Pegylated interferon and ribavirin therapy (Peg-IFN/RBV) was recommended as the first-line treatment in the past decades, but this regimen showed unsatisfactory results in terms of safety and efficacy especially in elderly patients. Recently, it was demonstrated that dual therapy with daclatasvir and asunaprevir was well tolerated and led to high sustained virological response (SVR) rates, irrespective of age. We conducted a study to evaluate the efficacy and safety of daclatasvir plus asunaprevir by involving elderly patients aged above 65 years. Methods : We retrospectively analyzed clinical data from chronic hepatitis C virus (HCV) genotype 1b patients treated with daclatasvir plus asunaprevir from September 2015 to December 2016 at Seoul St. Mary s hospital. 2 JKSHP, VOL.35, NO.4 (2018) Results : A total of 112 patients were treated with daclatasvir plus asunaprevir for chronic hepatitis C. Among them, 101 patients completed the whole treatment, and in 88 patients the amount of HCV RNA was measured after 12 weeks of treatment. There was no significant difference in SVR at 12 weeks between both the groups (p=0.68). Typically, 91.4%(32/35) of elderly patients and 94.3%(50/53) of nonelderly patients achieved SVR12. Common adverse events included elevation in transaminase level, headache, and gastrointestinal disorders. There was no statistical difference in the symptoms between the two groups. Conclusions : The combination therapy with daclatasvir plus asunaprevir exhibited similar rates of SVR12 in HCV elderly patients without leading to further adverse events compared to non-elderly patients. Therefore, it is proposed that daclatasvir plus asunaprevir therapy could be considered as an effective and safe treatment, even in patients aged over 65 years. [Key words] Chronic hepatitis C, DAA (Direct Acting Antiviral Agents), Daclatasvir, Asunaprevir C형간염바이러스 (hepatitis C virus, HCV) 감염은잠복기와급성감염기를거치며자연적으로소실되기도하나, 약 50~80% 에서는만성화된다. 만성 C형간염은 6개월이상감염이지속된상태로, 만성화된이후에는자연회복이드물고지속적인간손상을유발하여간경변증과간세포암종을초래할수있다. 1),2) C형간염의치료목표는궁극적으로는바이러스의증식억제또는박멸을통해간경변증및간세포암종으로의진행과관련사망을예방하는것이고, 단기적으로는지속바이러스반응 (sustained virological response, SVR) 을달성하는것이다. 1) SVR은치료종료후12주또는24 주에혈액에서 HCV RNA가검출되지않는것으로정의하며, 1) SVR을달성한경우장기적으로는환자의생존율이향상되고간암발생이감소하는것으로알려져있다. 3),4) 만성 C형간염은전세계적으로약 1억 7천만명의감염자가있으며, 국내유병률은약 1% 내외로추산된다. 5),6) 특히, 국내 C형간염항체양성율은연령에따라증가하는특징을보이며, 7) 고령환자에서간섬유화가더욱빠르게진행되어간경변증의발생확률이높아지므로, 8) 조기발견및적극적인항바이러스치료가중요하다. 1),3) 수년전까지 C형간염의표준치료로 peginterferon과 ribavirin 병용요법 (PR요법) 이사용되 었으나, 치료성공률이낮고, 인터페론제제의부작용및 48 주간의장기간치료와주사제적용으로인해순응도가낮으며, 유전자형 (genotype) 에따라치료효과의차이가있어사용이제한적이었다. 1) 특히, 고령환자에서치료반응율은약 30~45% 로, 젊은연령층의반응율 55% 에비해낮은것으로알려져있다. 이는고령일수록간질환이진행된경우가더욱흔하며, 혈구감소증등의부작용발생의빈도가상대적으로높아중도탈락또는감량하거나, 다른만성질환의동반으로인한것으로추정된다. 9) 최근, 바이러스에직접작용하는경구용 C형간염치료제 (direct-acting antiviral agents, DAA) 가출시되며, 90% 이상의높은치료성공률과낮은부작용발생률, 짧은치료기간등의장점이입증되며널리사용되고있다. 이중, nonstructural protein (NS) 5A replication complex inhibitor인 daclatasvir ( 다클린자정, 이하 DCV) 와 NS3/4A 단백분해효소억제제 (protease inhibitor, PI) 로작용하는 asunaprevir ( 순베프라캡슐, 이하 ASV) 는국내에가장먼저도입된 DAA로, 병용시유전자형 1b인만성 C형간염환자에서유효성과안전성이입증되었다. 10)-12) 특히, PR 요법과는다르게고령환자에서도젊은연령에서와유사하게높은치료성공률과내약성을나타내었으나, 12)-14) 각

3 박유경 : 고령의만성 C 형간염환자에서 Daclatasvir 와 Asunaprevir 병용요법의유효성및안전성평가 연구에는적은수의고령환자만이포함되어있어추가적인연구가필요한실정이다. 이에, DCV/ASV 병용요법의처방현황을살펴보고, 고령환자에서의유효성및안전성을평가해보고자한다. 연구방법 1. 연구대상 이러스반응 (end-of-treatment response, ETR) 율과치료종료후 12주까지추적관찰이완료된환자를대상으로 12주 SVR (SVR12) 률을두군간비교하였다. 또한 RAS (NS5A-L31, NS5A-Y93) 음성인환자에서 SVR12률을조사하고 baseline HCV RNA value, 이전 PR요법에대한 HCV 치료경험, 신질환유무가각군의치료효과에영향을미치는지평가하였다. 2015년 9월부터 2016년 12월까지가톨릭대학교서울성모병원에서 DCV/ASV 병용요법을시작한유전자형 1b형만성 C형간염환자를대상으로하였다. 1b 형을제외한유전자형, 외국인, 19 세미만환자는분석대상에서제외하였다. 2. 연구방법분석대상을 65세이상고령환자군과 65세미만환자군으로분류하여치료효과및이상반응발생여부를비교및평가하였다. DCV/ASV가처방된환자의전자의무기록 (Electronic medical record, EMR) 을후향적으로검토하여, 각항목별로두환자군간유의한차이가있는지분석하였다. 1) 자료수집항목 3) 안전성평가기준안전성평가를위하여 DCV/ASV 복용후 EMR상기록된이상반응을확인하였다. 약물이상반응의종류및빈도를분석하고, 일시투여중지혹은영구중단과관련있는이상반응의종류를조사하였다. 3. 통계분석연구대상자의기본특성은평균 ± 표준편차및비율로나타내었고, 두군간비교는연속형항목은독립표본 t-test, 범주형항목은 Chi-square test 또는 fisher s exact test를이용하여비교하였다. 통계분석은 SAS 9.4 Version (SAS Institute Inc. Cary, NC) 을이용하였으며, p value가 0.05 미만인경우통계적으로유의한차이가있는것으로평가하였다. DCV/ASV의초회처방일자를기준으로연령, 성별, HCV genotype, baseline HCV RNA value, NS5A 약제내성 (resistance-associated substitution, RAS) 검사결과, 이전 HCV 치료여부, aspartate transaminase (AST), alanine transaminase (ALT), total bilirubin, 신질환유무, serum creatinine (SCr) 및 blood urea nitrogen (BUN) 수치를확인하였다. 4. 피험자보호본연구는가톨릭대학교서울성모병원에서수행된단일기관후향적연구로, 본기관의기관윤리심의위원회 (Institutional Review Board, IRB) 의승인을받아진행하였다 ( 과제번호 : KC17RESI0565). 연구결과 2) 유효성평가기준 1. 대상환자의기본특성 HCV RNA 농도가정량한계 25 IU/mL 미만인경우를반응있음으로정의하고, 투여 24 주시점의혈중 HCV RNA가검출되지않은상태인치료종료바 2015년 9월부터 2016년 12월까지가톨릭대학교서울성모병원에서유전자형 1b형만성 C형간염으로 DCV 60 mg(1일 1회 ) 과 ASV 100 mg(1일 2회 ) 병용

4 JKSHP, VOL.35, NO.4 (2018) Fig. 1 Flow chart of study population selection 치료를시작한환자는 112명이었다. 이중, 투여초기에투여가중지된 11명을제외한 101명을대상으로 65 세이상고령환자군 (n=38) 과 65 세미만환자군 (n=63) 으로분류하여유효성을평가하였고, 안전성은전체환자를대상으로조사하였다 (Fig. 1). 대상환자의기본특성은 Table 1과같다. 65 세이상고령환자군의평균나이는 74.11±5.69세, 65 세미만군의평균나이는 51.43±10.60세였다. 성별, baseline HCV RNA value, RAS 검사, PR요법치료경험유무, ALT, 신질환유무, SCr, BUN에서두군간의유의한차이는없었다. AST의경우 65 세이상환자군에서유의하게높았다 (p=0.018). 2. 유효성평가 24주간 DCV/ASV 치료를완료한환자는 65세이상고령환자에서 84.4%(38/45), 65세미만에서 94.0% (63/67) 이었다. 기저질환을우선적으로치료하거나, 외래방문을중단하는등의사유로투여가중단되거나확인이불가한환자가 11명이었다. ETR율은고령환자군 92.1%(35/38), 65세미만환자군 98.4%(62/63) 로두군간유의한차이가없었으며 (Fig. 2A), SVR12률 에서도각군에서 91.4%(32/35), 94.3%(50/53) 로유의한차이를나타내지않았다 (Fig. 2B). 치료전 NS5A RAS가검출되지않은 L31(-)/Y93(-) 대상자에서 SVR12률은 65세이상환자군에서 93.9% (31/33), 65세미만군에서 95.9%(47/49) 로두군간차이가없었다 (p=1.00). 또한, 치료전 HCV RNA 농도및 PR 요법치료경험, 신질환유무등은각군의치료효과에영향을미치지않았다 (Table 2). 3. 안전성평가 DCV/ASV를 1회이상복용한전체대상자에서이상반응이발생한경우는 65세이상고령환자군에서 22.2%(10/45), 65세미만군에서 47.8%(32/67) 로 65 세미만군에서유의하게높았다 (p=0.006)(table 3). 주요이상반응으로는간효소수치상승, 두통, 위장장애등이었으며, 각증상에따른두군간의유의한차이는없었다. 이상반응으로인해투여가중지된경우는 65세미만군에서 4명으로, 간효소수치상승 3명, 두드러기 1명이었다. 이중 2명대상자는휴약후재시도하여투여를완료하였다. 고령환자에서는이상반응으로인해치료가중지된경우는없었다

8 JKSHP, VOL.35, NO.4 (2018) 군간유의한차이가없었으며, 흔한이상반응은간효소수치상승, 두통, 위장장애등으로국내에서진행된한후향적연구와유사한결과였다. 20) 이상반응으로인해투여를중지한대상자는 4명 (3.6%) 으로, 간효소수치상승 3명, 피부발진 1명이었다. 이중, DCV/ASV 투여로인해정상상한치의 5배를초과한 3/4등급의 AST 상승은 1명 (0.9%), ALT 상승은 3명 (2.7%) 에서발생하였으나, 2명에서는회복이후투여가재개되어 SVR을달성하였다. 일본인을대상으로한연구에서도 3/4등급의 AST 상승은 5.4%, ALT 상승은 7.2% 로보고된바있다. 21) DCV/ASV 투여시에는 ASV에의한잠재적인간독성의가능성으로인해 AST 또는 ALT 상승이특징적으로나타나므로간기능검사와모니터링이권장된다. 18),22) 이는투여중지의주된사유이나, 10),11) 투약중단시간효소수치가개선되는가역적반응으로알려져있다. 21),23) 본연구는단일기관환자의 EMR 기록을후향적으로검토하여분석한연구로서, 대상자수부족, 기록미비, 치료종료후환자방문누락등으로분석에제한이있었다. 또한, DAA는다양한약제와약물상호작용을유발할수있으므로치료전사용하고있는모든약제에대해상호작용확인이필요하나, 17) 본연구에서는약물상호작용을고려하지않았다. 추가로연구진행도중국내시장에도입된다른 DAA와의처방현황및직접적인효과비교, 경제성평가등은이루어지지않아한계점으로남아있다. 최근개정된만성 C형간염치료가이드라인에따르면, 유전자형 1b인만성 C형간염치료에서 DCV/ASV 이후시판된 ledipasvir/sofosbuvir( 하보니정 ), elbasvir/grazoprevir( 제파티어정 ), ombitasvir/ paritaprevir/ritonavir와 dasabuvir( 비키라정 / 엑스비라정 ), glecaprevir/ pibrentasvir( 마비렛정 ) 등의 12주치료는 DCV/ASV 24주요법보다권고등급이강하다 (A1). 18),19) DCV/ASV은다른약제들과동일하게높은근거수준을보이지만, 권고등급은약하다 (A2). 이는투여전필수적인 RAS 검사로인해사용이제한적이고, 상대적으로치료율이낮으며, 치료기간이길기때문으로사료된다. 그러나, DCV/ASV는국내시장에가장먼저출시된 DAA로한국인을대상으로한실제임상현장의연구 결과가많고, 최근한메타분석연구에서는신장애환자와고령자등에서높은 SVR률을보였다고보고하였으며, 특수상황등추가분석데이터가확보되고있다. 24) 본연구도 DCV/ASV 요법에서실제환자의현황을파악하고, 유효성및안전성을직접비교한연구로, 해당요법을고령환자에적용시유용한참고가될수있을것으로사료된다. 결론 DCV/ASV가투여된유전자형 1b형만성 C형간염환자를 65세이상고령환자군과 65세미만군으로나누어치료효과및안전성을비교평가한결과, SVR12 및이상반응의종류와발현율에서두군간유의한차이를보이지않았다. Baseline HCV RNA value, 이전 PR 치료경험, 신질환유무등은효과에영향을미치지않았으나, 치료전 NS5A RAS 음성인경우 SVR12 률이높았다. 따라서, DCV/ASV는 RAS 음성인환자에서연령에관계없이고령환자에서도높은반응율과내약성을기대할수있겠다. 감사의말씀본연구의통계분석은보건복지부의재원으로한국보건산업진흥원의보건의료기술연구개발사업지원에의하여이루어진것임 ( 과제고유번호 : HI14C1062). 참고문헌 1) C형간염진료가이드라인, 2015 대한간학회 2) Chen SL, Morgan TR.
