Med. 2008; 35(4): ] I. 서론 불임이란일반적으로결혼후원만한부부관계를가져도 1~2년이내에임신이되지않는경우로엄밀히말해병이라고단정지을수없지만일정기간이지나도록임신을못하는상태를말한다. 1,2 불임의 40~60% 가여자쪽에원인이있는경우로분류되며배란장애, 난관이상, 자궁이상등이포함된다. 2 이들장애중에조기난소부전 (Premature ovarian failure, POF) 은여성의불임을일으키는한원인이다. 미국에서 40세이전여성의약 1% 정도가 POF로진단받고있다. 3 POF는난소내난포들이사라지거나감소되면서영구불임으로진단받는경우를말하는데, POF 환자들은에스트로겐 (Estrogen) 분비저하로혈액내난포자극호르몬 (Follicle stimulating Hormone, FSH) 분비가 (>40 IU/ liter) 높고 6개월이상월경주기가없다. 현재까지알려진 POF의원인으로는감염 (Infection), 화학치료 (chemotherapy), 자궁경부외과수술 (pelvic sur- 주관책임자 : 최영석, 우 ) 서울특별시강남구역삼동 606-5, 포천중문의과대학교차병원여성의학연구소 Tel: (02) , Fax: (02) youngsokchoi@gmail.com * 본연구는보건복지가족부보건의료기술연구개발사업의지원에의해이루어진것임 (A080364). gery), 자가면역질병 (autoimmune diseases), 환경적요인, 유전적요인등다양하게존재한다. 그렇지만 POF의정확한유전적원인은밝혀져있지않다. 몇해전까지만해도 POF는성염색체 X (X chromosome) 와상염색체 (Autosome) 이상과연관이있다고보고되어왔다. X chromosome의일부인 POF (Premature Ovarian Failure)-1, POF-2, POF-3를포함하고있는유전자위 (loci) 의손실 (deletion) 이나재배열 (rearrangement) 은 POF를유발한다. 4~8 POF1B의점돌연변이 (point mutation) 는 POF1B가 nonmuscle actin filament 결합을방해함으로써이상이발생한다. 그리고 Turner 증후군환자와 Fragile X 증후군환자에게서도난소부전이관찰된다. Turner 증후군의경우 X chromosome 특정지역에존재하는유전자의 Haploinsufficiency에기인하는것으로여겨지고있다. 9,10 Fragile X 증후군환자의경우는 FMR1 (Fragile X mental retardation 1) 돌연변이와 FMR2 micro-deletion에의해조기폐경이유발된다. 11~13 그리고 X-linked된유전자중하나인 BMP15 (Bone morphogenetic protein 15) 의유전자사슬변이 (sequence variant) 도난소이상 (Ovarian dysfunction) 과연관되어있다. 14~16 그리고상염색체 (Autosome) 상에존재하는유전자의돌연변이들도 POF 환자와연관이있다. FSHR (FSH receptor), LHR (Luteinizing

2 대한생식의학회지 Hormone receptor), GDF9 (Growth differentiation factor 9), INHA (Inhibin), GALT (Galactose-1-phosphate uridyltransferase), FOXL2 (Forkhead transcription factor 2) 등이여기에포함된다. 17~24 그렇지만최근까지이러한유전적결함들이어떻게난소내난포형성과유지에관계가있는지분자생물학적접근이이뤄지지않았다. 본고찰에서는 Non-syndromic ovarian failure의유전학적이해를설명하기위해최근에발견된난자특이전사조절인자의유전자결핍생쥐모델과임상적으로보고된연구등을살펴보고그가능성에대해알아보고자한다. II. 본론 1. Factor in the germline alpha (FIGLA) FIGLA는난포의투명대 (zona pellucida, Zp) 단백질발현을조절하는basic helix-loop-helix (bhlh) 전사조절인자이다. 25 FIGLA 는생쥐에서배발달 13.5 (E13.5) 일째발현되며, 난소에서원시난포 (Primordial follicle) 형성에중요한역할을한다. 26 FIGLA 유전자결핍생쥐암컷은불임이지만, 수컷은정상이다. 26 FIGLA 유전자결핍은난소에서 primordial follicle 형성을방해하며, 태어나면서난소내난자들이빠른속도로사라지게만든다. 26 FIGLA 유전자결핍생쥐난소에서 Pou5f1 (Known as Oct4), Zp2, Pad6 등수많은난자특이유전자들이유의적으로감소되거나결핍된다. 27 Zp2는투명대 (Zona matrix) 를구성하는세가지인자인 Zp1, Zp2, Zp3 중하나로 Zp2 유전자결핍생쥐도난포형성이상으로인한불임이다. 28 Zp2 프로모터에 FIGLA 결합장소인 E-box (CANNTG) 가위치하고있다. FIGLA가직접이결합장소에결합하고프로모터활성을조절한다. 25 이것은 FIGLA 전사조절인자가난자의형태및형성에중요한유전자를조절함으로써난자의생리적역할을조절한다는것을의미한다. Zhao 등은 100명의중국인 POF 환자의염기서열분석을통해 FIGLA genomic DNA에서세개의유전자변이 (Variant) 을발견하였고두명의환자에서유전자 돌연변이 (missence mutation, c.11c A (p.a4e)) 을발견하였다. 29 이러한돌연변이의경우프레임시프트 (frame shift) 를일으켜 haploinsufficiency 를유도한다. 그리고한명의환자에서는유전자결손 (deletion, c delaca (p.140 deln)) 이발견된다. 29 이러한 missense 돌연변이와유전자결손은 FIGLA 전사조절인자의활성에영향을미치는데, FIGLA의 heterodimer 파트너인 TCF3 bhlh 결합을방해한다. 25 이러한연구는 FIGLA가난포형성초기에중요하며난자특이의유전자들을직접적으로조절함으로써난포의성장과유지에관여한다는것을의미한다. 2. Newborn ovary homeobox gene (NOBOX) NOBOX는호메오박스 (Homeobox) 도메인을가지고있는전사조절인자이다. 30 NOBOX는생쥐에서배발달 E15.5일째발현되며, 난소에서생식세포및원시난포 (Primordial follicle) 를포함한모든성장난포의난자내에존재한다. 30,31 NOBOX 유전자결핍생쥐수컷은정상표현형을보여주지만암컷은불임이다. 31 NOBOX 유전자결핍생쥐는생후 14일이내에난소에존재하던난자와난포를모두잃어버린다. 31 갓태어난 NOBOX 유전자결핍생쥐암컷의난소와정상생쥐의난소를비교했을때조직학적으로차이가없다. NOBOX 유전자결핍생쥐암컷의난소에는비슷한숫자의생식세포와원시난포를포함하고있다. 31 그렇지만생후 7일째가되면 NOBOX 유전자결핍생쥐난소에는정상생쥐의난소에서발견되는 Secondary follicle를볼수없고난포의숫자도현저히감소됨을보여주고있다 (Figure 1). 31 그리고갓태어났을때 NOBOX 유전자결핍생쥐암컷의난소가조직학적으로문제가없었지만유전학적으로많은변화가있다. NOBOX 유전자결핍생쥐암컷의난소에서 Gdf9 (Growth differentiation factor 9), Bmp15 (Bone morphogenetic protein 15), Mos, Pou5f1와같은수많은난자및생식세포특이적유전자의발현이현저하게감소된다. 31,32 NOBOX는호메오전사조절인자로 TAA/

follicle; EF, empty follicle. GTTG/A 에결합하여직접적으로유전자발현을조절한다.

33 NOBOX는이들각각의결합장소에강한결합력을보여주고있고 Gdf9 프로모터의활성을조절한다. 33 특이할점은난자에서 Gdf9의조절이다.

Gdf9은난자에서분비되며난자주변을둘러싸고있는 granulosa 세포들의성장에영향을미쳐난포의성장을조절하는매우중요한난자특이성장인자이다.

즉 NOBOX 전사조절인자는형태학적으로초기원시난포형성에는 영향을미치지않지만생후난포의생존과성장에중요한유전자의발현을조절함으로써난자에중요한역할을한다는것을의미한다.

GTTG/A 에결합하여직접적으로유전자발현을조절한다. 33 NOBOX 유전자결핍생쥐암컷의난소에서현저하게감소되는유전자들중 Gdf9은프로모터부분에 3개의 NOBOX 결합장소가존재한다. 33 NOBOX는이들각각의결합장소에강한결합력을보여주고있고 Gdf9 프로모터의활성을조절한다. 33 특이할점은난자에서 Gdf9의조절이다. Choi 등이 NOBOX가 Gdf9 유전자를직접적으로조절한다고보고할때까지 Gdf9의분자생물학적역할은알려져있지않았다. Gdf9은난자에서분비되며난자주변을둘러싸고있는 granulosa 세포들의성장에영향을미쳐난포의성장을조절하는매우중요한난자특이성장인자이다. 34~37 Gdf9 유전자가없다면암컷은불임되고난소내난포들은 primary 난포에서성장을멈춘다. 38 이것은 NOBOX 유전자결핍생쥐암컷의표현형과거의유사하다. 즉 NOBOX 전사조절인자는형태학적으로초기원시난포형성에는 영향을미치지않지만생후난포의생존과성장에중요한유전자의발현을조절함으로써난자에중요한역할을한다는것을의미한다. Qin 등은 96명의백인 POF 환자의염기서열분석을통해네명의환자에게서 NOBOX genomic DNA에서네개의유전자변이를발견하였고 NOBOX 호메오도메인부분에서두개의 p.arg355his와 p.arg360gln missense 돌연변이를찾아내었다. 39 이중에서 p.arg355his missense 돌연변이는 NOBOX의 DNA 결합능력을현저히감소시킨다. 39 그리고정상 NOBOX와일대일반응에서도돌연변이 NOBOX는정상 NOBOX 의 DNA 결합능력을절반으로감소시켰다. 39 이것은 p.arg355his missense 돌연변이가 haploinsufficiency를유도한다는것을의미한다. 3. LIM-homeobox gene 8 (LHX8) LHX8은호메오박스 (Homeobox) 도메인과 LIM

4 대한생식의학회지 도메인을가지고있는전사조절인자로서뇌발달에관여하는인자로발견이되었다. 40 그이후에 Choi 등은 LHX8가생쥐난소에서초기배발달 E13.5일째발현되며, 난소에서생식세포및원시난포 (Primordial follicle) 를포함한모든성장난포의난자내에특이적으로존재한다고보고하였다. 41 LHX8 유전자결핍생쥐수컷은정상이지만암컷은불임이다. 41 LHX8 유전자결핍생쥐는생후 7일이내에난소에 존재하던난자와난포를모두잃어버린다. 31 갓태어난 LHX8 유전자결핍생쥐암컷의난소는정상생쥐와비교했을때원시생식세포의숫자는차이가없었지만원시난포의숫자는감소한다. 41 그리고생후 3일째가되면 LHX8 유전자결핍생쥐난소에는정상생쥐의난소에서발견되는 primary follicle를볼수없고난포의숫자도현저히줄어든다. 41 갓태어난 LHX8 유전자결핍생쥐암컷의난소는조 A B Figure 2. 5 제 35 권제 4 호, 2008 최영석 직학적으로차이가거의없지만 Pou5f1, Gdf9, Kitl, Zp3와같은난자및생식세포특이적유전자의발현이유의적으로감소한다. 41 LHX8도난자특이호메오전사조절인자 NOBOX처럼 DNA에결합하는호메오도메인을가지고있다. LHX8는전사조절인자의 DNA 결합장소는 TG/AATTA 로 NOBOX의 DNA 결합장소인 TAA/GTTG/A와약간차이가있다. 41 이것은조직발달에시간적으로공간적으로작동하는호메오전사조절인자들이조직특이적으로작동하게하는원리이다. 특이한점은 LHX8 유전자결핍난자에서유의있게감소하는유전자중에 NOBOX가포함된다. 41 그리고 NOBOX 프로모터에한개의 LHX8 DNA 결합장소가존재하며 LHX8 에강한결합력가지고있다 (Unpublised data, Choi 등, 2009). 이것은 LHX8가난포성장과생존에필요한 NOBOX 전사조절인자의상위조절인자로서 NOBOX를직접적으로조절한다는것을의미한다. 그렇지만아직까지 LHX8와 POF와상관관계는보고된바가없다. III. 맺음말 POF의유전적원인을규명하고자하는연구가계속적으로진행중이었지만난자에특이적으로발현되는전사조절인자들이발굴되기전까지그노력은답보상태에있었다 (Figure 2). FIGLA는 10 여년전에발견되었지만얼마전 POF 환자군에서임상적자료가발표될때까지알려지지않았다. 그리고 Rajkovic 그룹에서 NOBOX, LHX8와같은새로운전사조절인자들을발굴하면서 POF의유전학적규명에관한연구는탄력을받기시작했다. 현재 FIGLA, NOBOX, LHX8 이세가지전사조절인자들이각각의하위조절메커니즘을갖는지아니면서로전사조절네트워크를형성하면서난자와난포의성장과생존에영향을미치는지명확하지않다. 그리고이종설에서는언급하지않았지만전사조절인자들외에도 PTEN (Phosphatase and tensin homolog) 과같은세포질인자들도 primordial follicle pool를조기에활성화시킴으로 POF가유발한다고보고되고있다. 42 PTEN은 Phosphatidylinositol 3-kinase (PI3K) 의주요 negative regulator로이는난포내난자가 PTEN-PI3K pathway를통하여성장과활성화를조절하는것으로여겨지고있다. 이처럼난포형성과생존조절메커니즘에필수적인인자들을발굴하고이를통하여유전자이상으로유발된 POF의원인을분자수준에서설명할수있다면 POF를미리진단예측하고치료예방할수있는실마리를제공할수있을것으로생각된다. 참고문헌 1.
