Complex precipitating behaviors such as calculation, writing, copying of complicated figure, and spatial construction have been recognized as triggering factor of epileptic seizures. We report a case of a patient with a very unusual form of complex reflex epilepsy in which seizures are usually induced by writing or spatial construction. Neurological examination was normal and brain magnetic resonance imaging showed mild brain atrophy without focal lesion. During long-term video-eeg monitoring, she was instructed to write a letter with the right hand. About 20 minutes after writing, she experienced similar aura and motion arrest and then showed an elevation of bilateral upper extremities like myoclonic jerk followed by generalized tonic clonic seizure. On ictal EEG, spike and wave complexes appeared from both centroparietal region followed by 10 Hz rhythmic activities for 20 seconds. Interictal EEG was normal. Ictal-interictal subtraction image of brain SPECT 99mTc-HMPAO showed hyperperfusion in the middle frontal gyrus and precentral gyrus of left frontal lobe and superior and inferior frontal gyrus of right frontal lobe. We suggest that this reflex epilepsy might be originated from both central areas with maximum on the left side. Oxcarbazepine was effective in controlling this reflex epileptic seizure. 2 손을 이용한 정신 활동시 유발되는 반사성 간질 1예 가 하였다고 한다. 결혼 후 아이와 복잡한 종이접기를 하 성 발작으로 이어지진 않았다. 이때의 발작 중 뇌파는 양 거나, 복잡한 그림을 베끼던 중에 강직 간대성 발작이 발 측의 central areas(c3, Cz, C4)에서 극파가 일시적으로 생했다고 한다. 그 외에는 자발적인 간질발작을 일으킨 관찰되었으며, 좌측에서 약간 더 우세하였다. 환자는 여 적이 없다고 한다. 환자는 오른손잡이였고 과거력상 정상 러 작업 중 오른손으로 편지를 쓰거나, 복잡한 조형(Rey 분만이며, 열성경련, 중추신경계 감염 및 두부외상의 병 Complex Figure)를 보고 그리기를 할 때 반사성 발작이 력은 없었다. 간질의 가족력은 없었다. 성장 발달 상황은 여려 차례 발생하였다. 발작간 뇌파에서는 정상 소견을 정상적이었고, 발작 이전까지의 학습 능력 또한 정상이었 보였다. 다고 한다. 이학적 검사 및 신경학적 검사, 혈액 검사 등 이후 편지를 20분 쓰고 있던 중 순간적으로 기분이 멍 에서 특이 소견은 발견되지 않았으며, 뇌자기 공명 영상 해지면서 하던 동작을 일시적으로 멈추는 증상이 5~6초 검사에서 경도의 뇌 위축 소견이 관찰 되었다(Fig. 1). 가량 있으면서 의식을 잃고, 양팔에 근간대성 경련양상의 24시간 Video-EEG monitoring 검사 중 환자는 오 상지거상이 나타나고 이내 이차적인 강직간대성 발작을 른손과 왼손으로 각각 편지 쓰기, 지문보고 베끼기, 지문 하였다. 발작 후 오심, 두통을 호소하였고 발작 후 수면 을 작은 소리와 큰소리로 각각 읽기, 암산하기, 편지 쓰는 을 보였으나, 발작 후 요실금이나 발작 후 혼동 등의 증상 시늉만 하는 등의 작업을 수행하도록 하였다. 환자는 편 은 없었다. 전조증상을 느끼면서 하던 작업을 멈추는 경 지쓰기 시작한 후 5분정도 경과 후 의식을 잃을 것 같은 우의 발작 중 뇌파에서는 양측의 전두정엽(centroparietal 느낌의 전조 증상이 나타났으며, 기간은 2~3초 동안 지 areas)에서 보이는 극서파가 3~4초 동안 관찰되었다. 이 속하였다. 이후 이런 전조 증상은 1분 동안 5~6회 정도 차성 강직간대성 발작으로 이어지는 발작 중 뇌파에서는 나타났었다. 이때의 발작 중 뇌파에서는 양측의 전두정엽 양측의 전두정엽에서 보이는 극서파가 6~7초 동안 관찰 (centroparietal areas)에서 보이는 극서파가 3~4초 동 되었고 이후 이차성 강직간대성 발작에 상응하는 뇌파 소 안 관찰되었다.
