Methods: This prospective study included 332 women who were urodynamically diagnosed as stress urinary incontinence from April 2005 to January Patients showing pelvic organ prolapse higher than stage II by the Pelvic Organ Prolapse Quantification system or patients with detrusor overactivity were excluded from this study. The patients were alternatively selected to undergo TOT (n=192) or TVT-O (n=140) procedure and followed up at 1, 3, 6, 12 months postoperatively to compare the treatment outcome. Three hundred and six of them were available at 1 year follow up, 185 had the TOT operation, 121 the TVT-O procedure. The student t-test, chi square test were used for statistical analysis. Results: There were no statistically significant differences in general characteristics between two groups. No differences were found between 2 groups in the cure rates, postoperative complication such as voiding difficulty, De novo urgency, urinary tract infection, vaginal mesh erosion except groin pain. The groin pain rate of two groups showed significant difference. (TOT; 3.78% vs TVT-O; 9.92%, P=0.030). Conclusions: This study indicates that the TOT and TVT-O procedures are safe and effective treatments for female stress urinary incontinence. However, TVT-O shows high incidence of groin pain rate and it may be related to nerve irritation by inflammation of surrounding tissue around the pudendal and obturator nerve, according to anatomical differences of the tape position in two procedures. Transobturator tape (TOT) and Tension-free vaginal tape obturator (TVT-O) 근위요도의과운동성또는요도의요자제기전약화나손상으로인한내인성요도괄약근기능부전에의해발생한다. 1 이는 30~79 세한국여성에서약 22.9% 의유병률을나타내는흔한질병으로환자의삶의질에큰영향을미친다. 2 복압성요실금의수술적치료로서 1996 년 Ulmsten 등에의해 Tension-free vaginal tape이도입된이후약 7년간의추적관찰을통해만족스러운결과를보였다. 3 그러나투관침이치골후방공간을통과함으로인해방광천공, 치골후방혈종, 장천공및혈관, 신경손상과같은심각한합병증이보고되면서투관침을치골후방공간을피하여폐쇄공을통과함으로써합병증을감소시키고시술후방광경을시행하지않아도되는수술법이개발되게되었다. 폐쇄공을통과하는수술법에는투관침의통과방향에따라 outside-in transobturator technique (TOT) 과 insideout technique (TVT-O) 이각각 2001년과 2003년에도입되었다. 4.5 하지만, 아직까지 TOT 와 TVT-O 의치료성적및합병증에대해서는각각단독으로 TVT 와비교한몇몇보고들만이있을뿐이며더욱이우리나라여성에서앞서언급한이두종류의경폐쇄공테이프시술을전향적으로비교한연구는없었다. 본연구에서는복압성요실금치료를위한 TOT 와 TVT- O의치료성적및합병증을전향적으로비교함으로써복압성요실금환자의치료방향설정에도움을주고자하였다. 연구대상및방법 2005 년 4월부터 2007 년 1월까지본원에내원하여요역동학검사상복압성요실금으로진단받은 332 명의환자를대상으로전향적으로연구를시행하였으며, 본연구는본원의임상연구윤리위원회의승인을받았다. 모든환자는표준화된비뇨부인과문진및철저한이학적검사와요역동학검사를시행받았다. 표준화된비뇨부인과문진에는나이, 신체질량지수 (BMI), 분만력, 폐경여부, 호르몬치료의기왕력여부및이전골반수술과거력이포함되었으며, 국제요자제학회 (International Continence Society) 의표준화된골반장기평가체계인 POP-Q (Pelvic Organ Prolapse Quantification) 시스템에의거하여병기를정하였다. 6 요역동학검사 (Dantec-5000; Copenhagen, Denmark) 는요류측정검사 (uroflowmetry), 다중채널방광내압측정검사 (multichannel cystometry), 발살바요누출점압력 (Valsalva leak-point pressure, VLPP) 과요도압력외형분석검사 (urethral pressure profilometry) 로이루어졌다. 다중채널방광내압측정검사는 7 Fr. microtip transducer catheter를이용하여측정되었고 VLPP 는충만방광내압측정검사동안방광용적 200~300 ml에서발살바법에의해요실금이발생하는당시의압력을측정하였으며복압은질식방법으로측정하였다. 연구대상은요역동학검사상복압상승시배뇨근수축없이불수의적으로요실금이있는환자중에서 322 명을대상으로하였으며, 동반수술없이집도의에의해 TOT 혹은 TVT-O 의수술방법중양자택일하여진행하였고, TOT 는 192명, TVT-O 는 140명에서시행하였다. POP-Q system 에서 stage II 이상의골반장기탈출증환자이거나배뇨근과활동성을보인환자는연구대상에서제외하였다. 수술에사용된 Mesh 는 TOT 의경우 Iris (polypropylene, Dow-medics Co. Ltd., Wonju, Korea) 제품을, TVT-O 의경우는 Gynecare (polypropylene, Ethicon Inc., Somerville, NJ) 제품을사용하였고수술법은일반적인시술순서및방법에준하였으며숙련된동일한수술자에의해이루어졌다. 환자들은수술후 1, 3, 6, 12개월이되는시점에추적관찰하였고각방문시마다비뇨기계증상이나다른문제점들에대해조사하였다. 332 명의환자중 1년간추적관찰에성공한환자는총 306 명으로각각 TOT 185 명, TVT-O 121명이었다. 요도괄약근부전증은최대요도폐쇄압 (maximum urethral closure pressure, MUCP) 20 cmh 2 O 미만이거나 VLPP 60 cmh 2 O 미만인경우로정의하였다. 완치는기침유발검사에서요누출이없고, 요실금에대한주관적인증상이없는경우로정의하였으며기침유발검사는 300 ml 의상온의식염수를방광에채운후바로누운자세에서기침을유발하여요누출이있는지를측정하였다. 수술후합병증으로는배뇨곤란, 새로발생한절박뇨, 요로감염, Mesh 에의한질미란, 서혜부통증발생에대해평가하였다. 배뇨곤란은수술후 1개월이상지속되어약물혹은수술적치료를요하는요정체나요류약화가있는경우로보았으며,

Chicago, 1L) 을이용하여 student t-test, chi-square test 를시행하였으며 P<0.05 일때통계학적으로유의한것으로판정하였다. 결과평균연령은 TOT 를시행받은환자군에서 57.02±10.42 세, TVT-O 를시행받은환자군에서 56.51±11.27 세로두군간에차이가없었다 (P >0.05). 신체질량지수, 분만력, 폐경환자의비율, 호르몬치료의기왕력이있거나현재호르몬치료중인환자의비율, 요도괄약근부전증이있는환자의비율및이전골반수술과거력모두두그룹간에통계학적으로유의한차이가없었다 (P >0.05; Table 1). 또한, 수술 1, 3, 6, 12개월후두그룹간에완치율을비교한결과두그룹간에통계학적으로유의한차이가없었으며, 두군모두 1년이상의추적관찰결과 80% 이상의완치율을보였다 (P >0.05; Table 2). 수술후합병증발생률을비교한결과, 배뇨곤란, 새로발생한절박뇨, 요로감염, 질미란발생은추적관찰기간동안두그룹에서통계학적으로유의한차이가없었다. 서혜부통증발생률의경우

Transobturator tape (TOT) and Tension-free vaginal tape obturator (TVT-O) TOT 군에서 7명 (3.78%), TVT-O 군에서 12명 (9.92%) 으로 TVT-O 군에서유의하게높았으며 (P<0.05), 방광천공, 장천공및치골후방혈종은두그룹모두에서발생하지않았다. 고찰 TOT 와 TVT-O 시술모두복압성요실금의치료에효과적인수술방법으로알려져있으나, 7 두시술법의치료성적과합병증을국내단일연구기관에서전향적으로비교한연구는보고된바는없었다. 본연구는복압성요실금치료를위한두경폐쇄공테이프시술의치료성적및합병증을비교하기위해 stage II 이상의골반장기탈출증, 배뇨근과활동성환자를제외하였고동반수술을진행한환자는연구대상에포함하지않아수술후합병증에영향을줄수있는혼란변수를최대한제거하였고이들을전향적으로비교연구하였다는점에서의의가있다. 연구기간내에복압성요실금을진단받은환자들을양자택일하여수술을진행하였으며무작위대조이중맹검법을사용하지는않았으나환자의수술전임상적특징은서로통계학적으로유의한차이없이유사하였다. 또한본연구에서는수술의효과를판단하는완치의개념을환자의주관적인증상뿐만아니라기침유발검사에서요누출이없음을확인하여판단하였으므로보다객관적인결과를뒷받침한다. 본연구에서는두군간에통계학적으로유의한차이없이 1년이상의추적관찰결과 80% 이상의완치율을보였고이를통해 TOT, TVT-O 시술모두안전하고효과적인수술방법이라결론지을수있다. 복압성요실금의치료법으로서 TOT 와 TVT-O 는기존 TVT 수술에비해방광천공, 치골후방혈종, 장천공및혈관, 신경손상을감소시킨다고알려져있으나 7 두시술간의합병증을비교한연구는부족한실정이며두시술방법의우위를논하기에는아직논쟁의여지가많다. 본연구에서배뇨곤란, 새로발생한절박뇨, 요로감염, 질미란의발생은추적관찰기간동안두그룹에서통계학적으로유의한차이가없는것으로나타났다. 배뇨곤란을호소한환자의경우 Alfuzosin (alpha-adrenergic blocker) 로대부분증상이호전되었고약물치료 후에도 1개월이상배뇨곤란이지속된 TOT 2예와 TVT-O 2예에서는외래에서마취없이요도를통하여헤가르경관확장기 (Hegar s dilator) 를이용하여테이프를이완하였고 8 증상이호전되었다. 새로발생한절박뇨증상이있는경우 Trihexyphenidyl Tolterodine 을처방하였고모든환자에서 1개월이후에증상이호전된것을확인할수있었다. 서혜부통증발생률의경우 TOT 군에서 7명 (3.78%), TVT-O 군에서 12명 (9.92%) 으로 TVT-O 군에서유의하게높았으나, 서혜부통증이발생한환자모두비스테로이드성항염제로잘조절되었고 4개월이상지속되거나테이프를제거할정도로심각한경우는없었다. 시신해부를통해 TOT 와 TVT-O 시술을해부학적으로비교한 Spinosa 의연구에의하면 TOT 시행시 mesh 의위치가심부외측음부혈관구조와 3 cm 이상의거리를확보할수있지만 TVT- O시술의경우이보다근접하게내측에위치함에따라폐쇄신경의후방가지를손상시킬가능성이높다고보고하였다. 9 Zahn 역시 Cadaver dissection 을통해 TVT-O시술시 mesh 가폐쇄공으로부터 1.3±0.44 cm 거리에위치하여 TOT 시술시 2.3±0.41 cm인것과비교하여유의하게근접한거리에위치하고, 좌골치골가지 (ischiopubic ramus) 로부터의거리는 TVT-O 에서 0.39±0.44 cm으로 TOT 에서 0.04±0.13 cm인것과비교하여유의하게 (P< 0.008) 먼거리에위치하기때문에신경및혈관손상의위험이높음을보고하였다. 10 또한 Achtari 등의연구에서도 tape 과폐쇄공간의거리가 TOT 보다 TVT-O 에서더근접하다는결과를보고했으며 11 Whiteside and Walters 또한비슷한결과를통해 TOT 의안전성을주장하였다. 12 한편 Abdel-Fattah M 등이두경폐쇄공시술 (Obtape TM, Mentor-Porges and TVT-O TM, Gynaecare, Johnson & Johnson) 의질미란발생률을후향적으로비교한연구에따르면 TOT 를시행받은군에서 TVT-O 군에비해유의하게높은질미란발생률을보였다. 13 이는 TVT-O 시사용된 Mesh (TVT-O TM, Gynaecare) 의경우제1형 (monofilament mesh, pore size >75 μm) 인데반해 TOT 시사용된 Mesh (Obtape TM ) 는제2 형 (monofilament mesh, pore size <75 μm) 으로, 제2형에비해제1 형의 mesh 에서대식세포 (macrophage) 와백혈구, 섬유모세포의이주 (migration) 및혈관신생 (neovascularization) 의과정이보

5 대한산부회지제 51 권제 11 호, 2008 다쉽게이루어지고 14,15 이로인해낮은염증반응을보이고조직치유가잘이루어지므로질미란의발생을줄인다고설명하였다. 본연구에서사용된 mesh 는 TOT 와 TVT- O 모두제1형이었으며두군간의질미란발생률에는차이가없었고 mesh 를제거하거나재수술이필요한경우는없었으며경구항생제및비스테로이드성항염제, 베타딘질정치료로 1개월이내에호전되었다. 본연구결과에따르면복압성요실금치료로서 TOT, TVT-O 시술모두안전하고효과적인수술방법으로치료효과및합병증또한유의한차이를보이지않았으나서혜부통증발생률은 TVT-O 에서더높게보고되었는데이는폐쇄신경및음부신경주변으로의염증과관련된자극과연관성이있다고생각되며이는앞서언급한 tape 의해부학적위치와관련이있다고추정된다. 1.
