2 신생아 청각선별검사의 의의 것과 선별검사의 방법은 evoked otoacoustic emi- 간이 짧으며 검사비가 저렴하기 때문에 청각선별검사 ssion을 이용하여 1차 검사를 하고 이상이 있는 경우 로 적합하다. 이 검사의 통과기준은 시행하는 기관에 뇌간유발검사로 2차 검사를 시행하는 2단계 청각선별 따라 다양하지만 검사결과가 주위 소음이나 신생아의 검사를 권고하였다. 움직임에 영향을 많이 받기 때문에 신뢰도를 확인하는 것이 중요하다. 검사결과 통과기준에 충족되어도 검사 신생아 청각선별검사 모델 신뢰도가 낮을 경우 재검사를 시행해야 한다. 검사의 신 뢰도가 인정되는 기준은 1) stimulus stability mea- 신생아 청각선별검사로 유발이음향방사검사와 뇌간 surement가 75% 이상, 2) peak stimulus가 77~83 유발반응검사가 가장 많이 이용되는데 이 중 한 가지 dpk SPL, 3) stimulus spectrum이 충분한 주파수대 검사만으로, 또는 두 가지 검사를 혼합하여 검사를 한 에 분포할 것, 4) noise level이 40 d SPL이하, 5) low 다. 유발이음향방사는 소리 자극에 의하여 와우 내에서 noise sample이 50 이상일 것 등이다. 이중 가장 중요 발생한 작은 소리로서 중이를 통하여 외이도에서 측정 한 항목은 2)와 5)로서 peak stimulus가 83 dpk 된다. 유발이음향방사는 와우의 외유모세포의 기능과 SPL을 초과하거나 low noise sample이 50 미만일 경 밀접한 관계가 있기 때문에 이 반응의 소실이나 이상 우 재검사를 실시해야 한다.9) 은 와우 병변에 의한 난청의 정도와 상관관계가 있다. TEOE검사만을 이용하여 신생아 청각선별검사를 유발이음향방사에는 짧은 시간동안 click음이나 tone 하는 protocol은 신생아가 퇴원하기 전까지 TEOE검 burst 자극을 주고 20 msec 후에 외이도에서 주파수 사를 반복 시행하여 통과하지 못한 신생아는 생후 4주 대역 별로 측정하는 transient evoked otoacoustic 에 외래에서 TEOE 재검사를 시행한다. 재검사에서 emission(teoe)과 두 개의 인접한 순음자극을 주 이상이 발견된 경우 진단적 뇌간유발반응검사를 시행 고 측정하는 distorsion product otoacoustic emission 하게 된다. TEOE와 뇌간유발반응검사를 혼합하여 (DPOE) 검사가 있다. 두 가지 검사 모두 신생아 청 신생아 청각선별검사를 시행하는 protocol은 TEOE 각선별검사로 이용되나 TEOE가 보다 널리 사용되 에서 통과하지 못한 신생아는 퇴원 전에 자동 뇌간유발 고 있다.8) 반응검사(automated auditory brainstem response) TEOE 검사를 이용한 신생아 청각선별검사는 30 를 시행받게 된다. 이 검사에 통과하지 못하면 생후 4 d이상의 난청이 있는 경우 이상 소견이 나타나기 때 주에 TEOE를 재검사를 하고 이상이 있는 경우 진단 문에 sensitivity가 높고 환자가 불편하지 않고 검사시 적 뇌간유발반응검사를 시행하게 된다.10) Fig. 1.
