The investigators postulated that ionizing irradiation could be applied to prevent restenosis caused by tissue hyperplasia in non-vascular luminal organs as it reduced coronary or peripheral arterial narrowing successfully. The authors combined β-irradiation using 188 Re-MAG 3 solution with balloon dilation for animal and clinical studies because this new treatment approach had the advantages such as low penetration depth of β-ray, self-centering irradiation, and mechanical effect of balloon dilation over using γ-irradiation with afterloading devices. In this article, the concept and mechanism of radioisotope balloon dilation, and animal and clinical studies using radioisotope balloon dilation are reviewed. (Nucl Med Mol Imaging 2006;40(2): ) Key Words : radioisotope application, stents and prostheses, balloon dilation 비혈관성 관강 장기는 식도, 기도, 요도, 및 담도 등과 같 은 혈관 외의 관강 장기를 지칭한다. 협착의 원인은 식도암, 기관지암, 또는 담도암과 같은 악성 종양에 의해 좁아지는 악성 협착과 수술, 외상이나 염증, 또는 스텐트(stent) 설치 후에 생기는 이차적인 자극에 의해 반응성으로 나타나는 조 직과증식(tissue hyperplasia)에 의해 좁아지는 양성 협착으 로 나눌 수 있다. 비혈관 관강 장기의 악성 협착은 장기에 따라 치료 방향 이 다르기는 하나 항암화학요법과 방사선 치료가 주된 치료 방법이라고 할 수 있다. 그러나 즉각적인 관강의 개통성 (patency)이 필요할 경우 스텐트 설치는 간편하면서도 안전 하게 쓰일 수 있는 시술이라고 할 수 있다. 반면 비혈관 관강 장기의 양성 협착은 스텐트의 사용이 Received: Accepted: Address for reprints: Ji Hoon Shin, M.D., Department of Radiology and Research Institute of Radiology, University of Ulsan College of Medicine, Asan Medical Center, #388-1, Pungnap-2dong, Songpa-gu, Seoul , Korea Tel: , Fax: jhshin@amc.seoul.kr 늘면서 스텐트 철선의 자극으로 인한 반응성의 조직과증식 이 주원인이며 식도 스텐트의 경우 비피복스텐트(bare stent)의 경우 50-60%까지, 피복스텐트(covered stent)의 경 우 0-13%까지 스텐트 설치 후 조직과증식이 생기는 것으로 보고되고 있고, 1,2) 요도 피복스텐트의 경우에도 33%까지 스 텐트와 관련된 조직과증식이 보고되었다. 3,4) 이러한 조직과 증식은 스텐트의 개통성을 떨어뜨려 스텐트를 추가로 설치 하거나 스텐트 제거를 불가피하게 할 수 있으며 따라서 이를 해결하려는 여러 가지 노력이 있었다. 2,3,5) 조직과증식 외에도 약물, 염증, 또는 외상과 관련된 양성 협착으로는 부식성 식도 협착, 결핵성 기도 협착, 그리고 외상 성 요도 협착이 대표적이라고 할 수 있다. 이와 같은 여러 장 기의 양성 협착의 경우 수술이 불가능할 경우 또는 수술적 방 법의 대용으로 풍선확장술과 스텐트 설치술이 쓰일 수 있는데 기존 풍선확장술은 재발률이 비교적 높은 것으로 알려져 있으 며 스텐트 설치술은 전술한 조직과증식의 문제점이 있다. 조직과증식 또는 기타 여러 가지 원인에 의한 비혈관 관 강 장기의 양성 협착은 동맥협착이나 켈로이드(keloid)와 같 은 과증식성 상태임을 가정하고 동맥협착이나 켈로이드 (keloid)에서 방사선 치료의 항증식성 특징은 비혈관 관강 106

2 신지훈. 비혈관성 관강 장기의 양성 협착 질환의 방사성동위원소 치료 장기의 조직과증식 억제에도 적용이 가능할 것이다. 6-8) 실제로 임상적으로 기존 풍선확장술 및 스텐트 설치술의 한계점을 극복하기 위해 방사선 치료를 양성 협착의 치료에 이용하려는 시도가 기도 및 요도에서 있었으나 9-12) 이는 모 두 γ-선인 192 Ir(Irridium-192)을 이용한 국소적 방사선 치료 방법이다. 저자는 혈관에서 쓰이는 β-선을 이용한 방사성동 위원소 풍선확장술을 이용해서 양성 협착을 줄이고자 하는 실험 연구와 이를 적용한 임상 연구를 저자들의 연구를 중심 으로 소개하고자 하며 따라서 아직 확실한 치료 방법으로 정 립된 단계는 아니라고 할 수 있다. 방사성동위원소 풍선확장술의 작용 기전 의 처리가 간편한 장점이 있다. 다른 β-선과 마찬가지로 188 Re 은 선원으로부터 2-3mm까지 투과하며 10mm 이상 투과하는 것은 거의 없어서 적은 투과력의 장점을 가지며 이는 환자와 술 자에 피폭 및 주변 정상 조직의 손상을 최소화할 수 있으며 액 상 형태로 존재하므로 풍선확장술과 병행하여 쓸 수 있다. 7) 관 상동맥의 경우에 최근 대표적인 두 논문에서 0.5-mm 조직 깊이에서 Gy 범위의 188 Re-mercaptoacetyltriglycine ( 188 Re-MAG 3)를 채운 풍선확장술이 6개월 추적 검사에서 관상동맥 재협착을 36% 감소시킨 것을 보고하였다. 22,23) 방사성동위원소 풍선확장술: 동물 실험을 통한 유효성 평가 동맥에서 방사선은 풍선확장술인 혈관성형술이나 스텐트 치료에 대한 세포증식 반응을 억제하며 그 기전으로는 세포 비활성화, 클론형성세포사(clonogenic cell death), 방사선 유 도 아폽토시스(apoptosis, 세포자멸사), 세포 외 매질 합성의 감소, 세포 이동의 장애, TGF(transforming growth factor)- 의 하향조절 등에 의한 것으로 알려져 있다 ) 이러한 방 사선의 작용 기전이 비혈관 관강 장기에서 스텐트 설치 후 이차적으로 생기는 조직과증식에도 적용되리라 가정하는데 이는 조직과증식이 조직학적으로 신생모세혈관의 증식, 수 많은 섬유아세포들과 염증 세포들 및 신생 혈관 증식으로 구 성된 결체 조직이고 여기에 내피 세포의 분열이 동반되면서 만성 염증에 의한 반응으로 세포들의 숫자가 증가하게 되는 것이기 때문이다. 16,17) 즉 혈관에서 스텐트 시술 후 협착이 생 긴 혈관 벽에 방사선을 조사하여 혈관 평활근세포의 증식 및 신생내막의 형성이 억제되어 재협착을 방지한다는 원리가 비혈관 관강 장기에서도 적용될 수 있다는 것이다. 혈관의 경우에 이러한 방사선의 치료 원리를 스텐트 설치 후의 재협 착을 방지하려는 시도로 1990년대 중반부터 임상에 사용되 기 시작하였다. 18) 초반부의 보고는 대개 192 Ir와 같은 γ-선 선원을 이용한 afterloading 방식의 국소적 방사선 치료였다. 여러 가능성 있는 기전 중 아폽토시스는 프로그램된 세포사 (cell death)를 말하며 방사선을 포함한 여러 자극에 의해 개 시되는 것으로 알려져 있다. 혈관 내피세포, 각막 색소상피 와 rat colonic crypt cell lines에서 방사선 유발 아폽토시스가 확인되었으며 19-21) 본 저자들은 토끼 식도에서도 상피세포와 점막하 조직에 방사선에 의해 유발된 아폽토시스를 확인하 여 조직과증식 억제 효과와 관련이 있는 것을 알 수 있었다 (submitted data). 반면 188 Re(rhenium-188)은 β-선을 방출하는 동위원소로 서 반감기가 17시간으로 비교적 짧고, 사용 후 방사성동위원소 육안적 및 현미경적으로 스텐트와 연관된 조직과증식이 억제될 것이라는 생각으로 잡견의 요도 및 가토의 식도에 스 텐트 설치 후의 방사성동위원소 풍선확장술을 시행하여 기 존의 풍선확장술을 시행한 군과 비교하였다 ) Tungsten-188 ( 188 W)/ 188 Re generator로부터 188 Re-p errhenate 용액을 얻어서 합성기를 이용하여 MAG 3를 합성 하여 188 Re-MAG 3를 만들고 원하는 방사선활성도와 용적을 갖도록 농도를 조절한다. 그 후 조영제를 섞어 조영제의 비 율이 30%가 되도록 한다. 풍선 표면에서 원하는 거리에서 원하는 방사선량이 조사되도록 조사시간을 결정한다. 1. 잡견 요도에서의 실험 잡견 요도는 인체의 요도와 직경이 비슷하고 실험하기도 용이하여 비혈관 관강 장기의 실험으로 많이 쓰인다. 초기 실험으로 10 mm(직경)-20 mm(길이)의 폴리우레 탄 피복 스텐트를 총 18마리의 개의 요도에 넣은 후 풍선카 테터 표면에서 1 mm 거리에서 15 Gy를 조사하는 실험군을 조영제로 채운 풍선확장술인 대조군과 비교하였는데 추적 검사를 달리하여 총 네 군으로 나누어 4주 또는 8주의 추적 검사 후에 피복 스텐트의 안쪽 부위(in-stent)와 바깥쪽 부 위(out-stent)에서 상피세포층의 수 및 유두상 돌출부위의 두께에 중점을 두어 병리 검사를 시행하였다. 24) 결과적으로 잡견 요도에 스텐트 설치 후 방사성동위원소 풍선확장술(15 Gy/mm 조직깊이)은 조영제만으로 풍선확장술을 시행한 대 조군에 비해 조직과증식을 시사하는 소견인 상피세포층의 수 및 유두상 돌출부위의 두께가 모두 적게 나타났다. 또한 스텐트 삽입 직후 방사성동위원소 풍선확장술을 시행했을 때 4주 후 희생된 군보다 8주 후 희생된 군에서 조직과증식 을 시사하는 소견이 더 적게 나타났다. 후속 실험으로 추적 검사를 12주로 늘리고 방사선조사량 107

There is esophagotracheal fistula (arrowheads). 을 1 mm 조직 깊이에서 20 Gy와 40 Gy를 실험대상으로 하 여 조영제로 채운 풍선확장술인 대조군과 비교하여 각 군마 다 5마리씩의 잡견을 사용하여 총 15마리를 대상으로 하였 다. 역행성요도조영술 소견을 4주 후 및 희생 직전에 시행하 였으며 초기 실험과 같은 방법으로 병리 소견을 얻었다. 25) 결과적으로 역행성요도조영술은 세 군 간에 유의한 차이가 없었으나 병리 소견적으로는 20 Gy 및 40 Gy에서 대조군에 비해서 12주 추적 검사에서 조직과증식을 가장 잘 반영하는 소견인 유두상 돌출부위의 두께가 세 군 간에 유의한 차이를 보였으며 대조군에 비해 20 Gy 군에서는 유의한 차이를 보 여서 방사성동위원소 풍선확장술이 조직과증식을 줄이는데 효과적이라는 것을 알 수 있었다.(Fig. 1) 2. 토끼 식도에서의 실험 토끼 식도는 비피복 스텐트를 설치했을 때 스텐트 철선 사이로 조직과증식의 형성이 매우 왕성하여 비피복 스텐트 설치 후의 방사성동위원소 풍선확장술의 효과를 보기에 적 합한 장기라고 할 수 있다. 토끼 30마리의 식도에 12 mm(직 경)-20 mm(길이)의 스텐트 설치 후 10마리씩 3개 군으로 나누어 제1군에는 188 Re-MAG 3를 채운 풍선확장술을 시행하 여 1-mm의 조직 깊이에서 20 Gy를 조사하였고 제2군은 40 Gy를 조사하였으며 제3군은 기존의 풍선확장술만 시행하였 다(submitted data). 스텐트 및 풍선확장술 시행 후 모든 토 끼를 6주 후에 희생하는 것으로 하였다. 희생 직전 식도조영 술을 얻었고 스텐트가 들어가 있었던 부위에서 직경의 백분 협착률을 구하였다.(Fig. 2) 희생 후에 정상 부위와 스텐트 가 있었던 중간 부위에서 병리학적 소견으로 상피세포층의 두께 및 근육층의 파괴 정도를 분석하였다. 아폽토시스를 규 명하기 위해 각 군 당 두 마리의 토끼를 대상으로 TUNEL (terminal deoxynucleotidyl transferase-mediated deoxy- 108

Esophageal specimen (C) from group III (control group) shows marked nodular tissue hyperplasia between the wire meshes of the stent. uridine triphosphate nick end labeling) 방법을 써서 정상 부 위와 스텐트가 있었던 중간 부위에서 아폽토시스의 정도를 평가하였다. 스텐트 설치 후 스텐트 이동과 같은 합병증은 없었다. 13 마리는 예정대로 6주까지 생존하였다. 직경의 백분 협착률의 평균은 제1군에서 32.6%, 제2군에서 25.2%, 그리고 제3군에 서 46.5%를 보여 대조군인 제3군에 비하여 제1군이나 제2군 에서 보다 적은 직경의 백분 협착률을 나타내었다. 육안적으 로도 식도 점막은 제1군과 제2군에서는 밋밋하였으나 제3군 에서는 스텐트 철선 사이로 결절성 돌출을 보였다.(Fig. 3) 식도의 천공(n=10)이나 스텐트 인접 부위의 적색 점막 변 화(n=9)는 제1군과 제2군에서만 관찰되었다.(Fig. 3) 상피 세포층과 근육층은 제1군과 제2군에서 제3군과 비교하여 현 저한 손상을 보여 통계학적으로 유의한 차이를 보였 다.(p<0.05) (Fig. 4) 아폽토시스는 제1군과 제2군에서 상피 세포층 및 일부 근육층에서 제3군과 비교하여 뚜렷한 증가 를 보였다.(Fig. 5) 결과적으로 방사성동위원소 풍선확장술 은 비피복 스텐트 설치에 의해 형성되는 조직과증식을 줄일 수 있으며 그 기전의 하나로 아폽토시스가 관여한다는 것을 Fig. 4. However, the patient was free of symptoms. 알 수 있었다. 방사성동위원소 풍선확장술: 임상 시험 동물 실험을 통해 방사성동위원소 풍선확장술이 요도 및 식도의 조직과증식을 줄일 수 있다는 가설을 확인한 후 요 도, 기도, 및 식도에서 기존의 풍선확장술이나 스텐트 설치 술에 잘 반응하지 않는 재발성 양성 협착을 대상으로 하였 다. 현재 각 장기별로 많은 숫자를 대상으로 한 것은 아니며 이 중 요도에서의 임상 시험에 대해서 그 결과를 소개하고자 110

6 신지훈. 비혈관성 관강 장기의 양성 협착 질환의 방사성동위원소 치료 한다 (submitted data). 요도내 절개술(visual internal urethrotomy)이나 스텐트 설치술에 잘 반응하지 않는 요도 협착은 비뇨기과 영역에 있 어서 난치 분야 중의 하나이다. 요도내 절개술은 요도 협착 의 일차적 치료라고 할 수 있으나 2년 내에 요도 내 반흔 형 성으로 인해 38-82%의 비교적 높은 재발률을 보인다. 4,11,27) 기존에 192 Ir 선원을 이용한 두 보고에서 요도내 절제술 후 국 소적 방사선 치료 방법이 효과적일 수 있다는 치료 결과를 바탕으로 본 임상 연구를 시행하였는데 방사성동위원소 풍 선확장술은 방사선 치료의 기능적 효과 외에 풍선확장술이 라는 기계적인 효과를 겸할 수 있고 관강 내에 균일하게 방 사선을 조사할 수 있는 장점을 가진다. 고농축된 188 Re-표지 방사성의약품의 가장 큰 장점은 비교적 짧은 시간 내에 방사 선 치료를 끝낼 수 있으며 요도의 경우 만약의 경우 방사성 동위원소가 체내에 유출되더라도 신속하게 소변으로 배출되 므로 환자에 대한 위험성을 최소화할 수 있다는 안정성에 있 어서 큰 장점이 있다. 총 5명의 외상성(n=4) 또는 수술 후 문합 부위(n=1) 협 착 환자를 대상으로 하였고 스텐트 설치나 요도내 절개술과 같은 치료 간격이 평균 2.2개월이었다. 1 mm 조직 깊이에서 20 Gy 또는 30 Gy의 조사량으로 치료하였다. 5명의 환자에 서 총 9번의 방사성동위원소 풍선확장술을 요도내 절개술 또는 기존 스텐트 제거 후에 시행하였고 시술 후 치료 간격 이 평균 10.7개월로 현저한 증가를 보였다. 16.2개월의 추적 관찰 중 2명의 환자에서는 더 이상 치료를 필요로 하지 않았 고,(Fig. 6) 3명에서는 재발하였다. 아직 초기 단계이고 다른 장기에서의 성적과 비교할 수 없지만 재발을 잘하는 요도 협착의 치료로 충분한 가능성이 있고 요도내 절개술 후 재협착을 막는데 효과적으로 보이며 이는 절개된 자리에서 반흔이 생기는 것을 막아주는 방사선 의 항증식 작용에 의한 것으로 여겨진다. 맺음말 방사성동위원소 풍선확장술은 혈관에서 스텐트 설치 후 스텐트 내의 재협착을 줄이기 위해 개발된 것으로 이를 비혈 관 관강 장기에 적용했을 때에도 조직과증식을 줄이는데 효 과적이라는 것을 알 수 있다. β-선을 이용한 방사성동위원소 풍선확장술의 장점은 요 약하면 첫째, 최대 투과 길이가 1 cm 이내로 짧아서 환자 및 시술자에 대한 피폭을 최소화할 수 있고, 둘째, 풍선확장술 과 병행하므로 풍선이 해당 관강에서 잘 펴져있다면 모든 방 향으로 균일한 방사선 조사를 가함으로 선량 분포의 불균형 을 줄이거나 없앨 수 있다. 셋째, 풍선확장술과 병행함으로 인해 기계적으로 관강의 열상(laceration)을 유도해 협착 부 위를 늘려 주고 열상이 된 점막 부위에 직접적인 국소적 방 사선 치료를 하는 역할을 할 것이다. 각 장기별로 방사선에 대한 감수성이 다르고 사람과 동물 간에도 장기의 조직학적 특성 및 감수성이 다르므로 동물 실 험 자료를 그대로 사람에게 적용하기 어려운 한계점이 있다. 따라서 보다 보편적인 임상적 활용을 위해서는 인체 내에서 의 최적정 방사선 조사량에 대한 연구가 뒤따라야 할 것이 다. 더불어 방사성동위원소를 공급해주는 generator가 고가 이므로 이를 쉽게 설치하기가 용이치 않은 문제점도 있다.
