Methods: We analyzed the number and medical costs of patients with AMD diagnosed by the National Health Insurance Corporation ( ). We also analyzed the number and medical costs of such patients who received anti-vegf treatment, and analyzed the frequency, period of use, and average medical cost of anti-vegf use in AMD patients. Finally, we evaluated the use of anti-vegf injections for new AMD patients. Results: The number of patients with AMD was 236,158 in 2009 and 537,528 in 2016, which represented a 2.3-fold increase over 8 years. Of these, the number of patients undergoing anti-vegf therapy increased steadily from 9,961 in 2009 to 35,762 in The mean number of cycles of ranibizumab or aflibercept per patient was 4.87 ± 3.37, and the mean interval between treatments was 2.89 months. On average, 6.2 injections were performed in the first year of diagnosis, and the frequency of use decreased with time, with an average of 1.2 cycles after 4 years of diagnosis. Among all AMD patients in 2016, the total medical cost of those treated with anti-vegf was 76.9 billion won, and the average medical cost per person was 2,162,145 won. Conclusions: The use of two drugs, ranibizumab and aflibercept, as reflected in public health insurance claims, steadily increased over the study period. Notably, there was a tendency to substitute aflibercept for ranibizumab. 2 - 조송희외 : 항혈관내피성장인자의국내사용현황 - 고생각되었던망막질환들도이제는치료및관리가가능해지고있다. 10 고령화로인해연령관련황반변성의유병률이점차증가하고있으며, 해당질환의표준적인치료법으로알려진항혈관내피성장인자유리체내주입술의사용도꾸준히증가하고있다. 11 그러나항혈관내피성장인자의국내사용현황에대한연구는거의없는실정이다. 사용현황에대한연구는환자들의치료패턴을파악할수있는중요한기초가되며, 향후보건연구에기초적인자료가된다. 따라서본연구에서는건강보험청구자료를활용하여연령관련황반변성의환자규모를파악하고나아가항혈관내피성장인자의유리체내주입술을받은환자들을대상으로치료패턴을알아보는등항혈관내피성장인자의국내사용현황을파악하고분석하였다. 대상과방법 본연구는헬싱키생의학연구윤리헌장을준수하며, 가톨릭대학교의과대학의기관윤리심사 (Institutional Review Board) 의승인을받았다 ( 승인번호 : VC17ZESI0207). 본연구에서는국내연령관련황반변성환자의규모를확인하고관련치료현황을파악하기위해국민건강보험공단의건강보험청구자료를이용하였다. 청구자료는 2007년 1월 1일부터 2016년 12월 31일까지연령관련황반변성을진단받은환자의전체의료이용내역이며연구대상자의전체의료 이용및사회 인구학적특성을건강보험청구자료와자격자료를통해파악하여자료원을구축하였다. 구체적인국민건강보험공단청구자료분석내용은이전의논문들에서참조할수있다 연령관련황반변성환자의규모와해당질환에서항혈관내피성장인자의유리체내주입술사용양상및의료비용을산출하기위해본연구에서는연구대상자를다음과같이정의하였다. 연구대상자는 2007년 1월 1일부터 2016년 12월 31일까지연령관련황반변성으로의료이용을한 18세이상성인환자이며상병코드 (H35.3, H35.30, H35.31, H35.32, H35.35, H353.9) 또는산정특례코드 (V201) 를바탕으로의료이용을한환자를모두포함하였다. 본연구에서는연령관련황반변성으로의료이용을한유병환자의규모와의료비파악뿐만아니라항혈관내피성장인자치료를받은환자의규모및의료비를확인하였다. 이를위하여연령관련황반변성을진단받고동시에라니비주맙 (Ranibizumab, Lucentis, Novartis AG, Basel, Switzerland) 과애플리버셉트 (Aflibercept, Eylea, Regeneron Pharmaceuticals, Inc., Tarrytown, NY, USA) 를포함하는항혈관내피성장인자약제를처방받은환자를연구대상자로정의하였다. 다만베바시주맙 (Bevacizumab, Avastin, Genentech, South San Francisco, CA, USA) 은비승인약제이므로건강보험청구자료에서확인할수없어, 본연구대상에서제외하였다. 급여대상이아니어서파악이불가능하므로항혈관내피성장인자약제중라니비주맙의건강보험급여적용시점이 Table 1. 3 - 대한안과학회지 2019 년제 60 권제 8 호 년임을감안하여해당시점부터의유병률을제시하였으며유병환자의특성과연간총의료비및 1인당평균의료비용은 2016년유병환자를기준으로제시하였다. 의료비용의경우관련비용을산출하기위하여진료과목코드가안과인명세서만을포함하였다. 본연구에서는앞서기술한유병환자뿐아니라신환자에대한분석을별도로시행하였다. 신환자는연령관련황반변성을진단받고동시에항혈관내피성장인자약제를처방받은환자들중에이전 3년동안연령관련황반변성상병진단또는관련치료내역이없는환자로정의하였으며, 연구기간동안최초로상병진단을받은청구명세서의요양개시일을신환자의입적일로정의하였다. 신환자의경우 3년의과거력제외기간을고려하여대상환자선정기간을 2010년부터 2016년까지로설정하였다. 통계방법은기술적분석방법 (descriptive method) 을사용하였다. 결 과 2009 년 1 월 1 일부터 2016 년 12 월 31 일까지연령관련황 Table 2. IQR = inter quartile range. 반변성으로인해의료이용을한환자의연도별분포는 Table 1과같다. 2009년 236,158명에서 2016년 537,528명으로환자규모는지속적으로증가하였으며남성에비해여성의비율이높게나타났다. 2009년의경우남성은 96,110명으로전체유병환자의 40.7% 를차지한반면여성은 140,048명으로 59.3% 를차지하였으며이러한양상은모든연도에서동일하게나타났다. 연령군별환자수의경우 70-79세연령군에서가장높았으며그다음으로 60-69세연령군에서높게나타났다. 2009년기준 70-79세환자는 79,389명으로전체환자의 33.6% 였으며 60-69세환자는 27.6% 를차지하였다. 대상자의세부특성및의료이용규모를파악하기위해 2016년유병환자를대상으로기저특성을확인하였다 (Table 2). 환자연령중앙값은 70세 (inter quartile range [IQR]: 61, 77) 였으며, 연령군별분포는 70-79세에서 34.9% 로가장높게나타났으며 60-69세연령군은 28.0% 를차지하였다. 여성환자가전체의약 60% 인 318,749명으로남성보다많았다. 건강보험가입자구분의경우직장가입자가 47.3% 로가장높게나타났으며의료급여수급권자는세대주와세대원을모두포함하여약 9% 를차지하였다. 건강보험자격자료의보험료 20분위를기준으로구분한소득분위의분포를확인한결과 16분위이상인고소득층이 45.5% 로가장높은구성비를보였으며, 6분위이상 10분위미만의환자가 15% 로가장적은비중을차지하였다. 요양기관종별의경우상급종합또는종합병원의이용환자가 2016년유병환자의약 68% 를차지하는것으로나타났다. 2016년연령관련황반변성유병환자의 1년의료비용은 3,279억원이었으며이중본인부담금은전체의료비용의약 22% 인 714억원, 공단부담금은 2,557억원으로나타났다. 1인당평균의료비용은 613,492원이었으며이중본인부담금은 133,589원, 공단부담금은 478,363원으로각각전체의료비용의 21.8%, 78.0% 를차지하였다 (Table 3). 항혈관내피성장인자주입술을시행받은환자들의연도별분포를살펴보면, 환자규모는 2009년 9,961명에서 2016년 35,762명으로지속적으로증가하였다 (Table 4). 2016년유병환자의경우환자연령중앙값은 74세 (IQR: 67, 79) 였으며, 연령군별분포는 70-79세에서 41.7% 로가장높게나타 Table 3. 4 - 조송희외 : 항혈관내피성장인자의국내사용현황 - 났다. 남성이약 60% 인 21,550명으로여성보다높은비중을차지하였으며 4분위이상인고소득층이 45.5% 로가장높은구성비를보였다. 의료기관종별의경우상급종합병원을이용한환자가전체의 56% 를차지하여가장높은비율을보였다 (Table 4). 2016년연령관련황반변성유병환자중항혈관내피성장인자치료를받은환자의전체의료비는 769억원이었으며이중본인부담금은총의료비용의약 11% 인 87억원, 공단부담금은 680억원으로나타났다. 1인당평균의료비용은 2,162,145원이었으며이중본인부담금은 244,898원, 공단부담금은 1,913,894원으로각각전체의료비용의 11.3%, 88.5% 를차지하였다 (Table 5). 연령관련황반변성의신환자는 2010년에서 2016년까지진단을받은동시에항혈관내피성장인자치료를받은환자중이전 3년동안관련질환의진단또는치료내역이없는환자를산출하였다 (Table 6). 전체연령관련황반변성신환자와 2016년신환자의기저특성을확인한결과전체신환자의중위연령은 69.8세이며연령군의경우 70-79세환자가 13,777명으로전체의약 40% 를차지하였다. 건강보험가입유형은직장가입자 ( 세대원 ), 지역가입자 ( 세대주 ) 순으로구성비가높았으며소득수준은 16분위이상인고소득층이약 45% 로가장높은비중을차지하였다. 신환자를 기준으로라니비주맙과애플리버셉트의연도별사용현황을확인한결과, 라니비주맙의사용량은애플리버셉트가급여된 2014년이후다소감소하는경향을보였다. 반면애플리버셉트는급여된해에 7,992건에서 2년뒤에 35,329건으로빠르게증가하는양상을보였다 (Table 7). 라니비주맙또는애플리버셉트의 1인당평균사용횟수는약 4.87 ± 3.37회로나타났으며최소사용횟수는 1회, 최대사용횟수는 14회였다. 마지막치료와그이전치료간간격을기준으로산출한평균치료간격은 2.89개월로확인되었다. 진단받은첫해에평균 6.2회의주입술을받았고, 시간이흐를수록사용횟수가줄어들어진단 4년이지난뒤에는평균 1.2회의사용횟수를보였다 (Table 8). 연도별 1인당사용횟수의경우 3회의빈도가가장높은비율을차지하였으며 2014년라니비주맙의건강보험급여횟수가 10회에서 14회로확대되면서 1년동안 10회이상치료를받은환자가증가하였다 (Table 9). 전체신환자중라니비주맙또는애플리버셉트를급여최대횟수인 14회를모두이용한환자는총 1,437명이었으며평균치료기간은 3.34 ± 1.29년으로나타났다. 14회치료를모두받은환자의연간 1인당평균의료비용은 6,363,457원으로전체연령관련황반변성유병환자의 1인당평균의료비에비해높았으며이중본인부담금은 669,041원으로의료비의약 11% 를차지하였다. 5 - 대한안과학회지 2019 년제 60 권제 8 호 - 으로기대된다. 연령관련황반변성의질병부담에관한다른나라의연구를살펴보면, 미국의경우 GDP 대비 300억달러의경제적인손실이황반변성으로인한시력손실로인하여발생하는것으로추정하였다. 15 또한캐나다의경우에는 GPD 대비 26억달러의경제적손실을예상하고있다. 16 전세계적으로는 2천 5백만명에서 3천만명정도가연령관령황반변성으로고통받고있는것으로추정하고있다. 17 이처럼연령관련황반변성은세계적으로도중요한문제로대두되었으며, 항혈관내피성장인자의유리체내주입술은연령관련황반변성의사실상유일한치료제로사용되고있다. 18 본연구에서는 년청구자료를활용하여연령관련황반변성에서항혈관내피성장인자 ( 라니비주맙, 애플리퍼셉트 ) 의장기사용현황을살펴보았다. 연령관련황반변성에서항혈관내피성장인자사용현황을파악하기위해국민 Table 7. Average numbers of intravitreal injections of ranibizumab and aflibercept according to treatment duration (years) after initial treatment Duration (years) < >4 Number 건강보험공단의청구자료 ( 연구관리번호 : NHIS ) 를활용하였다. 18세이상연령관련황반변성환자를대상으로연도별진료실인원, 진료비용, 항혈관내피성장인자약제사용경향등을파악하기위해요양개시일기준으로 2010년 1월 1일부터 2016년 12월 31일까지건강보험청구자료, 자격자료를이용하였다. 망막질환관련진료비용만을산정하기위해해당대상자들의안과명세서만으로한정하여분석하였다. 18세이상연령관련황반변성유병환자수는 2009년 236,158명이었으나 2016년기준 537,528명으로 7년사이 2.3배증가한상황이다. 2016년기준전체환자중 70대가가장높은비중 (34.9%) 으로차지하였고그다음으로 60대 (28.0%) 가높은비중으로차지하였다. 2016년연령관련황반변성유병환자들의전체의료비용은 3,279억원이었다. 이환자중항혈관내피성장인자유리체내주입술을한번이상받은환자는 2009년 9,961명에서 2016년 35,762명으로 3.6배증가하였다. 연령관련황반변성으로라니비주맙과애플리버셉트사용횟수를연도별로살펴보았으며 2010년 27,792회였던것이 2016년 63,115회로증가하였으며, 환자 1인당연간사용횟수는 3회, 평균치료간격은 2.89개월로나타났다. 사용횟수의증가는유병률증가로인한환자수증가뿐만아니라급여기준완화로인한 1인당치료횟수의증가를반영하는결과로판단된다. 또한라니비주맙의사용량은애플리버셉트가급여된 2014년이후다소감소하는경향을보인반면애플리버셉트는급여된이후로빠르게증가하는양상을보였다. 그러나라니비주맙의상대적비율은감소하였으나절대횟수는비슷한점을볼때, 2014년부터최대급여횟수가 14회로늘어나면서전체유리체주입술횟수가늘어났고, 이는라니비주맙이나베바시주맙의치료에반응하지않는연령관련황반변성환자에서애플리버셉트가대 Table 9. 6 - 조송희외 : 항혈관내피성장인자의국내사용현황 - 안으로각광받아사용량이늘어이러한결과가발생했을가능성이있다. 기존급여기준평생 14회는평균 3.34 ± 1.29년동안모두소진되는것으로나타났고이들환자들의연평균 1인당의료비는 636만원정도인것으로나타났다. 최근에는급여기준의완화로평생 14회의급여제한이없어졌다. 그러나 5회시술후교정시력이 0.1 이하의경우에는급여기준에서제외한다는새로운급여기준이생겼다. 많은수의연령관련황반변성환자의교정시력이 0.1 이하인경우를고려할때최근변경된급여기준하에서항혈관내피성장인자의사용이증가하게될것인지, 아니면감소하게될것인지추가적인연구가필요하겠다. 본연구에서연령관련황반변성진단받은환자군, 그리고실제주사치료받은환자군모두에서고소득층비율이높았다. 정확한이유를알수는없으나, 상대적으로저소득층군에서는진단비율이낮고, 그만큼치료기회가적기때문일수도있다. 즉의료불평등의문제를확인할수있었다. 그러나연령관련황반변성은실명과직결되는질환이기에의료비용뿐아니라생산성손실등을포함한사회, 경제적비용을증가시킬수있다. 따라서이에대한적극적인해결책이필요하다. 그러나이는본연구의범주에서벗어나므로추후연구에서논의할예정이다. 새로이진단되는환자들의수는연간 5,000명내외로일정한데비하여, 유병환자의수는 23만명에서 (2009년) 53만명으로 (2016년) 증가하였다. 발생률은증가하지않는데유병률이증가하는경우는환자치료법의발달로해당질환으로인한사망률이낮아지거나, 해당질환의완치가되지않는경우에발병률이증가하지않아도유병률이증가하게된다. 연령관련황반변성의경우질병으로사망하는경우가없고, 사실상완치가되어질환이없는상태로이동하지않기때문인것으로추론된다. 본연구에서는연령관련황반변성과항혈관내피성장인자의사용에대해알아보기위하여 18세이상의남녀를대상으로하였다. 일반적으로연령관련황반변성의유병률및관련인자를알아보는연구에서 40세또는 50세이상으로대상을제한하는경우와다르다. 이는본연구의목적은우리나라연령관련황반변성의치료에서항혈관내피성장인자의국내사용현황을파악하는것이기에, 특정연령대로대상을제한한다면일부사용현황이누락되는문제가생긴다. 본연구는몇가지제한점들을가진다. 이는건강보험청구자료, 자격자료등의변수가제한적인공공자료원을이용한연구이기때문이다. 첫째, 건강보험청구자료를주된자료원으로사용한연구였기에연령관련황반변성에관한임상적요인의활용이부족할수있다는제한점이있다. 둘 째, 건강보험청구자료상진단코드로만연령관련황반변성을정의한것역시한계점이다. 이는실제연령관련황반변성이아니지만환자와진료의편의상연령관련황반변성으로청구하는업코딩 (upcoding) 의가능성이있어연령관련황반변성환자수를과잉추계 (overestimation) 하게될위험성이있다. 19 그러나본연구에서는추가로항혈관내피성장인자를유리체내주입술로투여하는경우에한정하였으므로, 결과에큰영향을미치지는않았을것으로판단된다. 셋째, 베바시주맙은건강보험청구자료상에서확인할수없는제한점이있다. 베바시주맙은비승인 (off-label) 사용약제이므로, 정확한사용행태를알수없으며, 약제가격도의료공급자에따라천차만별인한계점이있다. 20 넷째, 건강보험청구자료에기반한연구는보험인정기준에따라환자들의유병률과항혈관내피성장인자의사용현황이변하게되는단점이있다. 예를들어 2014년에최대급여횟수가 14회로늘어나면서전체유리체주입술횟수가증가한것으로나타났으며이를해석하기어려운한계점이있다. 결론적으로지난 10여년동안항혈관내피성장인자유리체주입술이연령관련황반변성의주된치료로자리를잡았으나, 그약제들의국내사용현황에대한연구는부족한실정이었다. 이에본연구는공공자료원인국민건강보험공단건강보험청구자료를이용하여급여약제인라니비주맙, 애플리버셉트의사용현황을살펴본결과두약제의사용량은꾸준히증가하고있었다. 본연구의결과에서연령관련황반변성의국민보건에미치는영향의중요성을확인할수있었다. Drugs Aging 2009;26: = 국문초록 = 연령관련황반변성환자에서항혈관내피성장인자의국내사용현황 목적 : 연령관련황반변성환자에서항혈관내피성장인자의유리체내주입술의국내사용현황을알아보고자하였다. 대상과방법 : 국민건강보험공단의청구자료 ( 년 ) 를이용하여연령관련황반변성으로진단받은환자들을대상으로항혈관내피성장인자를주입받은환자들의규모와의료비를분석하였다. 연령관련황반변성환자들의항혈관내피성장인자주입술의사용횟수, 사용주기, 평균의료비용등을분석하였다. 또한신환자의규모와항혈관내피성장인자주입술의사용현황을조사하였다. 결과 : 연령관련황반변성유병환자수는 2009 년 236,158 명이었으나 2016 년기준 537,528 명으로 7 년사이 2.3 배증가하였다. 이중에서항혈관내피성장인자주입술을시행받은환자들의규모는 2009 년 9,961 명에서 2016 년 35,762 명으로증가하였다. 라니비주맙또는애플리버셉트의 1 인당평균사용횟수는약 4.87 ± 3.37 회로나타났으며평균치료간격은 2.89 개월로확인되었다. 진단받은첫해에평균 6.2 회의주입술을받았고, 시간이흐를수록사용횟수가줄어들어진단 4 년이지난뒤에는평균 1.2 회의사용횟수를보였다. 항혈관내피성장인자치료를받은환자의전체의료비는 769 억원이었으며, 1 인당평균의료비용은 2,162,145 원이었다 (2016 년기준 ). 결론 : 국민건강보험공단건강보험청구자료를이용하여급여약제인라니비주맙, 애플리버셉트의사용현황을살펴본결과두약제의사용량은꾸준히증가하고있었다. < 대한안과학회지 2019;60(8): > 조송희 / Songhee Cho 한국보건의료연구원경제성평가연구단 Economic Evaluation Team, National Evidence Based Health Care Collaborating Agency 764
