Objective: This study was designed to investigate efficacy and safety of different treatment regimens with the two-compound product (Daivobet ) and combination of calcipotriol 50 μ g/g ointment (Daivonex ) and methylprednisolone aceponate 0.1% cream (Advantan ). Methods: In total, 84 patients with mild to moderate psoriasis vulgaris were randomized to one of two treatment regimens: group A, the two-compound product once daily for 12 weeks; group B, calcipotriol in the morning and methylprednisolone aceponate in the evening once daily respectively for 12 weeks. PASI and Psoriasis life stress inventory (PLSI) were considered as the outcome measures. Results: The mean reduction in PASI from baseline to the end of 12 weeks of treatment was 62.6% for group A and 41.1% for group B. Group A achieved faster treatment response than B, and the maximum treatment effect for these patients was seen after 2 weeks. This effect was maintained with continued treatment with the twocompound product for up to 12 weeks. For both PASI and PLSI score at weeks 4 and 12, group A was statistically superior to group B (p < 0.05). Conclusion: The results confirm a higher efficacy and more rapid onset of action with the two-compound product compared with combination therapy. Key Words: Calcipotriol, Betamethasone, Compound product, Psoriasis 서 건선은 전 세계적으로 1-3% 정도의 유병률을 갖는 비 교적 흔한 재발성 만성피부질환으로 건선 환자들은 정상 인에 비하여 전반적인 삶의 질이 저하되어 있는 것으로 알려져 있다. 1,2 따라서 건선의 치료는 환자의 삶의 질 향 상에도 초점을 맞추어야 하며, 단기적으로 급성기를 억 제할 수 있는 빠르고 효과적인 치료뿐만 아니라 장기적 으로 유지할 수 있는 안전한 치료법이 필요하다. 일반적 론 으로 경도 혹은 중등도의 건선 환자 중 약 80% 는 국소치 료에 잘 반응하는데 1, 여기에는 비타민 D 유도체와 국소 스테로이드가 가장 많이 사용되어 왔다. 그러던 중 이 두 가지 제제를 아침과 저녁에 각각 도포하는 복합요법 이 단일요법에 비해 더욱 효과적인 것으로 보고되었고, 3 최근에는 이 두 가지 성분의 혼합 제제가 개발되었는데 이미 여러 연구에서 그 효과와 안전성이 검증되고 있 다. 4-9 저자들은 이 연구를 통해 우선 calcipotriol/betame- thasone dipropionate 혼합 제제를 이용한 단일요법과 calcipotriol 제제, methylprednisolone aceponate 제제 를 각각 아침, 저녁으로 도포하는 복합요법 사이의 효 과와 안전성을 비교하고자 하였으며, 또한 각각의 치료 효과가 환자의 삶의 질에 미치는 영향을 분석해 보고자 하였다.

2 대한건선학회지 Vol. 6, No. 1, 2009 연구대상 및 방법 결 과 1. 연구대상 2007년 11월부터 2008년 8월까지 본원 피부과 외래를 방문한 건선 환자 중 18세 이상이면서 Body Surface Area (BSA) 10% 이하, Psoriasis Area and Severity Index (PASI) 점수 10 이하인 경도 및 중등도 판상 건선 환자 84 명을 대상으로 하였다. 물방울양 건선이나 농포 성 건선, 박탈성 건선, 피부병변에 위축이 있는 환자, 임 산부, 수유부, 지난 4주 간 전신투여제 혹은 광선요법 치료를 받았거나, 지난 2주간 국소요법을 시행한 환자 는 제외하였다. 성별 분포는 남자 55명 (65.5%), 여자 29 명 은 (34.5%) 이었으며 연령은 18-69세까지로 평균 연령 40.7 세였다. 2. 연구방법 무작위적인 방법으로 84명의 환자를 42명씩 두 군으로 나 누었고, 12주 동안 A군은 건선병변 부위에 calcipotriol/ betamethasone dipropionate (Daivobet ) 연고를 1일 1회 도포하였으며, B군은 calcipotriol (Daivonex ) 연 고를 아침에 1 회, methylprednisolone aceponate (Advantan ) 크림을 저녁에 1 회 도포하였다. 치료 효과 는 초진 시와 2 주, 4 주, 8주 및 12주 후에 각각 PASI 점수 를 이용하여 측정하였으며, 매 방문 시마다 홍반이나 작열 감, 가려움증, 피부위축, 모세혈관확장 등의 부작용 여부를 조사하였다. 또한 초진 시와 4 주, 12주 후에는 Psoriasis Life Stress Inventory (PLSI) 를 이용하여 각각의 치료 효 과가 삶의 질에 미치는 영향을 비교하였다. 1. 임상 및 인구학적 특징 연령 분포는 A군 평균 40.7 세, B군 평균 40.6세로 두 군 간에 유의한 차이가 없었고, 성별 분포에서 A군에서 남녀 비가 28 : 14, B군에서 남녀 비가 27 : 15로 남자가 좀 더 많았지만 두 군 간에 유의한 차이는 없었다. 환자들 의 평균 이환 기간이나 초진시의 PASI, PLSI 점수에서도 두 군 간의 유의한 차이는 보이지 않았다 (Table 1). 2. 치료 효과 비교 치료 시작 전에 비해 12주 후 PASI 값의 평균 감소율은 A군이 62.6%, B군이 41.1% 로 나타났다. A군의 PASI 값 은 치료 전 평균 7.3에서 치료 시작 후 2주 만에 급속히 호전되어 4.14 를 나타낸 반면, B군에서는 치료 전 평균 7.08에서 2주 후 6.13으로 약간만 호전된 양상을 보였으 며, 2주째 두 군 간의 평균 PASI 값의 차이는 통계학적으 로 유의하였다 (p < 0.05). 그리고 이러한 PASI 값의 유의 한 차이는 4 주, 8 주, 12주까지 지속되는 경향을 보였다 (Fig. 1). 3. 통계 분석 두 군 사이에 PASI와 PLSI 값의 비교를 위한 통계 분석 은 SPSS for windows (Version 12.0, Chicago, IL, USA) 를 사용하였고, Mann-Whitney 검정을 이용하여 분석하 였으며, 처리된 결과의 유의수준은 p < 0.05 로 하였다. 3 양윤석 등 : 건선치료에서 Daivobet 에 관한 연구 3. 안전성 비교 대상 환자 중 부작용이 나타난 환자의 수는 A군에서 3명 (7.1%), B군에서 5명 (11.9%) 이었으며, 부작용은 가 려움증, 홍반, 작열감, 모세혈관확장 순으로 흔하게 나타 났다. 피부위축이나 탈색 반응은 관찰되지 않았고, 기타 부작용으로 혈압 상승, 소화기장애가 있었으나 본 치료와 직접적인 관련은 없는 것으로 생각된다. 부작용의 평가는 복수 응답이 가능하도록 하여 부작용을 보고한 환자들은 대개 가려움증과 홍반 또는 작열감을 동시에 보인 것으 로 조사되었다. 그러나 이러한 부작용으로 인하여 치료를 중단해야 하는 환자는 없었다 (Fig. 2). 4. 삶의 질에 미치는 영향 비교 치료 시작 전에 A군과 B군의 평균 PLSI 값은 각각 21.19, 21.5를 보여 삶의 질이 전반적으로 저하되어 있음 을 시사하였고, 4주 후 A군은 11.85, B군은 15.23으로 두 군 모두 감소하였으며 통계학적으로 유의한 차이가 있었 Fig. 2. Changes in Psoriasis Life Stress Inventory (PLSI) scores at baseline, 4 weeks, and end of treatment (*statistically significant, p < 0.05). 다 (p < 0.05). 그리고 이러한 차이는 12주 후에도 지속되 는 경향을 보였다 (Fig. 3). 고 국소치료는 경도 혹은 중등도의 건선에서 주된 치료법 이며, 중증 건선에서도 초기 치료로 사용된다. 10 건선의 국소치료에는 국소 스테로이드와 비타민 D 3 유사체가 가 장 흔히 사용되는데, 국소 스테로이드는 세포질의 글루코 코르티코이드 수용체에 결합하여 빠르게 핵 내로 전위되 어 염증에 관여하는 유전자들을 조절하는 반면, 비타민 D 3 유사체는 각질형성세포와 림프구세포의 비타민 D 3 수용체에 결합하여 표피의 과증식과 이상각화를 정상화 시키고, 염증세포의 세포자멸사를 유도한다. 11 또한 일반 적으로 국소 스테로이드는 효과가 빠르게 나타나지만 지 속적이지 못하고 부작용이 많다는 단점이 있고, 비타민 D 3 유사체는 부작용이 적으나 효과는 느리게 나타난다는 단점이 있다. 이러한 서로 다른 작용기전과 장단점의 측 면에서 볼 때, 두 가지 약제의 복합요법에서 상승효과를 기대해 볼 수 있겠고, 실제로 과거에 복합요법을 이용한 찰 연구들은 이를 뒷받침해주고 있다. 127명의 건선 환자를 대상으로 시행한 다기관 연구에 서 아침에 calcipotriene 1 회/ 일, 저녁에 halobetasol 1회 / 일 국소도포군이 calcipotriene 2 회/ 일 국소도포군 또 는 halobetasol 2 회/ 일 국소도포군에 비하여 2주 후 건 선의 평균 중증도가 통계적으로 유의하게 낮은 것으로 보고되었고, 12 이후 169명의 건선 환자를 대상으로 시행 한 무작위적 이중맹검 연구를 살펴보면 82명의 환자는 calcipotriol 과 betamethasone 을 각각 아침저녁으로 도 포하는 복합요법을, 나머지 87명의 환자는 calcipotriol 단일요법을 사용하였는데 4주 후 복합요법을 사용한 군 의 PASI 값이 유의하게 낮아 복합요법이 더욱 효과적인 것으로 증명되었다. 13 국내에서도 비슷한 연구가 진행되 었는데 calcipotriol 연고와 methylprednisolone acepo- nate 크림을 각각 아침과 저녁에 1회씩 도포하는 복합요 법이 calcipotriol 연고를 2 회/ 일 도포하는 단일요법에 비해 6주 후 평균 PASI 값이 유의한 차이로 감소한 것을 확인하였다. 3 과거 이 연구에서 국내에 bethamethasone 국소 제제가 없기 때문에 이와 비슷한 강도를 가지는 methylprednisolone aceponate 제제를 이용한 것으로 생각되는데 따라서 본 연구도 이와 같은 맥락에서 같은 제제를 사용하였다.

4 대한건선학회지 Vol. 6, No. 1, 2009 일반적으로 국소 스테로이드는 산성 환경에서 안정적 이고, 비타민 D 3 유사체는 알칼리 환경을 필요로 하여 이 두 가지 제제를 혼합하는 것은 매우 어려운 것으로 알려 져 있었다. 그러나 2004년 Simonsen 등 14 은 5% polyoxypropylene-15-stearyl ether라는 단일 기제를 사용하 여 calcipotriol 50 μg/g과 betamethasone dipropionate 0.5 mg/g 을 혼합하는데 성공하였고, 각각의 성분이 활성 화된 형태로 피부에 전달됨을 확인하였다. 이후 다수의 임상연구들을 통해 보통 건선의 치료에서 이 혼합 제제 를 하루에 한 번씩 도포하는 것이 calcipotriol 단일요법 이나 국소 스테로이드 단일요법에 비하여 훨씬 효과적인 것으로 밝혀졌다. 4-9 부작용으로는 4 주간의 임상연구에서 가려움증, 홍반, 작열감 등의 경미한 피부자극 증상이 약 2.6% 에서만 나 타나 비교적 안전한 것으로 알려졌고, 5 최근에는 이 혼합 제제만을 52주간 지속적으로 도포한 군이 calcipotriol 과 병행하여 교대로 도포한 군이나 calcipotriol 만 52주간 도 포한 군에 비해 적은 부작용과 높은 환자 만족도를 나타 낸 것으로 보고되어 장기간 사용 시의 안전성과 내성 없 는 지속적인 효과에 대한 측면도 검증되었다. 15,16 그리고 이러한 적은 부작용은 아마도 효과가 betamethasone 의 항염증 calcipotriol 의 자극증상을 완화시켜주는 것에 기 인한 것으로 생각된다. 한편 이 혼합 제제가 건선 환자의 삶의 질에 통계적으 로 유의한 긍정적 효과를 보였다는 보고가 있는데 이는 국소 스테로이드제와 비타민 D 3 유사체의 상승효과 뿐 만 아니라 하루 한 번 바르는 간편함이 반영된 결과라 생각된다. 17 본 연구에서는 두 군 모두 국소 스테로이드 제와 비타민 D 3 유사체를 사용하였지만, 혼합 제제를 이 용한 군에서 더 빠르고 높은 효과와 삶의 질 향상을 가져 와 혼합 제제가 두 가지 제제를 따로 도포하는 것에 비해 더 높은 상승효과를 나타내는 것을 증명하였다. 그리고 이러한 차이의 기전에 대해서는 추후 더 세밀한 연구가 필요하겠으나, 두 가지 성분이 동시에 흡수될 때 염증의 억제 반응이 각각 따로 작용하는 것에 비해 우세한 것으 로 추정해 볼 수 있고, 역시 하루 한 번 바르는 간편함이 환자들의 순응도를 높여 나타낸 효과도 무시할 수는 없 을 것으로 생각된다. 본 연구에서 삶의 질 측정도구로 사 용한 PLSI (Psoriasis Life Stress Inventory) 는 외적 결 점, 사회적 불명예, 질병과 그의 치료에 대한 신체적 견해 등과 연계된 스트레스를 측정하기 위한 15가지 문항으로 구성되며 전혀 아니다 (0) 에서 매우 그렇다 (3) 인 4단계 점수로 이루어져 있다. 이것은 건선에 특이적인 질문도구 로 건선 환자의 삶의 질 평가에 유용한 것으로 보고되어 있다. 18 최근에는 건선의 치료에 있어서 calcipotriol/betamethasone dipropionate 혼합 제제의 사용이 calcipotriol 또는 tacalcitol 단일제제의 사용에 비해 비용-효과적인 측면에서도 우월한 것으로 보고되고 있다. 19,20 본 연구에 서는 비용-효과 측정지표를 이용하여 직접 분석하지는 않았으나 A군과 B군에 소요된 비용이 대체적으로 비 슷했던 점에서 미루어보아 calcipotriol/betamethasone dipropionate 혼합 제제의 사용이 두 가지 단일 제제를 사용한 것보다 비용- 효과적인 측면에서도 더 나을 것으 로 유추해 볼 수 있었다. 결 경도 혹은 중등도 건선의 치료에서 calcipotriol/betame- thasone dipropionate 혼합 제제가 calcipotriol 제제, methylprednisolone aceponate 제제를 각각 따로 도포 하는 복합요법에 비해 더 빠르고 높은 효과를 보였고, 더 나은 삶의 질 향상을 가져와 비교적 안전한 치료법인 것 으로 생각된다. 론 REFERENCES 1.
