Method: Among 116 patients, 43 were received assisted reproductive technologies using natural ovulatory cycle, 38 and 35 were received clomiphene citrate only protocol and clomiphene citrate/gnrh antagonist combined protocol, respectively, and the clinical results were compared and analyzed. Results: In each group, basal levels of LH, FSH, E2 and FSH, E2 on hcg day injected were not different, but LH level and endometrial thickness on hcg injected day were decreased significantly and the pregnancy rate was increased significantly in clomiphene citrate/gnrh antagonist group. Conclusion: The pregnancy rate was increased significantly in clomiphene citrate/gnrh antagonist group compared with natural ovulatory cycle and clomiphene citrate only group. Key Words: Clomiphene citrate, GnRH antagonist 불임의원인에대한많은연구를통해남성인자, 난소기능의감소, 비기능성배란, 난관의이상, 복강인자, 자궁경부혹은면역학적요인, 자궁의이상, 전신질환등다양한요인들이밝혀지고있으나 1 아직 10~15% 는불임의원인을알수없다. 2 이중자연배란이되면서불임이되는경우가 60~ 70% 에달하므로이런불임환자에서인공수정술을시행하기위해서는정상배란주기에서과배란을유도하는경우가흔하다. 과배란을유도하는데사용되는 clomiphene citrate 는비스테로이드성선택적에스트로겐수용체조절 제 (nosteroidal selective estrogen receptor modulator) 로서 3 사용법이간단하고주사제가아닌내복약으로서비침습적이기때문에 20여년간난포자극호르몬과함께인공수태시술에서배란유도제로널리쓰이고있다. 4~6 Clomiphene citrate의작용은에스트로겐의촉진제와길항제의작용을부분적으로나타냄으로서시상하부에스트로겐수용체에작용한다. 7,8 시상하부는 clomiphene citrate의신호에의해체내에스트로겐을저농도로간주하여시상하부에서성선호르몬분비자극호르몬의박동성분비를증가시키고이에따 주관책임자 : 이우석, 우 ) 서울특별시동작구흑석동 224-1, 중앙대학교부속병원산부인과학교실 Tel: (02) , Fax: (02) , oblws@hanmir.com

2 정상배란주기의불임여성에서 Clomiphene Citrate 단독사용과 GnRH Antagonist 병합요법의임상적효용성 라뇌하수체에서황체호르몬과난포자극호르몬의농도가증가하게되어난소의과자극상태를유도한다. 9 또한황체호르몬의고농도상태는황체기에황체를보존하는역할을하므로불임의원인중하나인황체기결함 (luteal phase defect) 을교정할수도있다. 10~12 한편으로인공수태시술에서 clomiphene citrate를사용하는경우에배란율은높으나임신율이낮게나타나는데, 13~15 이는 clomiphene citrate가에스트로겐의존성조직인자궁내막과자궁경부에서항에스트로겐효과를가지기때문으로생각되며난소에서는난포의성숙을방해하여황체의파열을저해하기때문으로여겨진다. 16 또한 clomiphene citrate 에의해야기된황체호르몬의고농도상태로인하여인공수태시술시수정률이낮아지고 17~18 초기태아손실이야기될수도있다. 19 GnRH agonist는 clomiphene citrate의사용으로야기되는조기황체호르몬의급증 (premature LH surge) 과미성숙난자의배란등을막기위해사용되었다. GnRH agonist 장기요법은조기황체화현상을효과적으로예방할수는있으나뇌하수체의탈감작을유도하기위해미리 GnRH agonist를투여하여야하는데, 투여초기에불필요한성선자극호르몬상승을유도하게되며약 2주간의긴투약기간으로인해환자의신체적, 정신적, 경제적부담이문제가되었다. 20 또한과배란증후군과다태임신등의다른부작용도무시할수없다. 21 이러한부작용이대두됨에따라최근에는난소를과다하게자극시키던기존의방법대신, 자연배란주기에외인성인간폐경성선자극호르몬 (hmg) 또는난포자극호르몬을투여하여일부난포들의성장을발달시키는한편내인성난포자극호르몬은억제되지않도록하여외인성성선자극호르몬의필요량을줄이고, 약물투여기간을단축시킬수있는최소자극방법 (minimal stimulation protocol) 이소개되었다. 22 이에남성불임원인을제외한정상배란주기의불임환자에서임신율을향상시키기위해 clomiphene citrate를단독투여하는방법과 clomiphene citrate와 GnRH antagonist를병합하는용법을시행하여임신의효율성을비교검토하여유용성을보 고자본연구를시행하였다. 연구대상및방법 1. 대상환자본연구는 2002년 2월부터 2005년 7월까지중앙대학교부속병원산부인과불임클리닉을방문하여불임시술을받은환자들중남성불임의원인이아닌정상배란주기를가진 116명 (154주기) 의불임환자를대상으로시행하였다. 43명 (53주기) 에서는 clomiphene citrate를쓰지않고자연주기에의한임신을시도하였고, 38명 (58 주기 ) 에서는 clomiphene citrate를사용하여임신을시도하였으며, 35명 (43주기) 에서는 clomiphene citrate 와 GnRH antagonist를병합하여임신을시도하여이들을대상으로임상적결과를비교분석하였다. 2. 연구방법 1) 자연배란주기법군 (natural ovulatory cycle group) 대상환자들은월경제 2~3일에혈중 E 2, LH, FSH 농도를측정하였다. 배란이되는날을예측하기위해질식초음파를통해난포의발달과자궁내막의두께를측정하였고, 우성난포 (dominant follicle) 의지름이 16~18 mm일때배란을유도하기위해 hcg 5,000 IU를근주하였다. 2~3일후 IUI (intrauterine insemination) 를시행하였다. 2) Clomiphene citrate 단독요법군 (Clomiphene citrate only group) 자연주기법과마찬가지로대상환자들은월경제 2~3일에혈중 E 2, LH, FSH 농도를측정하였고월경주기제 5일부터제 9일까지 clomiphene citrate ( 파마구연산클로미펜정, 한국파마, 50 mg/tablet) 100 mg을매일경구투여하였다. 역시배란이되는날을예측하기위해질식초음파를통해난포의발달과자궁내막의두께를측정하였고, 우성난포가 16 ~18 mm일때배란을유도하기위해 hcg 10,000 IU 를근주하였으며 2~3일후 IUI를시행하였다

3 3) Clomiphene citrate/gnrh antagonist 병합요법이군 (Clomiphene citrate/gnrh antagonist group) 다른군의환자들과마찬가지로대상환자들은월경제 2~3일에혈중 E 2, LH, FSH 농도를측정하였고월경주기제 5일부터제 9일까지 clomiphene citrate 100 mg을매일경구투여하였다. 역시배란이되는날을예측하기위해질식초음파를통해난포의발달과자궁내막의두께를측정하였고, 우성난포가 16 mm일때 LH surge 를예방하기위해 GnRH antagonist인 cetrorelix (Cetrotide, Serono International, Germany) 0.25 mg을일회피하주사하였다. 이후우성난포가 18 mm일때배란을유도하기위해 hcg 10,000 IU를근주하였으며 2~3일후 IUI를시행하였다. 3. 통계적분석각군에서우성난포가 16 mm 이상이되어 hcg 를투여하는날의 E 2, LH, FSH와자궁내막의두께를측정하였고, 각군의임신율과유산율을조사하였다. 통계처리는 SAS (version 6.04, SAS-KOREA, Seoul, 1995) 통계프로그램을이용하였다. 모든측정치는평균 ± 표준편차로표시하였고, 자료의평균치는 Student-t 로비교검정하였으며, 유의수준은 p 0.05인경우를통계학적의의가있는것으로판정하였다. 결과 1. 각군의특성및호르몬수치대상환자의불임원인은자궁내막증, 난관인자, 불명확한원인으로분류되었으며각군간에유의한차이는없었다 (Table 1). 각각의군에서시술의시 행주기는자연배란주기법군에서 53주기, clomiphene citrate 단독요법군에서 58주기, clomiphene citrate/gnrh antagonist 병합요법군에서는 43주기였고평균나이는자연배란주기법군에서 32.4±3.6 세, clomiphene citrate 단독요법군에서 30.7±4.5세, clomiphene citrate/gnrh antagonist 병합요법군에서는 32.4±5.8세로모두유의한차이를보이지않았다. 불임의기간은각각자연배란주기법군에서 3.7± 2.6년, clomiphene citrate 단독요법군에서 3.6±3.4 년, clomiphene citrate/gnrh antagonist 병합요법군에서는 3.7±2.9년으로의미있는차이를보이지않았다. 각군에서월경 2~3일째측정한혈청 E 2, LH 및 FSH 수치는각군간에통계적으로유의한차이를보이지않았다 (Table 2). 2. 임상결과우성난포의크기가 16~18 mm로발달되어배란을위해 hcg를투여하는날짜는자연주기법군과 clomiphene citrate 단독요법군과 clomiphene citrate/ GnRH antagonist 병합요법군에서각각생리주기의 12.4±8.7일, 11.6±8.3일, 12.6±6.9일이었고통계적으로의미있는차이를보이지않았다. hcg를투여하는날의혈청 E 2 수치는자연배란주기법군에서 186.6±34.6 pg/ml, clomiphene citrate 단독요법군에서 284.6±57.8 pg/ml, clomiphene citrate/ GnRH antagonist 병합요법군에서는 275.5±45.3 pg/ ml로유의할만한차이가나타나지않았으나혈청 LH 수치는자연배란주기법군에서 17.5±3.9 U/L, clomiphene citrate 단독요법군에서 16.5±11.5 U/L, clomiphene citrate/gnrh antagonist 병합요법군에서는 14.9±10.4 U/L로 clomiphene citrate/gnrh anta- Table 1. Comparison of clinical responses Natural cycle CC only CC/GnRH antagonist p value hcg day (mensturational day) 12.4± ± ±6.9 NS E 2 on hcg day (pg/ml) 186.6± ± ±45.3 NS LH on hcg day (U/L) 17.5± ± ±10.4 <0.01 FSH on hcg day (U/L) 12.6± ± ±6.9 NS Endometrial thickness on hcg day (mm) 13.5± ± ±5.8 <0.05 Pregnancy cycle (%) 2/53 (3.77%) 3/58 (5.17%) 4/43 (9.30%) <0.01 No. of abortion No. of multiple pregnancy (twin) NS: not significant, CC: Clomiphene citrate, hcg: human chorionic gonadotropin, LH: Luteinizing hormone, FSH: Follicular stimulating hormone gonist 병합요법군에서의미있게저하되었다. 반면 FSH는자연배란주기법군에서 12.6±8.7 U/L, clomiphene citrate 단독요법군에서 11.6±8.3 U/L, clomiphene citrate/gnrh antagonist 병합요법군에서는 12.9±6.9 U/L로유의한차이를보이지않았다. 자궁내막의두께는자연주기법군에서 13.5±7.9 mm, clomiphene citrate 단독요법군에서 10.4±8.6 mm, clomiphene citrate/gnrh antagonist 병합요법군에서는 9.5±5.8 mm으로 clomiphene citrate를사용한두군에서의미있게감소하는것을관찰할수있었다. 각군에서주기당임신율은자연주기법군에서 53주기중 2명으로 3.77%, clomiphene citrate 단독요법군에서 58주기중 3명으로 5.1%, clomiphene citrate/gnrh antagonist 병합요법군에서는 43주기중 4명으로 9.30% 를보여 clomiphene citrate/gnrh antagonist 병합요법군에서통계적으로유의하게높은임신율을보였다. 유산이된경우는 clomiphene citrate 단독요법군에서 1례, 쌍태임신이 clomiphene citrate/gnrh antagonist 병합요법군에서 1례가관찰되었다. 난소과자극증후군등의합병증은어떤군에서도나타나지않았다 (Table 3). 고찰불규칙적인생리주기를가진불임여성에게 clomiphene citrate는생리주기를규칙적으로유도하여배란일자를예측하기쉽게만들수있어유용하며비용이저렴하고사용하기쉬울뿐만아니라주요전신부작용이없기때문에무배란성, 희소월경성

5 불임에있어서우선적인치료제이다. 12 최근에는정상적인배란주기를가진불임여성에게도다수의배란을유도하여임신의가능성을높이기위해 clomiphene citrate가사용된다. 19 그러나저성선자극성성선기능저하증 (hypogonadotropic hypogonadism) 시에는효과적이지못하며당뇨병, 갑상선이상, 고프로락틴증, 선천성부신증식증등의내분비적질환을먼저감별해야하며질환이있다면선행질환을우선적으로치료하고이러한치료로도무배란이호전되지않을때 clomiphene citrate를사용해야한다. 또한황체기결함이관찰되는경우 clomiphene citrate를투여하여프로게스테론의수치를상승시킬수있으며이로인하여배란전난포와황체의발달을촉진시킬수있다. Clomiphene citrate를투여했을경우전반적인생리주기에황체호르몬의상승이일어난다. 이런황체호르몬의상승은성선호르몬분비자극호르몬유사체를투여하여도나타난다. Clomiphene citrate는우선적으로시상하부의에스트로겐수용체에작용하며수시간동안작용하는에스트로겐에비하여 clomiphene citrate는수주동안효과를나타낸다. 21 이러한장기간의작용효과로체내의황체호르몬은높은농도로증가하여황체를유지하는데중요한역할을하게되며또한난포자극호르몬분비의파동과빈도가증가하고이에따라에스트로겐과프로게스테론이증가하게된다. Clomiphene citrate을사용하는경우배란의성공률은대략 57~91% 로추정되나임신율은 27~40%, 유산율은대략 25% 에이른다. 23 본연구에서 clomiphene citrate 단독요법군과자연주기법군의임신율에는유의할만한차이가관찰되지않았다. FSH/hMG을이용하는자궁내정자주입술이전에 clomiphene citrate를이용한자궁내정자주입술을시행한횟수가 4회이상을넘으면임신율이저하되고고위험-다태임신의빈도가줄어든다는보고가있다. 24 이러한결과에대한해석으로서첫째, clomiphene citrate는자궁경부점액의질적, 양적감소를야기한다. 22,24 둘째, clomiphene citrate에의해 estradiol 수치가증가해도자궁내막의두께는얇아지고내막의성장속도가느려진다. 25,26 본연구에서도자연 주기군에비하여 clomiphene citrate를사용하는단독요법군과 GnRH 병합요법군에서모두자궁내막의두께가유의하게감소하는것을관찰할수있었는데이는 clomiphene citrate가가지는항에스트로겐작용에의한것으로여겨진다. 자궁내막의적절한두께는착상에필수적인요소로서자궁내막의두께가 9~10 mm 이상인경우착상율은증가되고 6~8 mm 이하시는착상이거의불가능하다고알려져있다. 27,28 자궁내막두께감소를방지하기위해서는 tamoxifen이나 letrozole로대체하여사용할수있다. 20 셋째, 난포가파열이되지않거나늦어질수있다. 한연구에서 clomiphene citrate 150 mg을복용한그룹에서는높은빈도의난포비파열이보고되었으며 15 난포기의기간이길어져배란일이늦어지는현상이나타났다. 8,29 그외에도이전까지의연구에서염색체이상이나임상적인증상이보이지않는초기유산율의증가가높은빈도로나타난다고보고된바있다. 30,31 본연구에서도초기유산이 clomiphene citrate 단독요법군에서 1례관찰되었다. Clomiphene citrate의항에스트로겐효과를상쇄하기위해함께에스트로겐을투여하였을경우 clomiphene citrate를단독투여한것과비교하여체내에스트로겐의농도, 난포의크기와수, 자궁의용적등은변화하지않는것으로보고된다. 23 이는 clomiphene citrate가에스트로겐수용체에결합하여에스트로겐에경쟁적으로작용하기때문이다. 그러나에스트로겐을투여하였을경우자궁내막두께의증가를보이며이로인해유산율의감소를가져올수있다고보인다. 32 이는소위 'differential cell stimulation' 으로 clomiphene citrate 자체가자궁내막에서는자궁상피세포의증식을유도하나자궁의근층에서는별다른효과를야기하지않기때문이다. 33 Clomiphene citrate의이러한항에스트로겐효과는자궁내경부, 자궁내막, 난소와같은장기의말단부위에영향을미칠수있다. Clomiphene citrate의전신적인부작용은경미한편이고열감, 우울증, 눈부심이나반점, 복시등의시야이상등이드물게나타날수있으며심한경우시신경병증을야기하기도한다. 또한다태임신이증가되는경향이있으나선천성기형, 자연유산,

6 정상배란주기의불임여성에서 Clomiphene Citrate 단독사용과 GnRH Antagonist 병합요법의임상적효용성 난소과자극증후군과의연관성은보고되지않았다. 20 또한조기황체호르몬증가와 12주기이상사용하는경우난소암발생의증가가보고된연구도있었다. 34 현재까지의연구결과 clomiphene citrate는인공수태시술에서배란유도를위해널리쓰이고있으나그것이지닌항에스트로겐효과로인하여정상적인배란주기를가진여성에서는그사용의유용성은미미한것으로여겨진다. 즉정상적인배란주기를가진불임여성이 clomiphene citrate를복용할경우전체적인임신율, 각배란주기에따른임신율, 각환자의임신율등이 clomiphene citrate를사용하지않은여성보다감소하거나차이를보이지않는다. 12 Clomiphene citrate의사용에따르는임신율의저하와부작용으로인해도입된것이 GnRH agonist이다. GnRH agonist를사용하여뇌하수체의하향조절 (down regulation) 유도를통한조기 LH 급증및미성숙난자의배란현상을예방할수있게되었으며다수의성숙난자획득이가능해졌다. 그러나 GnRH agonist의이용에는장기간투여에따르는환자의경제적, 신체적, 심리적부담을유발하였을뿐만아니라난소과자극증후군의빈도가증가되었고, 과배란이후발생하는정신적스트레스, 복부불쾌감및다태임신과관련된산모와태아의합병증등에따른문제가제기되었다. 22 이러한부작용으로인하여이전의자연생리주기또는 clomiphene citrate 혹은성선자극호르몬을이용한 'minimal stimulation protocol ( 최소자극방법 )' 의개념이도입되었는데, 이는자연생리주기에호르몬을이용하는경우보다는덜침습적이고비용이적게든다는장점이있는반면, 주기취소율이높고, 황체형성호르몬급증시간을예측할수없다는단점이있다. 그러나최근개발된제 3세대 GnRH antagonist를후기난포기에사용하는경우, 난포기에서난포형성의억제가발생하지않으며, 조기황체형성호르몬급증을예방하고주기취소율을감소시킬수있었다. 35 GnRH antagonist는 GnRH agonist와는달리뇌하수체의성선자극호르몬수용체에직접작용하여, 성선자극호르몬의일시적인증가없이초기부터분비를억제하고, 수용체의탈감작이일어나지않기때문에 GnRH antagonist의작용이끝난후에즉시정상적인기능을회복하게된다. 또한 GnRH antagonist를이용함으로써외인성성선자극호르몬의용량및치료기간을줄여난소과자극증후군의발달위험도감소시킬수있으며기존의장기요법시관찰되는안면홍조, 수면장애, 두통등과같은저에스트로겐으로인한부작용도피할수있을것으로생각된다. 20 또한저반응군에서경제적부담을줄이고기존의성선자극호르몬유도체병합용법과유사한임신율을보여저반응군환자들에게새로운치료법으로이용되고있다. 36 GnRH antagonist를이용할경우, 전처치즉 suppression phase가필요하지않고다양한과배란유도방법이가능하다. 많은부부들이불임으로인해고통받고있으며인공수정시술등의치료를받고있으나실제로인공수태시술이성공하는경우는 15~20% 에지나지않는다. 11 현재불임치료에가장널리쓰이는약제인 clomiphene citrate는배란이상과관련된불임을가진여성에게초기치료에일차선택약제이며자궁내정자주입술과병행할때수정력을더욱증가시킬수있다. 그러나만약 clomiphene citrate의사용을 6주기이상시행해도임신이되지않는다면치료를중단하고불임의원인에대한보다심도있는조사를해야할것이다. 또한 clomiphene citrate와 metformin, 스테로이드, 외인성성선자극호르몬을사용하는것이효과적일수도있다. Clomiphene citrate 로치료받는여성은기초체온, 혈청내프로게스테론의농도, 소변의프로게스테론배출정도를감시하여그효과를관찰할수있을것이다. 본연구에서는자연배란주기를이용하는방법, clomiphene citrate를단독사용하는방법, 그리고 clomiphene citrate와 GnRH antagonist를일회병합하여사용하는방법에대한임신율을조사하여각각의임신율을비교분석하였다. 그결과 clomiphene citrate 단독사용법은자연배란주기법과의미있는차이를관찰할수없었으나 clomiphene citrate/gnrh antagonist 병합요법군에서는통계적으로유의하게높은임신율을보였다. 따라서, clomiphene citrate/ GnRH antagonist 병합요법이정상배란주기를가진

7 불임환자에서임신율을향상시킬수있을것이라는결과를얻었다. 한편, GnRH antagonist를반복해서사용하는경우의임상적결과에대한연구와 GnRH antagonist 자체가난포의성장이나자궁내막에미치는부작용에대한것은추가적인연구가필요할것으로생각된다. 참고문헌 1. Jonathan SB, Rebecca DR, Paula J, Eli Y. 13th ed. 9 = 국문초록 = 목 적 : 본연구는인공수정시술에있어서정상배란주기의여성에서 clomiphene citrate 와 GnRH antagonist 를병합하여임신을시도한방법을기존의 clomiphene citrate 단독으로사용한방 법과비교하여그임상적유용성을알아보고자하였다. 연구방법 : 116명의환자들중 43명에게는자연배란주기를이용하여인공수정시술을시행하였고, 38명과 35명에게각각 clomiphene citrate 단독요법과 clomiphene citrate/gnrh antagonist 병합 요법으로인공수정시술을시행하여그임상결과를비교분석하였다. 결과 : 각각의군에서기저 LH, FSH, E 2 와배란유도를위해 hcg를투여하는날의 FSH, E 2 는차이를보이지않았으나 clomiphene citrate/gnrh antagonist 병합요법군에서 hcg를투여한 날의 LH와자궁내막두께는의미있게감소하였으며유의한임신율의증가가관찰되었다. 결론 : 자연배란주기법군, clomiphene citrate 단독요법사용군에비해 clomiphene citrate/ GnRH antagonist 병합요법사용군에서유의하게임신율의향상을나타냈다. 중심단어 : Clomiphene citrate, GnRH antagonist
