다발성 요추간반 탈출증에서의 컴퓨터 적외선 전신 체열 촬영의 진단적 가치 인 저온 영역을 나타내었는데 이는 김영수 등의 radiculopathy 84.2%(16명)가 양측성 하지 방사통을 호소하여 DITI상의 type과 동일한 양상을 나타내었다. 일측성 다발성 신경근 검사 소견과 환자가 호소하는 부위가 일치하는 경우가 80.

6 다발성 요추간반 탈출증에서의 컴퓨터 적외선 전신 체열 촬영의 진단적 가치 인 저온 영역을 나타내었는데 이는 김영수 등의 radiculopathy 84.2%(16명)가 양측성 하지 방사통을 호소하여 DITI상의 type과 동일한 양상을 나타내었다. 일측성 다발성 신경근 검사 소견과 환자가 호소하는 부위가 일치하는 경우가 80.2% 병변은 2개 이상의 체온절에서 저온 영역을 나타낸 양상을 를 나타내었다. 요통을 포함한 임상 증상과의 DITI검사의 보였으며, 양측성 하부 요추부 병변은 양하지에 신경근 압 예민도는 88.6%로 매우 높았다. 박으로 체온절을 따라 저온을 나타내었다. 비특이성의 체온 수술을 시행한 139명의 일측성 단일 신경근 병변 양상 환 양상을 보인 경우도 11.4%가 있었는데 이는 김영수 등의 자중 134명의 환자에서는 방사선학적 소견과 일치하고 추 10.5%와 비슷하였다. 간반 조영술에서 이를 확인할 수 있어(Fig. 1) 단일 신경근 일측성 단일 신경근 병변을 나타낸 환자 155명에서 임상 감압을 하여 수술후 임상 증상이 호전됨을 확인하였으며, 증상과 비교하면 92.7%(142명)의 환자에서 병변 체온절과 화학적 수핵 용해술을 시행한 경우는 용해술 후의 증상 호 일치하는 부위에 일측 하지방사통을 호소하였으며, 105명 전으로 이를 확인할 수 있었다(Fig. 2, 3). 3명의 Failed (69.7%)의 환자에서 요통을 호소하였다. 일측성 다발성 병 Back Surgery Syndrome 환자와 급성 추간반 탈출증과 변을 나타낸 5명의 환자중 4명이 일측성 하지 통증을, 또 동반된 퇴행성 전방 전위증 환자 1명, 퇴행성 척추증에 의 한 양측성 하부 요추부 병변 양상을 보인 19명의 환자 중 한 척추관 협착증이 동반된 환자 1명에서는 병변부위의 신 Fig. 1. D1T1 revealed severe hypothermic on right anterior thigh same as pain drawing(right). 경근을 모두 감압하였다. 이와 같이 방사선학적 검사상 다 은 유발률을 나타내어 DITI의 임상 증상과의 전체적인 일 발성 추간반 병변이 있다 하더라도 환자의 증상이 일측성이 치율과 비슷하였다. 그러나 퇴행성 척추증과 동반된 협착증, 며 DITI 상에서 단일 신경근 병변이며 환자의 증상 부위에 Failed Back Surgery Syndrome 등과 추간반 병변이 같이 저체온 영역(hypothermic area)이 있을 경우에는 그 부위 있을 경우에는 진단적인 가치가 감소하였다. 177명의 수술 의 신경근을 감압해 줌으로서 임상 증상이 호전되었다. 또 환자중 DITI 소견과 추간반 조영술에서의 증상 유발 유무 한 일측성 다발성 신경근 병변 양상과 양측성 하부 요추부 와의 일치도를 분석하였는데 양 검사간의 연관성은 81.4% 신경근 병변 양상을 나타내어 수술 치료를 받았던 19명의 이었다. 환자에서도 DITI 소견과 일치하는 신경근 압박이 있음을 이와 같이 DITI 검사는 인체에 전혀 해를 주지 않고 환 확인하여 전체 수술 환자에서 DITI 소견과 수술 소견과의 자에게 통증을 주지 않는 비침습적인 검사로서 추간반 조영 일치도는 86.4%이었다. 그러므로 DITI 검사가 다발성 추 술과 진단상의 연관성이 높고, 임상 증상 및 수술 소견과도 간반 탈출증 환자의 증상 부위를 진단하고 수술 범위를 결 높은 일치도를 보여 다발성 추간반 병변에서 증상 부위 진 정하는데 매우 유용하였다. 그러나 다른 부위에 퇴행성 척 단에 유용한 검사로 사료되었다. 추 전방 전위증이 동반되어 있거나 척추관 협착증으로 인한 만성적인 증상이 있는 상태에서 추간반 탈출증이 일측성으 결 론 로 악화되면 이와 같이 DITI 상에서 일측성 단일 신경근 병변을 나타낼 수 있으므로 수술 결정시 방사선학적 소견과 종합하여 판정하여야 할 것으로 사료되었다. 추간반 조영술과 DITI와의 관계를 보면 추간반 조영술이 다발성 요추간반 병변에서 증상부위 진단에 DITI검사 및 추간반 조영술을 이용하여 각각의 임상적 효용성을 연구 분 석하여 아래와 같은 결론을 얻었다. 177예의 수술 환자에서 시행되어 135명에서 평소 증상을 1) 컴퓨터 적외선 전신 체열 촬영(DITI) 검사는 다발성 유발시킴으로서 76.3%의 임상적인 가치가 있었는데, 그중 추간반 병변의 환자에서 임상 증상과의 예민도는 88.6%이 재발성 추간반 탈출증에서는 90%의 높은 유발률을 나타내 었으며, 수술 소견과의 정확도는 86.4%이었다. 어 수술후 재발성 추간반 탈출증과 수술후 섬유 유착을 감 2) 추간반 조영술의 다발성 추간반 병변에서 임상적 효용 별하는 것에 특히 유용할 것으로 사료되었다. 또한 단순 다 성은 76.3%이었으며, 특히 재발성 추간반 병변 진단에 더 발성 추간반 병변이 있었던 환자에서도 83.1%의 비교적 높 욱 임상적 가치가 있었다.
