Recent studies have demonstrated a high signal intensity on diffusion MRI in TGA patients. In this study we reviewed and compared the use of electroencephalography (EEG) and diffusion-weighted imaging (DWI) in TGA in order to determine their sensitivity and to reveal clues about the etiology of this condition. Methods: Twenty patients (7 males and 13 females; aged 58.0±12.1 years, mean±sd) who had been diagnosed with TGA at Dongguk University International Hospital within a 2-year period (2007 and 2008) were included in the study. All patients underwent EEG and DWI. Results: The EEG of 12 of the 20 patients (60.0%) exhibited abnormalities; all 12 exhibited slowing on either the left side (n=6) or bilaterally (n=6). Spikes or sharp waves were detected in two patients. DWI revealed unilateral high signal intensities in the hippocampus of eight patients; five of these patients had left hippocampal lesions, and the other three had right hippocampal lesions. Four patients with a unilateral DWI lesion exhibited bilateral EEG abnormalities, and six patients exhibited only EEG abnormalities (without DWI abnormalities). Three patients had a high-signal-intensity lesion on DWI without EEG abnormalities. Five patients had normal EEG and DWI results. Interestingly, no patient had EEG abnormalities confined to the right temporal area. Conclusions: These findings suggest that left temporal dysfunction is important for the development of TGA. EEG might be complementary to DWI in TGA investigations, and may be superior at illustrating the associated memory dysfunction. 2 다. 17 최근국내증례보고에의하면확산강조영상에서우측에병변이보인환자두명에서극파가양측혹은좌측에서뚜렷하게확인되었다. 18 본연구에서는 TGA 환자에서뇌파이상의빈도와확산강조영상결과를비교해보았다. 대상과방법 2007 년부터 2008 년까지본원에온환자중에서 Hodges 가제안한 TGA 의진단기준에합당한환자 20명이포함되었다 (Table 1). 1 진단기준은우선명확한사건후기억상실증이발병중에있어야하며, 의식장애나자신의주체성에대한혼돈이없으며인지장애가기억상실에만국한되어야한다. 다른국소신경학적징후나증상이없으며, 간질발작이동반되지않으면서 24시간이내에증상이호전되어야한다. 그밖에최근에머리를다치거나간질발작의병력이없어야하며, 목격자로부터발병당시의증상에충분한정보를얻을수있어야한다. 모든환자는발병이전에신경계질환이없었으며모두오 른손잡이였다. 모든환자에서확산강조영상과뇌파검사를시행하였다. 남자는 7명, 여자는 13명이었고, 연령분포는 18세부터 69세까지로평균 58±12.1 세이었다. 뇌파검사는대부분 4일이내에시행하였는데증상발생후평균 4±6.9 (0-32) 일에시행하였고환자네명은각각 5일, 8일, 9일, 32일에시행하였다. 환자두명은 24시간뇌파모니터링검사를시행하였다. 판독은신경과전문의와신경과전공의가함께시행하였다. 확산강조영상 (FOV= mm, matrix= , B=1000, TR/TE=5400/77 msec, slice thickness=3 mm) 은발병후 9일내에모두촬영하였고평균 2±2.3 일에시행하였다. 환자세명은 B값을 1000과 2000, 3000으로각각촬영하였으며환자네명은 1000 과 2000 으로촬영하였다. 결과 뇌파검사에서이상이확인된경우는 12명이었다 (60.0%, Table 2). 환자 6명에서좌측측두엽서파가, 환자 6명에서양측측두엽에서파가보였고, 환자 2명에서는극파가발견되었 Table 1. Both co-registration and reconstruction processes were performed with Analyze 8.1 software (Mayo Clinic, Rochester, MN, USA).

4 TGA 는일과성뇌허혈증과는예후가달라서발병이후뇌졸중혹은심근경색발병률이일과성뇌허혈증환자보다유의하게낮은것으로알려져있는양성질환이다. 2 발병원인으로는뇌혈관질환이나간질발작혹은편두통에의한것일가능성이제시되었으나, 환자의 7% 에서 1년이내에간질로진행하는것이확인된것외에혈전색전증의증거는찾을수없었다. 2 본연구에서기존보고보다뇌파이상의비율이높았는데 22 이는증상발생부터뇌파시행까지시간이비교적짧으며심지어증상이있을때시행된경우도있고, 두명의환자는뇌파모니터링을시행하여뇌파이상을찾아낼수있었기때문으로추측한다. 환자두명에서극파같은간질모양파가보였다. 기존연구에서일반적으로서파가보이는경우가많지만간질모양파가보고된예가있으며, 임상양상은같았다. 18,23 본연구에포함된환자에서도임상적차이는없었다. 확산강조영상에서는편측해마에고신호강도가보이고뇌파에서는양측측두엽의서파가나타난나타난경우가있는데, 측두엽간질환자를대상으로한연구에서, 편측측두엽에서시작하여등쪽해마연결부 (dorsal hippocampal commisure) 를통해반대측으로전파되어양측에서뇌파이상이보였던경우가있었다. 18 이와는달리확산강조영상에서병변위치와뇌파이상의위치는우측측두엽인데, 뇌파에서는좌측측두엽에서서파가관찰된경우가있었는데두피뇌파의민감도가낮아동측의뇌파이상은보이지않고반대측의이상만보이는역설편측화 (paradoxical lateralization) 가능성도고려해볼수있다. 24 일반적인뇌자기공명영상으로는병변을확인하지못하지만, 최근개발된확산강조영상으로는해마의고신호강도를찾아낼수있다 촬영기법과촬영시기에따라발견확률이다르다고알려져있으며, Weon 등은환자 13 명에서 B값을 2,000 이상으로하고, 추적검사를시행하였더니모든환자에서병변을확인할수있었다고보고하였다. 6 이외에도단일광자방출촬영을통하여국소적인뇌혈류량의변화를보는검사에서좌측혹은양측반구의측두엽, 전두엽또는우측시상, 기저핵의혈류감소가있었다 본연구에서는확산강조영상보다뇌파에서이상을보이는빈도가높았는데, 이는확산강조영상의촬영시점과촬영기준과도연관이있을것이다. 따라서, B값을높이고, 영상단면의간격을작게하면더많은경우에서병변을확인할수있을것이다. 6 확산강조영상에서우측해마에만병변이있던환자도양측측두엽뇌파이상이나타났고, 이는환자의기억기능장애를반영하는것이라고생각한다. 결론적으로 TGA 의진단에있어서임상특징이중요하지만뇌파검사와확산강조영상에서보이는이상이질환의병태생리학적기전을규명하는데중요 하다고생각한다. 기억장애의원인이나기전을밝히는데뇌파검사가확산강조영상보다우수하다고할수는없지만상호보완적인검사로이용할수있다.
