에서항생제와연관된설사를경험하며그동안비교적양호한임상경과를보였던 Clostridium difficile 감염은 2003년이후변이종의전파에의해미국과캐나다를중심으로사망률과재발률이높은중증감염양상을보이기도한다. 1 Clostridium difficile 감염은매우빠르게증가하고있어미국에서는원내 Clostridium difficile-associated disease (CDAD) 로인해연간 10억달러이상의의료비가소비된다. 2 국내에선이들감염에대한관심의부족, 높은항생제사용률, 요양병원의증가등으로 상당히감염이많거나외국과유사할것으로추정되지만아직까지체계화된자료는없다. CDAD 치료에있어서가장중요한것은예방이며조기진단하여치료를시작하는것이다. 현재 CDAD의치료는 metronidazole 또는 vancomycin을바탕으로하고초기반응률은 90% 이상이지만치료후 15-30% 환자들에서는재발을경험하며, 초치료에실패하는빈도가점차증가하고있어새로운치료제에대한요구가증가하고있다. 특히 2003년부터유행하기시작한 BI/NAP/027균주는이전과달리치명적일뿐아니라기존약제에다른반응을보여많은주의를요한다. 이균주의유행은미국과유럽뿐아니라전세계적으로퍼지고있어유사한여건에놓인우리나라역시안전지대는아니다. 이글에서는 C. difficile 감염을중심으로항생제와연관된 연락처 : 한동수, , 경기도구리시교문동 한양대학교구리병원소화기내과 Tel: (031) , Fax: (031) hands@hanyang.ac.kr Correspondence to: Dong Soo Han, M.D. Division of Gastroenterology, Hanyang University Guri Hospital, 249-1, Gyomun-dong, Guri , Korea Tel: , Fax: hands@hanyang.ac.kr

2 6 대한소화기학회지 : 제 54 권제 1 호, 2009 설사에대해간략히정리해보고, 진단과현재연구중인치료법을중심으로살펴보기로한다. 본론 1. 정의항생제연관설사는특정한원인없이항생제투여후설사가발생하는것으로항생제노출 2-8일후장내에존재하는정상세균총의파괴에의해발생한다. 정상세균총은장관내에서비흡수탄수화물을발효시켜단쇄지방산 (short chain fatty acid) 을생성하는데항생제로인하여정상세균총이줄어들어탄수화물발효에이상이생기면장관내삼투압과산도가변해설사가유발된다. 대부분경미한설사가발생하고항생제를중단하면호전이된다. 항생제연관설사의약 20% 는 C. difficile 감염에의하며소수이지만 C. perfrigens, S. aureus, Candida albicans 등도원인균이될수있다. 3,4 2. 역학항생제연관설사의유발인자는광범위항생제의사용, 고령, 면역력이떨어지거나건강상태가나쁜환자, 입원력등이다. 이외에도위산억제제사용, 장관영양, 위장관수술, 항암제를사용할경우잘나타난다. 모든항생제가항생제연관설사를유발할수있지만특히광범위항생제인 cephalosporin, 광범위 penicillins, clindamycin 등이중요한유발인자이다. 5 CDAD에미치는위산억제제의영향은보고된여러연구가상반된결과를보여주기때문에결론지을수없으나적어도위산억제제를투여받는사람이다소전신상태가불량할것으로추정되기때문에어떤식으로든영향을미칠가능성이있다. 이외에만성염증성장질환환자에서더감염이쉽게일어나기도한다. 6 C. difficile은그람양성, 혐기, 포자형성균으로분변-구강경로를통해감염된다. C. difficile 감염은입원기간과비례하는데, 감염률이입원 2주째에는 10% 이지만, 4주이상입원하면 50% 까지증가한다. 새로균에노출된경우감염이발생하지만이전에감염되어보균상태에있는경우무증상으로있는경우가많다. 항생제영향없이도성인과같은정상세균총이형성되지않은신생아에선생후 12-18개월까지 C. difficile 집락 (colonization) 이형성될수있다. 이런경우대변에많은양의독소가배출됨에도불구하고장염이발생하는경우는매우드물며정상세균총이형성되는생후 2년후에는보균상태가사라진다. 7 건강한성인은혈청에독소 A에대한면역글로불린항체가있어 C. difficile 집락형성및질환발생을보호한다. 그러나항생제투여, 장수술, 항 암제치료로인해장내정상세균총에변화가생기면균집락이형성된다. 따라서 CDAD는이런집락형성에영향을줄수있는고령의입원환자중항생제투여를받거나항암치료를받는경우가장흔하게나타난다. 8 자연환경에서 C. difficile은열이나산에저항성이있는포자형태로존재하며포자들은병원침대및가구, 병실손잡이, 기구, 피부, 손톱, 환자와간병인의반지등에서검출될수있고이럴경우몇달이나병원에서잔존하게된다. 이후감수성있는환자가포자를섭취하면집락을형성하고이후포자들은대장에잠복하면서분열하여장내독을방출하게되며임상증상을유발한다. 항생제사용이나병원입원력이없이도지역사회감염의형태로발생할수있으며그빈도역시빠르게증가하고있다. 9,10 변등의보고에따르면국내에서도위험력노출없이지역사회감염의형태로감염될수있으며, 빈도또한 10.5% 로외국과유사하다. 지역사회감염군은병원감염군에비해항생제노출빈도는낮지만패혈증, 쇼크등의중증임상경과를보이고합병증의빈도또한의미있게높았다. 11 이는외국에서처럼중증 CDAD가출현할가능성이높음을시사한다. 국내에서중증감염이나변이종이중증감염에미치는영향에관한연구가미약하지만적어도산발적으로관찰되는증례가운데전형적인감염이아닌장마비, 독성결장, 천공이관찰되는것으로미루어변이종의출현가능성이높다 병인 C. difficile은두종류의독소 A (enterotoxin), 독소 B (cytotoxin) 를생성한다. 이들독소는장내단백을불활성화시켜장관세포의 cytoskeleton을파괴, 세포손상을유발하여체액을분비하고염증및세포자멸사를유도한다. 대부분균주는 19.6 kb 크기의 pathogenecity locus를가지며여기에 5개의유전자를포함하고있다. 균주의독소발현은조절인자인 TcdR과길항체인 TcdC, CodY유전자의상호작용에의해좌우된다. TcdC는독소를분비하는유전자를조절하는 TcdR에영향을미쳐독소발현을억제한다. 현재유행균주로문제되고있는 BI/NAP1/027균주는 fluoroquinolone 에내성이있으며기존균주에비해독소 A, B를 16-23배더분비하고 binary toxin을분비한다. 이들균주는 TcdC를인식하는유전자에 18-base pair 결손돌연변이가있거나, 염기서열 117번째에있는 1-bp 결손이있다. 15 최근 fluoroquinolone을많이사용하면서문제가더심각해지고있는데집단유행을한경우약반정도에서돌연변이종이관찰되고있으며이들은 moxifloxacin, gatifloxacin 등의퀴놀론계항생제에도내성을보인다. 16,17 대부분균주는독소 A, B를동시에분비하지만소수에서는독소 A 분비없이독소 B만분

3 박혜선외 1 인. 항생제연관설사의치료 7 비하기도하며이아형역시증상을유발한다. 18,19 4. 진단항생제연관설사의진단을위해서는최근 (6-8주이내 ) 항생제를사용한병력이가장중요하다. C. difficile 감염은임상증상과대변에서 toxin A 또는 B의검출, 대변배양검사에서 C. difficile이자라거나위막장염이보이면확진할수있다. 그러나위막대장염은 C. difficile 감염의심한형태일뿐이며대변내독소검사나배양검사에서양성인경우라도 50% 에서만장내위막을관찰할수있으며, 장마비가있는경우수월하게검사를시행할수없다는제한점이있다. 따라서내시경검사는위막장염이의심되는설사환자에서쉽고빠른시간내에시행할수있는진단방법이지만검사가음성이라고해서 C. difficile 감염이배제될수없다. 대변세포독소검사 (tissue culture cytotoxicity assay) 는예민도 %, 특이도 97% 로가장좋은진단방법이지만가격이비싸고시행하기위해적어도 48시간이상기다려야하기때문에임상에서이용하는데제한적이다. 현재대부분의병원에서는효소면역분석법 (enzyme immunoassay, EIA) 을이용하고있으며간단하고빠른시간에결과를알수있다. 그러나이검사법은특이도는높지만양성반응으로나타나는데적어도 100-1,000 pg의독소가존재해야하기때문에예민도가매우낮다. 20,21 처음검사해서음성으로나타난경우연소 3회측정하면예민도를 10% 정도올릴수도있다. 또한기관에따라독소 A, B 모두를검사하는경우도있지만독소 A만을검사하는경우도있으므로독소 A 음성, 독소 B 양성에의한위음성의경우를항상고려해야한다. 22,23 독소 A 음성, 독소 B 양성균주는우리나라에서도적지않은빈도로분리되고있기때문에향후 C. difficile 감염진단에유의하여야한다. 24,25 라텍스응집법 (latex agglutination assay) 은독소를확인하는것이아니라 C. difficile에필수적인세균효소인 glutamate dehydrogenase를확인하는것으로빠르고비용이저렴하지만 C. difficile 외에다른세균에서도같은효소를생성할수있기때문에민감도와특이도가좋지않다는단점이있다. 26 Stool toxin PCR (polymerase chain reaction) 은효소면역분석법과비교하여민감도와특이도가우수하기때문에효소면역분석법보다비용이높지만임상에서사용이증가하고있다. 대변내독소는매우불안정하여검체채취후실온에서 2시간이상방치하거나검사를바로시행하지않으면잘검출되지않는다. 대변독소검사는검사방법마다특이도, 민감도가다르고병원마다검사양성률이차이가있기때문에임상적으로 C. difficile 감염이의심되나효소면역법이음성인경우반드시세균배양및 PCR 검사를함께시행하여 진단에이용해야하며임상적으로 C. difficile 감염이의심된다면검사결과가나오기전조기에치료를시작하고검사결과를기다리는것이좋다. 5. 치료항생제연관설사의치료에앞서중요한것은빠른진단, 즉조기상태에서의심하는것이며항상병의중증도와위험도가높은그룹을파악하여야한다. 첫치료는설사를유발한약제를중단하는것으로시작된다. 대부분은저절로좋아지지만심한경우항생제치료가필요한경우도있다. CDAD의궁극적인치료는장의정상세균총을회복시켜 C. difficile를제거하는것이다. 치료방침은환자의상태에따라달라지게되는데젊고증상이경미한환자들은원인이되는항생제를중단하고임상경과를지켜볼수있지만대부분의급성 C. difficile 감염환자들은항생제치료가필요하게된다. 27 중증도의판단기준은연구자에따라다양하지만심한하복부통증, 설사, 복부팽만, 발열, 저볼륨증, 젖산증, 심한백혈구증가증이있으면중증으로본다. 28 1) 초치료로어떤것을선택할것인가? C. difficile 감염이의심되면유발가능한항생제사용을중단하고 metronidazole 또는 vancomycin 경구투여를고려한다. 임상에서는 metronidazole이 vancomycin에비해가격이낮고 vancomycin resistance enterococci (VRE) 발생가능성때문에일차치료제로 metronidazole을선호한다. Metronidazole은 500 mg 하루 3회 10-14일간투여하며, 경구 vancomycin은 metronidazole 일차치료에실패했거나투여금기일때사용한다. 그러나일차치료제로 metronidazole을투여하면 96% 에서호전된다고알려진초기연구와는달리 2000년이후 metronidazole 투여후치료실패및재발의빈도가증가하고있고증상호전반응이느리며 metronidazole 투여군도 vancomycin 투여군과유사한 VRE 동정률을보이는점등으로미루어일차치료제로어떤항생제를선택해야하는지는아직논란의여지가있다. 29 현재여러문헌에따르면경증감염에서 vancomycin이우월하다는증거는없지만심한설사, 백혈구증가, 신기능악화, 위막대장염, 발열등을동반한중증감염에서는치료실패율이적고증상호전이빠르기때문에경구 vancomycin (125 mg 하루 4회 ) 을일차치료약제로 10-14일사용하는것이권장된다. 최근기존균주보다더강력한균주가나타나고 metronidazole의효과가감소하는추세로미루어초치료약제로서 vancomycin 경구투여는유용할것이다. 30 Vancomycin은분자량이크기때문에경구투여시잘흡수되지않고대장에활성형으로존재하지만정맥주사할경우대장으로배출되지않아효과가없다. 다만장마비와같은경우에는약물분포를예측하기어

4 8 The Korean Journal of Gastroenterology: Vol. 54, No. 1, 2009 렵기때문에종종관장등의형태로도사용한다. 31 무증상보균자의경우항생제치료를하면균이관찰되지않다가일정기간이지나면다시균집락을형성하기때문에특별히시행하지않으며특히 metronidazole은효과가떨어진다 (Table 1). 32 2) 전격감염의치료항생제치료에도불구하고 2% 정도는전격감염으로사망하게되며특히고령, 동반질환이있는경우, 높은백혈구수치, 젖산증등이동반되었을경우사망률이높아져집중치료가필요하다. 전격감염에서병합치료의효과에대해서아직까지근거는부족하지만보통경구 vancomycin과정맥 metronidazole을함께투여하며장마비, 독성거대결장으로경구투여가불가능한경우정맥 metronidazole (500 mg 하루 4회 ) 와가능하다면 vancomycin 관장 (500 mg 하루 4회 ) 을함께투여해야한다. 33 C. difficile은독소에의해매개되는질환이므로독소를중화하거나독소와결합하여증상을완화시킬수있다는개념으로면역글로불린 ( mg/kg) 을사용해볼수있지만소규모연구뿐대규모전향연구가필요하다. 또한장천공, 복막염, 기관부전등이있으면즉시장부분절제술을고려해야하며수술후사망률은 35-80% 로예후는매우불량하다. 3) 반복되는감염의치료 C. difficile 감염환자중 15-30% 가재발을경험하게되는데대부분치료중단후 2-4주이내에발생하나일부에서는오랜기간이지나재발하는경우도있다. 고령또는과거감염력이있는사람에서잘나타나며아직까지 vancomycin에내성이있는 C. difficile은보고된바가없고 metronidazole에대한내성도매우드물기때문에재발은대부분재감염에의하고일부는완전히제거되지않는균주에기인한다. 항생제내성보다는충분한치료이후에설사가반복되는환자에서는항생제재치료시작전반드시변내독성검사 (stool toxin assay) 를시행하여감염후과민성장증후군, 염증성장질환, 현미경장염등임상증상이유사한질환과감별진단을해야한다. 또한증상이없거나경미한환자에서변내독성검사에서양성이라고해서즉시항생제치료를해서는안되며심한설사가동반되지않는한치료후추적검사를하는것은권유되지않는다. 처음재발하는경우기존에반응이좋았던항생제를그대로반복해서사용하지만이후재발하는경우에는다른항생제나 toxin binding agent, probiotics, 면역글로불린등을고려해볼수있다. 34 여러번재발한경우 6주간경구 vancomycin 펄스요법을 Table 1. Management of Antibiotics-Associated Diarrhea 9 하는것이좋다. 이는여러항생제를투여, 감수성또는저항성포자를제거하는방법으로항생제투여후항생제저항성포자가감수성이있는분열형태로변하면서항생제에반응하여제거하는방법이다. 경구 vancomycin (125 mg 하루 4번 ) 과 rifaximin ( mg 매일 ) 을 2주간격으로투여했을때좋은결과를보여주는것이한예이다 (Table 2). 4) 다양한치료방법 (1) 항생제치료 : 증상이있는감염에있어서 metronidazole이나 vancomycin이가장많이사용되지만이밖에다른항생제를사용할수있다. Rifaximin 은세균의 RNA 합성을억제하는비흡수항균제로서체내로흡수되지않아특히장기간항생제치료가필요한경우비교적안전하게사용할수있다는장점이있다. 동물과사람을대상으로한임상연구에서도 C. difficile 감염치료에있어서초치료및재발치료모두에서효과가입증되었지만내성균주의발현으로광범위한사용은제한적이다. 35 Nitazoxanide는효소의존전자전달을방해하는합성항기생충제로 metronidazole 및 vancomycin과비교하여효능에차이가없는것으로보고되었다. 36 이외에 bacitracin, ramoplanin, teicoplanin, 그리고 Rifampicin을이용한시도가있다. (2) Toxin binding agent: Cholestyramine 과같은 toxin binding agent는 C. difficile 독소와결합하여체외로배출시키면서장내정상세균총의불균형은회복시키지만항생제내성은발생하지않는다. 초기 cholestyramine을이용한연구에서는치료반응률이낮아항생제치료에보조적인역할만을하였으나최근개발된비흡수음이온중합체인 tolevamer 는 C. difficile 독소에대한높은결합능력이있어 metronidazole 또는 vancomycin과비교하여동등한치료효과가있는것으로알려져있다. 37 (3) Probiotics: Probiotics는파괴된장내정상세균총을회복시키고면역반응을증가시키며병원균과병원균의독소를제거한다. 최근메타연구에서 probiotics 예방투여는 CDAD 빈도를감소시킨다고보고하였다. 코크란분석 (Cochrane Review) 에따르면 probiotics을연구한방법이매우다양해서서로비교하는것이무리가있지만 Saccharomyces boulardii를항생제와병합투여할경우항생제로인한설사를줄이고재발을방지하는데효과가있으며, 38 다른균주역시가능성이있음을시사하고있다. 39 현재근거중심의학에서는항생제연관설사의치료나예방에 probiotics의유용성을다소폄하하고있는것이사실이나 40 감염이정상세균총과의불균형에서기인한다는점, 기존약제에잘반응하지않는감염의경우배우자의장내세균총을주입하여일부에서효과를보고있는점등을미루어좀더다양한연구가필요하다. (4) 면역요법 (Immunotherapy): 독소 A에대한면역글로불린생성이적을수록 CDAD의임상경과가심해지고재발률이높아지며, 동물실험에서면역글로불린을주입했을때임상경과가호전되고 C. difficile의장내집락형성이감소한다. 41 이러한사실을바탕으로현재전격감염이나재발감염에서다른치료방법에반응하지않는경우면역글로불린치료가시도되고있고일부에서좋은반응을보이고있지만대규모전향연구가필요한상태이다. 42 (5) Fecal bacteriotherapy: 정상인의건강한장내상 Table 2. 6 10 대한소화기학회지 : 제 54 권제 1 호, 2009 재균을이식하여정상세균총을회복시키면 C. difficile을제거할수있을것이라는개념을바탕으로, 항생제에잘반응하지않는재발 CDAD 환자에게건강한성인의변을관장또는비위관을통해주입하는방법이다. 대규모연구는아니지만환자의가족으로부터모은변을정제하여비위관이나대장내시경을통해주입한경우치료에잘반응하지않던환자에서투여한지 24시간안에임상증상이호전되고치료율이 90% 에달하는효과를보였다. 43,44 변이식을이용해장관세균총에변화를주려는시도는매우엄격한기준으로시행되고있으며아직까지대상수가많지않아결론을내리기어려우나장내항상성을유지하는측면에서난치 CDAD의치료로기대할수있다. 6. 예방 C. difficile 감염은치료보다예방이중요하다. 먼저불필요한항생제나광범위한항생제사용을자제하며항생제사용은사용기준에따른다. 대부분병원이나요양시설에서포자를통해감염되므로의료진및개인의위생관리가예방에가장중요하다. 다소논란의여지가있으나감염이의심되는환자는다른환자로부터격리하는것이좋다. C. difficile 은포자를통해감염되므로환자와접촉후알코올솜이나알코올용액으로손을씻거나기구를세척하는것은효과적이지않다. 의료기관종사자는감염환자와접촉시일회용장갑, 가운을착용하고접촉후에는반드시 chlorhexidine gluconate 성분이포함된비누로손씻기등을시행하여감염예방을위한질관리를해야한다. 또한환자가사용한시설은 sodium hydrochlorite, ethylene oxide, alkaline glutaldehyde 등의성분이포함된제제로소독하는것이예방에좋다 결론최근병독성이높은 C. difficile이증가하면서병원이나요양시설을중심으로하여 CDAD의발생률과이로인한사망률이높아지고있고이에따라조기진단, 효과적인치료법에대한관심과필요성이증가하고있다. 현재치료는비교적수월하지만재발또는불응증례들이늘어나고있으며, 임상상황이비슷한우리나라역시미국, 유럽에서일어난유행이일어날가능성이농후하다. 따라서국내역학을조사하여위험인자를분석하고국내에존재하는균주특성을파악하고이를토대로질병전파방지를위한체계적인교육과행정적인조치와대책이이루어져야한다. 참고문헌 1.
