This study evaluated the antibacterial activities of some essential oils (tee tree, rosemary, and lavender oils) against 18 clinical isolates of multidrug-resistant A. baumannii (MRAB). The carbapenemase screening Hodge test showed that all 20 strains of A. baumannii were resistant to imipenem. The identification of multidrug-resistant microbes was carried out using the VITEK system. The antimicrobial activity of essential oils was tested by a disk diffusion method against MRAB. In the disk diffusion method, tea tree showed the largest increase in inhibition size compared to lavender oil, and rosemary had no antibacterial effect. These results proved the antimicrobial effect of multidrug resistance A. baumannii. Tee tree oil would be a useful alternative natural product for the treatment and prevention of most common human pathogens and MRAB infections. This is expected to be used as an antimicrobial agent, such as hand disinfectant using natural essential oil in the future. Received: September 21, 2018 Revised: October 19, 2018 Accepted: October 26, 2018 서론에센셜오일 (essential oil) 은식물의 2차대사산물중인체에약리효과를나타내는성분으로이루어진휘발성유기화합물로, 주로식물의내분비선에서분비되며식물체내에서생합성되어생성된다. 이들은식물종이나부위에따라여러가지향기나특성이다르고수증기증류법, 용매추출법, 압착법등에의해추출한다. 오일은수소, 탄소, 산소등이화학물질이기본구조이며테르펜류, 에스테르, 알데하이드, 옥사이드, 케톤, 알콜등의다양한성분과함량이존재한다 [1]. 로즈마리 (rosemary) 는 Rosemarinus officinalis 학명으로유럽에서오랜세월동안기억을증진시키기위한상징처럼여겨지며 [2], 로즈마리입은항인간면연결핍바이러스 [3], 항염증 [4], 항종양 [5] 과항산화 [6] 를포함하여다양한생리활성인자에관여하는것으로알려져있다. 티트리 (tee tree) 는 Melaeuca alternifolia 학명으로감염된상처부위에탁월한효과가있으며, 항균효과가있다고알려져있다 [7]. 라벤더 (lavender) 는 Lavandula angustifolia 학명으로여러가지용도로사용되는것으로스트레스, 불면, 우울, 항산화에효과가있다고알려져있다 [8]. 최근천연물질의자연치

Antimicrobial Effects of Multidrug-Resistant Acinetobacter baumanii 유능력에대한관심도가높아짐에따라식물에서추출한에센셜오일의효과에대한연구가활발히진행되고있다 [9]. 따라서이들의항균력을평가하고자하였다. 최근 Acinetobacter baumanii는폐렴, 폐혈증, 심내막염, 요로감염, 창상감염, 수막염등다양한유형의감염증등주로면역능이저하된환자에감염증을유발하는기회감염균으로중환자실및병원환경에서흔히분리된다 [10]. Acinetobacter spp. 에의한감염으로인하여발생하는원내감염이병원내에서발생하는전체원내감염중 10-30% 의비율을차지하고있어세계적으로문제가되고있다 [11]. 또한광범위베타락탐아제 (extended-spectrum -lactamase, ESBL), AmpC -lactamase 등의효소를생성하는다제내성 A. baumannii균 (multidrug-resistant A. baumannii, MRAB) 의증가로인해카바페넴 (carbapenem) 의사용량이확대되어서결과적으로이약제에대한내성균주가증가되기시작하였다. Acinetobacter spp. 의 colistin에대한내성률은 2002년에서 2006년까지분리된 Acinetobacter spp. 중에서는 18.2% 가 colistin에내성을보였다 [12]. 이미페넴 (imipenem) 내성 A. baumannii에의한균혈증의사망률은 57.5% 로 imipenem 감수성 A. baumannii에의한균혈증의 20% 에비하여높으며, 이는균혈증초기에부적절한항생제치료로인한결과일것으로설명하고있다 [13]. 국내분리세균의내성률조사에의하면 A. baumannii의 carbapenem 내성률은 2001년 6% 에서 2004년에 17% 로증가하였고, 2007년 27% 로현저한증가세를보였으며, 2011년 국내종합병원을대상으로항생제내성모니터링 에서는 63% 의내성률을보였다 [14]. 2012년, 질병관리본부다제내성균감염관리지침에의하면 MRAB 는 carbapenem 계, aminoglycoside 계, fluoroquinolones 계항균제에모두내성을나타내는 A. baumannii 균으로감염병의예방및관리에관한법률에지정감염병으로포함되어있는다제내성균이다 [15]. 따라서, 본연구의목적은임상에서분리된다제내성 A. baumannii을분리하여일반적으로시판되는천연에센셜오일인라벤더 (lavender), 티트리 (tea tree), 로즈마리 (rosemary) 3 종류의항균효과를평가하여천연에센셜오일이내성에대한강한저항성을보고하고이러한장점을활용하여원내감염균으로알려진세균들에대해예방제로선택될수있는가능성을찾고자한다. 재료및방법 1. 대상균주본연구는 2013년 12월부터 2014년 2월까지충남지역일개대학병원의임상에서분리된 A. baumannii 균주 20균주를사용하였고 S1부터 S20으로순서대로명명하였고, 이중 18개의 MRAB균과다제내성이아닌 2개의 A. baumannii 균주를대상으로하였다. 이중동일환자에서반복분리된균주는수집대상에서제외하였다. 2. 에센셜오일일반적으로시중에시판되는 D사제품중티트리 (tee tree), 로즈마리 (rosemary), 라벤더 (lavender) 3개의제품을사용하여평가에이용하였다. 판매업체에서제공받은물질안전자료에따르면모두수증기증류법으로추출된오일이며, 공급회사의물질안전자료에따라실험을하였다. 에센셜오일의원산지와학명은 Table 1과같다. 3. 균주동정및항균제감수성시험임상검체로부터분리배양된균주는 Vitek 2 R automated instrument IDs ystem (biomérieux, Marcyl Etoile, France) 을이용하여세균동정과항균제최소억제농도를확인하였다. 4. Carbapenemase 선별시험법 (Hodge test) Carbapenemase 생성균주를 Hodge 변법으로선별하였다. 시험에사용된표준균주는 E. coli ATCC 25922로균주부유액탁도를 McFarland No 0.5로맞춘후 MacConkey 한천에면봉으로고루접종한다. 접종한균주가마른뒤배지의중앙에 imipenem 디스크 (10 g, BBL, Cockeysville, MI, USA) 를놓은후, A. baumanii를디스크로부터평판의가장자리로한줄로굵게접종하였다. 이후 37 C 산소성배양기에 18시간배양후시험균주접종선중앙쪽말단부가다른부위에비해서더넓게증식되면양성으로판정하였고, 이는 carbapenem 분해효소가있음을뜻한다. Vol. 50, No. 4, December 에센셜오일의디스크확산법 CLSI 가이드라인 M07-A10 [16] 에따라대상균주를 Mueller-Hinton agar에직접제조하여계대한후 37 C 배양기 (Vision scientific, Daejeon, Korea) 에서 시간동안배양한후집락을취하여 Mueller-Hinton broth 2 ml에현탁시킨다음 CFU/mL가되는 0.5 MacFarland 표준값을 densicheck 탁도계 (biomerieux, Marcy L Etoile, France) 로측정한후 Mueller-Hinton agar 에멸균된면봉으로고루분주하였다. 각각의허브오일 3종류를멸균된 6 mm 종이디스크를멸균된 petridish에세개씩담은다음일반적으로시판되는 D사의에센셜오일을각 10 L씩분주한뒤 15분간흡수시킨후또 10 L씩분주한후 1시간을건조시킨후 Mueller-Hinton agar에멸균된핀셋으로가볍게올려놓은후살짝눌러준후배지뚜껑을밑으로하여산소성배양기에서 35 C로 18 24시간배양을하였다. 이때 negative control 로 mineral oil를사용하였다. 배양후디스크주위의억제대직경 (mm) 을측정하였다. 이와같은실험은 4회반복시행하여평균을내었다. 결과 리지침에의해다제내성에부합하는균은 gentamicin에감수성을보인 1번과 3번균주를제외한 18개균주가다제내성균으로판정되었다 (Table 3). 3. Carbapenemase 선별시험법 Imipenem-Hodge 변법시험을시행하여 imipenem 내성이 -lactamase 생성에의한것인지검사하였고, 그결과는 20 개균모두양성을보여 carbapenem 계에내성이있는것으로확인되었다 (Figure 1). 4. 에센셜오일의항균효과티트리 (tee tree) 의항균효과는검체중 S18번검체가 23 mm 의억제대를보여가장효과가좋았으며, 평균값은 19.6 mm로나타났다. 라벤더 (lavender) 에서는 S17번에서 18.5 mm의억제대를보여가장효과가좋았으며, 평균값은 10.9 mm로평균값이나왔지만, tee tree 보다는항균효과낮은것으로나타났다. 로즈마리 (rosemary) 는항균효과가아주미미한것으로보이며 S1과 S17번이 8.5 mm 억제대를보여작은억제대범위를보였고그다음이 S3 번균이 8 mm로나왔고, 나머지균들은거의항균효과가없는것으로나타났다 (Figure 2). 천연에센셜오 1. 검체별특성임상에서검체의종류는 sputum 7건, urine 4건, urine culture 3건, open pus 3건, BAL (bronchoalveolar lavage) 2 건, blood 1건의순서로분리되었다 (Table 2). 2. 항균제감수성검사항균제감수성에서 carbapenem 계의항균제인 imipenem, meropenem 에서 20개균주모두내성을확인하였고, colistin 은모두감수성을확인하였다. Cephalosporin 계는 3세대항균제인 cefotaxime, ceftazidime 와 4세대 cefepime, monobactam 계인 aztreonam, ampicillin/sulbactam (AMP/S), trimethoprim/ sulfamethoxazole (SXT), penicillin계는 3세대인 piperacillin 모두내성을나타냈다. 반면, aminoglycoside계의 gentamicin은 carbapenem 내성을보인 20개균중 18개균은내성, 2개균은감수성을보였고, tetracycline 계인 minocycline 은감수성이 11균, 8균이중간내성인반면, tigecycline은 100% 감수성을보였다. fluoroquinolone 계인 ciprofloxacin 은모두내성이지만, 새로운 quinolone계항생제인 levofloxacin 은내성이 19건, 중간내성이 1건이었다. 이결과로질병관리본부다제내성균감염관 Table 2. CLSI 가이드라인 M100[17] 정도관리판정기준을참조하면 ciprofloxacin, imipenem이 21 mm, 22 mm 이상이면감수성으로판정하는것으로티트리의평균값 19.6 mm는항균효과가비슷한것으로추정된다. 고찰항균성이알려진천연물질로는주로 lysozyme, polylysine, protamine, conalbumun, avidin, 유기산, polyphenol 물질등이대표적이다 [18-22]. 고, Pseudomonas 균속에 대해서 lavender oil과 thyme oil 모 두 좋지 않은 결과로 나왔다. 이는 우리의 연구에서도 대상균주 는 다르지만, tee tree에 비해 lavender가 항균효과가 떨어지는 것이 비슷하였다.

(A) NC, negative control (mineral oil), (B) T (tee tree); L (lavender); R (rosemary). 고, Pseudomonas 균속에 대해서 lavender oil과 thyme oil 모 두 좋지 않은 결과로 나왔다. 이는 우리의 연구에서도 대상균주 는 다르지만, tee tree에 비해 lavender가 항균효과가 떨어지는 것이 비슷하였다. 그 이유로는 허브 에센셜오일에 포함된 항균 효과를 나타내는 terpinene과 같은 항균성분을 가진 2차 대사 산물의 함량이 다르기 때문일 것으로 추정된다. 본 연구에서 S17균의 경우 lavender 오일에서 다른 균에 비해 항균효과가 뛰어난 것으로 조사되었는데 이에 대해서는 성분분석과 다양한 원인분석이 필요할 것으로 사료된다. Antibacterial inhibition zone of essential oils against A. baumanii by disk diffusion method. 2014년 Mohaddese 등[25]은 임상에서 분리된 A. baumannii 균 32종을 대상으로 Oliveria decumbens, Pelargonium graveolens, Eugenia caryophyllata, Ziziphora alternifolia로서 100여종의 다른 물질들로 구성되어 있는 복합 tenuir, Trachyspermum copticum oils의 항균력을 평가하였 물이며, 항미생물작용의 terpinene-4-ol이 주성분이다[23]. 는데, O. decumbens와 E. caryophyllata, 그리고 T. copticum 그리고 Shin 등[23]의 연구에 의하면 tee tree의 주요성분으로 oils이 가장 효과적인 것으로 조사되었는데 그것은 thymol과 는 4-terpineol이 35.22%, -terpinene이 10.73%, - eugenol 성분이 함유된 페놀화합물 때문에 효과적인 항균력을 terpinene이 19.63%로 분석되었으나, 본 연구에 사용된 에센 발휘한 것으로 보고되었다. 위의 연구의 디스크 확산법에서 O. 셜오일은 판매처에서 제공받은 분석결과에 의하면 4-terpineol decumbens과 E. caryophyllata, 그리고 T. copticum oils의 은 라이브러리에 제외되었지만 -terpinene이 28.1% - 항균제 억제대가 mm, 13.3 mm, 18 mm로 보고되어, 본 terpinene이 26.57%로 테르펜 성분이 가장 높은 것으로 분석 연구의 tee tree oil의 결과인, 평균 19.6 mm 억제대 범위와 비 되었다. 슷한 항균력을 보여 결과적으로 tee tree oil은 O. decumbens Aminoglycoside계 항균제에 감수성으로 나온 S1과 S3균 와 비슷한 항균력을 발휘하는 것으로 추정된다. 에센셜 오일의 이 다제내성균이 아니지만, MRAB와 비교하여 보면 비슷한 항균 활성은 2004년 Nguefack 등[26]에 의하면 그람 음성균의 항균효과가 보여 다제내성과의 에센셜오일의 항균력과는 연 세포막의 침투성 증가와 ATP 감소로 lipopolysacharide (LPS) 관성이 없는 것으로 분석되었다. 2011년 Sienkiewicz 등[24] 방출로 사멸하거나 2009년 Bouhdid 등[27]은 칼륨의 유출로 은 임상검체에서 분리된 Staphylococcus, Enterococcus, 세포손상이 오고 세포벽의 구조와 성분의 변화가 사멸을 유도 Escherichia, Pseudomonas 균속을 대상으로 lavender oil 한다고 알려져 있으나 앞으로 좀 더 많은 에센셜오일의 살균 메 과 thyme oil의 항균활성을 비교하였는데 thyme oil은 커니즘에 관한 연구가 필요하다고 사료된다. Staphylococcus, Enterococcus, Escherichia에 대한 항균효 시료의 추출방법에서도 시판되는 에센셜오일은 수증기 증류 과가 좋았지만, lavender oil은 항균효과가 미비함을 보고하였 법으로 추출하였지만, 천연물로부터 정유를 추출하는 방법 중

Antimicrobial Effects of Multidrug-Resistant Acinetobacter baumanii 에성분파괴없이고순도정유추출법인초임계추출법으로사용하였다면, 더좋은항균효과가있을거라예상되고, 추출방법별항균효과의비교도앞으로연구가필요하다고할수있다. 따라서, 본연구의결과로다제내성 A. baumannii의항균효과를입증하였고, 티트리가항균효과가좋았으며, 라벤더의효과는미비하고, 로즈마리의효과는좋지않음을확인하였다. 향후천연에센셜오일을이용한손소독제와같은항미생물제재로활용이가능할것으로사료된다. 요약 Acinetobacter baumannii는광범위한항생제에대한저항성으로인해감염된환자의사망률이높아지는적색경보병원체로분류됩니다. 이연구에서다제내성 A. baumannii (MRAB) 의 18가지임상분리균주에대해일부에센셜오일 ( 티트리, 로즈마리, 라벤더오일 ) 의항균활성을평가하고자하였다. Carbapenemase 선별을위한 Hodge 시험법은 A. baumannii 의 20 가지균주가모두 imipenem 에내성이있음을보여주었습니다. 다제내성미생물의확인은 VITEK 시스템을통해수행하였다. 에센셜오일의항균활성은 MRAB에대한디스크확산방법으로평가하였다. 디스크확산방법에서 tee tree는라벤더오일에비해억제크기가가장크게증가했으며, 로즈마리는항균효과가없었다. 티트리오일은가장일반적인인간병원균및 MRAB 감염의치료및예방을위한대체천연제품으로유용할것으로보인다. 따라서이연구의결과는다제내성 A. baumannii의항균효과를입증했으며, 미래에천연에센셜오일을사용하는손소독제와같은항균제로사용될것으로예상됩니다.
