7 < 국문요약 > 국내말라리아에서 Chloroquine 내성및 pvmdr1 단염기변이 분석 배경 : 1997년이후비무장지대근처말라리아위험지역을중심으로시작한한국군의말라리아에대한 hydroxychloroquine/primaquine 예방요법은말라리아발생율감소에기여하고있으나, chloroquine 내성말라리아출현의위험성이있다. 이에연구자는예방약을적절히복용하였음에도불구하고발생한말라리아의빈도및 chloroquine 내성감시를시행하였고, pvmdr1 유전자단염기변이와 chloroquine 내성과의연관성을조사하였다. 방법 : 말라리아발생위험지역의한국군을대상으로예방을위하여 2006년 5월부터 hydroxychloroquine sulfate 400 mg이말라리아유행기에 1주마다투여되었다. 2006년 6월부터 9월까지비무장지대근처의경기도에주둔한한국군에서발생한 102명의환자를대상으로말라리아발생이전의예방요법의적절성을조사하였다. 이중 85명의환자를대상으로 28일간 chloroquine 치료내성감시를시행하였으며이들환자로부터분리된 Plasmodium vivax로부터 pvmdr1 유전자의단염기변이를분석하였다. 결과 : 102명의환자중예방요법이적절하였던환자는 66명 (64.7%) 이었고, 불규칙한복용 11명 (10.8%), 예방약을전혀복용하지않았던환자는 25명 (24.5%) 이었다. Chloroquine 치료내성감시를시행한모든환자에서 chloroquine 표준치료 3일이후의말초혈액도말검사에서말라리아원충은관찰되지않았다. pvmdr1 유전자분석을시행한 1

8 78예모두에서 F1076L 변이가관찰되었다. 결론 : 연구대상말라리아발생환자중 2/3가적절히예방약을복용하고있었다. Chloroquine의치료실패는없었으나, pvmdr1 유전자의 F1076L 변이가 chloroquine 예방실패와관련있을것으로생각된다. 핵심되는말 : 삼일열말라리아, 화학예방요법, 클로로퀸, 내성, pvmdr1 2

9 국내말라리아에서 Chloroquine 내성및 pvmdr1 단염기변이 분석 < 지도교수김준명 > 연세대학교대학원의학과 김영근 Ⅰ. 서론 Plasmodium vivax는아프리카이외지역에서발생하는말라리아의대부분을차지하며매년약 800백만명의감염자들이발생한다 1. Chloroquine은 P. vivax에의한말라리아의치료에매우중요한 1차약제이며, asexual stage 및 hypnozoite의박멸을위하여 primaquine이필요하다 2. 그러나 1989년 Papua New Guinea에서 chloroquine 내성 P. vivax가최초로보고된이후로동남아시아, 인도, 남미등에서지속적으로 chloroquine 내성이보고되고있다 3. Plasmodium falciparum에서 pfcrt와 pfmdr1 유전자가 chloroquine 내성에관여한다고알려져있지만 4,5, P. vivax의 pfcrt와상동유전자인 pvcg10는 chloroquine 내성과연관없다고알려졌다 6. 최근 Brega 등은태국, 인도네시아, 터키, 아제르바이잔, French Guyana 등 chloroquine 내성 vivax 3

10 말라리아가유행하는지역의 P. vivax 에서 Plasmodium vivax mdr (pvmdr1) 을동정하고변이의빈도가높음을보고하면서이유전자와 chloroquine 내성과의관련성을제시한바있어 7 이유전자의변이와 chloroquine 내성과의연관성에대한연구가필요하다 8. 국내에서는 1970년대말말라리아가박멸된것으로생각되었지만 9, 1993년 1증례가발생한이후급속도로증가하고있어공중보건학적으로문제가되고있다 10, 년이후비무장지대근처말라리아위험지역을중심으로시작한한국군의 hydroxychloroquine, primaquine 말라리아예방요법은말라리아발생율감소에기여하고있으나 12,13, 약물순응성 (compliance) 의문제및약물압력 (drug pressure) 등이 chloroquine 내성에기여할가능성이매우높다 14. 아직국내에서는 chloroquine 내성에대한감시가이루어진적이없어 chloroquine 내성이평가된적이없다. 국내에서말라리아의원인인 P. vivax의내성감시및 pvmdr1 유전자의변이를동정하여그상관관계를밝히는것은말라리아관리및치료를결정하는데도움이될것이다. 이에연구자는국내 vivax 말라리아의 chloroquine 내성양상및 pvmdr1 유전자의단염기변이를조사하고 pvmdr1 유전자의변이와 P. vivax의 chloroquine 내성의관계를연구하고자한다. 4

11 Ⅱ. 대상및방법 1. 대상 2006년 6월 1일부터 9월 30일까지 경기도의 말라리아 발생 위험지역을 담당하는 국군 벽제, 양주, 일동 병원에 내원하여 말라리아 진단을 받은환자를대상으로하였다. 한국군 내에서 발생하는 말라리아는 모두 군병원으로 전원하여 치료하므로 연구기간내경기도지역의한국군에서발생한모든말라리아환자를 포함한다. 말라리아예방을위하여 2007년 5월부터 9월까지 hydroxychloroquine sulfate 400 mg이 1주 1회투여되었고마지막 2주는 primaquine 15 mg 투여되었다. 연구에동의한환자를대상으로문진을통하여예방약 복용정도를확인하여모두복용하였을경우 complete compliance, 1회 이상복용하지않았을경우 poor compliance, 전혀복용하지않았던 경우 no compliance로 분류하였고, complete compliance에 해당한 환자에게서 말라리아가 발생하였을 때를 breakthrough malaria로 정의하였다. 말라리아의치료는 hydroxychloroquine sulfate 800 mg 경구투여후 6, 24, 48시간후 400 mg을투여하였으며연구에동의한환자를대상으로 28일간내성감시를시행하였다. 5

Chloroquine 내성감시 Chloroquine 내성감시는세계보건기구에서제시한방법을따랐다 15. 즉, 연구포함대상기준은 1) 나이 6개월이상, 2) P. vivax parasitemia > 250/µl, 3) 치료시작전 48시간이내발열, 4) 연구에동의하는자이며검사항목및 parasitemia여부, 분자유전학적검사시기등은아래의 Table 1과같다. Chloroquine 내성말라리아의진단 Chloroquine 내성은세계보건기구가제안한기준으로판단한다. 즉, 다음중하나이상을만족할때내성으로판단한다. 1) Parasitemia가지속되면서임상적으로악화될때 6

13 2) 치료 3일째부터치료 28일사이에 parasitemia가있으면서겨드랑이체온이 37.5 이상일때 3) 임상적인상태와무관하게치료 7일째부터치료 28일사이에 parasitemia가존재할때 4) 재발시 chloroquine 및 desethylchloroquine 혈중농도가 100 ng/ml 이상임에도 parasitemia가존재할때 4. pvmdr1 유전자동정및단염기변이분석가. DNA 추출수집된 RBC로부터 QIAmap DNA blood kit(qiagen) 를이용하여 DNA를추출한다. 방법은제조사에서제시한방법에따르며, 약간변형하여다음과같이한다. 1) 1.5 ml microcentrifuge tube에 QIAGEN Protease ( 또는 Proteinase K) 20 µl를피펫을이용하여옮긴다. 2) 전혈 200 µl를 microcentrifuge tube에첨가한다. 3) AL buffer 200 µl를첨가한후잘혼합한다. 4) 56 C에서 1시간동안 incubation 한다. 5) 간단히원심분리기를이용혼합한다. 6) 200 µl 100% 에탄올을첨가하여잘혼합한다. 7

14 7) 6번의시료를 QIAamp Spin Column 에적용하고, 1분간 8,000 rpm으로원심분리한후 2 ml 집합 tube에 QIAamp Spin Column를넣은후필터가포함된 tube는제거한다. 8) QIAamp Spin Column에 500 µl의 AW1 buffer를첨가한후 1분간 8,000 rpm으로원심분리한후, 필터가포함된 tube는제거한다. 9) QIAamp Spin Column에 500 µl의 AW2 buffer를첨가한후 3분간 14,000 rpm으로원심분리한후, 필터가포함된 tube는제거한다. 10) QIAamp Spin Column을 1.5 ml microcentrifuge tube에위치시킨후, 필터가포함된 tube는제거한다. 11) QIAamp Spin Column을열어 100 µl AE buffer 또는증류수를첨가한후, 상온 (15-25 C) 에서 1분간 incubation 후 1분간 8,000 rpm에서원심분리한다. 나. pvmdr1 유전자동정 10 x buffer 5 µl, dntps(10 mm) 1 µl, primer1 10 p moles 2 µl, primer2 10 p moles 2 µl, Taq polymerase (solgent SG1002) 0.25 µl, 증류수 µl, DNA 5 µl를첨가하고 94 에서 5분, 그후 94 에서 30초, 55 에서 30초, 72 에서 30초 35 cycle 시행후 72 에서 7분의조건으로 PCR을시행한다. 사용되는 primer는 Table 2와같다. 8

Chloroquine 감수성 P. vivax로알려진 Sal-1 strain 16 의 pvmdr1 유전자 (GenBank accession number AY618622) 의 444 bp (222번-665번) 와 784 bp (2753번-3536번) 와비교하여변이유무를분석하였다. 라. Chloroquine 내성과 pvmdr1 유전자의단염기변이와의상관관계분석연구대상환자로부터분리된 P. vivax에서 pvmdr1 유전자의단염기변이와 chloroquine 예방요법의적절성및내성감시결과와비교하여 chloroquine 내성과 pvmdr1 유전자의단염기변이와의 9

16 상관관계를분석하였다. 10

17 Ⅲ. 결과 1. 대상환자의일반적특성 연구기간중총 149명의말라리아환자가발생하였다. 이중연구에 동의하는 102명의환자에대하여항말라리아예방약의복용상태에 대하여파악하였다. 대상환자는모두남자로평균연령 22.5 ± 3.3세 (17-36세) 였으며, 주둔지역은 경기 북서부 ( 벽제병원 ) 44명 (44.7%), 경기중북부 ( 양주병원 ) 36명 (35.3%), 경기북동부 ( 일동병원 ) 23명 (22.6%) 이었다 (Table 3). 18 2. 항말라리아예방요법및예방실패 (breakthrough malaria) 대상환자 102명중예방약을모두복용한환자 (complete compliance) 는 66명 (64.7%), 불규칙하게복용한환자 (poor compliance) 는 11명 (10.8%), 전혀복용하지않았던환자는 25명 (24.5%) 이었다 (Table 3). 즉 102명의환자중예방약을모두복용하였음에도불구하고말라리아가발생한 breakthrough malaria (prophylaxis failure) 환자는 66명 (64.7%) 이었다 (Table 3). Parasitemia의중심수는 2250/µl (interquartile range ) 이었으며, 예방약복용이적절하였던환자에서 2057/µl (interquartile range ) 로그렇지않은환자의 4558/µl (interquartile range ) 에비하여낮았으나통계적으로유의하지는않았다 (p = 0.246, Mann-Whitney U test). 3. Chloroquine 내성 (treatment failure) 감시연구에동의한 102명의환자중 28일간추적관찰이되지않았던 (follow up loss) 15명및말초혈액검사에서 gametocyte만보였던 2명을제외한 85명의환자를대상으로 chloroquine 내성감시를시행하였다. 표준치료후 3일이후에말초혈액도말검사에서말라리아원충이발견된환자는없었다 (Table 4). 즉, chloroquine 치료실패는없었다. 12

20 Ⅳ. 고찰위험지역주둔한국군을대상으로시행하고있는대규모항말라리아예방요법은전체적으로말라리아발생을줄이는데기여하였지만 12-13, 예방요법의실패 (prophylaxis failure) 또는말라리아원충의 chloroquine에대한내성 (resistance) 과관용 (tolerance) 등이문제될수있어 chloroquine에대한내성감시가필요하다 12. 더욱이 chloroquine/primaquine 예방요법을시행받는대상군인의수는 2001년약 140,000명에서 2006년에는약 200,000명으로확대되었으나, 국군의무사령부의통계에의하면말라리아감염환자는 2001년 688명에서 2004년 159명으로감소하는경향을보이다가 2005년 205명으로증가하는양상을보이고있어예방요법의실패또는 chloroquine 내성의문제가대두되고있다. Chloroquine 예방요법은말라리아원충의 blood stage에작용하는약물로말라리아의 1차감염을예방한다 17. Chloroquine은 liver stage의말라리아원충에는작용하지않기때문에예방요법의실패는종종 late-onset malaria의형태로나타날수있는데 18,19, 이는 chloroquine 예방요법의실패가모두 chloroquine 내성을의미한다고할수없을것이다. 그러나, late-onset malaria가발생하였던환자와는다르게본연구에서는말라리아발생시에도지속적으로예방약을복용하고 14

21 있었으므로 late-onset malaria라기보다 breakthrough 말라리아라할수있을것이다. 66명의예방실패환자는전체말라리아발생환자에비하여는많은수이지만, chloroquine 예방요법을시행한약 200,000명에비하여매우적은수이기때문에예방실패를문제로여기지않을수있다. 그러나, chloroquine 내성말라리아유행지역에서의 chloroquine 예방요법은실패할가능성이매우높기때문에 20-21, 이런환자들에대한지속적인감시가필요하리라생각된다. 본연구의예방요법의적절성은환자를대상으로한문진을통하여이루어져기억편견 (recall bias) 이발생할가능성이있으나, 위험지역에서의말라리아예방요법은지휘관의책임하에각부대별로직접관찰투여 (directly observed therapy;dot) 가이루어지고있으므로문진에의하여예방요법의적절성을평가하는것은큰문제가없을것으로생각한다. Chloroquine 내성은엄밀히구분하기는힘들지만생물학적내성과임상적내성으로나눌수있다 3. 혈중 chloroquine 농도가 100 ng/ml 이상임에도말라리아감염이지속될때생물학적내성이라하며, 표준 1.5-g chloroquine으로치료후말라리아감염이지속될때를임상적내성이라할수있는데, 표준치료시 chloroquine의혈중최고 15

22 농도는약 2000 ng/ml 까지상승한다 3,15. 약물복용시적절한치료농도에도달하지않는이유로가짜약 (counterfeit drug), 불규칙적인복용, 구토등이있을수있다 3. 본연구에서는위의가능성이없고, 1주 1회 chloroquine 예방요법시최저약물농도가 400 ng/ml 이상유지되므로 20,22, 본연구에서발생한예방실패환자에서약물농도를측정하였다면 100 ng/ml 이상으로나타날가능성이높다. 이는본연구에서관찰된 breakthrough 말라리아가생물학적내성을나타낸다고할수있을것이다. 본연구에서는 chloroquine 내성감시와더불어 pvmdr1 유전자변이를분석하였다. P. falciparum의 pfmdr1 유전자는 ATP-Binding Cassette(ABC) transporter로분류되고 chloroquine, artemisinin, quinine, mefloquine, halofantrine 등에대한내성과관련있는것으로알려졌다 17,23. P. vivax의 pvmdr1유전자는 P. falciparum의 pfmdr1 유전자와상동유전자로 Brega 등에의해처음으로동정되었는데, pfmdr1에서 N86, Y184, Y939, F1039 codon의변이가 chloroquine 내성과관련있으며 7,17,23, 이에대응하는 pvmdr1 유전자의 N91, Y189, Y979, F1076 변이가 chloroquine내성과관련있을가능성이제기되었다 7. 본연구에서 pvmdr1 유전자를분석한모든환자에서 3226번째 DNA 서열에서 T가 C로치환되는변이가나타났다. 즉, 1076 codon의 TTT가 16

23 CTT로변환되어 phenylalanine이 leucine으로치환되는변화를나타내었다. pvmdr1 변이가예방실패와관련있다는것을명확하게하기위하여는 pvmdr1 변이가없는군에서예방실패가없는것으로나타나야하지만, 말라리아감염이발생하지않으면이를증명하기가현실적으로불가능하다. 또한예방약복용을하지않는민간인에서발생한말라리아를분석하여비교할수있지만, pvmdr1 있든지없든지 예방요법의효과를 측정할수없고, 국내말라리아유행지역이 한국군에서발생하는지역과민간인유행지역이인접해있고유행지역범위가좁아다른균주 (strain) 라할수없으므로대조군이될수없을것이다. 본연구에서치료실패는없었지만예방실패가있었고이환자를포함하여분석한모든 P. vivax에서 F1076L 변이가나타났으므로 pvmdr1의 F1076L 변이가 chloroquine 치료실패를유발하지는않는다할지라도, chloroquine 예방실패와관련있을것으로조심스럽게유추할수있을것이다. 예방실패가치료실패를의미하지는않는데, 이는 chloroquine 예방요법시혈중농도가보통 400 ng/ml이상유지되지만 3, 20,22, 치료시에는약 2000 ng/ml 도달하는 것과관련있을수있다 3. 위의사실과본연구의결과로 pvmdr1 의 F1076L 변이가있는 P. vivax 는 chloroquine 에대한감수성이떨어진다 17

24 유추할수있을것이다. 즉 ATP-Binding Cassette(ABC) transporter인 pvmdr1의변이를가지는 P. vivax는 chloroquine에노출되었을때 drug efflux pump가활성화되어 chloroquine에대한감수성이떨어져치료요법시의 chloroquine농도는극복하지못한다할지라도예방요법시의농도는극복할수있어 breakthrough 말라리아를유발할수있을것이라생각할수있을것이다. 본연구의제한점은말라리아예방요법의적절성을 chloroquine 및 desethylchloroquine의혈중농도를측정하지않고문진에의존하였다는는것이다. 그러나, 말라리아에대한대규모예방요법이시행되는한국에서처음으로시행된말라리아내성감시라는것과이와연관되어 pvmdr1 유전자의변이를분석한것은세계적으로처음시행된연구라는데의의가있다. 본연구를통하여한국에서발생한 P. vivax에서는 chloroquine의치료실패는발견되지않았으나, 말라리아발생환자중약 2/3가적절히예방약을복용하고있는것으로나타나이에대한지속적인감시가필요하리라생각된다. pvmdr1 유전자의 F1076L 변이를가지는 P. vivax는 chloroquine에대한감수성이떨어져예방실패와관련있는것으로생각된다. 18

25 Ⅴ. 결론한국군내발생한말라리아환자를대상으로예방요법의적절성및 chloroquine 내성감시를시행하였고환자로부터분리된 P. vivax의 pvmdr1 유전자의단염기변이분석을통하여다음과같은결과를얻었다. 1. 연구기간중발생한 105 명의말라리아환자중 chloroquine 의 규칙적복용 66 명 (64.7%), 불규칙복용 11 명 (10.8%), 미복용 25 명 (24.5%) 으로나타나 breakthrough 말라리아는약 2/3 에서나타났다 일 chloroquine 내성감시를시행한 85 명의대상환자모두에서 chloroquine 치료실패는관찰되지않았다. 3. pvmdr1 유전자단염기변이분석을시행한 P. vivax 에서 N91, Y189, Y979 변이는관찰되지않았으나 F1076L 변이는모두에서관찰되었다. 4. P. vivax 의 pvmdr1 유전자의 F1076L 변이는 chloroquine 치료실패와는관련없을지라도, 예방실패와관련있는것으로 생각된다. 19

26 본연구의결과로국내에서의 chloroquine의치료실패는없었으나, 말라리아환자중 2/3가적절히예방약을복용한것으로나타났으며, pvmdr1유전자의 F1076L 변이는 예방 실패와 관련있을 것으로 생각된다. 앞으로예방요법중발생한말라리아환자에서의혈중 chloroquine 및 desethylchloroquine 농도측정을통한예방의적절성의 객관적 평가를 통하여 진정한 breakthrough 말라리아의 검출이 필요하며 chloroloquine 내성 말라리아에 대한 지속적인 감시가 필요하다. 20

30 Abstract Chloroquine Resistance and Analysis of Single-Nucleotide Polymorphisms of pvmdr1 Gene in Korean Vivax Malaria Young Keun Kim Department of Medicine The Graduate School, Yonsei University (Directed by Professor June-Myung Kim) Background: Chemoprophylaxis with chloroquine (CQ) and primaquine has been used in the Republic of Korea (ROK) Army since 1997 to reduce the number of the malaria cases. It has contributed, in part, to the decrease in the number of malaria cases among military personnel. However, chemoprophylaxis may facilitate the development of CQ-resistant strains of Plasmodium vivax. We investigated the breakthrough malaria infection and the therapeutic efficacy of CQ in P. vivax in ROK. And the pvmdr1 gene polymorphisms and its relevance to CQ-resistance in P. vivax were evaluated. Methods: Chemoprophylaxis with hydroxychloroquine sulfate (400 mg, once per week) was started in May and continued throughout the transmission season. From June to September 2006, 102 soldiers with vivax malaria near the demilitarized zone in Gyeonggi-do, ROK, were enrolled. Breakthrough malaria was defined when a patient had taken all prophylactic doses every week prior to infection. The status of chemoprophylaxis was determined by the interview. In 85 patients, therapeutic efficacy was monitored during 28 days after standard CQ treatment. The single nucleotide polymorphisms of pvmdr1 in the isolates from these patients were analyzed. Results: Among 102 malaria cases occurring during study period, breakthrough malaria was observed in 66 patients (64.7%) who had complete compliance. Incomplete compliance was 11 cases (10.8%) and no compliance was 25 cases (24.5%). In all patients enrolled in therapeutic efficacy monitoring, parasitemia 24

31 had not been observed since 3 days after standard CQ treatment. Nucleotide polymorphism at 1 codon (F1076L) was found in all analyzed isolates. Conclusion: Breakthrough malaria was observed in about two thirds of malaria cases occurring in the area on chemoprophylaxis.
