All rights reserved. 호흡기내과의사를위한 COPD 리뷰 Review 고려대학교의과대학내과학교실 이상엽 Literature Review of COPD 2008 Sang Yeub Lee, M.D. Division of Respiratory and Critical Care Medicine, Department of Internal Medicine, College of Medicine, Korea University, Seoul, Korea 서 지난 1년간발표된 chronic obstructive pulmonary disease (COPD) 에관한논문을중요일반학술지인 New England Journal of Medicine, Lancet 과호흡기계중요학술지인 American Journal of Respiratory and Critical Care Medicine, Thorax, Chest, European Respiratory Journal 에서검색하였다. 역학, 발병기전, biomarkers, 치료영역에서모두 5편을선정하여설명하였고, 추가로읽어보아야할중요논문을제목만더소개하였다. 역 Proportional classificiations of COPD phenotypes. Thorax 2008;63: COPD 는만성기관지염, 폐기종, 천식등과같은여러가지다른 phenotype 을포함하고있다. 연구의목적은 COPD에서각 subgroup의 proportion 을평가하기위한것이다. 뉴질랜드 Wellington Respiratory Survey에참가한 3,500명의환자들중에서 50세이상의환자를대상으로 Address for correspondence: Sang Yeub Lee, M.D. Division of Respiratory and Critical Care Medicine, Department of Internal Medicine, Korea University Anam Hospital, 126-1, Anam-dong 5-ga, Seongbuk-gu, Seoul , Korea Phone: , Fax: Received: May. 29, 2009 Accepted: May. 30, 2009 론 학 하였다. 조사원이면담하여설문지를작성하였고, 폐기능검사, CT 검사를시행하였다. 469명이모든조사및검사를완료하여, 이들을대상으로결과를분석하였다. COPD 는 GOLD 지침에따라 post-bronchodilator FEV 1 /FVC <0.7 이면서, 기도폐쇄를설명할수있는다른원인병변이없는경우로정의하였다. 만성기관지염은기침과객담이 1년에최소 3개월이상적어도 2년이상있는경우로정의하였다. 폐기종은 CT 검사결과폐기종이있거나, post-bronchodilator FEV 1 /FVC<0.7이면서 TLCO/VA가정상치이하인경우 ( 헤모글로빈수치교정 ) 로정의하였다. 천식은기관지확장제투여후 FEV 1 이 15% 이상증가하거나, 1주동안측정한 peak flow variability 가 20% 이상또는의사가진단한경우 ( 천명음또는, 야간호흡곤란등 ) 로정의하였다. 469명중 96명 (20.5%) 이 COPD 였다. COPD 96명중 53 명 (55.2%) 이천식이었고, 가장흔한 phenotype이었다. 만성기관지염과폐기종이각각 29/96 (30.2%), 31/96 (32.3%) 였다. COPD 96명중한가지 phenotype만있는경우는 35/96 (37.0%), 한가지이상 phenotype을가진경우는 36/96 (37.5%), 천식, 만성기관지염, 폐기종중어느한가지 phenotype 에도속하지않는경우는 25/96 (26.0%) 였다 (Table 1). COPD 와 No COPD 를포함한전체연구대상에서는 Table 1과같이모두 16개의가능한 phenotype이존재하며, 이결과를가지고 proportional Venn diagram을만들면 Figure 1과같다. 결론적으로, 이연구는 COPD의 phenotypic subgroup 의 proportion 에따른분류를제공하였다. 이결과는앞으로 COPD 의기전과치료에대한연구에사용할수있을것이다. 1

The isolated clear circle represents subjects with COPD who did not have an additional defined phenotype of asthma, chronic bronchitis or emphysema. 1989년 Snider 1 가 COPD 의구성성분에대한 non-proportional Venn diagram을제안하였고, 이후이 diagram 은 COPD 의개념을시각적으로표현할수있는훌륭한방법이었다. 그러나 Snider의연구에서는현재에서와같이 post-bronchodilator spirometry 로 COPD를진단한것이아니었고폐기종을진단할때 CT 검사를이용하지않았기때문에현재에는제한점이있다. 또한이 diagram 의여러구성성분의 proportion 에대해서궁금증이많았다. 이연구는임상자료, 폐기능검사, CT 결과를토대로 proportional Venn diagram 을제안하였다. 결과에의하면 COPD 의 55% 가천식이었고가장흔한 phenotype 이었다. 비록뉴질랜드에서천식의유병률이높긴하지만, 기존의연구와는다른놀라운결과이다. 하지만이연구의결과를해석하는데주의가필요하다. 첫째는, COPD 진단정의를단순히 post-bronchodilar FEV 1 /FVC<0.7 이고기관지확장증이나결핵같은기도폐쇄를설명할수있는다른질환이없는경우로하였는데, 비록이정의가 GOLD 지침 2 을근거로하였지만, postbronchodilator FEV 1 /FVC<0.7 인모든천식환자에 COPD 진단을붙일수있는것은아니다. 둘째는, 천식의진단기준도마찬가지로의심스럽다. 의사가천식으로진단한경우또는 bronchodilator reversibility>15% 를진단기준으로하였다. 그런데증상이심하지않고, 기도폐쇄가있는경우, 처음에는의사들이주로천식진단을붙이는경향이있고나중에는결국 COPD 로판명되기도한다. 또한, COPD 환자에서도다른천식특징이없으면서 reversibility가있는경우가있는데, 단지, 이결과만가지고천식진단을붙이는경우도있다. 그럼에도불구하고이연구는 COPD 에대해서기존에우리가가지고있던관점이올바른것인가에대해서다시한번생각해볼시간을가지게하는주목받아야할연구라고생각된다. Malhotra D, Thimmulappa R, Navas-Acien A, Sandford A, Elliott M, Singh A, et al. Am J Respir Crit Care Med 2008;178: 흡연에의한 oxidative stress 는 COPD 의발생기전에있어서중요한역할을한다. Nuclear factor erythroid 2-related factor 2 (NRF2) 는 redox-sensitive transcription factor로써 antioxidant defense system 을조절한다. 세포내에있는 NRF2는 inhibitor 인 Kelch-like ECH-associated protein 1 (KEAP1) 과결합되어있다. DJ-1은 NRF2의 stabilizer로서 NRF2 의효과를촉진시킨다. 이연구의목적은 COPD 환자의폐조직에서 COPD 의중증도, oxidative stress, NRF2-dependent antioxidant level 사이의연관성을확인하는것이다. Non-COPD 26명, GOLD 명, GOLD 명을대상으로하였다. Non-COPD 와비교해볼때, COPD 에서는 NRF2-dependent antioxidants (HO1, NQO1, GCLM proteins) 와 glutathione level이유의하게감소되어있었다. Oxidative stress markers (GSH, TBARS) 가유의하게감소되어있었고, NRF2 protein 이유의하게감소되어있었다. KEAP1은차이가없었지만, DJ-1 level은유의하게 감소되어있었다. NRF2-dependent antioxidants, DJ-1은 COPD 의중증도와음의상관관계가있었다. 결론적으로, COPD 에서는 NRF2 antioxidant defence pathway 가감소되어있고, 이것은 NRF2의 stabilizer인 DJ-1의감소때문이다 (Figure 2). NRF2는 COPD의발생기전에서중요한치료표적이될수있을것이다. Oxidative stress는 proinflammatory transcription factor인 NF-κB를활성화시키고, 이것은 CXCL1, CXCL8, MMP-9, TGF-β 와같은여러가지 inflammatory genes 을활성화시킨다. 또한 oxidative stress는활성화된 inflammatory genes 을억제시키는 HDAC2 를억제시킨다. 이와같은 oxidative stress 는폐에서 exogenous, endogenous antioxidant 에의해중화된다. Exogenous antioxidant에는비타민 C, 비타민 E와같은 dietary antioxidants 가있는데효과가미약하다. Endogenous antioxidants에는 superoxide dismutases, glutathione peroxidase, glutathione reductase, catalase, heme oxygenase-1 등이있고, 이것은 NRF2 에의해조절된다. COPD 환자에서 oxidative stress를감소시키는것은염증을감소시키고, steroid 저항성을반전시킬수있는좋은방법이지만, 현재사용가능한 antioxidants 인 N-acetyl cysteine 은 COPD의진행과악화를감소시키는것에있어서실망스러운결과를보이고있다 3. 따라서, NRF2 level을정상으로회복시키는것이매력적인치료방법이될수있을것이다. 이연구는 COPD 에서 oxidative stress 의조절에있어서새로운지견 Figure 2. 4 SY Lee: Literature review of COPD 2008 을제공하고있으며, 앞으로 COPD 치료의훌륭한치료방향이될수있을것으로생각된다. BIOMARKERS Circulating fibronectin to C-reactive protein ratio and mortality: a biomarker in COPD? Man SF, Xing L, Connett JE, Anthonisen NR, Wise RA, Tashkin DP, et al. Eur Respir J 2008;32: COPD에서폐의항상성을유지하기위해서는 inflammatory process 와 repair process 사이의균형이중요하다. 점차 COPD 는 systemic disease 로서의중요성이강조되고있고, 혈액표본에서 biomarker 를발견하려는노력이증가하고있다. Systemic inflammatory pathway 의중요한 biomarker는 C-reactive protein (CRP) 이고, wound repair Figure 3. In this example, the hazard ratio would be 3. pathway의중요한 biomarker 는 fibronectin 이다. 이연구의목적은 CRP와 fibronection이 COPD의예후를예측할수있는 biomarker 가될수있는지를확인하는것이다. 캐나다의 Lung Health Study 에참여한 5,887 명의흡연자를대상으로하였다. 이들은 mild-to-moderate COPD 환자들이다 (FEV 1 /FVC<0.70, 55%<FEV 1 <90%). 이들중 4,787 명이 CRP, fibronection 검사를모두시행받았고, 추적관찰기간의 median 은 7.5년이었다. Fibronectin level은 all-cause mortality와음의상관관계를보였으나유의성은없었다 (p=0.055). CRP level은 all-cause mortality 와유의한양의상관관계를보였다 (p=0.004). Repair 와 inflammation 의균형을평가하기위하여 fibronectin/crp ratio을구하였고, all-cause mortality와의관련성을분석하였다. Fibronectin/CRP ratio와 all-cause mortality 사이에는 L-shape 의연관성이있었다. 사망위험도는 fibronectin/crp ratio가 148에도달할때까지계속감소하였고, 이수치이후에는변화가없었다 (Figure 3). 결론적으로 fibronectin/crp ratio가 COPD 의 all-cause mortality과관련성이있었다. 그러나, 다른 biomarker 와는달리관련성은 L-shape을나타내었다. 이연구에서, fibronectin 과 CRP는한번측정되었다. 따라서 fibronectin/crp ratio의재현성에제한점이있을수있고, mortality 와의직접적인연관성은좀더연구가필요할것으로생각된다. 그리고, fibronectin, CRP 모두 lung specific markers 가아니기때문에혈액에서측정한결과가과연 COPD 폐의현상을직접반영하는것인지도확실하지않다. 또한 mild-to-moderate COPD 를대상으로한것이기때문에 severe COPD 는결과가다를수있을것으로생각된다. 그럼에도불구하여이연구는기존의시각과는달리, COPD가 systemic disease로서 inflammation 과 wound repair 의균형이라는새로운관점을제시한의미있는연구라고할수있겠다. Zheng JP, Kang J, Huang SG, Chen P, Yao WZ, Yang L, et al. Lancet 2008;371: 이연구의목적은항염증, 항산화효과를가진 mucolytics인 carbocisteine 이 COPD의급성악화를감소시킬수있는지를평가하는것이다. Multicenter, randomized, double-blind, placebo-controlled, parallel-group study 이다. Postbronchodilator FEV 1 /FVC<0.7, 25%<FEV 1 <79%, 40 80세, 최근 2년동안 2회이상급성악화를경험한환자를대상으로하였다. 중국의 22개병원에서 709명이등록되었고, carbocisteine군이 353명, placebo군은 354명이었다. Primary endpoint 는 1년간 exacerbation rate 이다. 악화는다음과같이정의하였다. 즉, 적어도 2일이상두가지이상의주증상 ( 호흡곤란악화, 객담의농성증가, 객담의양증가 ) 이있거나, 주증상한가지와한가지이상의부증상 ( 상기도감염, 발열, 천명음증가 ) 이있는경우로하였다. Carbocisteine 은하루 1,500 mg (2 250 mg, 하루 3회 ) 을투여하였고, 속효성또는지효성기관지확장제, 흡입스테로이드등과같은 COPD 의일반적치료가모두허용되었다. Carbocisteine 군에서 placebo군과비교할때, 연간환자당악화의수가유의하게감소되었다 (1.01 vs. 1.35, risk ratio 0.75, p=0.004). SGRQ score도 carbocisteine 군에서개선되었다 (total score units, symptom score units, activity score units). 그리고이효과는 COPD 중증도, 흡연, 흡입스테로이드의사용과는연관성이없었다. 결론적으로 carbocisteine 은 COPD 악화의예방에가치있는치료방법이다. 비록현재까지근거는충분하지않지만 mucolytics 는 COPD 환자에서객담배출을용이하게하기위해서수년동안사용되어왔다. 또한이약제의 cysteine residues 는항산화효과가있기때문에 COPD 의악화를감소시키는 목적으로도사용할수있다. 이것에대한여러임상시험이있었지만시험디자인이좋지않았으며, 대규모연구인 BRONCUS 연구에서는 N-acetyl-cysteine 이 placebo 에비해서 ICS를사용하지않는환자에서 exacerbation 을감소시켰지만 primary outcome이아니었다 3. PEACE 연구는 carbocisteine을사용하였고, exacerbation이 primary outcome이었다. 매일 1,500 mg carbocisteine을투여한군에서 1년간평균 exacerbation rate가 1.35회에서 1.01회로약 24.5% 감소하였다. 하지만몇가지해석에주의할점이있다. Exacerbation 의원인에대해서정보가없다는것이다. Carbocisteine 이어떤종류의 exacerbation 을예방하는것인지에대해서평가가되어야할것이다. 마찬가지로, exacerbation 을예방하는효과의기전에대한설명이필요하다. 즉, 객담배출을용이하게해서인지, 아니면항산화효과때문인지에대한평가가필요할것이다. 그럼에도불구하고이연구는기존의약제를가지고비용효과적이고안전하게 exacerbation 을줄일수있는새로운치료방법을제시했다는것에의의가있다고생각된다. A 4-year trial of tiotropium in in chronic obstructive pulmonary disease. Tashkin DP, Celli B, Senn S, Burkhart D, Kesten S, Menjoge S, et al. N Engl J Med 2008;359: 이연구의목적은 tiotropium 이 COPD 환자의 FEV 1 의 rate of decline 을감소시킬수있는지를평가하는것이다. Moderate-to-very severe COPD 환자를대상으로한 4-year, randomized, double-blind, placebo-controlled, parallel-group 연구이다. Primary endpoint 는기관지확장제투여전, 그리고투여후연간 mean FEV 1 의 rate of decline이다. 37개국 487개병원에서 5,993명을대상으로하였고, tiotropium 군이 2,986명, placebo군이 3,006명이었다. 매일 1회 tiotropium 18 μg 이투여되었고, 흡입항콜린제를제외한모든호흡기약제의투여가허용되었다. 기관지확장제투여전과투여후 mean FEV 1 의 rate of decline 은 tiotropium 군과 placebo 군사이에차이가없었다 ( 기관지확장제투여전 30±1 vs. 30±1 ml/yr, p=0.95, 기관지확장제투여후 40±1 vs. 42±1 ml/yr, p=0.21). Placebo 군과비교해서, tiotropium 군에서기관지확장제투여전과투여후 mean FEV 1, mean FVC 의유의한개선이있었다. 또한 tiotropium 군에서 SGRQ score가개선되었고, 악화의평균횟수가 14% 감소되었다 (p<0.001). 결론적으로, COPD 환자에서 tiotropium 의투여는폐기 5

6 SY Lee: Literature review of COPD 2008 능, 삶의질, 급성악화를개선시켰으나, FEV 1 의 rate of decline은감소시키지못하였다. 이연구에서 tiotropium 군에서 mean FEV 1 의 rate of decline 은 40 ml/yr 였는데, 이수치는이전에연구들과비교해보면, EUROSCOP 연구 4 의 57 ml/yr, ISOLDE 연구 5 의 50 ml/yr, BRONCUS 연구 3 의 54 ml/yr 에비해서낮은수치이다. 그리고 TORCH 연구의 post hoc analysis 의결과보다도낮다 6. 연구자들은이것이다른연구디자인과는달리, 항콜린제를제외한모든종류의호흡기약제를허용하였고, 또한이미많은수의환자에서지속적으로금연하였기때문일수있다고설명하고있다. 속효성항콜린제, 흡입스테로이드, 항산화제 (N-acetylcysteine) 등을사용한이전의연구에서도 FEV 1 의 rate of decline 에대한유의한효과가증명되지않았기때문에 3-5,7-9, 현재까지 COPD의치료에서 disease-modifying therapy 는금연이유일한방법이다 8. 또한사망률을감소시키는데에있어서도, 금연과일부선택된환자에서산소치료, lung-volume reduction surgery 만이현재까지증명된유일한치료방법이다.
