1 종설 J Kor Sleep Soc / Volume 6 / June, 2009 기면증의진단 이주헌 한림대학교강동성심병원신경과 Diagnosis of Narcolepsy Ju-Hun Lee Department of Neurology, Hallym University, College of Medicine Narcolepsy is a sleep disorder, which is characterized by excessive daytime sleepiness (EDS) that is typically associated with cataplexy and other REM sleep phenomenon such as sleep paralysis and hypnagogic hallucination. Narcolepsy is distinguished into three different entities, narcolepsy with cataplexy, narcolepsy without cataplexy, and narcolepsy due to medical condition. Multiple sleep latency test (MSLT) following overnight polysomnography is s standard method for evaluating EDS and plays essential role in the diagnosis of narcolepsy. However, to provide a valid assessment of sleepiness or narcolepsy, the MSLT should be performed under appropriate condition using a qualified recording procedure. In this article, current diagnostic criteria of narcolepsy and recommended recording methods for reliable MSLT will be summarized. Key Words : Narcolepsy, Multiple sleep latency test 서론수면질환의정확한진단을위해서는병태생리학에근거한명확한분류체계가필수적이다 년미국수면질환협회 (American Sleep Disorders Association) 에의해 International classification of sleep disorders, 1 st edition "(ICSD-1) 이발표되었고, 수면질환에대한그동안의연구결과를반영하여 2005 년 International classification of sleep disorders, 2 nd edition (ICSD-2) 로개정되어, 1 현재의수면질환의진단기준은 ICSD-2 에근거하고있다. 기면병 (narcolepsy) 은전형적으로허탈발작 (cataplexy) 과수면마비또는입면환각과같은렘수면현상 (REM sleep phenomenon) 을동반하는주간과다졸림증 (excessive daytime sleepiness) 을특징으로하는아직원인기전이불확실한질환이다. 이종설에서는 ICSD-2 에근거한기면병의진단기준및그병인론적배경을서술할것이며, 아울러기면병의가장핵심적인진단도구인수면잠복기반복검사 (multiple sleep latency test, MSLT) 의시행방법에대해자세히소개하고자한다. 본론 MSLT 시행방법과주간과다졸림증의정의 * Address of correspondence Ju-Hun Lee Department of Neurology, Kangdong Sacredheart Hospital, 445 Gil-Dong, Kangdong-Gu, Seoul, Korea, Tel: Fax: leejuhun@hallym.or.kr 기면병이나특발성과다수면 (idiopathic hypersomnia) 과같은중추신경계에서기인되는수면질환의진단을위해서는주간과다졸림증을신뢰성있게정확하게측정하는것이가장중요하다. 2 이주헌 측정할수있는표준적인검사이다. 검사-재검사신뢰도 (test-retest reliability) 와판독-재판독또는판독자사이의신뢰도 (intrarater and interrater reliability) 도높은것으로알려져있다. 그러나, 신뢰성있는 MSLT 검사를위해서는다음과같은검사조건들을준수하여검사가시행되어야한다. 2 1 수면일지 (sleep log) 나활동기록기 (actigraphy) 를이용하여, MSLT 검사전 2주동안적절한야간수면이이루어졌는지와일주기리듬장애등다른수면질환여부를확인하는것을권장한다. 2 MSLT 검사전최소한 15일동안각성효과가있거나렘수면을억제하는등수면에영향을주는약물을금해야한다. 3 MSLT 검사바로전날야간수면다원검사를통해수면무호흡증후군과같은주간과다졸림증의다른원인을감별해야하고, 적절한야간수면 ( 수면다원검사에서최소한 6시간이상의수면 ) 이이루어졌는지확인해야한다. 야간수면다원검사는지속기도양압 (cont- Inuous positive airway pressure) 의압력역가측정 (pressure titration) 을위한방법으로진행되어서는안된다. 4 MSLT 는야간수면다원검사종료 ( 기상 ) 후최소한 1.5 시간지나서시작하며, 2시간간격으로짧은낮잠 (nap) 검사를 5회반복하게된다. 4회만시행하고마칠수있으나, 4회낮잠검사중최소한 2회의수면개시렘 (sleep-onset REM period, 이하SOREMP) 이관찰되는경우가아니라면신뢰성이떨어지므로 5회의낮잠검사를모두마쳐야한다. 5 검사당일흡연은각각의낮잠검사시작전최소 30 분부터는금지한다. 카페인이함유된음료도금해야하며, 아침식사는첫번째검사최소 1시간전까지마치는것이좋고, 점심은두번째검사직후가볍게하도록한다. 6 MSLT에사용되는전형적인몽타주 (montage) 는뇌파 (C4-A2, C4-A1, O1-A2, O2-A1), 전기안전도 (EOG), 턱근전도, 그리고심전도이다. 7 각각의낮잠검사마다눈금조정 (calibration) 을하고 나서, 환자로하여금눈감고편안히잠을청하도록요청한후, 바로조명을끄고검사시작을표시하도록한다. 각각의낮잠검사사이에환자는잠을자서는안되며, 검사자가이를지켜보고확인하도록한다. 8 수면잠복기는조명을끈후부터어떤단계의수면이던지첫번째수면이관찰될때까지소요시간이다. 즉, 1단계수면 (sleep stage 1) 이비연속적으로한번 (1 epoch) 만관찰되어도수면시작이라고평가할수있다. 각각의수면검사는첫번째수면이관찰된후 15분, 또는수면을취하지못하는경우검사시작 20 분후에종료한다. 수면을취하지못한경우에수면잠복기는 20분으로계산한다. 9 SOREMP 는수면시작후 15분이내에렘수면이관찰되는것으로정의한다. MSLT 결과 8분이하의평균수면잠복기 (mean sleep latency) 를보이면주간과다졸림증이있다고정의한다. 이정의는기면병을진단하는데최선의수치를기준으로정해진것으로, 3,4 진단기준을단순화한다는명분으로특발성과다수면과같은타질환에서도그대로적용하고있다. 전체기면병환자의평균수면잠복기는 3.1±2.9 분으로알려져있으며약 90% 에서 8분이하의수면잠복기를보이는것으로알려져있다. 5 기면병의진단기준기면병의가장특징적인증상은허탈발작이다. 기면병은허탈발작의동반여부에따라허탈발작동반기면병 ( 이하, narcolepsy with cataplexy ) 과허탈발작비동반기면병 ( 이하, narcolepsy without cataplexy ) 로나뉜다. ICSD-1 과달리 ICSD-2 에서이처럼기면병을둘로나누어정의한이유는, narcolepsy with cataplexy 의경우거의모든환자에서 HLA DQB1*0602 를가지고있으며약 90% 에서뇌척수액히포크레틴-1( 이하, hypocretin-1) 이매우낮거나검출되지않는반면, narcolepsy without cataplexy 에서는환자의 40% 에서만 HLA DQB1*0602 양성이며겨우 10% 에서만뇌척수액 hypocretin-1 이저하되어있어, 양군사이 10 수면

3 기면증의진단 에병태생리학적차이가있을가능성을높이인정하였기때문이다. 5 한편, 다른내과적및신경학적질환으로인해발생한속발성기면병 (narcolepsy due to medical condition) 도있어, 기면병은 narcolepsy with cataplexy, narcolepsy without cataplexy, 그리고 narcolepsy due to medical condition 으로구분할수있다. Narcolepsy with cataplexy Narcolepsy with cataplexy 는유럽이나북미에서는 % 의유병률을보인다. 6-8 조사방법의차이가있겠지만일본에서는 % 로다소높은유병률을보이고, 9 반면이스라엘에서는 0.002% 의낮은유병률을보이는것으로알려져있다. 10 ICSD-2 에서는 narcolepsy with cataplexy 를진단하는데, 허탈발작이전형적이어야함을강조하고있다. 또한, 기면병에흔히동반되는수면마비, 입면환각, 야간수면장애 (disrupted night sleep), 자동행동 (automatic behaviors) 등의증상들은비특이적증상이기때문에진단기준에포함되지않는다. 기면병환자들에서야간수면다원검사를시행하면, 렘수면이 10분이내로빨리관찰된다든가수면잠복기가 20분이내로줄어든소견을보이는경우가많지만, 이또한비특이적소견들이기때문에진단기준에포함되지않는다. HLA DQB1*0602 의경우도일반인에서흔히관찰되는소견이므로 narcolepsy with cataplexy 의진단기준의하나로사용되지않는다. Narcolepsy with cataplexy 는거의매일반복되면서최소한 3개월이상지속되는주간과다졸림증을호소하면서전형적인허탈발작의병력이있으면진단이가능하다 (Table 1). 주간과다졸림증은보통거의매일짧게반복적으로일어나며, 도저히억제할수없고, 신체활동이활발하지않을때잘일어난다. 흔히꿈을동반하기도하기도하는데, 잠에서깨고나면잠시정신이맑아져서수시간까지도졸림증이없어지는청량효과 (refreshing effect) 가있다는것이중요한특징이다. 허탈발작은감정변화에의해유발되는 ( 의식소실을동반하지않는 ) 갑작스런근긴장의일시적인소실을의미한다. 허탈발작은감정변화중즐거운소식이나웃음과같은긍정적인감정고조또는분노등에의해잘유발되는반면, 스트레스나공포등에의해서는 거의유발되지않는다. 11 횡경막이외에모든가로무늬근 (striated muscle) 에서발생할수있어갑자기쓰러질수있으며, 대부분수초에서수분지속되다가즉각적으로완전히회복된다. 그러나, 전신적인근긴장의소실이아니라, 얼굴근육이나눈꺼풀또는턱이처지거나팔또는다리만마비되는등부분적으로근긴장이소실되는경우도있다. 또한, 10분이상지속되는경우도있으므로병력청취시에주의해야한다. 허탈발작의발생빈도는개인차가심하여하루에도수없이반복되는환자가있는반면, 일년에한번이하로드물게발생하거나, 발병시점에몇차례나타났다가그후에없어지는경우, 또는간헐적으로군집성으로발생하는경우도있다. 또한, 허탈발작을치료하기위해항우울제를복용하다가갑자기중단한경우특히심할수있어주의해야한다 ( status cataplecticus ). 허탈발작이 narcolepsy with cataplexy 의진단에있어진단적가치가있는특징적 ( pathognomonic ) 인증상이므로, 주간과다졸림증과전형적인허탈발작이있는경우 MSLT 검사를 ( 진단을위해 ) 필수적으로시행해야하는것은아니다. 그러나, 가능하다면꼭 MSLT 검사를통해기면병의임상적진단을확인할것을권장하고있다 (Table 1). MSLT 결과전날야간수면다원검사에서최소한 6시간이상의야간수면이확인된상태에서, 평균수면잠복기가 8분이하이며 SOREMP 이 2회이상이관찰될때기면병을확진할수있게된다. 그러나, narcolepsy with cataplexy 환자에서도 17-26% 에서 SOREMP 가 2회미만으로관찰될수 Table 1. The diagnosis of narcolepsy with cataplexy should, whenever possible, be confirmed by nocturnal polysomnography followed by an MSLT; the mean sleep latency on MSLT is less than or equal to 8 min and two or more SOREMPs are observed following sufficient nocturnal sleep (minimum 6 h) during the night prior to the test. Alternatively, hypocretin-1 levels in the CSF are less than or equal to 110 pg/ml or one-third of mean normal values. D. 4 이주헌 있고평균수면잠복기가 8분을초과하는경우도있어, 3,4 이런경우에는뇌척수액 hypocretin-1 을측정하여 narcolepsy with cataplexy 를확진할수있다. 뇌척수액 hypocrtetin-1 이 110 pg/ml 또는검사기관정상수치의 1/3 미만의소견을보이면 narcolepsy with cataplexy 를진단할수있다. 110 pg/ml 미만의뇌척수액 hypocretin-1 소견을보일때 narcolepsy with cataplexy 진단의특이도 (specificity) 는 99%, 민감도 (sensitivity) 는 87-89% 인것으로알려져있다 따라서, 상기한것처럼 MSLT 에서확진이되지않거나, 여러가지이유로 MSLT 를시행하지못하거나, 신뢰성있는 MSLT 검사를못하는경우에 MSLT 대신뇌척수액 hypocretin-1 을측정하여 narcolepsy with cataplexy 을확진할수있다. Narcolepsy with cataplexy 의병인론기면병개모델 (canine model of narcolepsy) 과달리기면병환자에서 hypocretin 유전자의변이가보고된경우는극히드물지만 15, hypocretin 기능부전이 narcolepsy with cataplexy 의병인에관계된것임은분명한사실로보인다. 기면병환자의사후뇌조직에서외측시상하부 (lateral hypothalamus) 에있는 hypocretin 신경세포의선택적인감소및 hypocretin 펩티드 (peptides) 와프리프로히포크레틴리보핵산 (pre-prohypocretin RNA) 의감소소견이보고되었으며, 16,17 허탈발작이동반된기면병환자의대부분에서뇌척수액 hypocretin-1 이감소되어있다. 12,13,18 다만, 약 10% 의 narcolepsy with cataplexy 환자에서뇌척수액 hypocretin-1 이감소되어있지않아이들의병인은확실치않으나, 이들도 hypocretin 수용체나그아래단계의문제를가지고있을수있다. 한편, hypocretin 신경세포가파괴되는원인은아직정확히밝혀져있지않다. 대부분의환자에서인종과관계없이 HLA DQB1*0602 유전자를가지고있기때문에, 자가면역질환일것이라는학설이가장유력하지만, 아직혈청이나뇌척수액에서 hypocretin 과관련된어떠한자가항체 (autoantibody) 도발견된적이없다. 그러나, 발병초기에일시적으로짧게자가면역반응이일어났다가사라졌을가능성이나세포매개자가면역반응 (cell-mediated autoimmunity) 을배제할수는없다. Narcolepsy without cataplexy 현재까지의연구에서는전체기면병환자의 20-40% 가 narcolepsy without cataplexy 라고보고하고있지만, 5 narcolepsy without cataplexy 의유병률은아직도불확실하다. 지역사회백인들을대상으로시행된최근의한조사에의하면, 일반남성의 5.9% 와여성의 1.1% 에서 MSLT 에서평균수면잠복기가 8분이하이면서 SOREMP 이 2회이상관찰되었다. 이들남성들에서졸림증이나입면환각, 허탈발작유사증상등이많았지만, 모두전형적인허탈발작이없었다. 19 따라서, 이런사실을고려하면, narcolepsy without cataplexy 의유병률이실제보다훨씬평가절하되어있을가능성이있다. 진단에있어 narcolepsy with cataplexy 와의차이는전형적인허탈발작이없다는것이다. 또한, 뇌척수액 hypocretin-1 수치도대부분정상이다. 따라서, 반드시 MSLT 를통해서기면병소견을확인해야만진단이가능하다 (Table 2). Narcolepsy without cataplexy 의병인론 Narcolepsy without cataplexy 와 narcolepsy with cataplexy 가공통된병인을가진같은질병인지, 아니면두질환이별개의질병인지가핵심적인관심사이다. Narcolepsy without cataplexy 환자의소수에서는뇌척수액 hypocretin-1 이감소되어있어이들의경우 narcolepsy with cataplexy 의연장선상에서병인을설명할수있을지모르지만, 대부분의환자에서는 hypocretin-1 수치는정상이어서서로다른 12 수면

5 기면증의진단 기전에의할가능성이있다. 12,20 또한, 긴수면시간동반특발성과다수면 ( idiopathic hypersomnia without long sleep time ) 과의경계도불명확하여, 실제상당히이질적인환자들로구성된진단군일가능성이많다. Narcolepsy due to medical condition 정확한유병률은알수없으나, 현재까지 100여증례가문헌에보고될정도로드물다. 21 허탈발작이동반되는경우도있고그렇지않은경우도보고되었는데, 허탈발작이동반되는기면병의경우시상하부 (hypothalamus) 에병변이많았다. Narcolepsy due to medical condition 로진단하기위해서는우선거의매일반복되고최소한 3개월이상지속되는주간과다졸림증을호소하면서설명이가능한내과적또는신경학적질환이있어야한다. 또한, 전형적인허탈발작, MSLT 에서기면병에합당한소견 (8분이하의평균수면잠복기와 2회이상의 SOREMP), 또는뇌척수액에서 hypocretin-1 이 110pg/mL 미만이라는 3가지소견중하나이상이있어야진단이가능하다 (Table 3). If cataplexy is not present or is very atypical, polysomnographic monitoring performed over the patient s habitual sleep period followed by an MSLT must demonstrate a mean sleep latency on the MSLT of less than 8 min with two or more SOREMPs, despite sufficient nocturnal sleep prior to the test (minimum 6 h) 3. Hypocretin-1 levels in the CSF are less than 110 pg/ml (or 30% of normal control values), provided the patient is not comatose C. A significant underlying medical or neurological disorder accounts for the daytime sleepiness. D. The hypersomnia is not better explained by another sleep disorder, mental disorder, medication use, or substance use disorder. 결론만성적인주간과다졸림증은개인의일상및사회생활에큰영향을미치는중요한문제이다. 주간졸림증의원인은매우다양하며, 기면병은그중하나일뿐이다. 따라서, 기면병의진단에우선적으로중요한것은주간졸림증의다른원인들을감별하는것이다. 그감별을위해서병력의청취도중요하지만, MSLT 를위에설명한원칙에따라정확히시행하는것이중요하다는것을다시한번강조하면서종설을마친다.
