In Korea, most cases of imported malaria infections are caused by Plasmodium falciparum and P. vivax. Only one report of imported P. malariae infection has been published thus far. Here, we describe a case of imported P. malariae infection that was confirmed by peripheral blood smear and nested PCR targeting the small subunit ribosomal RNA (SSU rrna) gene. A 53-yr-old man, who had stayed in the Republic of Guinea in tropical West Africa for about 40 days, experienced fever and headache for 3 days before admission. The results of rapid malaria test using the SD Malaria Antigen/Antibody Kit (Standard Diagnostics, Korea) were negative, but Wright-Giemsa stained peripheral blood smear revealed Plasmodium. To identify the Plasmodium species and to examine if the patient had a mixed infection, we performed nested PCR targeting the SSU rrna gene. P. malariae single infection was confirmed by nested PCR. Sequence analysis of the SSU rrna gene of P. malariae showed that the isolated P. malariae was P. malariae type 2. Thus, our findings suggest that when cases of imported malaria infection are suspected, infection with P. malariae as well as P. falciparum and P. vivax should be considered. For the accurate diagnosis and treatment of imported malaria cases, we should confirm infection with Plasmodium species by PCR as well as peripheral blood smear and rapid malaria antigen test. 에서백혈구수 7,020/μL, 혈색소 14.5 g/dl, 혈소판 80,000/μL로혈소판감소증을보였다.

로정상범위보다약간증가되어있었다. 입원당일말라리아신속항원검사 (SD Bioline Malaria antigen P.f/Pan rapid kit, Standard Diagnostics Inc., Yongin, Korea) 는음성결과를보였으며말초혈액도말검사에서도말라리아원충이발견되지않았다.

말라리아원충의농도는현미경 1,000 배율에서백혈구 200개당원충의수를센후자동혈구분석기 (XE-2100 hematology analyzer, Sysmex, Japan) 로측정한백혈 구수로 μl당원충의수를간접계산하였고 109/μL로계산되었다.

환자는치료 6 일째부터증상이호전되었으며치료 9일째부터말초혈액도말검사에서말라리아원충이발견되지않았다. 치료 12일째에서도말초혈액도말검사에서말라리아원충은관찰되지않았으며혈액검사와생화학검사도모두정상으로회복되었다. 2.

2 에서백혈구수 7,020/μL, 혈색소 14.5 g/dl, 혈소판 80,000/μL로혈소판감소증을보였다. 혈액생화학검사 ( 총단백, 알부민, 포도당, 총빌리루빈, 아밀라아제, 요소질소, 크레아티닌, 나트륨, 칼륨, 염소 ) 는모두정상이었으나, AST 45 IU/L ( 참고치 7-38 IU/L), ALT 44 IU/ L ( 참고치 4-43 IU/L), 젖산탈수소효소 369 U/L ( 참고치 U/ L) 로정상범위보다약간증가되어있었다. 입원당일말라리아신속항원검사 (SD Bioline Malaria antigen P.f/Pan rapid kit, Standard Diagnostics Inc., Yongin, Korea) 는음성결과를보였으며말초혈액도말검사에서도말라리아원충이발견되지않았다. 환자는해열제를복용하였으나 39 의고열을보이며호전되지않았다. 입원 6일후다시말라리아신속항원검사를하였으나음성이었으며, 말초혈액도말검사에서정상크기의적혈구내에밴드형의말라리아원충이관찰되었다 (Fig. 1). 말라리아원충의농도는현미경 1,000 배율에서백혈구 200개당원충의수를센후자동혈구분석기 (XE-2100 hematology analyzer, Sysmex, Japan) 로측정한백혈 구수로 μl당원충의수를간접계산하였고 109/μL로계산되었다. 환자는메플로퀸 ( 처음 800 mg 투여후 400 mg을 6시간간격으로 4회투여 ) 을 4일간투여하였다. 치료 5일째말초혈액도말추적검사에서 52/μL 농도의말라리아원충이지속적으로발견되고다시열이발생하여메플로퀸 1 g을다시 3일간투여하였다. 환자는치료 6 일째부터증상이호전되었으며치료 9일째부터말초혈액도말검사에서말라리아원충이발견되지않았다. 치료 12일째에서도말초혈액도말검사에서말라리아원충은관찰되지않았으며혈액검사와생화학검사도모두정상으로회복되었다. 2. 말라리아종 특이이중중합효소연쇄반응 (Nested PCR) 사일열말라리아감염및혼합감염여부를확인하기위하여 Snounou 등 [4] 이발표한 18s rrna 유전자를이용한이중중합효소연쇄반응을일부변형하여중합효소연쇄반응을수행하였다. 전혈 200 μl 로부터 QIAamp DNA blood Mini Kit (Qiagen, Hilden, Ger- A B C D Fig. 1. 3 many) 를사용하여 DNA를추출후, rplu5/rplu6 프라이머를사용하여 1차중합효소연쇄반응을수행하였다. 그결과 1,100 bp의중합효소연쇄반응산물이확인되어말라리아원충으로판독되었으며 (Fig. 2), 4종의말라리아원충을감별하기위해종-특이프라이머 4종 ( 열대열말라리아, rfal1/2; 삼일열말라리아, rviv1/2; 난형열말라리아, rova1/2; 사일열말라리아, rmal1/2) 을사용하여 2차중합효소연쇄반응을수행하였다. datp, dctp, dttp, dgtp 각각 200 μm, Tris-HCl (ph 9.0) 10 mm, MgCl mm, KCl 40 mm, Taq polymerase (Bioneer, Daejeon, Korea) 1 μl, 각각의프라이머 1 μl, DNA 2 μl이포함된 20 μl reaction mixture를이용하여 2차중합효소연쇄반응을시행하였다. 양성대조군으로질병관리본부말라리아기생충과에서보유하고있는삼일열말라리아, 열대열말라리아, 난형열말라리아가확인된검체를이용하였으나, 사일열말라리아는양성대조검체를구하지못하였다. 열대열말라리아, 삼일열말라리아, 사일열말라리아는동일한조건으로 1차및 2차중합효소연쇄반응을시행하였으며난형열말라리아는중합효소연쇄반응조건을약간달리하였다. 1차중합효소연쇄반응과정은 95 에서 5분동안 predenaturation 후 94 에서 30초동안 denaturation을하고, 60 에서 1분동안 primer annealing, 72 에서 1분동안 extension을진행하고 5분동안 72 에서유지하며 30 cycle을시행하였다. 난형열말라리아는 94 에서 30초동안 denaturation 후, 45 에서 30 초동안 primer annealing, 72 에서 1분동안 extension 후, 7분동안 72 에서유지하였다. 2차중합효소연쇄반응에서는 1차중합효소연쇄반응산물 1 μl가포함된중합효소연쇄반응 mixture를 92 에서 20초동안 denaturation, 58 에서 20초동안 primer annealing, 72 에서 20초동안 extension하는조건으로수행하였다. 난형열말라리아는 94 에서 30초동안 denaturation, 45 에서 30초동안 primer annealing, 72 에서 1분동안 extension을하는조건으로시행하였다. 증폭 된중합효소연쇄반응산물의크기는각각열대열말라리아 205 bp, 삼일열말라리아 120 bp, 난형열말라리아 800 bp, 사일열말라리아 144 bp로확인되었다 (Fig. 2). 3. 염기서열분석중합효소연쇄반응결과 144 bp의사일열말라리아산물이확인되었으나, 정확한판정을위하여염기서열분석을시행하였다. 염기서열을 NCBI BLAST: National Center for Biotechnology Information Basic Local Alignment Search Tool ( nih.gov/blast.cgi?cmd =Web&PAGE_TYPE=BlastHome) 에서검색한결과, AF 사일열말라리아 type 2 small subunit ribosomal RNA 유전자와 99% 일치하여사일열말라리아에서유래한서열임이확인되었다. 고찰 말라리아는 Plasmodium 원충에의해전파되는감염병으로열대, 아열대및일부온대지역에서발생하며각종마다그분포지역에차이를보인다. 열대열말라리아는주로열대지역에서발생하며, 삼일열말라리아는열대, 아열대, 온대지역에까지널리발생하며, 열대열말라리아와삼일열말라리아가전체발생의 95% 이상을차지한다. 난형열말라리아는서아프리카중부, 중동, 파푸아뉴기니, 인도네시아등지에서주로발생하나, 베트남, 캄보디아, 미얀마에서도감염환자가보고된바있다 [5, 6]. 사일열말라리아는사하라사막이남, 서남아시아, 서태평양의섬들에서주로발생하는것으로알려져있다 [2]. 우리나라에는삼일열말라리아가주종을이루고있으나, 최근에는해외여행의증가와해외근로에따른해외유입형열대열말라리아가간혹보고되고있다 [7]. 질병관리본부에따르면해외유입형말라리아환자는 2002년부터 2010년 Plasmodium spp P. vivax P. falciparum P. ovale P. malariae 1,000 bp 500 bp 100 bp Fig. 2. 4 까지 353명으로보고되었으며, 원충종류별로는열대열말라리아 (145 명, 41.1%), 삼일열말라리아 (128명, 36.3%), 삼일열말라리아와열대열말라리아혼합감염 (10명, 2.8%), 사일열말라리아 (8명, 2.3%), 난형열말라리아 (5명, 1.4%), 미상 (57명, 16.1%) 이발생하였다 [3]. 사일열말라리아는모기체내에서다른종의말라리아보다성장속도가느려 [8] 그발생빈도가높지않다 [2, 9]. 토착형으로국내에서발생한사일열말라리아에대한보고는 Hiraba 등 (1930) 이충남서산에서처음 14예보고이래 1961년보사부 ( 현보건복지부 ) 말라리아팀에서마약중독환자로부터 1예검출까지약 43예를들수있다 [2, 10]. 해외유입형사일열말라리아감염은 2011년질병관리본부의통계에서 8명이집계되었으며 ( 년까지보고된통계임 )[3], 증례로보고된것은현재까지한증례 [2] 가유일하다. 사일열말라리아는다른종에비해원충의농도가낮아진단이늦어질수있으며, 다른종과의혼합감염이빈번히일어난다. 또한만성신증후군을유발하여사망률을높이는경우도있으므로정확한진단이요구된다 [2, 11]. 본증례의환자도처음내원당시현미경검경에서말라리아원충이관찰되지않았으며, 말라리아신속항원검사에서도음성을나타내어치료시기를놓칠위험이있었다. 말라리아신속항원검사키트는말라리아의감염여부를간편하고신속하게확인할수있으나, 원충의농도가낮거나삼일열말라리아감염이아닌경우위음성을보일수있다 [6, 12]. Moon 등 [6] 의해외에서유입된난형열말라리아보고에서난형열말라리아의원충의농도가 50/μL 이하일때, 신속항원검사에서음성을보였으나, 본증례에서원충의농도는 109/μL로저농도말라리아혈증이아니었다. 또한 Kang 등 [13] 의서아프리카에서유입된난형열말라리아원충의농도는처음내원시와두번째내원시각각 583/μL, 2,478/μL로계산되어저농도말라리아혈증이아니었으나신속항원검사에서위음성소견을보였다. 삼일열말라리아에대한신속항원검사키트의민감도와특이도에관한연구는있으나 [14] 아직삼일열말라리아이외의다른종에대한민감도와특이도연구는미흡한실정이다. 이번증례의사일열말라리아와 Kang 등 [13] 의증례난형열말라리아에서신속항원검사위음성소견으로보아삼일열말라리아감염이아닌경우에신속항원검사에서위음성가능성을염두에두어야한다 [12]. 신속항원검사위음성의다른원인으로검사표적이되는효소를분비하는유전자에변이가있을가능성을생각해볼수있다. 최근에는해외여행이나해외근로로장기체류하는인구가점점늘고있어해외유입말라리아가더증가할것으로생각된다. 해외유입말라리아로의심시삼일열말라리아와열대열말라리아뿐만아니라흔하지않은사일열말라리아나난원형말라리아도염두에두고진단을하여야하며, 정확한진단과치료를위해말초혈액도말검사및신속항원검사외에중합효소연쇄반응검사로말라리아 의정확한종을동정하는것이필요할것으로사료된다. 요약 최근말라리아풍토병지역으로한국인여행자나해외근무자의수가증가하였고그에따라해외유입형말라리아감염자의수도증가하고있다. 국내에서해외유입형말라리아는열대열말라리아와삼일열말라리아에의해대부분발생하고있으며지금까지사일열말라리아감염에대한보고는한증례가유일하다. 저자들은말초혈액도말검사와 small subunit ribosomal RNA (SSU rrna) 유전자를표적으로하는이중중합효소연쇄반응을이용하여확진된유입형사일열말라리아 1예를보고하는바이다. 환자는 53세남자환자로서아프리카기니에서약 40일간체류하였고, 입원 3일전부터발열과두통을호소하였다. SD Malaria Antigen/Antibody Kit (Standard Diagnostics, Korea) 를이용한말라리아신속항원검사에서음성의결과가나왔으나 Wright-Giemsa로염색한말초혈액도말검사에서말라리아원충이관찰되었다. 종확진및혼합감염여부를확인하기위해 SSU rrna 유전자를표적으로한이중중합효소연쇄반응을시행하여사일열말라리아단독감염을확인하였다. 사일열말라리아의 SSU rrna 유전자를염기서열분석한결과, 분리된사일열말라리아는사일열말라리아 type 2로확인되었다. 그러므로해외유입형말라리아로의심시열대열말라리아나삼일열말라리아뿐만아니라사일열말라리아도염두에두고진단을하여야하며, 정확한진단과치료를위해말초혈액도말검사및신속항원검사외에중합효소연쇄반응검사로말라리아의종을확진하는것이필요할것으로사료된다. 감사의글 본연구는질병관리본부의기술적협조를받아진행되었습니다.
