Department of Internal Medicine, Institute of Gastroenterology Yonsei Universiη College of Medicine, Seoul, Korea Background/Aims: The serum Iipase/amylase ratio has been proposed to distinguish acute episodes of alcoholic from nonalcoholic pancreatitis. We assessed the clinical utility of this ratio in differential diagnosis of eti이 ogies of acute pancreatitis. Methods: We retrospectively reviewed all records with the diagnosis of acute pancreatitis at the Yonsei University Severance Hospital between January 1990 and June A total of 138 charts were reviewed. For a patient to be included in the subsequent analysis, the following criteria were met: 1) the patient had typical symptoms of pancreatitis, and serum levels of amylase or lipase were elevated, 2) serum lipase and amylase were analyzed within 48 hours of the onset of symptom, 3) serurri creatinine was less than 2.5 mg/dl on admission. Twenty one patients satisfied the requirement for inclusion in the study. Data collected from the charts included age, sex, serum amylase and Iipase (from this the lipase/amylase ratio was calculated), duration of abdominal pain before admission, length f hospital stay, various laboratory parameters, severity according to the Atlanta classification and positive scores according to Ranson s criteria. The Iipase/amylase ratio was calculated on serum amylase and lipase (expressed as m비 tiples of the upper limit of normal), each of which was obtained at least within 48 hours of the onset of symptom. Eleven patients had acute alcoholic pancreatitis and the remaining 10 patients had acute biliary pancreatitis. Results: All patients with alcoholic pancreatiti~ were male, while there were 4 male and 6 female patients in biliary pancreatitis group. There was no difference in the serum amylase, Iipase and Iipase/amylase ratio (p=0.4813) between the two groups. Key Words: Lipase, Amylase, Serum Lipase/Amylase ratio, Acute pancreatitis 서 론 춰 l 장엽의감별진단에유용한지를평가해보고 자하였다. 급성춰l 장엽은대부분과다한음주나담석에의해유발되므로1. 2 내원초자세한병력의청취및각종영상진단을이용하여그원인을일부 1. 대상 대상및방법 규명할수있다. 특히담석혜장염의경우는복 부초음파검사나전산화단층촬영, 내시경적 역행성담춰 l 관조영술 (ERCP) 을시행하여담석을 발견하면확진할수있으나, 초음파검사의 우는총담관결석진단에있어민감도가낮고 ERCP 는쉐장염을악화시킬수있는위험성이 있어급성기에모든환자에게일률적으로시행 할수없는한계가있다. 경 전통적으로사용되던 혈청 amylase, lipase 와더불어근래 methemalbumin 4, 보체계단백질, C-reactive protein 5, phospholipase A2 6 키 trypsinogen activation peptide 8, leukocyte elastase 9, tumor necrosis factor 10 등이임 상에이용되면서쉐장염의진단및중등도예측 에있어각각의효용성에대한평가가이루어지 고있다. 한편 Gumaste 등 11 이급성춰 l 장염의초 기에측정한혈청 lipase 와 amylase 를이용하여 각효소치를정상상한치로나눈뒤산출한 lipase/amylase 비 ( 이하니 A 비 ) 가알콜섭취에의한 급성춰l 장염의진단에유용하다고보고한이후 여러저자들의보고는그결과가상이하다 1-14 이에저자등은급성쉐장염환자들을대상으로 혈청 lipase/amylase 비를후향적으로산출하여이 칸 l 월부터 1995 년 6월까지연세의대부속세브란스병원에내원하여임상증상및혈청 amylase의증가를통해급성쉐장염으로진단받은 138예중내원당시혈청 creatinine치가 2.5 mg/dl 이하이면서증상발현 48시간이내에혈청 amylase와 lipase가동시에측정된총 21 예를대상으로하였다 2. 방법대상환자의입원기록을분석하여성별, 연령, 혈액학적검사소견, 혈청생화학검사소견, 증상발현에서내원까지의시간, 입원기간, 각종영상검사소견등을조사하였다혈청 lipase/amylase ( 니A) 비는각환자의혈청 1ipase, amylase치를각효소의정상상한치로나눈수 ( 배수 ) 를분자, 분모로하여산출하였다. 임상적중등도의판정은 Ranson s criteria, 및 At1anta 분류 를사용하였다대상환자를두군으로분류하였는데, 알콜군 (1 1 예 ) 은복부초음파검사나전산화단층촬영상담석의증거가없으면서규칙적으로일정량 ( 주당소주 2홉 2 병 ) 이상의음주를하는환자로하였으며, 담석군 (1 0예 ) 은음주력이없는환자로 비가춰 l 장염의원인중알콜성쉐장염과담석 서 상기방사선검사상담석 ( 담낭석또는담관

OI98). 알콜군에서는 hematocrit이담석군에비해의미있게높은수치를나타냈다 (p=0.0043). 그러나연령, 증상발현에서내원까지의기간및기타검사실소견은양군간에통계적으로유의한차이가없었다 (Table 1). Ranson score

Li pasefamylase 비 NS 혈청 amylase 치는양군이비슷한수치를보 였으며정중값은알콜군 408, 담석군 IUfL 였고, Iipase 치는담석군에서더높았으나통계적 인 유의성은없었다혈청 L/A 비도담석군에서 높은경향을보였으나양군간의차이는통계적으로유의하지않았다 (Table 3, Fig. 1). 혈청니A 비 2.0을기준으로했을때알콜성쉐장염의진단에있어 υa비의민감도, 특이도는각각 8 1.8%, 0% 였으며, 3.0을기준으로했을때민감도 8 1.8%, 특이도 0% 로 2.0을기준으로했을때와동일하였다. 찰고급성춰l 장엽에서영상진단검사를시행치않고 asp따tate 없ninotr없1Sferase(AS T), alanine aminotransferase (AL T), 총 bilirubin, alkaline phosphatase(alp) 등흔히측정되는효소를이용하여담석춰l장염과알콜성쉐장염을감별하려는시도가계속되어왔으나그성적은일정치않았다 1 1 알콜성춰l 장염에서혈청 amylase치가다른원인에의한춰l 장염에서보다상대적으로낮음을이용하여 Gumaste 등 11은상승한혈청 lipase와 amylase를각효소의정상상한치로나눈수 ( 배수 ) 를분자, 분모로이용한니A비를소개하였다이들의보고에의하면 L/A비값 2.0을기준으로했을때, 얄콜성춰l장염진단의민감도와특이도은각각 91%, 78% 였다. 이들은혈청 amylase 치가증상발현후 2 일을전후로급격히감소함을고려하여증상발현후 48시간이내에혈액을얻어 L/A비를계산해야함을강조하였다 Gumaste 등 11 의결과는 Tenner 등 12에의하여추인되었는데, 후향적분석이었으나알콜군이비알콜군에비해통계적으로유의하게낮은혈청 amylase~l 와높은 L/A비를나타냈고, L/A비값 3.0을기준으로했을때알콜성급성춰l장염의진단에있어민감도, 특이도는각각 59%, 78% 였다. 알콜성춰l 장염에서혈청 amy lase치가낮은이유는명확치않으나만성음주자의춰l 장실질이알콜에의한만성적인손상상태에있는것이원인이아닐까추측하고있다. 즉알콜에의한만성 손상으로쉐실질에섬유화, 선 (gland) 조직의손

5 송건훈외 5 인 - 급성춰 l 장염에서혈청 Li pase/amylase 비 241 실등이발생하여급성쉐장염시혈청 amylase 치의상송도둔화되지않나추측한다 12, 1 3 그러나 Tenner 등 1 2의보고이후로혈청니A비의효용성 을추인하는보고는없는실정이다, Pezzilli 등 1 3 은알콜성쉐장염환자군에서혈청 amylase, lipase 치가담석군에비해유의하게낮다는사실 을확인했을뿐, 니 A 비는알콜군, 담석군, 비알 콜비담석군세군간에의미있는차이를관찰할 수없었다 King 등 14 에의하면알콜군, 비얄콜군 간에니 A 비의유의한차이는없었고 1 lipase 치의 혈청 amylase, 정중값은비알콜군이알콜군에비해 유의하게높았는데, 비알콜군환자의 60% 가담 석증환자였다. 본연구는환자군을알콜군, 담석군의두군 으로제한함으로써과거연구에서제기된비알 콜군환자구성의차이에따른혼란요인을방지 하고자하였으며, Ranson s criteria 및 Atlanta 분 류에따른임상적중등도가양군간에통계적으 로차이가없다는사실을확인한상태에서혈청 니 A 비를산출하였다, 또한양군의환자에서증 상말현에서내원까지기간의정중값은각각 12 시간, 9 시간이었고, 혈청 amylase, lip ase 의측정 이증상발현후 48 시간이내에행해진환자들 만을분석대상에포함시킴으로써시간변화에 따른혈청 amylase 치의변동이 L/A 비산출에미 치는영향을최소화시키고자하였다 본연구 에서양군간혈청 amylase, lipa se 치의분포양 상은외국연구자들의결과 와상이하였다. 즉혈청 amylase 치는양군간에유사했던반면 lipase 치는당석군이훨씬높았는데, 그원인은 확실치않으나이러한양상때문에니 A 비도담 석군에서높은경향을나타냈다. 이결과를설 명하기위해서는몇가지가설이가능한데가장 타당성이있는것은국내담석쉐장엽환자들의 초기혈청 amylase 치 상승이외국환자들에비 해상대적으로적다는사실이다. 이는외국비 알콜쉐장염환자들의발병초기혈청 amylase 정중값이 IU/L 의 범위를보이는데 반해 본연구에서담석쉐장염군환자들의 amylase 정중값은 IU/L 에불과했던점으로 미루어추측할수있다. 이렇게외국과는다른 초기쉐장효소의상승양상은인종에따라손상 에대한쉐장의반응이상이하기때문에나타나 는현상일가능성이있으며, 국내급성쉐장염 환자의사망율이 3. 5% 로서외국환자의사망율 에비해현저히냥은사실 16 과도밀접한관계가 있을가능성이있다 본연구에서 υa 비가양군간에유의한차이 가없었을뿐아니라외국연구자들이 보고한 L/A 비 2.0, 3.0 을기준으로알콜성춰 l 장염의진 단에있어민감도, 특이도를산출했을때, 민감 도는높으나특이도가 0% 에불과하여웨장염의 원인규명에는큰도움을주지못하는것으로 생각되었다. 목적. 요 약 혈청 lipase/amylase( 니 A) 비가급성춰 l 장 염의원인중알콜성쉐장염과담석춰 l 장염의 감별에유용한가를평가하고자하였다. 대상및 방법. 급성쉐장염으로진단된환자들중증상 발현 48 시간이내에혈청 lipase, amylase 가동시 에측정된환자 21 예를대상으로업원기록을통 해임상적특성을파악하였고, 혈청 L/A 비를후 향적으로산출하였다. 결과 : 총 21 예중알콜군 (1 1 예 ) 은전예가남성인반면, 담석군 (1 0 예 ) 은 남녀비가 1: 1. 5 로서양군간의성비의차이는통계 적으로유의하였다 (p=0.oo39). 균은알콜군 ::!:: 미 U 범위 : 석군 ::!:: IU/L( 범위 혈청 amylase 치의평 ), 담 ) 였으며, 혈 청 lipa se 치의평균은알콜군 ::!:: IU/L( 범위 ), 담석군 ::!:: IU/L( 범위 ) 로 lipase 치는담석군에서 높았으나, 두효소모두양군간에의미있는차 이는없었다. 혈청니 A 비의평균은알콜군 6.03 ::!:: 4. 16( 범위 ), 담석군 6.63 ::!:: 3. 16( 범위 :
