We aimed to compare the efficacy and outcome between transobturator and retropubic mid-urethral sling procedures. Materials and Methods: We reviewed the chart of 186 consecutive women who had sling procedures with a follow-up of at least 1 year from 2001 to The retropubic sling procedure was perfomed on 76 women assigned to retropubic group. A total 110 women received transobturator slings assingned to transobturator group. The preoperative evaluations, the indices of voiding, and postoperative complications were analysed by Student's t-test and chi-square. Results: Two groups were similar in their preoperative characteristics and the perioperative parameters except the period of hospitalization and operative time. There were significant difference between the 2 groups in terms of the postoperative obstructive and iatrogenic complications. group had shorter hospitalization and operative time and lower complications than retropubic group. Conclusions: Because transobturator sling procedures showed better results and less complications than retropubic slings, it is a safe and efficient surgical treatment for female SUI. (J. 2 김태흥 권종규 이신영 김태형 학적기전에대한이해의증가로안전하고간편하며덜 침습적인많은수술방법들이소개되고있다. 슬링수술 은 Delancey (2) 가 hammock hypothesis' 로복압성요실금의 병태생리를기술함에따라가장효과적인치료로알려지 면서대중화되었다. 1995년에 Ulmsten 등 (3) 에의해고 안된 tension-free vaginal tape (TVT) (Gynecare, Ethicon, Somer-ville, USA) 는많은연구에서여성복압성요실금의 안전하고매우효과적인치료법으로보고되고있다 (4). 2001년에 Delorme (5) 은방광, 장, 혈관등의손상이적은 더안전한경폐쇄공경로 (TOT; trnasobturator tape) 를이 용한방법을고안하였으며 2003년에 Leval (6) 은폐쇄공 을 inside-out 방식으로통과하는 TVT-O 를소개하였다. 이에저자들은여성복압성요실금환자에서최근개발 된질강을통한요실금교정술중치골후경로를이용 한방법 (retropubic sling procedure; TVT & SPARC) 과경 폐쇄공경로를이용한방법 (transouturator sling procedure; Monarc & TVT-O) 의효과와안전성을비교하기위해조 사하였다. 대상및방법 2001년 12월부터 2006년 1월까지본원에서여성복압 성요실금으로치골후경로술식과경폐쇄공경로술식 을시행받은환자중 1년이상추적관찰이가능했던 186 명을대상으로후향적으로조사하였다. 본원에서치 골후경로술식 ( 시행시기: 2001년 12월 2005년 12 월, 중앙값 2003년2 월) 으로는 TVT, SPARC 을시술하였으며, 경폐쇄공경로술식 ( 시행시기: 2003년 3월 2006년 1 월, 중앙값 2005년 1 월) 으로는 Monarc와 TVT-O를시술하였 으며두명의술자에의해시술되었다. 치골후경로술식과경폐쇄공경로술식각각에대하 여수술전환자특성, 병력 ( 연령, 분만력, 체질량지수, 골반수술병력), 수술후퇴원까지기간, 수술시간, 배뇨 지표 ( 수술전저장증상, 신체검사, 요역동학검사, 수술 후저장증상), 수술후폐색성합병증 (100cc 이상잔뇨, 도뇨관삽입술), 수술후합병증 ( 방광천공, 요도손상, 요로감염, 질천공, 혈색소손실) 을조사하였다. 수술방법은TVT 는Ulmsten 등(7), SPARC은Deval 등(8) 이기술한방법으로, Monarc은outside-in, TVT-O는inside-out 방식으로하였으며, 마취는척추마취, 전신마취또는국 소마취를이용하였으며, 수술시간은마취유도와회복시 간을제외한술기시작부터끝날때까지의시간으로하 였다. 각군의모든환자에서수술중방광손상여부를 알기위해방광경검사를시행하였다. 도뇨관은수술다 음날제거하여자가배뇨를시도하였으며배뇨후잔뇨를 측정하여잔뇨량이 100ml 이상인경우또는도뇨관유치 를 1일이상한경우또는테이프제거를한경우를폐 색성합병증으로분류하였다. 마취방법과관계없이수술 다음날배뇨곤란이없는경우퇴원하였다. 수술후외래 에서객관적인성공률을묻는질문을시행하였고결과 판정은 Stamey (9) 의기준을따랐으며수술후요실금이 전혀없는상태를완치로, 수술전에비해요실금이호전 되었으며특별한치료가필요없을정도의경도의요실 금이있는경우는호전으로, 수술후요실금의정도가변 하지않거나환자가수술결과에만족하지못한경우를 실패로정의하였으며, 호전이상을수술성공으로간주 하였다. 통계분석은 Student's t-test와 chi-square를 Microsoft Office Excel 2003 을이용하여분석하였다. 1. 대상군의특성 결 과 치골후경로술식을받은환자 76명중 Gynecare TVT 와 SPARC 술식을받은환자는각각 55 명, 21명이었으 며, 경폐쇄공경로술식을받은환자 110명중 Monarc와 TVT-O 술식을받은환자는각각52 명, 58 명이었다. 치골 후경로술식군과경폐쇄공경로술식군의평균나이는 각각 51.8±8.9, 50.3±9.5 세, 평균분만력은 2.5±0.9, 2.3±1.1 회, 술전골반수술력은 27.6%, 28.2% 로두군간에유 의한차이는없었다. 수술후재원기간은 3.2±2.5 일, 2.3±1.6 일 (p=0.03), 수술시간은 31.6±11.9 분, 26.7±6.3분 (p=0.004) 로통계적으로유의하게경폐쇄공경로술식군 에서짧았다 (Table 1). 2. 배뇨증상및배뇨지표 빈뇨, 요절박, 절박성요실금의빈도는치골후경로술식 군과경폐쇄공경로술식군에서각각 43.6% 와 74.5%, 132 대한배뇨장애요실금학회지

4 김태흥 권종규 이신영 김태형 의한차이는없었다. 수술후빈뇨, 요절박, 절박성요실금은 두군사이에차이는없었으며, 수술전에비해서는통계적 유의성은없지만배뇨증상의호전을보였다 (Table 2). 차이가없었다 (Table 3). 4. 폐색성합병증 3. 수술성공률 치골후경로술식군과경폐쇄공경로술식군에서완 치, 호전, 실패는각각 86.8% 와 89.1%, 7.9% 와 7.3% 그리 고 5.3% 와3.6% 로, 성공률은 94.7% 와96.4% 로두군에서 100cc 이상의잔뇨를보인경우, 도뇨관삽입술이필요 한경우, 테이프제거술이필요한경우는치골후경로 술식군과경폐쇄공경로술식군에서각각 25% 와 17.3%, 21.0% 와 4.5%, 1.3% 와 0% 로전체적으로 47.4% 와 21.8% (p<0.001) 로경폐쇄공경로술식군에서통계적으로유의 Table 3. 5 여성복압성요실금환자에서경폐쇄공경로와치골후경로를이용한술식의단기치료성적비교 하게폐색성합병증이적게관찰되었다 (Table 4). 5. 수술후합병증 방광천공, 요도손상, 요로감염, 질천공과같은합병증 은치골후경로술식군과경폐쇄공경로술식군에서각 각 7.9% 와 0.9% (p=0.025) 로경폐쇄공경로술식군에서 통계적으로유의하게낮게관찰되었다. 두군에서각각 었다 (Table 5). 혈색소손실은 1.6g/dl, 1.4g/dl로두군사이에차이는없 고 현재까지여성복압성요실금에대하여다양한치료가 시도되어왔고수술적치료가가장효과적인것으로알 려져왔다. 슬링수술은 Delancey (2) 가 hammock hypothesis' 로복압성요실금의병태생리를기술함에따라널리대중 화되었으며사용되는재료와술기에따라서여러가지 방법이소개되어왔다. 1993년 Petros와 Ulmsten 등 (10) 이 제시한 Integral theory를기초로 1996년 Ulmsten이보고하 면서부터본격적인중부요도슬링수술이시작되어현재 까지이와유사한수술법및재료가개발되고있다. SPARC와 TVT가대표적이라할수있으며효과와안전 성면에서기존의요실금수술에견줄만하다 (11). 최근에 는방광및방광이외의장기손상을줄이기위한대체 접근수술법으로서개발된폐쇄공을통한시술 (Monarc, TVT-O) 도증가추세이다. 다양한중부요도슬링수술이개발되면서이들수술법들 간의치료효과를비교한연구보고도점차증가하고있 다. detayrac 등 (12) 은 1년간기존의 TVT와 TOT의무작 위, 전향적비교연구에서두군간의수술성공률에는 차이가없으나, TOT가수술시간도짧고방광천공도없으 며, 술후요폐도현저히적으므로더효과적인방법이라 고하였다. Morey 등 (13) 은다기관연구에서 transobturator (TO) 술식과 transabdominal (TA) 술식에서패드사용개수 에서비슷한감소효과 찰 (TO 89.0% vs TA 86.0%) 를보인 다고하였다. 본연구에서는이들보고와같이경폐쇄공 경로술식군의수술성공률은 96.4% 로치골후경로술 식군의 94.7% 와차이가없었다 (Table 3). 수술후재원기 간이나수술시간도경폐쇄공경로술식군에서치골후 경로술식군보다짧게나타났다 (Table 1). TVT 수술은슬링이방광경부나요도를압박하지않은 상태에서배뇨시에만중부요도에역동적비틀림 (dynamic kinking) 을주어서요자제를이루기때문에원리상수술 후배뇨곤란이발생할가능성이낮다. 그러나 TVT 수술 후에도배뇨곤란합병증은 (14) % 로보고되고있다 요폐는슬링수술시너무많은긴장으로중부또는 근위부요도에비정상적모서리가생겨방광경부의역류방 지판막현상이나타나거나, 슬링재료의말림에의한융기 형성이요도를압박하여생길수있다 (15). 수술후생 기는요폐문제를해결하기위해요도확장, 약물요법, 방 광경부절개및 urethrolysis 등이시행되고있다 (16-18). Morey 등 (13) 에의하면 100cc 이상의잔뇨, 도뇨관유치, urethrolysis 등의시행이필요한폐색성합병증이 TO군에 서 (11.0%) TA 군보다 (18.3%) 유의하게낮다고보고하였 다. 본연구에서도마찬가지로경폐쇄공경로술식군에 서 (21.8%) 치골후경로술식군보다 (47.4%) 폐색성합 병증이적게관찰되었다 (Table 4). 중부요도슬링수술과연관된합병증은방광천공, 장천 공, 요로감염, 출혈, 혈종, 질천공, 요도손상등이있다 (19). 이중가장흔한합병증은방광천공이며이는연구 자들에따라다양하며평균 5.4% (1.1 21%) 로보고되고 있다 (4,20). 본연구에서도이러한합병증이경폐쇄공경 로술식군에서치골후경로술식군보다더낮게관찰되 었다 (0.9% vs 7.9%). (Table 5). 이상으로여성요실금환자의치료에경폐쇄공경로 술식이수술시간측면이나수술성공률과술후요폐및 합병증측면에서치골후경로술식보다양호한결과를 나타내어경폐쇄공경로술식이보다안전하고효율적인 방법으로생각한다. 하지만술자의차이와시기적차이 는술자의숙련도와경험의축적측면에서후향적연구 의한계점이라할수있다. 결 경폐쇄공경로술식에서치골후경로술식보다수술 시간과재원기간이짧았으며, 폐색성합병증과수술후 합병증이더적었다. 따라서경폐쇄공경로술식으로보 다안전하고효율적으로여성요실금을치료할수있을 것으로생각한다. 론 Vol. 11, No. 2,
