But, there were a few clinical reports about the myocardial protection of inhalation agents. So we investigated the cardiac protection of sevoflurane in comparison with total intravenous anesthesia (TIVA). The study is a retrospective unrandomized study via the medical record review. Methods: The records of 102 patients who received off-pump CABG were reviewed. One patient group received TIVA by midazolam and sufentanil continuous infusion (TIVA group, n = 68), and the other patient group received an inhalational anesthesia by sevoflurane (sevoflurane group, n = 34). Except maintenance of anesthesia, two groups of patients received an identical surgical, anesthetical, and postoperative care. At arrival in the intensive care unit, and after 1, 2, 3 and 5 days, serum cardiac enzyme levels were measured. Results: All the median values of cardiac enzyme concentrations were lower in the sevoflurane group than TIVA group. (Anesth Pain Med 2012; 7: ) Key Words: Cardiac anesthesia, Myocardial protection, Preconditioning, Sevoflurane. 서 전통적인 심장 수술의 마취 관리는 고용량의 아편 유사 제를 사용하여 혈역학적 변화를 최소화 하는 것이었다[1,2]. 이는 아편 유사제가 비교적 심기능 억제가 적으면서 효과 적으로 통각 반응을 억제할 수 있기 때문이었다. 하지만, 이런 방법은 아편 유사제의 약효가 풀릴 때까지 장기간의 중환자실 관리를 필요로 한다. 이에 경제적인 동기에서 심 장 수술후 조기각성(fast-track cardiac anesthesia)이 시도되었 다. 심장 수술후 조기각성이란 수술중 작용시간이 긴 아편 유사제의 사용을 지양하여 수술후 환자의 중환실 체류기간 을 짧게 하는 것이다. 이런 심장 수술후 조기각성은 여러 임상 실험 결과, 사망률과 이환율에서 전통적인 심장 수술 마취와 차이가 없다고 밝혀졌다[3-5]. 심장 수술후 조기각성 은 일반적으로 흡입 마취제나 작용시간이 짧은 정맥 마취 제를 사용해 왔다. 최근에 흡입 마취제의 심근 보호 효과가 동물 실험뿐 아 니라 임상 실험에서도 보고되고 있어서[6-10] 심장 수술의 마취에서 흡입 마취제의 사용이 각광받고 있다. 국내에서도 흡입 마취제의 심근 보호 효과가 동물 실험에서 보고되고 있으나[11], 임상 연구에서는 결과 보고가 많지 않은 상황이 다. 본원에서는 체외순환 없는 관상 동맥 우회 수술(off-pump coronary artery bypass graft, off-pump CABG)의 마취 방법을 론 320

2 길남수 외 9인:Sevoflurane and TIVA during OPCAB 321 midazolam과 아편유사제의 지속 정주에 의한 전정맥마취에 서 sevoflurane 흡입 마취로 바꾸어 시행하고 있다. 이에 저 자는 off-pump CABG 마취방법에 따라 심근 보호 효과가 다른지 여부를 후향적 관찰 연구를 통하여 비교해 보고자 했다. 대상 및 방법 2006년 7월부터 2007년 4월까지 정규 off-pump CABG 수 술을 받은 환자 102명을 대상으로 하였다. 본원에서는 offpump CABG의 마취 방법을 2007년 1월부터 midazolam과 아편유사제의 지속 정주에 의한 전정맥마취에서 sevoflurane 흡입 마취로 바꾸어 시행하였다. 자료 수집은 의무 기록 조 회를 통하여 이루어졌다. 심장 판막 질환, 불안정 협심증, 심부전, 중등도 이상의 간질환(AST or ALT > 150 U/L)이 나 신장질환(creatinine > 1.5 mg/dl), 또는 폐쇄성 폐질환(예 상치 50% 이하의 FEV 1)이 있는 경우, 신경학적 질환력이 있는 경우, 이전의 심장 수술 경력이 있는 경우, off-pump CABG 수술 외에 다른 수술을 동시에 받는 경우, 혈역학적 불안정으로 심실보조장치(ventricular assist device)와 같은 기 계적 보조장치가 필요한 경우는 대상에서 제외하였다. 수술 전 aspirin 이외의 항혈소판 제제는 중단하였다. 마취 유도는 midazolam 0.1 mg/kg, etomidate 0.2 mg/kg, vecuronium 0.12 mg/kg, sufentanil 2 μg/kg 정주로 시행하였 다. 환자들은 마취방법에 따라 두 군으로 분류하였다. 마취 유지는 midazolam 5 μg/kg/min, sufentanil 2.3 μg/kg/hr 지속 정주로 유지하거나(total intravenous anesthesia [TIVA]군), 아 니면 sevoflurane 1 3% 흡입과 remifentanil 1 μg/kg/min 지 속정주로 유지하였다(sevoflurane군). 마취 방법 이외에 수술 후 중환자실 및 병동에서의 환자 관리 방법에는 변화가 없 었다. 수술 직후와 수술 1, 2, 3, 그리고 5일 후, 총 5회에 걸쳐 혈액 검사를 통해 Creatine kinase-mb (CK-MB), Lactate dehydrogenase (LD)와 Troponin I 농도를 측정하였다. 채혈은 중환자실에서 침습적 동맥압 감시 카테터를 통하여 하였다. 수술 전후의 심초음파 소견에서 심박출율(ejection fraction)을 비교하였다. 남녀 성비 및 좌주간부 관동맥 질환(left main artery disease)와 같이 두 군간의 빈도차이에 대한 비교는 카이 제 곱 검정을 시행하였고 그 외의 환자의 나이, 체질량 지수, 이환된 관상동맥의 수, 그리고 수혈양과 같은 평균치의 비 교는 Student t-test를 시행하였다. 수술 전후의 심박출율은 paired t-test를 통하여 비교하였다. CK-MB, LD, Troponin과 같은 수술 후 심근 효소 수치의 비교는 Mann-Whitney test 를 사용하였다. 통계 프로그램은 SPSS 12.0 (SPSS, Leuven, Belgium)을 사용하였고, P값 0.05 미만인 경우를 통계적으로 유의한 것으로 판단하였다. Table 1. ( ) U/L 대 231 (154 1,303) U/L (P = 0.005) (Fig. 2)], 수술 후 1일과 3일의 troponin I [각각 ( ) ng/ml 대 1.12 ( ) ng/ml (P = 0.029), ( ) ng/ml 대 ( ) ng/ml (P = 0.047) (Fig. 3)]의 혈중농도는 통계적으로 유의한 차이가 있었다 (Fig. 1). 수술 후 심박출율의 차이는 유의하지 않았으며 수 술 전후로 심박출율의 변화량도 유의한 차이를 보이지 않 았다. 양쪽 군 모두에서 특별한 치료를 필요로 할 정도의 수술 후 신부전은 없었다. 또한 경련이나 뇌허혈과 같은 심 각한 신경학적 손상도 없었고, 위장관계 증상도 없었다. 인 공호흡기 거치 기간(평균)은 sevo군 1.0일, TIVA군 4.4일이 었고 P = 0.002로 통계학적 차이는 있었으나 중앙값은 일치 허혈에 의한 심근 손상은 직접적인 허혈로 인한 손상과 허혈이 끝나고 재관류가 시작될 때 free radical oxygen에 의 한 재관류 손상 두 가지 기전으로 일어난다. 그러나 두 가 지 형태의 손상을 구분하기는 어려우며 허혈은 종종 재관 류와 동반되며 재관류는 허혈 후에 일어나는 것이기에 허 혈-재관류 손상(ischemic-reperfusion injury)으로 통칭하고 있 다[12]. 이와 같은 허혈-재관류 손상을 최소화하기 위하여 심근에 대한 ischemic preconditioning이라는 개념이 소개되었 다. Murry 등은[13] 개를 대상으로 한 실험에서 5분씩 4번의 일시적인 관상동맥 차단과 재 관류를 시행한 다음 40분의 관상동맥 차단 후 심근 경색의 크기가 ischemic preconditioning을 시행하지 않은 경우와 비교하여 75% 가량 감소하 는 것을 보고하였다. Preconditioning으로 기대되는 심근 보 호기능의 기전은 허혈 자극으로 인하여 adenosine, bradykinin, opioids, noradrenaline, free radicals과 같은 스트레스 매 개체에 의해 세포 내 신호 전달체계에 영향을 미쳐 결국은 미토콘드리아와 근섬유막의 ATP-sensitive K + channel을 활 성화시켜 심근 보호 효과가 나타나는 것으로 알려져 있다. Preconditioning의 다른 방법으로 흡입마취제를 관상동맥 차단 전에 심근에 투여하여 ischemic preconditioning과 비슷 한 심근 보호기능이 나타나도록 하는 것을 마취제에 의한 전처치(anesthetic preconditioning)라 하며, 현재 이와 관련된 여러 흡입마취제에 대한 연구가 진행되어 왔다[6-11]. 최근 에는 흡입 마취제에 의한 preconditioning에서도 ischemic preconditioning과 유사하게 미토콘드리아와 근섬유막내의

4 길남수 외 9인:Sevoflurane and TIVA during OPCAB 323 ATP-sensitive K + channel의 활성과 protein kinase C, adenosine A 1 receptor, α와 β adrenoreceptor의 자극이 관여하는 것으로 보고되고 있다[14]. 물론, 마취제에 의한 전처치는 심근 허혈전에만 흡입 마 취제를 투여하는 것으로 전 수술 과정에서 흡입 마취제를 사용하는 일반 흡입 마취와는 투여방식이 다르다. 하지만, 임상 상황에서 흡입마취제에 의한 심근 보호효과가 나타난 다면 이는 anesthetic preconiditioning 효과가 가장 큰 기여를 할 것으로 생각된다. 본 연구 결과는 전정맥 마취에 비해 sevoflurane을 사용한 흡입 마취가 심근 보호에 더욱 효과적임을 보여주고 있다. 모든 수술은 한 집도의에 의해 이루어진 off-pump CABG였 으며 해당 집도의를 보조하는 수술 인력이나 마취 인력도 연구 기간에 걸쳐 모두 같은 집단이었다. 그리고 수술 방법 이나 혈역학적 관리를 위한 방법도 일정하였다. 수술 전 협 착이 있는 관상동맥의 수가 비슷하였고 좌주간부 관동맥 질환의 비율도 비슷하였다. 그리고 수술 전 심초음파 소견 에서 심박출율의 차이도 유의하지 않았으며, 수술 전 대동 맥내 풍선펌프를 거치한 환자의 비율도 비슷하였다. 수술 중 수혈된 혈액양도 두 군에서 차이가 없었는데 이는 두 군 사이에 외과적 술기의 수준 차이가 없었음을 반영한다. 두 군에서 가장 큰 차이점은 midazolam과 sufentanil 정주 에 의한 마취 유지를 sevoflurane 흡입으로 바꾼 점이다. 물 론 sevoflurane군에서 sufentanil 대신 remifentanil을 마취 유지 에 사용했다는 점도 차이가 있으나 두군 모두 마취 유도에 는 sufentanil을 썼다는 점, 그리고 두 약제 모두 아편계 약 제라는 점에서 midazolam과 sevoflurane의 차이 만큼 크지 않을 것이라고 생각한다. 따라서, 두 군에서 나타난 심근 보호 작용의 차이는 sevoflurane 흡입마취와 전정맥 마취방 법의 차이에서 비롯되었을 가능성이 가장 크다고 할 수 있 겠다. 이러한 결과는 이전의 sevoflurane의 심근 보호 효과에 관한 연구들과 일치하는 것이다[8,11,14,15]. 그러나, 심근 보 호 효과는 심근 효소의 혈중 농도에서만 보였고 심초음파 상의 심박출율 소견에서 찾아볼 수는 없었다. 양 군간 중환자실 체류 시간은 통계학적인 차이를 보이 지 않았다. 인공호흡기 거치 기간은 통계학적인 차이가 있 었다. 그러나 수술 후 중환자실 체류 기간이나 인공호흡기 거치 기간과 같은 것은 병원 내 병동 상황, 혈관 조영실의 검사 일정 등 여러 가지 비 의학적 요인에 의해 결정되는 것이 많아서 양 군의 비교에 적절하지 않을 수 있다. 본 연구와 같이 off-pump CABG에서 sevoflurane의 심근 보호 효과를 알아보기 위해 심근효소를 관찰한 임상실험을 살펴보면 Conzen 등은[16] off-pump CABG 후 24시간 동안 의 troponin I의 농도 비교를 통하여 sevoflurane이 propofol보 다 심근 보호 효과가 크다고 보고하였다. 반면에 Law-Koune 등은[17] bispectral index (BIS) 값을 비슷하게 맞춘 sevoflurane-remifentanil 마취와 propofol-remifentanil 마취를 비교하 였는데 off-pump CABG 후 36시간 동안의 troponin I 농도 곡선의 아래 면적(area under curve)의 유의한 차이가 없었다 고 보고하였다. Law-Koune 등의 연구에서는 BIS값에 따라 sevoflurane을 MAC 수준으로 낮게 사용하였고 BIS 값도 양 군에서 유의하게 차이가 있는 등의 문제점이 있다. 이렇게 심근효소에 관한 연구들 사이에서는 상반되는 결과 가 있지만, Landoni 등이[18] 22건의 연구들을 메타 분석한 결과는 sevoflurane의 심근 보호 효과로 부작용 발생률과 사 망률을 낮춘다는 것이다. Landoni 등이[18] 분석한 연구들은 심장 판막 수술과 심폐기를 사용하는 CABG 등 다양한 심 장 수술이 포함되어 있으며 심근 보호 효과도 심근 효소가 아닌 심초음파 소견이나 혈역학적 지표 등 다양한 방법으 로 비교된 연구들이 포함되었다. 본 연구의 대상 환자들은 정규 off-pump CABG 수술을 받는 비교적 중증도가 낮은 환자를 대상으로 하였다. 그러 나 수술 전 대동맥내 풍선펌프를 거치한 환자는 대상에 포 함시켰는데 이것은 병원의 진료 방침상 좌주간부 관동맥 질환이 있는 경우에는 환자의 중증도와 상관 없이 마취 유 도 직전에 국소 마취하에서 대동맥내 풍선펌프를 거치하였 기 때문이다. 이번 연구의 가장 큰 제한점은 후향적 연구(retrospective study)로서 전향적 무작위 비교연구(prospective randomized trial)에 비하여 검증 능력이 떨어진다는 것이다. 그리고 두 군에서 sufentanil과 remifentanil이라는 다른 아편계 약제로 마취 유지를 했다는 점도 영향이 있을 수 있다. 또 다른 제 한점으로는 각 군간의 환자수의 차이가 많다는 것이다. 2007년 5월 이후로 심장 마취 시 마취 유지 방법이 다시 한번 바뀌었기에 sevoflurane군을 더 모을 수는 없었다. 마지 막으로 결과 값에 가외치(outlier)들이 있었는데 예를 들어, TIVA군에서 troponin I 농도가 100 ng/ml 이상으로 측정된 환자가 있었다. 이런 극단적인 가외치에 해당하는 값이 결 과에 영향을 미칠 수도 있다고 생각되지만, 해당 환자의 ICU 도착 직후, 수술 후 1, 2, 3일의 troponin I의 농도는 각 각 , 100, 129.9, ng/ml로 일관되게 일정 수준을 유지하고 있어서 검사 방법상의 기술적 오류라고 생각되지 는 않기에 결과에 포함시켰다. 그리고 해당 환자의 포함여 부가 통계적 유의성에 영향을 미치지는 않았다. 이러한 한계점에도 불구하고 본 연구는 실제 심장 수술 및 수술 후 환자 관리에서의 흡입 마취제의 심근 보호 효 과를 관찰한 것으로써, 임상 바로 적용 가능하다는 점에서 의의를 찾을 수 있다. 그리고 본 연구 결과는 sevoflurane의 심근 보호 작용에 관한 정황적 증거로서 기존의 연구 결과 들을 공고히 해주고 있다. 보다 정확한 검증을 위해서는 대 규모의 임상 실험이 필요할 것이다. 결론적으로, 본 연구는 off-pump CABG에서 sevoflurane흡

본 연구 결과는 전정맥 마취에 비해 sevoflurane을 사용한 흡입 마취가 심근 보호에 더욱 효과적임을 보여주고 있다. 모든 수술은 한 집도의에 의해 이루어진 off-pump CABG였 으며 해당 집도의를 보조하는 수술 인력이나 마취 인력도 연구 기간에 걸쳐 모두 같은 집단이었다. 그리고 수술 방법 이나 혈역학적 관리를 위한 방법도 일정하였다.
