The immunoglobulin E (IgE) reactivity to each pollen allergen is known to be variable according to the region. We determined the major IgE binding components of these tree pollens in sera of adult patients with allergic rhinitis. Methods: Allergic rhinitis patients, of whom specific IgE level to birch and/or alder pollens ( > ku/l by ImmunoCAP) were included. The protein bands of two pollen extracts were determined by sodium dodecyl sulphate-polyacrylamide gel electrophoresis, and their IgE-binding components were identified by IgE immunoblot analysis. The binding specificity and cross-reactivity between two pollens were evaluated by IgE enzyme linked immunosorbent assay (ELISA) inhibition test. Results: Six IgE binding components were found in birch pollens in which two (14 kda and 17 kda) were major components. Two IgE binding components were found in alder pollens in which the 17 kda was a major component. The IgE binding component to the major allergen component of 17 kda was observed in 90.3% of the study subjects sensitive to alder pollens and 72.7% of them sensitive to birch pollens. The ELISA inhibition tests showed significant inhibitions with additions of birch/alder pollen extracts. Conclusion: We identified two major IgE binding components (17 kda and 14 kda) from birch pollens and one component (17 kda) from alder pollens. Significant cross reactivity was noted between these two pollens. ( 2013;1: ) Keywords:, Birch, Allergens, Immunoblotting, Cross reactions 서론알레르기를유발하는원인알레르겐중화분은집먼지진드기다음으로중요한흡입항원이며화분과연관되어발생하는알레르기질환을화분증 (pollinosis) 이라부른다. 알레르기를유발하는수목화분에는나자식물 ( 소나무과, 측백나무 ) 와피자식물 ( 자작나무과, 참나무과, 버드나무과, 느릅나무과 ) 화분이주된원인이며, 특히피자식물에속하는자작나무 (birch) 와오리나무 (alder) 화분이흔한항원이다. 1,2) 이는동서양에서공통적으로관찰되어, 북유럽, 미국, 북서아프리카, 동아시아와오스트레일리아에서는자작나무, 오리나무, 개암나무 (hazel) 와참나무 (oak) 화분이알레르기증상을나타내는중요한요인중하나이며 3) 우리나라에서도자작나무, 오리나무, 참나무화분등이알레르기를일으키는주요원인수목알레르겐으로알려져있다. 4-6) 국내의수목화분농도를살펴보면소나무, 참나무, 오리나무, 자작나무등이주를이루는데이들중약 70% 는소나무화분이차지하고있다. 모든지역에서소나무화분의농도가가장높게나타나고있으나, 소나무화분으로인한알레르기유발가능성은다른종 Correspondence to: Hae-Sim Park Department of Allergy & Clinical Immunology, Ajou University Hospital, Ajou University School of Medicine, 164 World cup-ro, Yeongtong-gu, Suwon 443-3, Korea Tel: , Fax: , hspark@ajou.ac.kr This study was supported by the National Research Foundation (NRF) and a grant from the Korean government (MEST, ). 2 윤문경외 자작나무와오리나무화분의주요항원 류의화분에비해매우낮다. 반면자작나무와같은수목화분및돼지풀등과같은잡초화분은그발생량은적으나알레르기를유발할가능성은매우높아화분알레르기환자에게특히주의해야할식물로보고된다. 7) 그러나유럽에서는인종에따른주요수목화분항원에대한 immunoglobulin E (IgE) 결합성분 (binding component) 의양상차이를보고한바가있으나, 8) 국내에서는알레르기환자의주요수목화분항원에대한 IgE 결합성분을보고한연구가없었다. 따라서본연구는우리나라알레르기비염환자를대상으로자작나무화분과오리나무화분의주알레르겐에대한 IgE 결합성분을규명하고, 오리나무와자작나무화분항원사이의교차반응을조사하고자하였다. 대상및방법 1. 재료 1) 연구대상 2009년부터 2012년까지아주대학교병원에알레르기비염으로내원한환자들중, ImmunoCAP (Phadia, Uppsala, Sweden) 으로측정한자작나무혹은오리나무화분항원에대한혈청특이 IgE 값이 ku/l을초과하는환자들을대상으로자작나무군 (33명 ) 과오리나무군 (31명 ) 을선택하였다. 2) 화분항원추출자작나무화분은 Betula pendula (Allergon, Engelholm, Sweden), 오리나무화분은 Alnus incana (Allergon) 를사용하였다. Phosphate buffered saline (PBS) 에화분 1 g을넣고 4 C에서 24시간동안항원을추출한후, sonicator를사용하여항원에초음파를 30초씩 3번반복하여작용시켰다. 그후, 추출액을 2,000 rpm에서 분간원심분리하여그상층액을모아 0.45 μm filter를이용하여분리하였다. 분리된화분항원에서 Bradford assay로단백질을정량하고 sodium dodecyl sulphate-polyacrylamide gel electrophoresis (SDS-PAGE) 로단백질을확인한후, 이항원을 ELISA 억제시험 (enzyme linked immunosorbent assay inhibition test) 과 immunoblot 에이용하였다. 2. 방법 1) SDS-PAGE에의한화분항원의확인각각의화분항원을 sample buffer (0.5 M Tris ph 6.8, glycerol, % SDS, 0.5% bromophenol blue, 2.5% β-mercaptoethanol) 에희석하고 5분간 0 C에서가열하였다. 표지자와각각의화분항원을 4 20% Tris-glycine gel (Novex, Invitrogen, San Diego, CA, USA) 에서 120 V, 2시간동안전기영동하여화분항원단백질을분리하였다. 2) IgE Immunoblot에의한화분항원의결합 IgE 성분분석각각의화분항원을 μg/ml의농도로 SDS-PAGE 법으로항원단백질을분리한후, poly-vinyl difluoride membrane (PVDF membrane, Millipore Co., Bedford, MA, USA) 에 250 ma로 2시간전이시켰다. 단백질이전이된 PVDF membrane 을 4 mm 간격으로절단한다음절단된각각의 membrane에비특이적결합을방지하기위해서 20% skim milk-tris buffered saline-tween20 (skim milk-tbst) 를이용하여 12시간이상 4 C에서작용시켰다. 환자와대조군의혈청을 2시간동안처리한후, TBST로 4회세척하고 biotinylated antihuman IgE 항체 (Vector Laboratories Inc., Burlingame, CA, USA) 를 % fetal bovine serum-phosphate buffered saline (FBS-PBS) 를이용하여 1:1,000 v/v로희석하여상온에서 1 시간작용시켰다. 이후 TBST로 3회세척한후각각의 membrane에 1X AP color development buffer와 AP conjugate substrate kit (Bio-Rad laboratories Inc., Hercules, CA, USA) 의혼합액을넣어단백대가나타날때까지반응시켰다. 3) ELISA 억제시험 5 μg/ml 농도의화분항원을 96 well microplate (Corning Inc., Corning, NY, USA) 에각 well 당 0 μl씩넣고 4 C에서 12시간이상반응시켰다. ImmunoCAP 측정결과, 자작나무화분항원과오리나무화분항원에대해서동시에 ku/l을초과하는환자의혈청에억제제로자작나무화분항원과오리나무화분항원을각각 0, 1,, 0 μg/ml씩가하여 4 C에서 12시간이상반응시켰다. 비특이적결합을방지하기위해 % FBS-PBS를각 well당 200 μl씩넣어 2시간작용시켰다. 0.05% PBS-Tween20 으로 3회세척후각각의화분항원을반응시킨환자혈청을 well 당 50 μl씩넣어상온에서 2시간반응시켰다. ELISA 방법으로각각의화분항원에결합하는 IgE 흡광도를측정하였다. 특이 IgE 항체결합의억제도 (%) 는 ([ 대조군의흡광도 억제제가포함된 sample 의흡광도 ]/ 대조군의흡광도 ) 에 0을곱한값으로정하였다. 결과 1. 연구대상자의임상적특성연구대상자는모두알레르기비염환자로, 자작나무혹은오리나무화분항원에대한혈청특이 IgE 값이 KU/L 이상이었다. 자작나무군 33명과오리나무군 31명의임상적특성은 Table 1과같다. 자작나무군의연령분포는 25세에서 53세사이, 남성비율이 60.6% 였으며오리나무군의연령분포는 19세에서 43세사이였고남성이 74.2% 를차지하였다. 알레르기비염과기관지천식을함께가지고있는환자의비율은자작나무군이 63.6%, 오리나무군은 22.6% 였다

Atopy was defined for subjects who had more than one positive response to common inhalant allergens (i.e., tree mixture, grass mixture, mugwort, ragweed, cat fur, dog fur, Dermatophagoides pteronyssinus, Dermatophagoides farinae, and Alternaria) on the skin prick test. SDS-PAGE법에의한화분항원의확인자작나무화분항원의단백질은약 14 kda, 17 kda, 24 kda, 35 kda에서단백대가나타났으며, 이들은각각 Bet v 2, Bet v 1, Bet v 3 와 Bet v 6로알려져있다. 오리나무화분항원의단백질은 9 kda, 17 kda, 18 kda, 22 kda에서단백대가나타났다 (Fig. 1). 3. IgE Immunoblot 에의한화분항원의결합 IgE 분석자작나무화분항원내에는 kda, 11 kda, 14 kda, 17 kda, 35 kda, 38 kda의 IgE 결합단백대를보였으며, 오리나무화분항원내 에는 17 kda과 14 kda의 IgE 결합단백대가관찰되었다 (Fig. 2A, B). 이단백대들중각군의 50% 이상에서 IgE 결합단백대를나타내는항원을주알레르겐 (major allergen) 으로판정하였다. 자작나무군 33명중 26명 (78.8%) 은자작나무화분항원에서단백대 17 kda에 IgE가결합하였으며 25명 (75.8%) 은 14 kda에결합하였다 (Fig. 3B). 오리나무군 31명중 명 (90.3%) 이오리나무화분항원에서단백대 17 kda에 IgE가결합하였다 (Fig. 3A)

ELISA 억제시험에의한화분결합 IgE 분석 자작나무와오리나무화분항원에대한 ImmunoCAP 검사결과 가모두양성인환자의혈청을대상으로시행한 ELISA 억제시험결 과, 자작나무화분항원에결합한 IgE 는억제농도 0 μg/ml 에서 오리나무화분항원에의해 82%, 자작나무화분항원에의해 98% 억제되었으며, 오리나무화분항원에결합한 IgE 는억제농도 0 μ g/ml 에서오리나무화분항원에의해 48%, 자작나무화분항원에 의해 51% 억제되었다 (Fig. 4A, B). 억제제로넣어준화분항원의농 도가 1 μg/ml, μg/ml, 0 μg/ml 로증가할수록각각최대 50% 이상의유의한억제증가를관찰하였다. Immunoglobulin E (IgE) enzyme linked immunosorbent assay inhibition results for alder (A) and birch (B) pollen extracts coated wells with serial addition of alder pollen extracts or birch pollen extracts from a patient who shows positive ImmunoCAP of alder and birch. 자작나무와오리나무는참나무목이며이들화분은높은항원 단백질을포함하며유럽과일부아시아에서계절성화분알레르기 를유발하는주요원인항원중의하나이다. 국내환자를대상으로 시행된수목화분연구는대부분피부단자시험을통해이루어졌으 며, 주로수목화분감작률혹은수목화분과음식항원과의교차 반응성에대한연구였다. 9) Kim 등 ) 에따르면, 부산지역호흡기알 레르기환자에서흡입알레르겐피부단자시험을시행한결과수목 화분 (tree pollen), 잡초화분 (weed pollen) 및목초화분 (grass pollen) 의감작률은각각 22.6%, 11.1%, 9.1% 로화분알레르겐중에수 목화분의감작률이가장높았다. 또한, 경기남부지역에서 1999 년, 2005 년, 2008 년알레르기질환으로피부단자시험을시행한환자들 을대상으로 년간화분알레르겐감작률의변화를조사한결과, 시간이경과함에따라전반적으로감작률이증가되는양상이었다. 특히오리나무화분의경우, 통계적으로유의한감작률의증가를 확인하였다. 5) 국외보고에서도수목화분알레르기환자의대부분 Percent inhibition of birch-specific IgE binding (%) B 이자작나무와오리나무에주로감작되어있지만, 11) 유럽 6개국에서자작나무주알레르겐에대한 IgE 결합성분의빈도를조사하였으며, 프랑스, 스웨덴, 오스트레일리아, 프랑스, 스위스, 이탈리아에서각각 Bet v 1 (17 kda) 성분에대한양성률은 0%, 98%, 90%, 65%, 62% 였으며, Bet v 2 (14 kda) 에대한양성률은각각 25%, 12%, 30%, 20%, 43%, 33% 로각나라간차이가있었다. 이에저자들은국내성인알레르기비염환자를대상으로수목화분중가장높은감작률이보고된자작나무와오리나무화분의주알레르겐성분및이들의 IgE 항체와의결합양상을조사하였다. 수목화분의항원에대한혈청특이 IgE immunoblot 을통해, 자작나무에서 17 kda과 14 kda가환자들의 78.8% 와 75.8% 로주항원으로나타나는것을확인하였으며오리나무에서는환자들의 90% 이상이 17 kda에 IgE가결합하는것을확인하였다. 이들항원중각화분증환자군의 50% 이상에서 IgE 결합단백대를나타내는항원을주알레르겐 (major allergen) 으로간주하였을때, 국내알레르기비염환자중자작나무화분에감작된경우 17 kda (Bet v 1) 과 14 kda (Bet v 2) 에 IgE가결합하는부분이주알레르겐으로, 오리나무화분에서는 17 kda 성분이주알레르겐으로생각한다. 자작나무화분의주항원으로 Bet v 1 (17 kda) 과 Bet v 2 (14 kda) 이잘알려져있으며, Bet v 1은식물에서파생된음식과참나무목에속하는다른나무의화분항원과에피토프 (epitope) 를공유하여높은교차반응을나타내는주요항원이다. 3) Pauli 등 12) 은피부단자시험에양성을보이는환자의 82% 에서 Bet v 1과결합하는 IgE 항체가존재함을보고하였고, Jarolim 등 13) 은자작나무화분에감작된환자의 95% 이상에서 Bet v 1에대한 IgE가나타나며, 이들중 60% 의환자에서는 Bet v 1에대한 IgE 항체가단독으로검출된다고보고하였다. 또한 Bet v 2는수목화분에알레르기를보이는환자의 20% 에서양성으로나타나며식물알레르겐과중요한교차반응을나타낸다고보고하였다 ) 하지만저자들이국내환자를대상으로시행한이번연구에서는, Bet v 1 감작률은 78.8% 로 Jarolim 등 13) 과비교해서낮은수치였으며 Bet v 1에만단독양성을보이는경우는없었다. 이처럼국내와국외결과간차이가나는이유는, 자작나무화분의성분과이에반응하는환자들의인종적차이를생각해볼수있다. 먼저, 자작나무화분의차이에대해살펴보면, 자작나무는그종류가 30종이넘으며이들중에서유럽과아시아에서화분증을일으키는자작나무로는주로 B. pendula와 Betula platyphylla로알려져있다. 국내에널리분포된자작나무종은 B. platyphylla이며, 17) B. pendula와 B. platyphylla는분류학적으로매우가까운종으로항원의주알레르겐은 Bet v 1으로동일하다. 비록저자들이연구에서사용한자작나무화분항원은유럽에주로분포하는 B. pendula이지만, 최근에알레르기와관련해서광범위하게연구되고있는종이고 B. platyphylla와주알레르겐이동일하기때문에자작나무화분의차이로외국과다른패턴을보이지 219

Major IgE binding components to birch and alder 는않을것으로생각된다. 따라서본연구에서국내환자들의주알 레르겐감작률과패턴이유럽에서보고된결과와차이를보이는것 은인종간의차이가있을것으로생각한다. 현재까지오리나무화분에대한국내의연구는보고된바가없 으며, 오리나무화분항원에결합한 IgE 인 17 kda 과 14 kda 은자작 나무화분의주항원과동일한성분의항원일가능성이높을것으 로생각된다. Park 등 18) 에의해다종화분에대한피부반응양성환 자들의화분간교차반응이보고된바있으나, 우리나라에서알레 르기를일으키는주요수목항원인오리나무와자작나무의화분 항원에대한교차반응은연구된바없다. 따라서본연구에서는화 분항원에대한 IgE 결합부분중주항원뿐만아니라모든 IgE 결합 부분을양적으로측정하는 ELISA 억제시험을시행하였다. 그결 과, 자작나무화분항원과오리나무화분항원을억제제로반응시켰 을때, 두항원에서모두억제도가 50% 이상이고억제제의농도가 증가할수록유의하게억제도가증가하였다. 이는자작나무와오리 나무화분항원간의교차반응성이존재함을시사한다. 이는오리 나무화분의주항원이참나무목의다른나무들과교차반응이있 으며, 특히자작나무과에속하는자작나무나개암나무등과강한 교차반응을나타낸다는보고와일치한다. 11) 이러한결과는 Niederberger 등 3) 에의해보고된 Immunoblot 억제시험의결과인, 자 작나무, 오리나무, 개암나무, 참나무화분의 Bet v 1 및 Bet v 2 에결 합하는 IgE 에피토프공유로설명할수있겠다. 저자들의연구결 과에서는자작나무와오리나무항원사이의교차반응은 50% 이 상나타나는공통된 IgE 결합항원인 17 kda 에의한것으로생각된 다. 이렇게공통된결합항원이있으므로분류학적으로가까운수 목화분알레르기를진단하는데이용가능하며, 알레르겐면역요 법을위한항원선정시자작나무와오리나무가모두양성이라면 자작나무를선택하여면역요법을시행할수있는근거를제시하고 있다. 19) 결론적으로, 국내알레르기비염환자의자작나무화분항원에 대한주 IgE 결합성분은 17 kda (Bet v 1) 과 14 kda (Bet v 2) 이며, 오리나무화분항원은 17 kda 으로확인되었다. 자작나무화분항원 과반응하는 IgE 결합성분은 Bet v 1 단독으로나타나는경우는없 었으며대부분 Bet v 2 와함께나타났고, 오리나무와자작나무화 분항원에대한알레르기를진단하고알레르겐면역요법을시행할 때두화분간의교차반응성을고려해야한다.
