Methods: Genomic DNA was extracted from peripheral blood in 1,020 of infertile Korean women. Genotypes of FSHR at Thr307Ala (T/A) and Asn680Ser (N/S) were screened by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) analysis. Clinical outcomes related to the genotypes of FSHR were evaluated in IVF-ET cycles (n=302) with controlled ovarian hyperstimulation (COH) of infertile women under 40 years old. Results: In a population of 1,020 infertile Korean women, the frequency of TT/NN, TA/NS and AA/SS for the major variant Thr307Ala and Asn680Ser was 44.80%, 41.96% and 10.49%, respectively. There was no significant difference in characteristics of ovarian response and clinical pregnancy rate among the major genotypes of FSHR in IVF-ET cycles with COH. However, implantation rate of AA/SS patients was significantly higher than that of TT/NN patients (24.5% vs 15.7%, p<0.05). Conclusion: This study showed that FSHR genotype was not directly associated with ovarian response in IVF-ET cycles with COH. The relationship between clinical outcomes and FSHR genotypes of patients should be substantiated by further studies. [Korean. J. Reprod. 2 난포자극호르몬수용체의유전자형과임상결과와의관련성 대한생식의학회지 은개인별로각기다르기때문에난소의반응정도를예측에대한필요성이대두되었다. 과배란유도과정에서여러가지원인에의해난소의저반응혹은난소과자극증후군이나타날수도있다. 현재까지제안된여러예측표지인자가운데기저혈중난포자극호르몬 (basal FSH) 의농도가가장유용하게사용되고있으며, 이외에도 inhibin-b, anti- Mullerian hormone, 환자의나이, 난소의크기등이제시되고있으며, 유전적, 환경적요인들도난소반응에영향을주는것으로알려져있다. 5~12 근래에활발히진행되고있는난포자극호르몬수용체유전자다형성 (FSHR polymorphism) 에대한연구들에서난포자극호르몬의투여에따른난소의반응과체외수정및배아이식술에서의임상결과가이수용체 (FSHR) 의유전자형과관련성이제기되고있다. 13~18 이러한난포자극호르몬수용체의유전자다형성중 Thr307Ala는 307번째코돈 (codon) 의시토신 (C) 이구아닌 (G) 으로바뀌어단백질합성시트레오닌 (Thr) 이아닌알라닌 (Ala) 으로합성되며, Asn680Ser의경우는 680번째코돈의아데닌 (A) 이구아닌 (G) 으로바뀌어아스파라긴 (Asn) 대신세린 (Ser) 으로단백질을합성되게된다. 이에따라난포자극호르몬수용체의 307번째코돈의유전자형을 Thr/Thr과 Thr/Ala 그리고 Ala/ Ala으로구분할수있고, 680번째코돈의유전자형을 Asn/Asn과 Ser/Ser 그리고 Asn/Ser으로구분할수있다. 이러한난포자극호르몬수용체의유전자형에따라같은기간동안동일한양의난포자극호르몬을투여했을때각기다른난소의반응정도를보일뿐만아니라혈중에스트라디올수치에서도차이를나타낸다고보고하고있다. 15,17,18 따라서체외수정을위한과배란유도를시작하기이전에각환자의난포자극호르몬수용체의유전자형을확인하여난소의반응을예측하고적절한호르몬투여용량을결정할수있다면, 저반응또는난소과자극증후군및관련합병증등을예방할수있으며, 이는중요한임상적의의를갖는다고할수있다. 이에본연구에서는한국불임여성의난포자극호르몬수용체유전자형의분포빈도를파악하고, 체외수정시술에서난포자극호르몬수용체유전자형에따른과배란유도과정에서의난소반응과각종임상결과와의관련성을알아보고자하였다. 연구대상및방법 1. 연구대상연구대상은제일병원아이소망센터에내원한여성불임환자 1,020명을대상으로난포자극호르몬수용체의유전자형, 즉 Thr307Ala과 Asn680Ser을조사하였으며, 이들중에서과배란유도와체외수정시술을시행한 302주기에서의임상결과를이들의유전자형에따라비교하였다. 임상결과는여성환자의나이가 40세미만인경우만을포함하였으며, 착상전유전진단을시행한경우는제외하였다. 2. Genomic DNA의분리연구대상자의말초혈액을항응고제 EDTA 가처리된유리관에채취한후 2700 rpm으로 10분간원심분리하고백혈구세포층만을분리하여 DNA 추출전까지 -20 에보관하였다. Genomic DNA는 Aquapure genomic DNA blood kit (Bio-Rad, Hercules, CA, USA) 를이용하여추출하였으며, 종합효소연쇄반응을수행하기전까지 -20 에보관하였다. 3. 난포자극호르몬수용체의유전자형분석을위한종합효소연쇄반응중합효소연쇄반응 (PCR) 은 PCR buffer [10 mm Tris-HCl (ph 8.3), 50 mm KCl, 1.5 mm MgCl 2 ], 0.2 mm dntps (Roche Diagnostics GmbH, Mannheim, Germany), 10 pmol primers (Bioneer, Daejeon, Korea; Table 1), 0.5 IU Neotherm DNA polymerase (Genecraft Co., Munster, Germany) 그리고 50~100 ng의환자 genomic DNA를혼합하여최종 20 µl의반응액에서수행하였다. Following PCR amplification, digestions of each PCR product were carried out with the restriction enzymes Bsu36I (A) and BsrI (B) to distinguish the Thr307Ala and Asn680Ser, respectively. The sizes of the digested products for each restriction enzyme which allow the individual's genotype to be determined are written in 'Methods and Materials'. Biosystems, Foster City, CA, USA) 에서처음 95 에서 5분간반응한후, 94 에서 40초, 65 에서 40초, 72 에서 50초의주기를 40회반복한후최종적으로 72 에서 10분간반응시켰다. 반응이끝난 PCR 산물은 2% agarose gel 전기영동으로 FSHR-307과 FSHR-680 부위에대한증폭여부를확인하였다. 4. 제한효소단편길이다형성 (restriction fragment length polymorphism, RFLP) 분석증폭된 FSHR-307 PCR 산물과 FSHR-680 PCR 산물을각각제한효소 Bsu36I와 BsrI (New England Biolabs Inc., Ipswich, USA) 로각각 37 와 65 에서 2시간동안처리한후 3% agarose gel에서전기영동하여그양상을분석하였다. FSHR-307 유전자형은증폭된 364 bp의 PCR 산물에 Bsu36I를처리하였을때 364 bp의 band만존재시 Thr/Thr (TT) 유전자형으로, 328 bp와 36 bp의 band만존재시 Ala/Ala (AA) 유전자형으로, 364 bp 외에 328 bp와 36 bp band가동시에존재시 Thr/ Ala (TA) 유전자형으로판정하였다. FSHR-680 유전자형의경우, 증폭된 520 bp의 PCR 산물에 BsrI를처리하였을때 520 bp의 band만존재시 Asn/Asn (NN) 유전자형으로, 413 bp와 107 bp의 band만존재시 Ser/Ser (SS) 유전자형으로, 520bp 외에 413 bp와 107 bp band가존재시 Asn/Ser (NS) 유전자형으로판정하였다 (Figure 1). 5. 과배란유도및임상결과확인과배란유도는 gonadotrophin-releasing hormone (GnRH) agonist (Suprafact, Aventis, France, 또는 Leucrin, Abbott, France) 또는 GnRH antagonist와 recombinant FSH (puregon, Organon, Netherland 또는 Gonal-F, Serono, Switzerland) 또는 highly purified FSH (Metrodin, Serono, Switzerland) 및 human menopausal gonadotropin (Pergonal, Serono, Switzerland) 을사용하였다. 초음파상에서직경 18 mm 이상의난포가

4 난포자극호르몬수용체의유전자형과임상결과와의관련성 대한생식의학회지 개이상일때, human chorionic gonadotropin (hcg; Pregnyl, Serono, Switzerland) 10,000 IU를주사하였고, hcg 투여 35~36시간후질식초음파를이용하여난자를채취하였다. 회수된난자는 10% Human Serum Albumin (HSA) 가첨가된 G-Fert TM 배양액 (Vitrolife, Sweden) 에서 4~6시간배양후일반적인체외수정을시도하거나세포질내정자주입술 (intracytoplasmic sperm injection, ICSI) 을시행하여수정을유도하였다. 경우에따라서는일반적인체외수정과세포질내정자주입술을병행하는분할수정법을적용하기도하였다. 19 체외수정후 16~18시간에전핵형성유무로수정을확인하였다. 수정된배아는 3~5일간 GIII series (Vitrolife, Sweden) 의 G1, G2 배양액에서체외배양하여자궁내이식을실시하였다. 임신의확인은배아이식후 12일째혈중 β-hcg 가 5 miu/ml 이상이고, 추적검사에서지속적인상승을보이며, 임신 5~6주사이에태낭 (gestational sac) 이확인되면임상적임신 (clinical pregnancy) 으로판정하였다. 이식한배아에대한태낭의비율로착상률을계산하였다. 임신후추적조사를통해분만결과를확인하였다. 6. 통계분석통계적인분석은 Student's t-test와 chi-square test를이용하였으며, p<0.05인경우통계학적유의성이있는것으로판단하였다. 결과총 1,020명의여성환자들을대상으로난포자극호르몬수용체의 Thr307Ala와 Asn680Ser의유전자형빈도를조사해본결과 97.25% (992/1,020) 가 Thr307-Asn680과 Ala307-Ser680으로연관되어있었다. 주요유전자형으로 TT/NN 군이 44.80% (n=457), TA/NS 군이 41.96% (n=428) 그리고 AA/SS 군이 10.49% (n=107) 의분포로조사되었다소수유전자형을가지고있는 28명 (2.75%) 의경우는 TA/NN 군이 1.86% (n=19), AA/NS 군이 0.29% (n=3), TT/NS Table 2. Distribution of FSH receptor (FSHR) genotypes in infertile Korean women FSHR genotypes (307/680) No. of infertile women TT/NN Major genotypes TA/NS AA/SS Subtotal TA/NN AA/NS Minor genotypes TT/NS AA/NN TA/SS Subtotal Total 1, 군이 0.29% (n=3), AA/NN 군이 0.20% (n=2), 그리고 TA/SS 군이 0.10% (n=1) 빈도의유전자형으로확인되었다 (Table 2). 체외수정및배아이식술을위한 302주기의과배란유도에서환자의평균나이는 33.1±3.2세였으며, 평균 14.2±9.4개의난자를채취하여 275주기 (91.1%) 에서평균 3.3±0.9개의배아를이식하였다. 전체적인임상적임신율, 착상률과분만율은각각 38.2% (105/275), 17.7% (161/910), 31.6% (87/275) 를나타내었다. 과배란유도에서각각의유전자형에따른난소반응과임상결과는 Table 3에요약하였다. 각군간에대상환자의나이는통계적으로유의한차이가없었으며, 난소반응에관련된사용한난포자극호르몬의용량과이에따른혈중에스트라디올의농도와채취된난자의수등에서도통계적으로유의한차이가나타나지않았다. 과배란유도과정에서에스트라디올농도가 3000 pg/ml 이상을나타낸고반응군과 1000 pg/ml 이하를나타낸저반응군의빈도는 TT/NN 유전자형에서각각 26.3% 와 19.6% 그리고 TA/NS 유전자형에서각각 25.2% 와 12.6% 로 %

6 난포자극호르몬수용체의유전자형과임상결과와의관련성 대한생식의학회지 련성이있다는결과도보고되고있다. 10 임상적으로난소반응을정확하게예측할수있다면, 과배란유도의진행방법과난포자극호르몬의초기투여량결정에많은도움이될수있다. 근래에난포자극호르몬수용체의유전자형과난소반응과의관련성에대한연구가활발하게진행되고있다. 몇몇의연구들에서난포자극호르몬수용체의유전자형에따라생리주기와난포자극호르몬에대한반응정도가상이하게나타남을보고하고있으며, 14~18 과배란유도과정에서의난소과자극증후군과의관련성에대해서도가설을제시하고있다. 20 그러나 Sudo 등과 Klinkert 등에의해수행된난구세포에서의난포자극호르몬에대한직접적인연구결과에서는각각의유전자형에따른 camp 농도의변화에는차이가없음이보고되었다. 21,22 이에반해서독일의 Simoni 교수연구팀의다양한연구결과들에서는임상적으로난포자극호르몬수용체의유전자형에따라난소반응이차이가있음이지속적으로보고되고있으므로, 23,24 보다명확한결론을내리기위해서는이에대한추가적인연구가필요할것으로생각된다. 난포자극호르몬수용체의유전자형에따른난소의반응은분자적측면의여러가설들로설명되고있다. 유전자형에세포표면의수용체의친화성과활성도그리고신호전달과정에서변화가나타날수있고, 이는유전자발현과인산화조절을포함한주변의여러가지변화를유발할수있다. 25~27 특히본연구에서의난포자극호르몬수용체의유전자형에따라변화되는아미노산인아스파라긴은해당과정을통해수용체의발현이촉진되지만, 세린은인산화과정을촉진하는등수용체의이러한변이가표적세포의호르몬활성에영향을미칠수있다는연구결과들과관련이있을수있다. 25 근래의난포자극호르몬수용체의유전자형빈도에대한연구들에서지역과인종에따라그빈도가상이하게나타나고있음을 Klinkert 등의연구에서보고하였다. 22 대표적인유전자다형성을나타내는 680번째코돈의유전자형에서유럽인들에 서는 Asn/Asn의빈도가 16~38% 를 Ser/Ser 의빈도가 17~39% 를나타내는반면, 동양의일본인에서는 Asn/Asn 빈도가 41%, Ser/Ser의빈도가 12% 로보고되었다. 21 국내에서 2004년에보고된 172명을대상으로한연구에서는 Asn/Asn 빈도가 42%, Ser/ Ser의빈도가 11% 로보고된바있다. 28 본연구에서도 Asn/Asn 빈도가 45%, Ser/Ser 의빈도가 11% 로나타나일본인과기존의국내연구와유사한빈도를보여주고있다. 이상의결과들을요약해보면동양인에서유럽인에비해상대적으로 Asn/Asn 빈도가높고, Ser/Ser의빈도가낮음을알수있다. 국내에서수행된난포자극호르몬수용체유전자형에따른과배란유도와체외수정시술에서의임상결과에대한연구에서 Asn/Asn 유전자형의환자들에서 Ser/Ser 유전자형에비해통계적으로유의하게많은수의채취난자와높은임상적임신율을보고되었다. 28,29 그러나최근네덜란드에서수행된연구에서는 Ser/Ser 유전자형의환자들에서 Asn/ Asn 유전자형에비해통계적으로유의하게높은임신율과착상률을나타내고있다. 21 본연구에서는네덜란드의결과와유사하게 Ser/Ser 유전자형에서통계적으로유의하게높은착상률을나타내었다. 이렇게상이한결과에대해서는향후추가적인연구를통해보다명확한결론을제시해야될것으로생각된다. 결론적으로본연구에서는비교적많은수 (n= 1,020) 의한국여성에서난포자극호르몬수용체의유전자형빈도를확인하였으며, 대표적인세가지 TT/NN (44.80%), TA/NS (41.96%) 그리고 AA/SS (10.49%) 유전자형에따른과배란유도과정에서난소반응양상의비교에서는통계적으로의미있는결과를얻지는못하였다. 그러나각각의유전자형에따라난소반응과임상결과가다소차이가나타남을확인할수있었으며, 이에대한추가적인연구를통해한국불임여성의난포자극호르몬수용체유전자형에따른적절한과배란유도방법개발하고, 보다명확하게임상결과와의관련성을밝히고자한다

J Hum Genet 2006; 51: = 국문초록 = 목적 : 본연구에서는한국불임여성의난포자극호르몬수용체유전자형의분포빈도를파악하고, 체외수정시술에서난포자극호르몬수용체유전자형에따른과배란유도과정에서의난소반응과각종임상결과와의관련성을알아보고자하였다. 연구방법 : 연구대상은제일병원에내원한여성불임환자 1,020명을대상으로난포자극호르몬수용체의유전자형, 즉 Thr307Ala (T/A) 과 Asn680Ser (N/S) 을조사하였으며, 이들중에서과배란유도와체외수정및배아이식술을시행한 302주기에서의임상결과를이들의유전자형에따라비교하였다. 결과 : 대상환자에서난포자극호르몬수용체의 Thr307Ala와 Asn680Ser의유전자형빈도는 TT/NN 군이 44.80% (n=457), TA/NS 군이 41.96% (n=428) 그리고 AA/SS군이 10.49% (n=107) 의분포로조사되었다. 과배란유도에서의각유전자형에따른대상환자의나이, 난소반응에관련된사용한난포자극호르몬의용량과이에따른혈중에스트라디올의농도와채취된난자의수등에서통계적으로유의한차이가나타나지않았다. 그러나배아이식후 AA/SS 유전자형환자에서의착상률이 TT/NN에비해통계적으로유의하게높게나타났다 (24.5% vs 15.7%, p<0.05) 결론 : 대표적인세가지 TT/NN, TA/NS 그리고 AA/SS 유전자형에따른과배란유도에서의난소반응은통계적으로의미있는차이를나타내지않았다. 그러나각각의유전자형에따라난소반응과임상결과가다소차이가나타남을확인할수있었으며, 이에대한추가적인연구가필요할것으로생각된다. 중심단어 : 난포자극호르몬수용체, 유전자형, 과배란유도, 난소반응, 임상결과
