The current epidemiology of CDI in the community could help to understand the outpatient healthcare environment and to extend infection control measures to outpatient settings. Methods: C. difficile isolates in NHIS Ilsan Hospital from 2012 to 2014 were included in this study. Clinical characteristics, acquisition types, and previous antimicrobial therapy were obtained via Electronic Medical Records. C. difficile culture was performed only in unformed stool. Toxin was positive by enzyme-linked fluorescent immunoassay (ELFA) in 247 specimens. In addition, toxin B and binary toxin gene were detected by PCR in 57 specimens. CDI was defined by toxigenic C. difficile isolation in unformed stool. Results: In the previous 3 years, 251 unduplicated C. difficile cases have been detected; 168 healthcare facility-associated hospital onset (HCFA-HO), 45 healthcare facility-associated community onset (HCFA-CO), and 38 community-associated (CA). Toxin positive rates by ELFA for toxin A&B were HCFA-HO 50.6% (84/166), HCFA-CO 41.9% (18/43), and CA 42.1% (16/38). Toxin positive rate by PCR for tcdb were HCFA-HO 62.9% (22/35), HCFA-CO 69.2% (9/13), and CA 100% (9/9). No binary toxin (cdta/cdtb) was detected in 57 cases. Conclusion: Community-associated CDI may be underestimated in Goyang province, Korea, especially by commonly used ELFA toxin assay. The spread of community-associated CDI should be recognized as an increasing burden of public health. (Ann Clin Microbiol 2016;19:7-12) Key Words: Clostridium difficile toxin B, Communityacquired infection, Epidemiology INTRODUCTION Clostridium difficile는항균제연관설사 (antibiotic associated diarrhea) 와위막성대장염 (pseudomembranous colitis) 의원인균으로잘알려져있다. 의료기관관련 C. difficile 감염은항균제의사용과기저질환등이주요위험인자로알려져있고 [1,2], 지난수세기동안계속증가해왔고증상이심한경우도많아졌다 [3]. 미국질병관리본부 (Centers for Disease Control and Prevention, CDC) 에의하면한해동안미국에서약 250,000건의 CDI가발생하고 14,000 명이사망하였다고하며, C. difficile을주의를긴급히필요로하는병원체로규정하고있다 [4]. 국내보고에따르면 2008년전국적역학조사에서독소생성 C. difficle이배양된의료기관관련 CDI는 10,000환자-일 (patient- days) 당 2.7건이었다 [5]. C. difficile는주로의료기관관련감염으로알려져있으나최근에는 C. difficile의지역사회감염에관심이높아지고있다 [6-8]. 부산지역병원에서분리된전체 CDI 의 22.6% 가지역사회에서발병하였으나, 의료기관이용과관련이있는감염이었고, 7.1% 가의료기관이용과관련이없는지역사회감염이었다고한다 [9]. 2006년부터 2010년까지응급실을내원한 272명의설사환자에서 C. difficle 배양을실시한결과 13.2% (36/272) 의환자에서 C. difficle이분리되었으며, 독소생성균주의 40.6% (13/32) 가최근병원입원력이있는환자들에서분리되었고, 59.4% (19/32) 가병원이용력이없거나퇴원한지 12주이후에증상이발생한환자에서분리되었다고한다 [10]. 이러한국내자료들로볼때우리나라에서도지역사회관련 CDI의증가가우려되는상황이다. 본연구에서는고양시를중 Received 6 October, 2015, Revised 6 January, 2016, Accepted 7 January, 2016 Correspondence: Heejung Kim, Department of Laboratory Medicine, Yonsei University College of Medicine, Yongin Severance Hospital, 225 Geumhak-ro, Cheoin-gu, Yongin 17046, Korea. MATERIALS AND METHODS 2012년에서 2014년까지경기도북부고양지역의 740병상규모의종합병원인국민건강보험공단일산병원환자를대상으로하였다. 한환자에서중복분리된 C. difficile은제외하였고후향적전자차트 (electronic medical records, EMR) 분석을통해임상적인특징을파악하였다. 감염형태의분류는 Society for Healthcare Epidemiology of America (SHEA) 지침 [11] 과 Friedman 등 [12] 이제안한의료기관관련감염에대한정의에따라 healthcare facility-associated hospital onset (HCFA-HO), healthcare facility-associated community onset (HCFA-CO), community-associated (CA) 3군으로분류하였다. HCFA-HO는입원 48시간이후균이분리된경우로, HCFA- CO는입원 48시간이전균이분리되고하나이상의의료기관관련위험인자 (12주이내의료기관에서퇴원, 30일이내항암치료정맥주사, 30일이내혈액투석 ) 있음으로, CA는입원 48 시간이전균이분리되고의료기관관련위험인자가없는경우로하였다. CDI는설사나묽은변에서독소생성 C. difficile이분리되는것으로 [13], 이전항균제사용력은 C. difficile 분리 30일이전에적어도 2일이상항균제치료로정의하였다. C. difficile 배양은설사나묽은변만을발색배지 chrom ID C. difficile agar (biomerieux, Marcy- l'etoile, France) 에접종하여혐기성조건으로 48시간배양하였고, 세균의동정은특징적인검은색집락, 냄새, 그람염색성, RapID32A (biomerieux) 를이용하였다. 독소생성여부는일차적으로효소형광면역검사법 (enzyme-linked fluorescent immunoassay, ELFA) 인 VIDAS Clostridium difficile Toxin A&B (biomérieux) 로하였고, 2013 년 10월부터 2014년 4월까지배양이의뢰된모든검체중 C. difficile이자란 57검체는 PCR을이용하여분리된균주에서 cytotoxin인 toxin B를생성하는 tcdb 유전자와임상증상이심각한 CDI와관련이있는 binary toxin을생성하는 cdta와 cdtb 유전자를검출하였다 [14]. RESULTS 지난 3년간의뢰된 C. difficile 배양은중복의뢰를제외하고 2,190건이었고, 배양양성이 251건 (11.5%, 251/2,190) 이었다. 배양양성인경우 HCFA-HO군에서 168건 (66.9%, 168/251), HCFA- CO군에서 45건 (17.9%, 45/251), CA군에서 38건 (15.1%, 38/251) 이분리되었다 (Fig. 1A). 연도별분리율을살펴보면 2012년 11.3% (78/689), 2013년 9.6% (68/712), 2014년 13.3% (105/789) 이며, HCFA-CO 군과 CA군을모두포함하는지역사회분리건수는 2012년 25건 (HCFA-CO군 16건과 CA군 9건 ) 에서 2014년 35건 (HCFA-CO군 16건과 CA군19건 ) 으로 C. difficile 분리건수에서 CA군이차지하는비율이 2012년 11.5% 에서 2014년도에는 18.1% 로증가하였다 (Fig. 1B). 지역사회분리균주중 HCFA-CO군은 82.2% 에서 C. difficile 분리전에항균제처방을받았으며, CA군은 23.7% 에서 C. difficile 분리전에항균제를사용하였다 (Table 1). 특히 HCFA-CO 군에서는 3 계열 (class) 이상의항균제를처방받은경우가전체항균제처방받은경우의 24.3% 로 CA군에비해통계적으로유의하게높았고 (P=0.01, χ 2 square, Table 1), 제1, 2세대 cephalosporin과 fluoroquinolones이흔히사용되었다 (Table 1). HCFA- CO군의 C. difficile 분리와관련된기저질환으로는만성신질환 (chronic renal disease) 이 40% 로가장흔하였으며, 그다음이악성신생물 20%, 신경과질환 (neurologic disorder) 13.4% 및재활 Fig. 1. Abbreviations: HCFA-CO, healthcare facility-associated community onset; CA, community-associated community onset. 치료 10.5% 순이었다. 무증상의정착 (colonization) 을구분하기위하여검사가가능하지않았던 4건을제외하고 247건에대하여독소생성여부를 ELFA법으로시험한결과, HCFA-HO군은 50.6% (84/166), HCFA-CO군은 41.9% (18/43), CA 군은 42.1% (16/38) 에서양성을보였다. ELFA법으로독소 A와 B의생성여부를판단하면지난 3년동안 CDI는 HCFA-HO 84건, HCFA-CO 18건및 CA 16건이었다. 일부에서만시험한결과이기는하지만 PCR을이용한 tcdb 유전자검출은 ELFA법보다 3군에서모두양성률이높아서 HCFA-HO 62.9% (22/35), HCFA-CO 69.2% (9/13), 및 CA 100% (9/9) 였다. PCR로 binary toxin 생성유전자 cdta와 cdtb 를시험한결과 57균주모두에서검출되지않았다. DISCUSSION 최근 CA-CDI의경향에대한외국보고에의하면의료기관을이용하지않은지역사회에서발생한 CA-CDI가전체 CDI의 20% 이상을차지하고, 유병률 (incidence) 은인구 100,000명당 20-30으로, CA-CDI는해마다증가하고있다고한다 [15] 년에서 2012년까지프랑스병원에서치료받은 136명의성인 CDI 환자를후향적으로분석한결과 19% 가 CA였고, 81% 가의료기관관련감염이었다 [16]. 특히이연구에서는지역사회감염이좀더임상적으로심각한경우가많았으며이는지역사회감염 C. difficile 균주가병독성이좀더강하거나, 지역사회에서 CDI에대한인식이충분하지않아서병원감염에서보다늦게의심하고검사가늦어지는경향이있기때문으로설명하고있다 [16]. 아시아에서도지역사회 CDI에대한보고들이있는데, 일본의최근현황을보면 2010년에서 2014년까지연평균환자 100,000당 1.4의 CA-CDI 유병률을보였고, 2013년에는환자 100,000당 3.4로증가하였다 [17]. 싱가폴에서는 2011년에서 2012년까지 66환자중절반인 33명이지역사회에서발생한 CO-CDI였고, 이중 9명 (27.3%) 이 CA-CDI로확인되었다고보고하였다 [18]. 특히임상적인특징이나예후는차이가없었으나 CO-CDI의경우의료기관에서발생한 HO-CDI에비해진단이늦었다고보고하고있다 [18]. CDI의중요한위험인자로항균제사용이잘알려져있다 [1,2,15-17]. 미국의 9개지역에서얻은자료를회귀모형 (regression model) 분석하여외래환자의경구용항균제처방과 CA-CDI 유병률과의관계를살펴본연구에의하면, 20세이상의성인에서경구용항균제처방을 10% 감소시키면 CA-CDI 를 17% 감소시킬수있고, penicillin과 amonxicillin/clavulanic acid이감소하는것이특히도움이된다고하였다 [19]. 지역사회 CDI의증가를막기위해서는지역사회에서의신중한항균제사용이중요한전략이되어야하겠다. 지역사회 CDI가증가하면, 독소생성 C. difficile가입원당시에검출되는것이증가할수있다. C. difficile는증상이있는환자의의료기관내전파가잘알려져있으나, 무증상의 C. difficile 보균자도감염원이될수있다 [20]. 의료기관내무증상의보균자와 C. difficile 설사환자의비는 4:1로추정되며 [21,22], 입원당시이미 C. difficile을보균하고있거나병원입원기간내에 C. difficile을획득하여무증상의보균자가된다고한다 [23]. 본연구에서지역사회에서 C. difficile가분리된환자중의료기관이용과관련된 HCFA-CO 군은항균제사용도많았고, 1 세대나 2세대 cephalosporin과 fluoroquinolones 같은여러가지
4 10 Ann Clin Microbiol 2016;19(1):7-12 계열의항균제를처방받은반면, CA군은대부분항균제처방력과동반질환이적었다. 전통적인위험인자가없으며의료기관이용과관련이없는지역사회에서분리된 C. difficile이적지않은것을볼때, 관심을충분히기울이지못하는사이이미지역사회에 C. difficile이상당히전파되었을가능성이있다. 독소생성결과는기존에알려진대로 ELFA법과 PCR법간에민감도의차이가컸는데 [24,25], ELFA법을이용하여 C. difficile 독소를검출할경우지역사회감염을실제보다적게진단할위험성이있다. 본연구에서 ELFA법으로는 HCFA-CO군과 CA군이각각 41.9% 와 42.1% 로유의한차이를보이지않았다 (P=0.421, χ 2 square). PCR법으로는 HCFA-CO 군과 CA군이각각 69.2%, 100% 로차이를보였으나, 시험한균주수가충분하지못하여결론을내리기힘들었다. 최근에는 CDI의치료가점점어려워재발의가능성이높아지고있고 [26], 약 10-40% 환자에서치료후에도설사변에서독소를생성하는 C. difficile가검출되고있어 [27], C. difficile 감염관리는점점어려워지고있다. Binary toxin을생성하는 C. difficile도문제인데, 이에의한 CDI는임상증상이좀더심각하고, 30일치명률 (mortality) 이더높다고알려져있다 [28]. 본연구에서실험한 57균주에서 binary toxin 생성유전자인 cdta 와 cdtb은검출되지않아아직까지이지역에서 binary toxin 생성 C. difficile은드문것으로생각되었다. 본연구진행에몇가지제한점이있었다. 첫번째, 지역사회감염의정의가잘확립되어있지않아 Society for Healthcare Epidemiology of America (SHEA) guidelines [11] 과 Friedman 등 [12] 의분류를사용하였다. 두번째, CDI는독소생성 C. difficile의분리가확인되어야하는데, 일차적으로사용한 ELFA 독소검사법이민감도가충분치않아 CDI가실제보다적게파악되었을가능성이크다. 일부균주에서시행한 PCR 결과를보면 HCFA-HO, HCFA-CO, CA 3군모두에서 ELFA법보다양성율이높아, 정확한 CDI 현황을파악하기위해서는독소검사법의민감도를고려해야하겠다. 세번째, 후향적으로본원의 EMR 검토를통해자료를얻었기때문에타의료기관이용에대한모든자료를얻지못하였다. 다만대학병원이나 3차의료기관에비해본원은지역사회병원으로지역주민의충성도가높은특징은고려되어야하겠다. 마지막으로연구의대상이일부지역에국한되어전체적인자료를대변하지못하는제한점이있다. 본연구는경기도일부지역에국한되는자료이기는하지만 C. difficile의지역사회감염에대한최근현황연구로서의의가있다. 본연구에서적지않은수의 C. difficile이지역사회에서분리되는것을확인할수있었으며, 지난 3년간추이를보면전체지역사회에서분리균주수는차이가없었으나, 지역사회 C. difficile 분리가차지하는비율이 2012년도 11.5% 에서 2014년도 18.1% 로증가하고있었다. 이는흔히항균제사용과관련된의료기관감염의원인으로잘알려진 C. difficile의지역사회확산 을우려하게하는것으로공중보건에새로운부담이될수있다. 결론적으로고양지역에서지역사회 CDI는드물지않으며앞으로, 지역사회감염균으로서 C. difficile에관심을가지는것이필요하겠다. 검사법에따라실제보다적게진단될수있으므로주의가필요하며, 설사환자에서 C. difficle 검사를고려할필요가있어보인다. 앞으로전국적인규모로지역사회 CDI현황에관한연구가요구된다. 6 12 Ann Clin Microbiol 2016;19(1):7-12 = 국문초록 = 공중보건에부담이되는 Clostridium difficile 지역사회감염의증가 1 국민건강보험일산병원진단검사의학과, 2 연세대학교의과대학진단검사의학교실, 세균내성연구소김영아 1, 임정훈 2, 최민혁 2, 김희정 2, 이경원 2 배경 : 유럽과북미를중심으로지역사회 Clostridium difficile 감염의증가가보고되고있지만, 국내보고는아직충분치않은실정이다. 최근의지역사회 CDI 현황에대한자료는의료기관내감염으로알려져왔던 CDI의지역사회로의확산과감염관리필요성에대한근거가될수있다. 방법 : 2012년부터 2014년까지국민건강보험공단일산병원에서분리된 C. difficile 균주를대상으로하였다. 임상적인특징, 지역사회또는의료기관에서감염, 의료기관관련인자및이전항균제사용력등은전산화된의무기록을검토하였다. 설사변이나비고형변에서만 C. difficile 배양을실시하였다. 독소생성여부는일차적으로효소형광면역측정법으로검사하였고, 추가로 57 검체에서 toxin B와 binary toxin 생성여부를 PCR로확인하였다. CDI는비고형변에서독소생성 C. difficile의분리로정의하였다. 결과 : 지난 3년간의뢰된 C. difficile배양은중복의뢰를제외하고 2,190건이었고, 배양양성이 251건 (11.5%, 251/2,190) 이었다. 배양양성인경우 healthcare facility-associated hospital onset (HCFA-HO) 군에서 168건 (66.9%, 168/251), healthcare facility-associated community onset (HCFA-CO) 군에서 45건 (17.9%, 45/251), community-associated (CA) 군에서 38건 (15.1%, 38/251) 이분리되었다. 연도별분리율을살펴보면 2012년 11.3% (78/689), 2013년 9.6% (68/712), 2014년 13.3% (105/789) 이며, HCFA-CO군과 CA군을모두포함하는지역사회분리건수는 2012년 25건 (HCFA-CO군 16건과 CA군 9건 ) 에서 2014년 35건 (HCFA-CO군 16건과 CA군 19건 ) 으로, 분리건수에서 CA군이차지하는비율이 2012년 11.5% 에서 2014년도에는 18.1% 로증가하였다. 형광효소면역검사법으로독소 A와 B를검출한결과, HCFA-HO 군 50.6% (84/166), HCFA-CO군 41.9% (18/43) 및 CA군 42.1% (16/38) 의양성률을보여 3군간에큰차이는없었다. 총 57주에서추가로 PCR로 tcdb 독소유전자를검출하였는데 HCFA-HO 군 62.9% (22/35), HCFA-CO군 69.2% (9/13), CA군 100% (9/9) 로독소검사법에따른차이를보였다. 좀더심각한임상증상과관련된 binary toxin (cdta/cdtb) 유전자는 57주모두검출되지않았다. 결론 : 고양지역에서지역사회 CDI는드물지않으며, 특히검사법에따라실제보다적게검출될수있으므로주의가필요하다. 지역사회에서 CDI의확산은공중보건에새로운부담이될수있으므로관심을기울여야하겠다.
