Although gastrointestinal lymphoma is usually secondary to nodal lymphoma, it can present as a primary gastrointestinal lymphoma with the majority being in the stomach (74.8%). In South Korea, the most frequent histological subtype of gastric lymphoma is extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue (MALT), followed by diffuse large B-cell lymphoma. Gastrointestinal lymphoma typically presents with nonspecific symptoms, and endoscopic findings are quite variable. So, the diagnosis is mainly dependent on the histopathological evaluation. Treatment of gastrointestinal lymphoma is dictated primarily by the histopathological type and the stage of the disease. This review will discuss the histopathological classification, staging systems, clinical features and treatment of gastrointestinal lymphoma. Dawson 등 1 은위장관및이와관련된배액부위림프절에국한되고간이나비장및기타림프절침범이없고, 단순흉부촬영상이정상이며말초혈액검사의백혈구가정상인경우를원발성위장관림프종으로정의하였다. 한연구에의하면 371예의원발성위장관림프종에서 277예 (74.8%) 가위에서발생하였다. 2 이어서소장 32예 (8.6%), 회맹부 26예 (7.0%), 위장관의다발성침범 24예 (6.5%) 의순서였고, 직장결장부위는 9예 (2.4%) 에불과하였다 년 Kim 등 3 이보고한국내림프종발생현황을살펴보면전체림프종중비호지킨림프종이 95.4% 였고림프절외림프종이 69.6% 였다. 림프절외림프종중에서는위림프종이가장많았는데, 세포형으로는 MALT 림프종이 56.1%, 미만성거대 B-세포림프종이 35.1% 였다. 3 본론 1. 분류와병기 1966년 Rappaport 4 는종양세포의크기와조직학적양상을기준으로최초의근대적인림프종분류법을제안하였다. 1974년 Lukes와 Collins 5 는림프종을세포의기원에따라 B- 세포형, T-세포형, 대식세포형으로세분하였고, 유럽에서는 Lennert에의한 Kiel 분류법 6 을이용하였다. 1980년대초미 158

2 이윤정, 이준행 : 위장관림프종 159 국국립암연구소는여러분류법을비교하고예후및치료의통일된공식을제공하기위하여실용공식 (National Cancer Institute working formulation) 7 을발표하였다. 그러나여러분류법이혼재되어사용되는문제는계속되었다. 이를해결하기위하여 1994년형태학적, 면역학적, 분자유전학적, 임상적특성을모두고려한 Revised European American Lymphoma (REAL) 분류법 8 이제안되었고비로소림프종분류법이통일되었다. 2001년세계보건기구 (World Health Organization, WHO) 에서는 REAL 분류법을발전시켜형태학적소견, 세포의기원 ( 면역표현형으로결정 ), 임상소견과유전자형 ( 핵형, 바이러스유전자존재 ) 까지고려한새로운분류를제안하였다. 이방법에서는세포의기원에따라 (1) B-세포종양, (2) T-세포와 NK-세포종양및 (3) 호지킨림프종의범주로분류하였다. 9,10 위장관에서발생하는림프종중호지킨림프종은매우드물다. 위장관의비호지킨림프종은림프절림프종에이용되는방법을약간변형하여분류하고있다 (Table 1). 10,11 위장관림프종의병기는다양한방법으로표현할수있다 Table 1. (Table 2, 3). 호지킨림프종의분류를위하여개발된 Ann Arbor system은위장관림프종의병기체계로는한계가있다. 예를들어위 MALT 림프종의경우, 초음파내시경검사로평가한심달도가 Helicobacter pylori 제균치료후예후를예측하는중요인자임에도불구하고 Ann Arbor system에는포함되어있지않다. 이러한단점을극복하고자제안된것이 Paris staging systems이다 (Table 3). 12 Paris staging systems 은원발장기의심달도와림프절침범뿐만아니라원격장기의침윤양상도자세히분류하고있다. 13 올바른병기설정을위해적절한문진과신체검진은필수적이다. 검사실검사로는일반화학검사, 간기능검사, 신기능검사, 포도당및요산수치측정을포함한혈청화학분석, 전해질검사, 혈청 LDH 및 beta-2 microglobilin 검사를시행하며이와더불어혈정단백전기영동검사로특정림프종의파라단백질 (paraprotein) 을확인한다. 골수검사는림프종의골수침범및치료효과판정을위해시행한다. 일부림프절림프종과비장변연부림프종 (nodal and splenic marginal zone B-cell lymphomas) 을제외하고골수침윤은많지않는데, 위장관림프종에서도골수침범은매우드물다. 14 목, 가슴, 복부및골반을포함하는컴퓨터단층촬영을시행하여원격전이여부를확인한다. 미만성거대 B-세포림프종, 소포성림프종 (follicular lymphoma), 외투세포림프종 (mantle cell lymphoma) 의경우 fludeoxyglucose (FDG)-PET가도움이된다 위 MALT 림프종은다발성으로발생할수있을뿐만아니라, 미만성거대 B-세포림프종으로전환될수있으므로내시경검사도중광범위하게조직검사를시행하는것이좋다. 18 내시경초음파는종양의침윤정도에대한상세한정보를제공하여 T, N 병기결정에 CT보다우수하다. 19,20 2. 위 MALT 림프종위 MALT 림프종의개념은 1983년 Isaacson과 Wright에의해체계화되었고, 최근 2008년 WHO 분류에서는림프절외변연부림프종 (extranodal marginal zone B-cell lymphoma of MALT) 이라는이름으로불리고있다. 위 MALT 림프종의진단과치료에대한적지않은혼선 Fig. 2. Extranodal marginal zone B-cell lymphoma of mucosaassociated lymphoid tissue (MALT). 이있는데이는위 MALT 림프종에대한개념이변하였기때문이다. 엄밀하지는않지만개념적으로설명하면다음과같다 (Fig. 1). 위 MALT 림프종이라는질환이처음정립될무렵에는 B-세포림프종을 (1) 고도요소가없는 MALT 림프종 (MALToma without high grade component), (2) 고도요소를가진 MALT 림프종 (MALToma with high grade component), (3) 미만성거대 B-세포림프종 (= 광범위큰 B세포림프종, diffuse large B-cell lymphoma) 의 3군으로구분하였다. 이중두번째에해당하는고도요소를가진 MALT 림프종은 H. pylori 제균치료에반응하는경우도있지만, 대부분은미만성거대 B-세포림프종과비슷한경과를보인다. 따라서최근에는고도요소가없는 MALT 림프종만을림프절외변연부림프종이라고부르고나머지두군은미만성거대 B-세포림프종으로통칭하고있다. 병리학교과서에서는위장관미만성거대 B-세포림프종을 MALT 림프종요소를가진경우와 MALT 림프종요소가없는경우로나누기도한다. 전통적으로위 MALT 림프종의내시경적특징은 특징이없는것 이라고가르치고배워왔다. 그만큼다양한모양을

이윤정, 이준행 : 위장관림프종 161 가질수있다는의미일것이다. 특히 고도요소를가진 MALT 림프종 을위 MALT 림프종에포함시키던시절에는진행위암과비슷한형태를보인경우도있었다. 그러나 고도요소를가진 MALT 림프종 을 MALT 림프종에포함시키지않게되면서, 위 MALT 림프종은대부분위염이나조기위암과비슷한모양을보이게되었다 (Fig. 2).

4 이윤정, 이준행 : 위장관림프종 161 가질수있다는의미일것이다. 특히 고도요소를가진 MALT 림프종 을위 MALT 림프종에포함시키던시절에는진행위암과비슷한형태를보인경우도있었다. 그러나 고도요소를가진 MALT 림프종 을 MALT 림프종에포함시키지않게되면서, 위 MALT 림프종은대부분위염이나조기위암과비슷한모양을보이게되었다 (Fig. 2). 내시경적특징에따른분류법은많은저자들이제시하고있으나필자는다음과같이나누고있다. (1) gastritis-like MALTomas, (2) multifocal atrophy pattern MALTomas, (3) multinodular MALTomas, (4) ulcerative MALTomas, (5) polypoid MAL- Tomas, (6) early gastric cancer-like MALTomas. 실제로는약간지저분하면서심하고불규칙하게분포하는미란성위염과비슷한형태를보이는경우가가장많다. 위 MALT 림프종의병리학적특징은다음과같은 5가지소견으로정리할수있다. (1) 정상적인 Peyer s patch의림프조직을닮은반응성림프여포가있고, (2) marginal zone/ monocytoid B-세포들이있으며, (3) 림프상피성병소 (lymphoepithelial lesion), 즉 3개이상의변연부세포 (marginal zone cell) 가상피혹은위선에침범하여상피세포의호산성변성을보이는병변을보이며, (4) 소림프구와형질세포침윤과함께 (Dutcher 소체를가질수있다 ), (5) 여포아세포 (centroblast) 와면역아세포 (immunoblast) 가출현한다. 실제로이와같은소견들은디지털적인단계를가지기보다는아날로그적인스펙트럼을가지고있으므로 Wotherspoon 등 21 이나눈 scoring system이자주사용된다. 병기결정을위한검사 (CT, 22,23 EUS, 골수검사등 ) 후, 점막이나위점막하층에국한된 MALT 림프종으로진단되고 H. pylori 감염이있으면제균치료를시행한다. 24 제균치료후완전관해에도달하는비율은대략 75% 정도로알려져있다. 25,26 최근에는건강검진을통하여경미한 MALT 림프종이많이진단되고있으므로과거보다제균치료의효과가더욱좋을가능성이있다고생각된다. 치료성공여부는 1 2개월후조직검사를시행하여확인할수있으며대개 1년이내에관해에도달한다. 제균치료로관해에도달하지않는예는대부분 (1) 점막하층을넘어침범되어있거나, (2) 고도요소를가지고있거나, (3) 내시경초음파에서위주위의림프절이침범되어있거나, (4) t(11;18)(q21;q21) 와같은염색체이상이존재하는경우이다. 전향적무작위대조연구에서조기위림프종 (stage IE, IIE) 환자를대상으로항암화학요법과방사선치료및수술적치료의효과를비교하였는데모든군에서 100% 의완전관해를보였다. 27 전체생존율은통계학적차이가없었으나, 무병생존율은항암화학요법군에서우수하였다. 27 현재제균치료에반응하지않는 MALT 림프종과 H. pylori 음성림 프종에대해서는방사선치료가가장많이이용되고있는데, 15 20회에걸쳐총 Gy의방사선을위와위주위림프절에조사할경우 90% 이상의완전관해율을보고하였다 수술은제균요법에반응이없는 MALT 림프종중방사선요법이나항암화학요법의대상이되지않거나, 출혈, 천공, 폐쇄등합병증을동반한경우에제한적으로고려된다. 15,27,32 3. 미만성거대 B-세포림프종미만성거대 B-세포림프종은최근에는 광범위큰 B세포림프종 으로번역되기도한다 ( 대한의사협회의학용어집 5판, 2008). 미만성거대 B-세포림프종은전체림프종에서가장흔한형태로서, 약 40% 를차지한다. 2011년통계자료에의하면우리나라에서비호지킨림프종은인구 10만명당 6.3명이발생하는데, 42.7% 가미만성거대 B-세포림프종이었다. 림프절이외에위장관, 피부, 뼈, 중추신경계, 갑상선, 고환등을침범할수있다. 33 미만성거대 B-세포림프종세포는일반림프구보다 4 5배까지크고핵에균열이있는세포와없는세포가섞여있다. 미만성거대 B-세포림프종은 MALT 림프종요소를가진경우와 MALT 림프종요소가없는경우가있는데발생기전과예후가약간다를것으로추정된다. 34 최근 FDG-PET 가위림프종진단에유용하다는보고가있었는데치료후에도약간의 18 F-fluorodeoxyglucose 섭취가남을수있으므로주의가필요하다. 35 미만성거대 B-세포림프종은치료하지않으면빠르게진행하는공격적인종양으로, 환자는명치부위통증, 체중감소, 식욕부진, 오심, 구토등위암과유사한증상을호소할 Fig. 3. 39 이러한결과를바탕으로최근에는수술로치료하는예가매우적으며, 대부분항암화학요법을일차적으로사용하고있다. 항암화학요법에방사선요법을더하는것은치료결과를향상시키지는못하였다. 40 4. 대장 T-세포림프종위장관 T-세포림프종은장병증과연관된경우 (enteropathy-associated) 와그렇지않은경우가있다.

5 162 Korean J Helicobacter Up Gastrointest Res: 제 12 권제 3 호 2012 수있다. 내시경에서는진행성위암과구분이중요하다 (Fig. 3). 과거에는완전절제가가능한경우수술이우선적으로선택되었다. 수술과함께항암화학요법혹은방사선치료를병합하는것이좋다는보고도있었다. 36,37 그러나 cyclophosphamide plus doxorubicin plus vincristine plus prednisone (CHOP) 기반복합항암화학요법단독치료군과수술과항암치료를병행한환자군을비교한대규모연구에서양군간생존율의차이는없었다. 38 또한수술에비하여항암화학요법으로치료받은환자의삶의질이월등히좋았다. 39 이러한결과를바탕으로최근에는수술로치료하는예가매우적으며, 대부분항암화학요법을일차적으로사용하고있다. 항암화학요법에방사선요법을더하는것은치료결과를향상시키지는못하였다 대장 T-세포림프종위장관 T-세포림프종은장병증과연관된경우 (enteropathy-associated) 와그렇지않은경우가있다. 41 서구에서의원발성대장림프종은거의대부분 B-세포에서기원하는것으로보고되었으나국내에서는 T-세포에서기원하는원발성대장림프종을종종경험할수있다. 42 필자는우리나라원발성대장림프종의임상상과세포기원에따른차이점을알아보고자국내 13개병원에서 Dawson 기준 1 에의하여진단된원발성대장림프종환자 97명 ( 남 : 여 =65:32, 평균연령 51.3세 ) 의임상상을후향적으로분석하였다. 내시경소견은종괴형, 궤양형, 침윤형, 궤양성종괴형및궤양성침윤형으로분류하였다. 세포의기원을알수있었던 94예중 78예 (83.0%) 가 B-세포, 16예 (17.0%) 가 T-세포에서기원하였다. 진단시평균연령은 T- 세포림프종에서 42.1세, B-세포림프종에서 53.0세였다 (P=0.011). 환자의주소는 B-세포림프종에서는대부분복통 (74.4%) 이었던반면 T-세포림프종에서는혈변 (37.5%) 과설사 (25.0%) 가흔하였다. 체중감소나야간발한의빈도는세포의기원에따라차이가없었으나발열은 T-세포림프종에서흔하였다 (P<0.001). 내시경을시행한 77명의환자에서육안소견에서악성종양을의심할수있었던경우는 T-세포림프종에서는 42.9%, B-세포림프종에서는 71.4% 였다 (P=0.061). 내시경소견에서궤양형이나궤양침윤형의빈도는 T-세포림프종에서 B- 세포림프종에비하여높았다 ( 각각 62.5% 및 11.6%). T-세포림프종은 B-세포림프종에비하여다분절에분포하였으며 ( 각각 50.0% 및 1.3%) 천공이많았고 ( 각각 25.0% 및 3.8%) 장중첩증은드문경향을보였다 ( 각각 6.3% 및 30.8%). 전체환자의 1년생존율은 78.6%, 3년생존율은 62.9%, 5년생존율은 55.4% 였다. T-세포림프종은 B-세포림프종에비하여예후가불량하였다 (P<0.001). 결론적으로우리나라에서원발성대장 T-세포림프종은비교적젊은나이에발생하고혈변, 설사, 발열등의증세를일으키며대장내시경에서다분절에분포하는다발성의궤양으로관찰되므로악성질환을의심하기어려워진단에주의가필요하다. 원발성대장 T-세포림프종은 B-세포림프종에비하여천공이흔히발생하며예후가불량하다 NK/T-세포림프종원발성위장관 NK/T-세포림프종은매우드물고예후가 Fig. 4. Burkitt lymphoma of the stomach. 나쁜질환이다 (Fig. 4). 삼성서울병원에서경험한원발성위장관 NK/T-세포림프종 14예중내시경사진을확보할수있었던 11예 ( 위 3예, 식도 2예, 십이지장 1예, 대장 5예 ) 를분석하였다. 43 비교적젊은환자들이었으며 ( 중앙연령 42 세 ), 주증상은위장관출혈 6예, 복통 4예, 속쓰림 4예등이었다. 내시경소견은궤양침윤형 4예, 궤양형 3예, 표재성 / 미란형 3예등이었다. Stage III 이상이 64% 였으며중앙생존기간은 9개월이었다 외투세포림프종 (mantle cell lymphoma) 외투세포림프종은보통광범위한다발성림프절종대와골수침윤으로발현한다. 외투세포림프종은진단당시부터전신적질환인데, 80% 이상에서위장관을침범한다. 대장혹은회맹장부근이가장흔하지만십이지장과위등전위장관을침범한사례가많다. 내시경검사에서전형적인소견은대장의림프종성용종증 (lymphomatous polyposis) 이지만일견정상으로보이는위, 십이지장, 대장점막에서조직검사를해도외투세포림프종이침윤된것을확인할수있다 (Fig. 5). 예후는매우나쁘다. 삼성서울병원에서경험한외투세포림프종 19예의 27개병소를분석하였다. 44 내원시증상은복통 (7예) 과출혈 (5예) 이많았다. 내시경으로확인가능한병소의위치는대장이 9예로가장많았고위와대장이 7예, 회장과대장이 1예, 위가 2예였다. 용종성종괴 (polypoid mass) 형태가가장흔했고 (48.1%), 이어서침윤형 (33.3%), 궤양형 (14.8%) 의순이었다. 44 버킷림프종은호염기구성세포질을가진단일형태, 중간크기의세포들로구성된 B세포신생물로고분화분획을가지며 8번염색체의 c-myc 유전자의비조정과전위가특징적이다. 이질환은아프리카에서발생하는지역성아프리카형을제외하고는소아와면역이저하된경우에주로발생하고, 가장흔한호발부위는복부이며주로말단회장과결장에서발생한다. 드문질환인위장관버킷림프종은매우진행이빠르며공격적이나비교적항암화학요법에반응이좋다. 위장관침범은드문편이지만위궤양, 위점막하종양, 회맹부종괴그리고회장궤양등으로나타날수있다 (Fig. 6). 1995년 1월부터 2007년 7월까지삼성서울병원에서버킷림프종으로진단받은 80명 ( 성인 47명, 소아 33명 ) 중위장관버킷림프종으로진단된성인환자는 20명이었다. 45 가장흔한증상은복통 (50%) 과속쓰림 (30%) 이었다. 위병변은 11 예 (55%), 대장병변은 4예 (20%), 위와십이지장동시병변 3예 (15%), 위와대장동시병변 1예 (5%) 그리고위, 십이지장, 대장을침범한경우가 1예 (5%) 였다. 내시경영상이확인가능한 13예에서궤양병변이가장흔했으며 (39%), 다음은궤양종괴형과궤양침윤형이동일한빈도로많았다 (23%). 대다수의환자는진단당시진행된상태였으며, Musshoff stage IV E (60%) 가많았다. 20명모두항암화학요법을받았고 5년생존율은 64% 였다. 45 결 위장관림프종은대부분비호지킨림프종으로내시경검사와겸자생검으로쉽게진단되는경우가많지만간혹주의하지않으면진단이지연될수있다. 조직형에따른적절한치료가좋은결과를얻는지름길이다. 론 참고문헌 1.
