This paper is the study for the correlation between thermal differences and severity in EMG in the carpal tunnel syndrome. Materials and Methods: We analyzed 6 patients, 12 hands of preoperative electrophysiological study, preoperative thermography, postoperative clinical results, postoperative thermography. Results: We divided mild, moderate, severe lesion group under the guidance of electrophysiological study. Preoperative thermography showed the ΔT is +0.7 in mild group, in moderate group, in severe group with hyperthermic typre and in severe group with hypothermic pattern. The more severe in the electrophysiological grade, the more hyperthermic in the preoperative thermography except hypothermic pattern within severe grade. Conclusion: Preoperative thermography is proportionate with electrophysiologic grade. We can make a estimate of the severity of carpal tunnel syndrome by the preoperative thermography. Postoperative thermography taken 3 to 6 months after surgery demonstrates the converging stage to normal thermal distribution state. CTS는일반인구의약 1% 에서발생하는것으로알려져있다. 2,5) 수근관증후군의진단은환자의병력과이학적검사에기초하여쉽게진단의인상을받으며, 확진은전기생리적검사로한다. 보존적치료가일부의환자에게서가능하지만, 대부분수술이확실한치료법이다. 수술결과의평가는수근관증후군의중증정도를고려하여야하지만, 현재까지널리알려진중증도의평가기준은없다. 본논문은수술전, 후의체열촬영과수술전근전도검사를토대로하여수근관증후군의중증도를평가하는방법과그에따른수술후결과에대하여연구하였다. 2 Ho-Yeol Zhang: Carpal Tunnel Syndrome: Correlation between Thermal Differences and Severity in EMG Study 대상및방법 2005년부터 2007년까지수술한 6명의환자 12개의손목을대상으로하였다. 이들모두는수술전근전도검사를시행하였고, 체열촬영은 4명에서시행하였다. 수술후체열촬영은 6명모두수술후 3개월이나 6개월째에촬영하였고, 수술후근전도검사는시행하지않았다. 수술전체열촬영에서온도를측정하였고, 이와근전도검사간의상관관계를분석하였고, 수술후체열촬영과수술후환자의결과와비교분석하였다. 었다. 이들모두는전기생리적검사를하였으며, 전기생리적검사의결과 mild, moderate 혹은 severe median nerve lesion around wrist로 3가지로분류한것과온도차를구분하였다 (Table 1). 근전도검사상병변이 mild인경우는 1쪽의손목으로온도차가 +0.7 로전박부위에비하여손바닥의온도가 1. 체열측정부위수근관증후군의경우는양측에발생하는경우가대부분이기때문에양손간의온도차는의미가없기때문에증상이있는손바닥의중앙부위의온도에서증상이없는전완부의중앙부위의온도를뺀온도를 data로사용하였다 (Fig. 1). 즉, ΔT = Tpalm - Tantecubital로하였다. 이를이용하여수술전과수술후의손의온도변화를측정하였다. 2. 전기생리적검사수술전근전도및신경전도속도검사는본원재활의학과에서시행하였으며, 이신경전도속도의느린정도에따라서 mild, moderate, severe compressive lesion으로분류하였다. 결 과 1. 수술전팔과손의온도차와근전도검사와의관계 수술전체열촬영을한경우는모두 4 명, 8 개의손목이 Fig. 1. 장호열 : 수근관증후군 : 온도차와근전도검사상의중증정도와의비교 고온이었고, 병변의정도가 moderate인경우는 2쪽으로평균 +0.87 로더고온을보였으며, severe lesion의경우에는체열촬영이두가지패턴으로나뉘어서, 고온의경우는 3쪽으로평균 +1.87 로가장높은고온을보였으며, 2 쪽의손에서는오히려평균 -1.68 의심한저온을보였다. 이들의예는 Fig.

3 장호열 : 수근관증후군 : 온도차와근전도검사상의중증정도와의비교 고온이었고, 병변의정도가 moderate인경우는 2쪽으로평균 로더고온을보였으며, severe lesion의경우에는체열촬영이두가지패턴으로나뉘어서, 고온의경우는 3쪽으로평균 로가장높은고온을보였으며, 2 쪽의손에서는오히려평균 의심한저온을보였다. 이들의예는 Fig. 2와같다. 즉, 근전도상 median nerve 의병변이심해질수록고온을보였고, 중증일경우일부는저온으로바뀌었다. 2. 수술후임상결과와체열촬영의변화수술후일상결과는 6명, 12쪽의손의증상은모두소실되었고, 환자모두에서만족함을표시하였다. 체열촬영은수술후 3개월에서 6개월사이에측정하였으며, 수술한 6명의 12쪽의손목모두를촬영하였다. 수술후전박과손바닥의온도와의평균은 였다. 이들을다시수술전의 mild, moderate, severe인병변별로구분을하여살펴보면아래와같았다 (Table 2). 수술전 mild lesion의경우는수술후약 1 가더고온으로변하였고, moderate lesion의경우는수술후약 0.5 더고온으로변한반면, severe lesion이면서 hyperthermic pattern을보인경우는수술후고온을보이지만, 그정도는수술전에비하여오히려 0.8 정도가낮아졌다. 또한 severe lesion이면서저온이었던경우는수술후고온으로변하였으며, 온도차는 2.5 이상높아졌다 (Fig. 3). 5 장호열 : 수근관증후군 : 온도차와근전도검사상의중증정도와의비교 은전기생리검사로는알수없는교감신경계의이상을측정하는것이며, 수술전, 후의변화를측정할수있다. 수근관증후군환자의손바닥의온도분포가정상인보다고온인지저온인지알아보았다. 장등에의하면 control ( 대조표준 ) 100명의상지와상반신을 108구획으로구분하여각구획의온도평균과좌우의온도차를구하였다. 8) 이에의하면전박부의중앙의평균온도는 31.36±1.41 였고, 손바닥중앙의평균온도는 31.81±1.48 였다. 이에의하면대조표준의경우는전박부중앙의온도보다손바닥중앙의온도가 0.45 높아서대조군의 ΔT = 였다. 본연구에서수술전 ΔT는 mild, moderate, severe 군으로변하면서 +0.70, +0.80, 를나타내어대조표준보다는 ΔT가고온을보였으며, 전기생리적검사상 grade 가높을수록더고온을보였고, severe grade의일부 hypothermic 군에서나타난 은대조표준보다매우낮은것이었다. 즉, 수근관증후군환자의 ΔT는대조표준보다높으며, 전기생리적 grade가높을수록온도차가더높아지며, severe group의경우는대조표준보다심한저온을보이는 group도나타날수있다. Carpal tunnel release 수술후온도변화는수술후 3개월에서 6개월에측정한것이었으나, 수술전 mild, moderate, severe lesion의경우에서 ΔΔT가 +1.07, 로증가하여대조표준보다더증가하는양상을보였으며, 수술전 severe lesion군중에서고온의군은 로변하여정상과가까운범위로하강하였으며, 수술전 severe lesion 군중에서저온의군은 ΔΔT는 로변하여결국 ΔT 는 로되어대조표준군보다약간의고온으로변하였다. 수술후환자의임상증상은바로소실되지만, 손바닥의온도는 mild와 moderate lesion군에서는모두에서더증가하였고, severe lesion군에서는 hyperthermic type과 hypothermic type 모두에서정상과가까운쪽으로온도변화가있었다. 즉, severe lesion의경우가오히려빠른시일내에정중신경의기능이정상화되고, 이와함께손바닥부위에분포하는교감신경의기능이 mild나 moderate lesion 군보다빨리정상화되는것을알수있었다. 결 론 의정도는비례한다. 전기생리적검사상 Severe lesion의경우는손바닥의심한고온이나저온을나타낸다. 수술후 3-6개월후온도변화는 mild lesion일경우는수술후고온화가가중되며, severe lesion 일수록고온화가약화되며, 대조표준과가까워진다. 수술전체열촬영으로전완부중앙과손바닥의온도차로정중신경압박의정도를예측할수있다. 수술후체열촬영은손바닥의혈액순환이정상화되는정도를측정할수있다. 참고문헌 1.
