We carried out this study to investigate the association between age, obesity index and PSA in Korean men over 40 years old. Materials and Methods: From April 2004 to April 2005, 1,147 men over 40 years old who visited our Heath Promotion Center were enrolled. The medical history were obtained through history taking and questionnaires. Physical parameters such as height, weight and waist and hip circumferences were measured and body fat (%) was checked using body composition analyzer. We checked serum chemical tests including lipid profiles, liver and renal function test, PSA and urinalysis. Results: According to body mass index (BMI), 496 males were classified as obese (BMI>25kg/m 2 ) and 651 as normal group. In the normal group, men 65 years old or older had higher log PSA than men younger than 65 years (p=0.002), but in the obese group, there was no such difference according to age. In multi-logistic regression analysis controlling life style-related parameters such as smoking, drinking or regular exercise, the log PSA increased by for an increase of 1 year. But, obese group showed less increase of log PSA than normal group. Conclusions: Obesity can intervene in the relationship between serum PSA and age. Slower increase of serum PSA per year in obese men over 65 years old than in normal men might suggest that it takes more time for serum PSA to reach its threshold for prostatic biopsy in this group. This result might suggest that prostate cancer in this group would be found in a more advanced state, causing a negative impact on treatment outcomes or prognosis of these men. (Korean J Urol Oncol 2007;5: ) Key Words: Obesity, Prostate-specific antigen, Prostate cancer 서 우리나라에서도비만은주요건강문제로 2001년국민건강영양조사에의하면전국민의 30.6% 가체질량지수 25kg/m 2 논문접수일 :2007 년 9 월 13 일, 채택일 :2007 년 11 월 30 일교신저자 : 최희정, 을지대학교의과대학가정의학교실대전시서구둔산 2 동 1306, Tel: , Fax: ohinia@eulji.ac.kr 론 이상의비만인것으로알려져있다. 특히비만한여성은 1998년 28.1% 에서 2001년 29.4% 로 1.3% 증가한반면, 비만한남성은 1998년 25.1% 에서 2001년 32.4% 로 7.3% 증가하여, 남성에서그증가폭이매우큰것을알수있다. 1 비만은다양한만성질환과연관이있으며, 일부암에서도발생이나진행에있어관련이있는것으로알려져있다. 우리나라중앙암등록본부에서발표한전국암발생률자료에따르면, 전립선암은남성의 10대암중 6위를차지하며, 전체암의 3.0% 를차지하는것으로나타났다. 2 서양인을대상으로 138

2 김은탁외 : 성인에서혈청전립선특이항원과비만지표, 나이와의관련성 139 하였던많은대규모역학연구에서비만과전립선암의관계는일관성이없는것으로나타나비만과전립선암의관계는아직불분명하다. 3-6 그러나비만과전립선암에의한사망위험은강한연관성이있는것으로나타나, Snowdon 등 7 은정상체중인남성에비해과체중인남성에서치명적인전립선암의발생위험이 2.5배더높았다고하였고, 미국암학회에서시행한대규모전향적연구인 CPS II (Cancer Prevention Study II) 에서는체질량지수 30kg/m 2 이상인남성에서전립선암에의한사망위험이 20-34% 증가하고체질량지수의범주가증가함에따라사망위험도증가하였다고하였다. 8 전립선특이항원 (prostate-specific antigen; PSA) 은전립선조직에대해높은특이도를가지는종양표지자로전립선암의선별검사뿐아니라, 진단, 병기결정, 질병의경과나치료에대한반응을관찰할목적으로널리사용되고있다. 그러나전립선암에대한특이도가높은것은아니어서, 직장수지검사가정상이면서 PSA치가 4-10ng/ml인경우조직검사에서전립선암으로진단되는경우가 21% 에불과한것으로나타나, 9 비용-효과적측면이나조직검사에따르는높은이환율을줄이기위해 PSA 밀도나증가속도, 유리 / 총 PSA 비를측정하는등의다양한방법들이시도되고있다. 서양남성을대상으로한연구에서 Tompson 등 10 은 PSA 농도와체질량지수간에연관성이없다고보고하였으나, Barqawi 등 11 은모든연령에서체질량지수가높을수록총 PSA 농도와 PSA 복합체의농도가낮았다고하였다. 본연구에서는우리나라 40세이후의남성을대상으로나이와비만지표들이 PSA 농도와어떠한연관성이있는지알아보고, 나이나비만지표가 PSA 농도에미치는영향을조사하기위해시행하였다. 대상및방법 1. 대상및방법 2004년 4월부터 2005년 4월까지건강검진을위해내원한 40세이상의남성 1,147명을대상으로하였다. 대상자들중전립선암이나전립선수술을받은병력이있는남성, 5-알파환원효소억제제나테스토스테론과같은호르몬관련약물을복용하는남성, AST나 ALT가 100mg/dl 이상인남성과소변검사에서백혈구가고배율시야당 5개이상관찰되는남성은제외하였다. 모든대상자의과거병력과현재병력, 약물복용력과건강행태등은자가기입설문지와의사의문진을통해조사하였다. 기본적인신체검사항목과검사실검사는전날저녁부터최소 12시간이상금식후시행하였다. 신체계측항목으로는신장과체중, 허리둘레, 엉덩이둘레를측정하였고, 체성분분석기 (InBody 4.0, Biospace, Korea) 를이용하여체지방량 (body fat%) 을측정하였다. 체질량지수 (body mass index; BMI) 는 Quetelet Index를이용하여체중 (kg) 을신장의제곱 (m 2 ) 으로나누어계산하였다. 채혈한혈액은검사실로보내어간기능과신장기능이포함된일반화학검사등을시행하였다. 혈청 PSA는 Direct chemiluminescent를이용한 Sandwich immunoassay 방법으로측정하였다. 체질량지수와허리둘레에의한비만의분류는아시아-태평양지침을기준으로하여체질량지수 25kg/m 2 이상인경우를비만으로하였고, 12 허리둘레가 90cm 이상이거나허리 / 엉덩이둘레의비가 0.9 이상인경우를복부비만으로정의하였다. 13 한편, 체지방률에의한비만의분류는체지방률 25% 이상인경우를비만으로하였다. 14,15 2. 통계분석모든측정치는연속형자료로구하여평균값과표준편차로제시하였으며, 편중분포를보이는 PSA는로그값을취하여분석하였다. 체질량지수에따른정상군과비만군의일반적인특성과이들특성의차이를알아보기위해연속형변수의경우는 Student s t-test를, 범주형변수인경우는 chi-square test를하였다. 비만지표에따른정상군과비만군에서 log PSA의비교와체질량지수에따라나눈정상과비만군각각에서 65세미만과 65세이상의 log PSA의비교는 Student s t-test를이용하였다. 한편, 혈청 PSA 농도 2.5ng/ml을기준으로하여두군으로나눈후, 흡연력이나음주량, 규칙적인운동여부등의생활행태관련지수와간기능, 신장기능에의한차이를비교하기위해연속형변수인경우는 Student s t-test를, 범주형변수인경우는 chi-square test를시행하였다. 또한흡연력이나음주량, 규칙적인운동여부와같은생활행태관련지수들을통제하고나이와비만이 log PSA에미치는영향을알아보기위해다중회귀분석을시행하였다. 분석에사용한통계프로그램은 SAS for windows (version 9.0) 이었다. 결과 1. 체질량지수에따른정상군과비만군의일반적인특성전체대상자 1,147명중체질량지수에따라정상으로분류된남성은 651명 (56.8%) 이었고, 비만으로분류된남성은 496명 (43.2%) 이었다. 평균연령은정상군이 53.9±9.0세, 비만군이 51.3±7.9세였다. 정상군에비해비만군에서상대적으로고혈압과당뇨병이많았고, 주 3회이상음주를하는

General characteristics of subjects and association with body mass index Variables Normal Obesity p-value* No. of subjects Age (years) 53.9± ±7.9 <0.001 Hypertension (%) 224 (34.4) 254 (51.2) <0.001 Diabetes (%) 75 (11.5) 92 (18.6) <0.001 Regular exercise (%) 233 (36.9) 197 (40.6) Smoking status Current smoker (%) 277 (43.8) 172 (35.3) Ex-smoker (%) 209 (33.0) 198 (40.7) Non-smoker (%) 147 (23.2) 117 (24.0) Alcohol frequency No (%) 189 (29.0) 112 (22.6) 1-2 times/week (%) 405 (62.2) 299 (60.3) < times/week (%) 57 (8.8) 85 (17.1) Waist (cm) 83.0± ±5.0 <0.001 Waist/hip ratio 0.88± ±0.04 <0.001 Body fat (%) 19.8± ±3.5 <0.001 *p-value by Student's t-test (continuous variables) or chi-test (categorical variables). 경우도많았다 (Table 1). 2. 비만지표에따른 log PSA 의비교 체질량지수에의해비만으로분류된남성의평균 log PSA 는 0.23±0.58 (PSA 0.95±0.67ng/ml) 로정상남성의 0.11±0.70 (PSA 1.23±1.82ng/ml) 보다낮은농도를보였다 (p=0.002). 그러나허리둘레, 허리 / 엉덩이둘레비에의한복부비만이나체지방률에따른정상과비만군에서 log PSA는유의한차이가없었다 (Fig. 1). 정상군과비만군에서나이에따른 log PSA를비교한결과, 정상군에서 65세미만의평균 log PSA는 0.16±0.64 (PSA 1.09±1.14ng/ml) 로 65세이상의 0.19±0.97 (PSA 2.16± 3.90ng/ml) 보다낮은농도를보였으나 (p=0.002), 비만군에서는이와같이나이에따른 log PSA에차이가없었다 (Fig. 2). 3. 혈청 PSA 농도에따른분류에서생활행태관련지수와간기능, 신장기능의비교 혈청 PSA 2.5ng/ml를기준으로, 이이하의 PSA 측정치를보였던남성은 1,076명 (93.8%) 이었고, 2.5ng/ml를초과하는남성은 71명 (6.2%) 이었다. 혈청 PSA가 2.5ng/ml 이하인남성의평균나이는 52.4±8.4세로 2.5ng/ml 초과했던남성의평균나이 57.7±9.9세보다적었고, 현재흡연비율이나주당음주횟수등이상대적으로더많은것으로나타났다. 간기능이나신장기능은이두군간에차이가없었다 (Table 2). 4. 나이와비만지표가 log PSA에미치는영향흡연력이나음주량, 규칙적인운동여부와같은생활행태관련지수들을통제하고나이와비만이 log PSA에미치는영향을파악하기위해시행한다중회귀분석에서, 나이가 1 세증가할때마다 log PSA는 0.010이증가하였으며, 비만군에서정상군보다 log PSA가더낮았다 (Table 3). 고찰이연구에서혈청 PSA는체질량지수에따른정상군에서나이에따른증가를보였으나, 비만군에서는이러한차이를보이지않았다. 비만은 PSA나직장수지검사와같은선별검사나조직검사와같은진단적검사시영향을줌으로써비만한남성에서높은전립선암관련사망률이나재발률과같은나쁜예후를나타낼가능성을생각해볼수있다. 전립선암과비만의관계는아직불분명하며, 서양인을대상으로시행했던대규모역학연구에서도이들관계에일관성이없는것으로나타났다. 135,000명의스웨덴건설직공들을 18년간추적관찰했던코호트연구 3 에서는체질량지수가증가할수록전립선암의발생위험이증가하고비만이전립선암의발생률보다는사망률과더강한연관성이있다고한반면, Health Professionals Follow-up Study 4 에서는비만이전립선암의발생위험과연관이없었을뿐아니라, 오히려 60세이하의남성이나전립선암의가족력이없는남성에서는체질량지수가 30kg/m 2 이상인경우암발생위험이낮았다고하였다. 한편, Porter와 Stanford 5 는비만이전립선암의위험과역상관관계를나타내었다고하였고, Baltimore Longitudinal Study of Aging 연구 6 에서는허리 / 엉덩이둘레비가증가할수록전립선암의위험이증가하는경향을보였으나체질량지수와는무관하였다고하였다. 이러한연구결과들로인해비만과전립선암의위험사이에어떠한관련이있는지결론을내리기어려운상태이나, 비만과전립선암에의한사망위험은강한연관성이있는것으로나타나, 비만이전립선암에있어시작보다는전파과정에더강한영향을미칠가능성이제시되고있다. PSA: prostate-specific antigen, BMI: body mass index. 으로선별검사나병기결정, 전립선암환자의경과관찰에널리사용되고있으며, 치료가가능한조기전립선암을발견함으로써암에따른이환율이나사망률을감소시킨것으 로알려져있다. 16 그러나혈청 PSA는전립선비대증이나전립선염, 암의진행단계와같은요인들에의해변화할수있으며, 전립선암이있다하더라도 PSA 농도가 4ng/ml 미만인경우가 20-25% 나되는것으로나타나, 17 조직검사를결정하는역치를낮추어야한다고주장하는연구자들도있다. 한편, 혈청 PSA의대사나배설은주로간에서이루어지는데, 간기능이나신장기능이혈청 PSA에미치는영향에대해서는아직까지논란이있다. 이연구에서는혈청 PSA를 2.5ng/ml 기준으로나누었을때, 두군간에간기능이나신장기능관련수치에차이가없었지만, Ku 등 18 은건강한남성에서간기능검사중 AST와 γ-gt, 빌리루빈은혈청 PSA에영향을주지않았으나, ALT와알칼리성포스파테이스등은관련이있다고하였다. 이연구의제한점은첫째, 전립선비대증에대한검사를하지못하였다는것이다. 전립선비대증을가진남성들을대상으로했던한연구에서전립선의크기는비만정도와나이가증가함에따라 40-80g까지증가한다고하였으나, 19 1,369명의전립선비대증을가진환자와 1,451명의정상인을대조군으로했던또다른연구에서는체중이낮은사분위

Multiple regression analysis on log PSA Variables β S.E. p-value* Age <0.001 Obesity Smoking status Current smoker Ex-smoker Alcohol frequency 1-2 times/week times/week Regular exercise *p-value by multiple regression analysis models adjusted for age, obesity, smoking, alcohol and exercise, compared to nonsmoker, compared to non-drinker, compared to irregular exercise or none, β: parameter estimate, S.E.: standard error, PSA: prostate-specific antigen. 수에속하는남성에비해높은사분위수에속하는남성에서전립선비대증의교차비가 0.71 (95% CI, ) 이었고, 생애동안체질량지수의변화가 6.1kg/m 2 이상이었던남성은 1.6kg/m 2 미만이었던남성에비해전립선비대증의교차비가 0.74 (95% CI ) 로나와, 비만이전립선비대 증과역상관관계에있다고하여, 비만과전립선비대증에대하여는아직까지논란의여지가있다. 둘째로는선별검사를위해건강증진센터에내원한남성들을대상으로하였기때문에선택편견으로인해결과에왜곡현상이나타났을가능성이있다는것이다. 그러나이연구에서는비만이혈청 PSA와나이와의관련성에영향을줄수있다는것을알수있었다. 비만한노인남성은체중이정상인노인남성에비해혈청 PSA가낮으며, 특히 65세이상의비만한노인남성에서는나이에따른혈청 PSA의증가가관찰되지않기때문에혈청 PSA가조직검사여부를결정하는역치에이를때까지더긴시간을소요하게되고, 따라서더진행된상태에서암을발견할가능성이높아재발률이나사망률과같은총체적인예후가나쁠가능성이있음을시사한다. 향후에는비만한남성에서혈청 PSA에영향을미치는요인을파악하기위해전립선크기나남성호르몬등을측정하여남성호르몬이혈청 PSA에어떠한영향들을나타내는지에대한연구도필요할것으로생각한다. 결론이연구에서는비만이혈청 PSA 농도와나이와의관계에

6 김은탁외 : 성인에서혈청전립선특이항원과비만지표, 나이와의관련성 143 영향을줄수있음을알수있었다. 65세이상의비만한남성에서는나이에따른혈청 PSA 농도의증가가관찰되지않았는데, 이러한결과는조직검사를결정하게되는혈청 PSA 농도역치에도달할때까지더긴시간을소요하게되어진행된상태에서암을발견하게되고그에따라재발률이나예후에있어나쁜결과를초래할가능성을시사한다.
