Some studies have suggested that treatment using antidepressant medications augmented with estrogen can be more effective than using antidepressant medication alone. However, data about the estrogen augmentation of selective serotonin reuptake inhibitor (SSRI) medication is insufficient. We investigated the effect of hormonal replacement treatment (HRT) as an augmentation in postmenopausal women with depression who were already being treated with SSRIs. Methods:This was an 8- week prospective, open trial study. The subjects were 13 patients who met the DSM-IV criteria for major depressive disorder or dysthymia. The menses of all subjects had ceased for at least one year, and all subjects had serum levels of follicular stimulating hormone (FSH) greater than 40 miu/ml. All subjects were already taking sertraline ( mg) or paroxetine (20-30 mg);during the study they received sequential combined hormone replacement treatment (1.25 mg estrogen +5 mg medroxyprogesterone). Subjects continued their SSRI medication at their original dosages throughout the study. Mood and anxiety symptoms were measured with the Beck Depression Inventory (BDI) and the State-Trait Anxiety Inventory (STAI). Menopause-related symptoms were assessed using the Women s Health Questionnaire (WHQ). Overall clinical symptoms were also assessed using the Symptom Checklist 90-Revised (SCL-90-R). Serum levels of luteinizing hormone (LH), FSH, prolactin, estradiol, and progesterone were measured. Measurements were conducted at three times: baseline, four weeks, and endpoint. Results:The subjects mean age was 53.54±5.38 years and mean postmenopausal duration was 4.46±3.67 years. During the HRT, BDI and Trait Anxiety Inventory scores decreased significantly. On the WHQ subscales, scores for somatic symptoms, vasomotor symptoms, sexual behavior, and sleep problems were significantly improved. Hormonal levels of LH, FSH, and estradiol changed significantly. (Korean J Psychopharmacol 2008;19(4): ) KEY WORDS:Postmenopause Depressive disorder Hormone replacement treatment Estrogens. 서론 실험및임상연구를통하여 estrogen 은기분에영향을줄수있는것으로인식되어왔다. 1) 동물연구에서생식호르몬들이기분장애에관련된일부신경전달물질들을조정하는결과가보고되었으며, 2) 폐경여성에난소의추출물 (ovarian extracts) 을투여하여기분이고양된다는보고이래기분장애여성에서도 estrogen 의잠재적항우울효과가관심을끌게되었다. 3) estrogen은다양한신경조절효과를갖고있으며, 치료불응성우울증, 산후우울증, 폐경기우울증여성들에대한하나의치료적사용으로연구되어왔다. 긍정적효과를보인많은연구들에서는 estrogen 단독치료에초점을두었고, 기존의보고들은주로 estrogen 단독요법에서의 estrogen 항우울효과에대한연구들이다. 1) estrogen 은중추신경계에광범위한영향을미치며, 특히 serotonin 체계의활성을촉진한다는증거들이제시되고있는바, 2,4,5) 세로토닌재흡수억제제 (selective serotonin reuptake inhibitor, 이하 SSRI) 와같은항우울제에대한 estrogen 의증강치료 (augmentation therapy) 는임상적인유용성을갖는다. estrogen 에항우울제를추가하여그결과를보고한많은연구들이있으나각기다른연구디자인 6-11) 을가지며, 그에따라일관되지않은각기다른결과들을보고하고있다. 12) Rasgon 등 10) 의연구에서는기존의 fluoextine 을사용하였을때반응이없거나부분적인반응만을보인 6명의폐경주위기 (perimenopause) 여성들에게 estrogen 을추가하였을때 1명은관해를보였고, 다른 5명도추가적인반응을보였다고하였다. Thase 등 13) 이보고한통합분석 (pooled analysis) 에서는연령이높은여성군 (50 세이상 ) 에서는 SSRI 에대한반응이상대적으로낮았으나, 호르몬대치 요법을받는경우이러한연령에따른치료반응의차이가없어서, 결과적으로호르몬대치요법이 SSRI 치료반응을증강시키는것으로나타났다. 폐경기간 (menopausal duration) 은호르몬의생리적활성과관련성을가지므로, 여성에서항우울제에 estrogen 증강치료를할때고려되어야한다. 1) 그러나기존의연구에서는대부분이폐경주위기 (perimenopause) 와폐경후기 (postmenopause) 의구분이되지않거나, 6,9,11) 폐경주위기여성을대상 10) 으로하였다. 그러나폐경후기의여성에대해서는그결과들은일치되지않고있으며, 전향적인연구디자인을갖추고있으면서관련되는호르몬들의농도를전반적으로평가한연구들은아주제한적이다. 12) 호르몬증강치료를받는여성들에서는아직도 estrogen 의항우울제에대한증강치료에대한지침이제시되지는않고있으며, 호르몬의종류, 용량, 투약경로, 치료기간, 대상환자들의폐경상태 (menopausal status) 등의변인들에따라치료효과가달라질수있다. 본연구에서는폐경후기우울증여성을대상으로하여, estrogen을 25일간투여하면서 progesterone 을월경주기후반의 10일간투여하는주기적호르몬병합대체요법 (sequential combined hormone replacement therapy) 을선택하여, 전향적으로기존의 SSRI 의항우울효과에대한단기증강치료를시도하여치료결과를보려하였다. 대상및방법 1. 연구대상본연구는고려대학교구로병원에서시행된단기간, 전향적, 개방표지연구이다. 이연구는헬싱키선언에입각하여실시되었으며, 본연구의목적과방법에대한설명을들은후모든참여자들은연구참여에관한서면동의를하였다. 환자들은 Diagnostic and Statistical Manual 218 Korean J Psychopharmacol 2008;19(4):

3 이문수등 of Mental Disorders, 4 th edition( 이하 DSM-IV) 에의거하여주요우울증또는기분저하증으로진단을받았으며, 폐경기간이최소 1년이상이면서혈중 folliclestimulating hormone( 이하 FSH) 농도가 40 IU/L 를초과한 13명의여성이연구에등록되었다. 각연구참가자는 sertraline(50~150 mg) 이나 paroxetine(20~30 mg) 을최소한 4주이상용량의변경이없이사용하고있었다. 2. 약물치료각연구참여자들은연구기간동안기존의항우울제약물의용량변동이없는상태에서, 8주간주기적 estrogen 과 progesterone 병합요법방식의호르몬대체요법 (hormone replacement treatment, 이하 HRT) 을받았다. 이방식에서는 1.25 mg conjugated equine estrogen 을 2회에걸친 25일간의주기 (1~25 일 ) 동안지속적으로처방하면서, 각주기의후반부 10일간 (15~25 일 ) 에는추가적으로 5 mg medroxyprogesterone acetate 를투약하였다. 3. 평가각평가는연구시작, 4주, 8주종료시점에서총 3회에걸쳐이루어졌으며, 다음의조사및측정이이루어졌다. 1) 자가보고형설문조사연구참여자로부터의정보는다음의자가보고형척도들로부터얻었다. Beck 우울척도 (Beck depression inventory, 이하 BDI), 상태- 특성불안척도 (State-trait anxiety inventory, 이하 STAI), 여성건강설문지 (the Women s Health Questionnaire, 이하 WHQ), 간이정신진단검사 (Symptom Checklist 90-Revised, 이하 SCL-90-R). (1) BDI 14) 우울증상들은 21항목으로되어서우울증의정도를정량화할수있는 BDI 로평가되었다. 각항목의점수를합산하여총점을내며점수범위는 0~63 점이다. 와특성불안을구분하여측정하는총 40문항으로된검사이다. (3) SCL-90-R 16) SCL-90-R 은전반적정신건강을평가하기위하여사용되었다. 이선별검사도구는잠재적인정신과적 / 심리적질병들의선별을위하여사용될수있으며, 5점척도 (0= 전혀없다, 4= 아주심하다 ) 로된 90개의항목들을포함하고있어서 9개의영역에서의증상들을평가한다 ( 신체화, 강박증, 대인민감성, 우울, 불안, 적대감, 공포불안, 편집증, 정신증 ). 또한 3가지의전체척도를산출할수있다 : 전체심도지수 (Global Severity Index, 이하 GSI), 표출증상심도지수 (Positive Symptom Distress Index, 이하 PSDI), 표출증상합계 (Positive Symptom Total, 이하 PST). (4) WHQ 17) 폐경주위기와폐경기의여성들이경험하는심리적, 신체적증상들을상세하게조사하기위해, 원래의 9개의영역, 36개의문항으로구성된 WHQ 18,19) 중척도의원저자가폐경기여성에서의연구를위하여선택 20) 한 6개의영역 ( 신체화증상 -7문항, 혈관운동증상 -2문항, 성행동-3 문항, 수면장애 -3문항, 우울감-6문항, 불안한감정 / 두려움 -4문항 ) 25문항으로구성된설문을사용하였다. 각질문은지난 4주간의경험에대하여답하게되어있으며 0~3 점까지의 4점척도로되어있다. 본연구에서는각영역별항목점수를합산하여산출하였으며, 각영역별점수가클수록장애가심한것을의미한다. 2) 호르몬농도측정본연구에서는혈중 estradiol( 이하 E2), progesterone, luteinizing hormone( 이하 LH), FSH, prolactin이방사면역측정법 (radioimmunoassay, 이하 RIA) 을사용하여측정되었다. 혈액채취전날밤부터금식을지시하고당일오전 9시에서오전 11시사이에혈액을채취하여검사를실시하였다. (2) STAI 15) 불안증상들은 STAI 로평가되었으며이척도는상태 4. 분석본연구에서는 3회에걸친각평가치의변동을반복측 219

4 폐경후기여성에서 SSRI 에의 Estrogen 증강치료 정분산분석 (repeated measures analysis of variance) 을사용하여평가하였고, 연구참여종료시점에서의평가치의변화량과각폐경기간과의상관관계를보았다. 통계분석은컴퓨터용통계프로그램인 SPSS 10.0 for windows 를사용하였으며통계적유의성은 p<0.05 로하였다. 결과 1. 대상자의인구학적특성및자가보고형설문조사결과의변화본연구참여자들의기본적인구역학적자료및연구시작시의각평가결과가표 1에제시되어있다. 연구기간동안, BDI(Greenhouse-Geisser correction, F= , df=2, p<0.01), STAI 중에서특성불안 (Greenhouse-Geisser correction, F=6.888, df=2, p<0.01) 은유의하게감소하였다 ( 그림 1). 이러한 BDI 및특성불안의경우에는시간상으로도, 기저치와 4주째의측정치사이에서도각기유의한차이가있어서, 본호르몬대치요법 4주째에이미유의한효과를보였다. 5 이문수등 FSH:p<0.01). 기저치의각호르몬농도 (LH, FSH, prolactin, estradiol, progesterone) 와치료에따른각평가치의변화량 (BDI, SCL-90-R 의우울증상점수, WHQ 의우울감영역 ) 간의상관을다시보았으나통계적으로유의한상관은보이지않았다. 3. 평가치의변화량과각폐경기간과의상관관계폐경기간과치료에따른각평가치의변화량간에는 Score changes BDI* SAI TAI* Figure 1. Measurement WHQ 척도중에서신체화증상 (r=-0.583, p=0.037), 불안증상 (r=-0.623, 0.023), 우울증상 (r=-0.586, p= 0.035) 이유의한부적상관이있었으며, SCL-90-R 척도중에서는우울 (r=-0.584, p=0.036), GSI(r=-0.642, p=0.018), PSDI(r=-0.592, p=0.033) 와유의한부적상관을보였다. 4. 안전성평가전체연구기간동안에호르몬대치요법으로인한이상반응을호소한환자는유방압통을호소한경우가한명있었으며, 보고된이상반응의심각성은경미한수준으로, 이상반응으로인해치료를중단한환자는없었다. 고찰 여성들은남성보다더높은우울증의유병률을보인다. 기존연구들에서내재한내분비호르몬상의변화가취약성을가진여성에서기분상의문제를가져올가능성이제시된바있다. 21) 내분비호르몬중의하나인 estrogen 의경우에는세로토닌활성을증가시켜서일부항우울제의효과를증강시킬수있을것으로생각된다. 4) 그러나, 어떻게항우울효과를증가시킬것인지에대한지침은아직없다. 1) 이전의연구들은호르몬대치요법이기분과인지기능에긍정적인효과를갖는다고보고하여왔다. 22) 그러나대규모로시행된장기간의예방연구인 Somatic symptoms* Depressed mood Anxiety/fear Vasomotor symptoms* Sleep problems* Sexual behavior* Scores 4 2 Figure 2. LH:Luteinizing hormone, FSH:Follicle Stimulating Hormone. women s health initiative 연구는호르몬대치요법을사용하는것이위약에비하여더큰건강상의위험을가져온다고보고하면서, 23) 폐경후기의여성들에서 estrogen 과 progesterone 의병용요법이치매나경도인지장애의위험성을감소시키지못한다고하였다. 24) 그러나각기다른대상군들에서기분장애에대한호르몬치료의효용성에대해서는아직확실하지않다. 1) estrogen이단독으로사용이된연구 7) 도있고, 반면에 progesterone과같은다른호르몬과병합하여사용되기도하였으며, 대상군도우울증환자군을대상으로한연구 7) 와비우울증일반인 25) 들을대상으로한것으로나뉘고, 또한폐경주위기 1) 와폐경후기의여성간의구분이되지않은경우가있어서기존연구들의결과를일반화하는데에는많은어려움이있다. 이중에서기존에우울증으로항우울제를처방받는폐경후기의여성을대상으로하여전향적으로추가적인호르몬대치요법의효과를평가한연구들은아주제한적이다. Schneider 등 7,8) 은기존에항우울제를투약받는 60 세이상의여성주요우울증환자들에서 estrogen 대체요법이 SSRI 들-fluoxetine, sertraline-에대한치료반응을증강시켰다고하였다. estrogen의용량에따라서기분에대한효과에서도차이가있을수있다는보고가있어서, Sherwin 등 26) 은 95명의건강한폐경기여성들을대상으로 estrogen 을투여한연구에서, mg의 estrogen 은생리적인대 222 치용량으로홍조증상들을조절하기에충분하나, 1.25 mg 고용량의 estrogen 은초생리적인혈장 estrone 농도를유발하면서기분에대한효과가발현된다고보고하였다. 본단기간의연구도 1.25 mg의상대적인고용량을사용하였으나, 주의할만한부작용이없이치료 4 주째부터추가적인기분호전효과가있었다. estrogen 이갖는증식반응은자궁이나유방에서는자궁암의위험성을높이는등의문제를가져올수있기때문에, estrogen과 progesterone을병합하여사용하는경우가많다. 27) 본호르몬대치요법의일부로사용된 medroxyprogesterone 의경우에도 estrogen 과함께사용시에, 항우울효과를감소시킨다는보고 25) 가있었으나최근의대규모의연구에서는 medroxyprogesterone이 estrogen 의항우울효과를경감시키지않았으며, 28) 본연구에서도별다른부작용을유발하지않았다. 본연구에서일차적으로우울증상을평가한 BDI 는유의한증상의변화를보여주었다. 본연구에서유의한변화를보인치료지표들 (BDI, 특성불안, WHQ 중신체화증상, 성행동, 수면장애 ) 의경우에는치료시작 4주째에유의한변화를보이고있으면서, 8주까지지속이되었다. 따라서본연구에서의초생리적인용량의 estrogen 이포함된 HRT 는치료 4주부터는유의한추가적항우울효과를보이는것으로생각할수있겠다. 그러나, 본연구결과의일반화를위하여는객관적인평가자척도를사용 Korean J Psychopharmacol 2008;19(4):

7 이문수등 하여추가적인많은대상군을포함한연구가필요하다. SCL-90-R 의경우에는우울증상및기타영역에서증상점수가감소하였으나, 통계적으로는유의하지않았다. 이때감안하여야할것은본연구에포함된대상군들의임상적인특성이다. 연구참가자들은 sertraline(50~ 150 mg) 이나 paroxetine(20~30 mg) 을이미최소한 4주이상용량의변경이없이사용하고있었으나연구참여시점에서의 BDI 점수가평균 점으로비교적높은군에속하고있었으며, 따라서다른항우울제치료에대해서도반응이적은치료불응성의환자들이포함되었을수있다. 또한부가적으로는참여자의숫자가적어서통계적유의성을얻지못하였거나, SCL-90-R 이범용적으로쓰이는전반적정신건강을평가하는척도인관계로, 이미항우울제를처방받고있는폐경후기여성에서의 HRT 에의한추가적인증상변화를민감하게반영하지못하였을가능성도생각하여볼수있다. 본연구에서사용한 WHQ 는주로폐경후기와폐경주위기여성에서경험하는여러신체, 심리적증상을다루는삶의질평가도구로그신뢰도, 타당도는기존의많은연구에서입증된바있으며삶의질을평가하기위하여일반화되어사용되는척도이다. 17) 기존의 223 명의폐경후기여성자원자에서경피 (transdermal) estradiol(50 μg/24 h) 의효과를이중맹검, 무작위, 위약통제방식으로 12주에걸쳐서평가한연구 29) 에서 WHQ 척도에서유의한호전이있었던것이보고된바있으며, 다른연구에서는저용량의 estrogen 으로도혈관운동증상뿐만아니라신체증상, 불안한감정 / 두려움, 우울감, 수면장애증상들에서유의한감소가있었다고한다. 30) 본연구에서도조사된 6개영역중에서신체화증상, 혈관운동증상, 성행동, 수면장애등총 4개영역에서유의한효과가있었으며통계적유의성은없었으나우울증상에서도감소를보였다. 본 HRT 에서여성의 WHQ 에서유의한효과를보인것은 HRT 가단순히일부폐경증상들의호전뿐만아니라, 더전반적인신체적, 심리적안녕과함께삶의질을개선시키는효과를보여줄수있는가능성을제시하는것이다. 기저치호르몬농도에따라서치료결과에다른영향이있는가를알아보기위하여, 기저치의각호르몬농도와각평가치의변화량간의상관을보았으나유의한 상관이나오지않아서, 본연구결과로는호르몬농도와우울증상치료효과간의직접적인연관을지을수는없는것으로나타났다. estrogen의부족은우울증에대한취약성을증가시키는것으로생각된다. 31) 그러나혈중 estrogen 의농도증가가직접적으로생리적인효과도증가함을의미하지는않는다. HRT시에우울증이없는폐경기간이오래된여성은폐경기간이짧은여성에비하여 estrogen 농도가유의한증가를보임에도불구하고, 더우울감을호소한다고보고되고있으며이러한현상은폐경기간이오래되면서 estrogen 결핍이오래지속되고, 뇌에서의 estrogen 수용체기능에장애가초래되어서유발된호르몬과중추신경계반응간의해리 (dissociation) 현상으로이해되고있다. 32) 따라서, 폐경기여성에서호르몬대체요법의효용성을평가하기위해서는폐경기간도고려되어야한다. 본연구에서는폐경후기여성만을대상으로하였으며, 폐경기간과치료에따른각평가치의변화량간에 WHQ 척도중우울기분, 신체화, 불안증상과 SCL-90-R 척도중에서우울증상및 GSI, PSDI 전체척도지수가유의한부적상관을보였으며, 상관관계의강도도중등도를보이고있어서본연구에서는폐경기간이길어짐에따라서 SSRI에대한 estrogen 증강치료에의한변화의폭이적었음을보여주었다. 이는폐경기간이짧을수록 SSRI 에대한 estrogen 증강치료의항우울효과가증대될가능성을제시하는것이다. 따라서향후에는초기폐경후기 (early postmenopause) 와후기폐경후기 (late postmenopause) 를구분하여각기동질적인군들을구성하여, 위약을사용한대조군들을포함한더많은대상자들에서의장기간의추적관찰을통한치료결과의평가가뒤따라야할것으로생각된다. 본연구에는 SSRI 의종류가통일되지못하였으며, 대조군없이, 연구참여자의수가 13명으로적어서, 본결과를일반화하는데에는한계가있다. 그러나본연구는고용량의 estrogen 을폐경후기여성들로만이루어진우울증환자군에서추가적으로사용하였으며우울증평가뿐만아니라, 폐경주위기와폐경기의여성들에서심리적, 신체적증상들을연구하기위한도구인 WHQ까지이용하여조사가이루어졌다는데에의의가있으며, 폐경후기여성에서의 SSRI 에대한 estrogen 의증강치료효과 223

8 폐경후기여성에서 SSRI 에의 Estrogen 증강치료 가있을가능성을실제전향적인평가에서제시하였다. 224 요 본연구에서는단기간의전향적인개방표지방식으로 SSRI 를투여받고있는폐경후기우울증여성환자들에서호르몬대체요법을실시하여호르몬농도변화및우울증상과폐경기관련증상들의변화, 폐경이후기간과의상관성을조사하였다. DSM-IV 에따라주요우울장애나기분저하증으로진단을받았으며, 폐경기간이최소 1년이상이면서혈중 FSH농도가 40 IU/L 를초과한 13명의여성이연구에등록되었다. 각연구참가자는 sertraline(50~150 mg) 이나 paroxetine(20~30 mg) 을투여받고있었고, 연구기간동안에는항우울제약물의용량을고정하여, 8주간호르몬대체요법중주기적 estrogen 과 progesterone 병합요법방식을사용하였다.
