3 항응고제 및 혈전용해제와 관련된 뇌실질내출혈 parin(용량 7,5 25,U)을 24 2시간에 총용량 사용 후 신경학적 증상이 발현되기까지의 기간은 최소 시 22,5 25,를 warfarin(용량 2.5 5mg)을 72시 간에서 최고 6년의 분포(례에서 6년간 warfarin 복용 후 간 8일간 경구 복용하거나, urokinase(용량 2 5만 발생)를 보였고, TPA를 사용한 례에서 약물 주입 후 시 U)를 6 44시간에 총용량 6 3만U, 또는 tissue 간, heparin을 사용한 경우 평균 69시간 후, urokinase를 plasminogen activator(용량 65mg)를 시간에 걸쳐 각 사용한 경우 평균 7시간 후, warfarin을 사용한 경우 평균 각 정맥 혹은 동맥을 통하여 주입하던 중 발생하였다. 약물 96시간 후에 신경학적 증상이 관찰되었다(Table 4). ICH location and amount Location 6. 임상소견 출혈발생 후 관찰된 임상증상으로는 의식저하가 6례 (89%)로 가장 많았고, 두통이 3례(7%), 구토가 3례(7%), 운동기능 저하가 2례(%), 구음장애가 례(6%)에서 발생하 였고, 두개내출혈이외의 다른 부위의 출혈은 관찰되지 않았다. 출혈이 발생한 당시 환자의 수축기혈압의 평균은 33mmHg, 이완기 혈압의 평균은 83mmHg이었고, 수축기혈압이 6 mmhg이상의 고혈압을 보인 경우는 3례(7%)에 불과하 였다. 따라서 혈압의 상승이 출혈 유발의 중요한 요소로 사 료되지는 않았다. 5 항응고제 및 혈전용해제와 관련된 뇌실질내출혈 의 정도에 따라 소량출혈(3cc 미만)이 7례, 중등도 출혈 다발성 혈종인 경우 66.7%의 사망률을 보여 단발성의 8.3% (3 6cc)이 례, 대량출혈(6cc이상)이 례였다. 항응 의 사망율 보다 매우 나쁜 예후를 보였다. 고제에 따른 출혈량은 TPA에서 5cc로 가장 적었고, he- 9. 치료방법 치료는 신경학적 상태 및 전신상태와 뇌전산화단층촬영을 parin 사용 환자에서 평균 28.5cc, warfarin 사용환자에서 평균 29cc, urokinase 사용환자에서 평균 35cc이었다. 출 혈의 발생위치는 엽상부가 8례(45%)로 가장 많았고, 다음 으로 기저핵부 4례(22%), 시상부 4례(22%), 소뇌부 2례 (%)순이었다(Table 5). 뇌실질내출혈은 8례 중 5% 인 9례에서 동반되었다. 바탕으로 치료방법을 결정하였다. 혈종 양이 3cc 미만인 7 례와 혈종 양이 3 6cc인 환자 중 고령 및 전신상태가 불 량한 5례에서 뇌실외배액술 및 보존적 요법을 시행(66%)하 였고, 혈종 양이 3 6cc인 경우 3례에서 뇌정위적혈종제 거술(22%)을, 혈종 양이 3 6cc인 경우 2례와 6cc 이 8. 뇌단층촬영소견 뇌단층촬영소견에서 엽상부, 기저핵부, 시상부, 소뇌부 등 상의 례에서 종괴효과 및 신경학적 증상이 진행되어 개두 에서 고밀도의 혈종이 관찰되었고, 혈종 주변 부위에 뇌부 행한 례와 개두술을 시행한 례에서 수술 후 재출혈 및 혈 종으로 인한 저밀도의 소견과 혈종의 양에 따라서 다양한 종의 불완전제거로 인해 이차적으로 뇌정위적혈종제거술과 정도의 종괴효과(mass effect)가 관찰되었다. 뇌실질내출 개두술을 실시하여 혈종을 제거하였다. 술(22%)을 시행하였다(Fig. 3). 뇌정위적혈종제거술을 시 혈의 5%의 경우에서 측내실, 3뇌실, 4뇌실 등에 고밀도. 합병증 치료 중 합병증은 폐렴 2례, 폐부종 례, 뇌막염 례, 재출 (hyperdensity)의 뇌실내출혈이 보였으며, 혈종의 모양은 단발성혈종(single hemoatoma)인 경우가 2례이었고, 다 발성혈종(multiple hematoma)인 경우가 6례였다(Fig. 2). 혈 례, 뇌경색 례, 독성간염 례로 총 5명의 환자에서 발 생하였다(Table 7). 뇌경색은 일과성 뇌허혈증으로 urokin- A B C D Fig. 3.
