Introduction 1934 년이비인후과의사인 Costen 이처음으로 ENT 질환에포함되지않는귀주변의통증과그와연관된여러복합적인증상들을학계에발표한후, Temporomandibular Disorder(TMD) 는오랫동안치과분야에서가장흔한논란의대상이되어왔다. 잘못된 dental occlusion 이 TMD 의주된 etiologic factor 라는가정하에 dental practitioner 들은성공적으로 TMD 를치과질환으로분류하여 TMD 환자를치료하는특권 (?) 을누려왔다. 하지만 TMD 는비교적새로운임상분야이기때문에이에대한정확한정보가부족하고따라서 TMD 에대한치과의사들의올바른이해가부족하다. 많은환자들이 TMD 로

2 인하여고통을받고있지만인식부족으로인하여치료를찾지않거나치료를원한다하여도 TMD 를전문적으로치료할수있는임상의가많지않기때문에환자들이적절한치료를받지못하고여러치과를전전하는경우가많다. TMD 는또한치과임상에서환자와임상의사이에서아주흔한의료분쟁의원인이되기도한다. 그것은아직도 TMD 의발생원인이나그병리적기전이정확히이해되고있지않기때문이다. 우리는가끔환자로부터지난치과진료이후에갑자기 TMJ 에서소리가난다거나혹은턱이아파서입을벌릴수없게되었다거나하는 complaint 를듣고몹시당황스러웠던경우를경험하게된다. 치과치료기간중에발생한 TMD 는모두치과의사의잘못된진료때문인가? TMD 의발병률은상당이높다. 역학조사에따르면거의 50% 이상의일반인이한두가지이상의 TMD 증상을경험하게된다고보고되고있다. 지금이순간에도여러분의환자중에는치과치료와관계없이 TMD 증상이시작되는환자가있을수도있다. 특히교정치료와같이장기간의치료기간을요하는경우에있어서, 많은수의환자에있어서치료기간중우연의일치로 TMD 증상이시작될확률은상당이높다. 모든치과의사가 TMJ specialist 가될필요는없겠지만환자에대한교육과이해를돕고임상에서만나게될 TMD 환자의적절한조치를위하여, 적어도 TMD 에대한기본적인지식과올바른이해는필요하다고본다. 다음은 TMD 에대한이해를돕기위하여 Okeson 의 textbook 을바탕으로최근에발간된 journal 들과 research 결과들을모아종합정리한것이다. Definition of TMD 역사적으로 Temporomandibular Disorder 는여러다른이름으로정의되어왔다. 그것은시대에따라 TMD 에대한증상과그의원인이학자들에따라다르게주장되어져왔기때문이다. 1930, 1940 년대에는, 처음주창자인 Costen 의이름을딴 Costen s Syndrome 이란이름이널리사용되었다. 그후 Temporomandibular Joint Disturbance 란이름으로불리다가 1960 년대엔 Shore 에의하여 Temporomandibular Joint Disfunction Syndrome, Ramfjort and Ash 에의하여 Functional Temporomandibular Joint Disturbance 란이름등으로불리게된다. 그밖에 Pain Dysfunction Syndrome, Myofacial Pain Dysfunction Syndrome(MPD Syndrome), Temporomandibular Pain Dysfunction Syndrome. 등여러다른이름으로정의되어왔다. 이와같이 TMD 에대한다양한용어와통일되지않은증상에대한정의는임상가에게이를정확히이해하는데많은어려움을주게된다. 마침내 1982 년 ADA 주체로열린 conference 에서, Temporomandibular Disorder(TMD) 를공식적인이름으로채택하게되어용어의단일화와증상에대한정의의통일을이루게되었다. 일반적으로 temporomandibular disorder 하면 temporomandibular joint 내에국한된 disorder 로생각되기쉬우나 1982, 1990 에열린 TMD conference 에서는 TMD 를 masticatory muscle 의 pain, 그리고 temporomandibular joint 내의여러복합적인증상들을포함하는종합적인 disorder 로정의하고있다. 즉 TMD 를잘이해하기위해서는 2 개의 category 로나누어서생각할필요가있다 1. Masticatory Muscle Disorder 2. Temporomandibular Joint Disorder. 일견 muscle pain 과 temporomandibular joint 내의 disorder 와는별다른연관이없는

것처럼보이나, 많은 TMD 환자에있어서 masticatory muscle pain 과 TMJ disorder 는공통적인증상으로나타나며상호원인과결과를일으키는밀접한관계를갖고있다. (Muscle pain 과 TMJ 와의관계는 Etiology of TMD 에서좀더자세히설명될것이다.

3 것처럼보이나, 많은 TMD 환자에있어서 masticatory muscle pain 과 TMJ disorder 는공통적인증상으로나타나며상호원인과결과를일으키는밀접한관계를갖고있다. (Muscle pain 과 TMJ 와의관계는 Etiology of TMD 에서좀더자세히설명될것이다.) Anatomy of the Temporomandibular Joint TMD 를잘이해하기위해서는먼저 TMJ 의 anatomy 와각조직들의정상적인 function 을잘이해하는것이필수적이다. TMJ 는 mandibular condyle 과 temporal bone 의 mandibular(glenoid) fossa 그리고이두 bone 사이에서 cushion 역할을하는 articular disk 로이루어져있다. 그리고이주요기관들이제위치에서서로정상적인기능을할수있도록도와주는여러 ligament 와 muscle 이부착되어있다. 다음은 TMD 와밀접한관계가있는 TMJ 의조직들을간략하게설명한것이다.(fig 1, fig 2) fig.1 : Anatomy of TMJ (Lateral View). Capsular ligament: Joint전체를둘러싸고있으며단단한 collagen fiber로되어있어 condylar head를 glenoid fossa에서벗어나지않도록 holding하는역할을해준다. Lateral and mesial discal ligament: Articular disc를 mesial and lateral 쪽으로 condylar head에부착시켜 condyle의움직임에따라 articular disc가따라움직이도록하는역할을해준다. Retro-discal tissue: Articular disc의 distal 쪽에연결되는 soft connective tissue로많은 blood vessel과 nerve tissue가분포되어있어 joint 내에 nutrition과 synovial fluid를공급해준다. Superior and inferior retrodiscal lamina: Retrodiscal tissue를둘러싸고있는단단한 fibro-tissue로 articular disc을 temporal bone과 condylar head에연결부착시켜주는역할을한다. Inferior lateral pterygoid muscle: Lateral pterygoid plate에서 condylar neck으로연결되며 mouth opening 시 mandible을전방으로당겨주며 condylar head가전방으로 sliding 하도록한다. Superior lateral pterygoid muscle: Greater sphenoid wing에서시작 articular capsule과 articular disc 그리고 condylar head에연결된다. 이 muscle은 inferior lateral pterygoid muscle과는반대의작용을하며다른 elevator muscle과함께 mouth closing 때수축을한다. 특히강한저작작용이나 clenching grinding 같은 power stroke시에많은수축을보인다. Sign and Symptom of TMD TMD 환자로부터들을수있는가장흔한 complaints 는안면근육의 pain 과 tenderness, pain around ears, joint noise (clicking, popping), difficulty of mouth opening, headache 등이다. 이러한 TMD 증상은 joint 외부, 즉 masticatory muscle 에서오는 pain 과순수하게 joint 내부에서기인되는증상으로나눌수있으며 joint 내부의

5 증상은다시 articular disc 의 derangement 로인한증상과그리고 joint 내부조직의 inflammation 으로인한증상으로크게나누어진다. Muscle pain은가장흔한 TMD 증상중하나이다. Muscle pain이일어나는 mechanism에대해서는아직도논란이계속되고있지만일반적으로과도한운동후 muscle내 artery가수축되며그로인하여 metabolic waste가 muscle 내에쌓이게되고이때통증의원인이되는 bradykinin이나 prostaglandin 같은 algogenic substance가분비됨으로서통증이일어난다고알려져있다. Masticatory muscle disorder는임상적으로 5가지다른형태로나타난다. muscle splinting, local muscle soreness, myofacial trigger point pain, myospasm, myositis. 이중 myofacial trigger point pain은 muscle내에서 trigger point라는 hypersensitive한 muscle bundle에서시작되며 muscle pain의 50% 이상을차지하는가장흔한형태이다. 이 muscle pain의원인과발생 mechanism은아직정확히밝혀지지않고있으나 emotional stress, trauma 등으로인한 muscle hyperactivity로 muscle이피로해지고이로인하여 muscle 내혈액순환이감소됨으로서 metabolic waste가쌓이고 trigger point가 alogogenic substance에의하여자극되므로서일어난다고알려져있다. 이 trigger point pain의가장특이한점은이 pain이지속되는동안 central excitory effect가발생하여 headache과같은 referred pain이발생하며이것이많은 TMD 환자들에게 headache이동반되는원인이된다. Internal Derangement: TMJ의 articular disc는강한섬유질의조직으로혈관과신경조직을포함하고있지않아저작압으로부터효과적으로 joint 조직을보호해주는 cushion 역할을해준다. Articular disc는 discal ligament나 retrodiscal lamina 등으로 condylar head에연결되어 condylar head 가움직임에따라함께따라움직이도록되어있다. 그러나많은 TMD 환자에있어서 discal ligament가늘어나제대로 function을하지못할때이 articular disc가제자리에있지못하고 condyle head와 articular fossa 사이에서벗어나있는경우가많이있는데이러한상황을 internal derangement라고한다.(fig 3) fig. 3 Internal derangement 는 TMD 환자에서볼수있는가장흔한 joint 내의 disorder 로그자체가하나의 TMJ disorder 이면서또한 TMJ 내에서일어나는여러이차적인증상의원인과출발점이되기도한다 년 Farrar 는 arthrogram 의도움으로그동안 mystery 로여겨졌던 TMJ 에서

6 나는 clicking, popping sound 의원인이 disc 의 derangement 로인한현상이란것을설명하였다. Okeson 의 textbook 에이에대한자세한 mechanism 이잘설명되어있다 (fig. 4). 전방으로전이되어있던 disk 가 mouth opening 시 condylar head 가전방으로 sliding 되면서 articular disk 가 condylar head 상방으로회복되면서 clicking sound 를내게되며다시 mouth closing 시 condylar head 가다시 articular fossa 로돌아갈때 disk 가다시이탈되면서 clicking sound 가다시발생한다. fig. 4 전방으로이탈되어있는 disk는 condylar head의전방 sliding을제한함으로서 mouth opening을크게할수없는원인이되기도한다. Disk가전방으로이탈되어있는경우 condylar head는결국 retrodiscal tissue를누르게되는데 retrodiscal tissue는많은 blood vessel과 nerve tissue를포함하고있는 soft tissue로서정상적인교합압에도쉽게 damage를입거나 inflammation을일으키기쉽다. 이러한 retrodiscal tissue의 damage가계속되는경우 retrodiscal tissue가파괴되어 condylar head가 articular fossa에직접마찰하게되며결국 condylar head나 articular fossa의 bone 조직이마모되거나파괴되는 degenerative joint disease로발전하게된다.(fig. 5)

7 fig. 5 Inflammatory Joint Disorder : 환자가 TMJ에심한통증을호소하는경우 joint내의 inflammation을의심할수있다. TMJ의 inflammation은발생부위에따라주로 3 종류로나누어진다. Synovitis : Joint cavity의내벽은특수한 endothelial cell로구성된 synovial tissue로둘러쌓여있다. 주로갑작스러운 trauma로인하여이 synovial tissue에손상을받게되면염증반응을일으키게되는데이것이 synovitis이다. Capsulitis : Joint의외벽을둘러싸고있는 capsular ligament가 trauma로인하여 damage를받은경우생기는 inflammation. Retrodiscitis : Retrodiscal tissue의 inflammation. Retrodiscal tissue가 acute trauma로인하여 damage를받거나, 무엇보다가장흔한경우는, internal derangement가있는환자에있어서정상적인저작압에의해서 condylar head가 retrodiscal tissue에 damage를가함으로서 retrodiscal tissue에 inflammation이발생할수도있다. Retrodiscitis는다른여러가지 TMJ 증상의원인이되기도한다. Retrodiscal tissue의 inflammation으로인한 swelling으로갑작스러운 occlusion의 change가올수도있으며심한통증으로어느날갑자기입을벌리지못하는 closed lock의직접적인원인이되기도한다. Etiology of TMD TMD 에대한오랜논란의초점은결국 TMD 의원인이무엇인가? 에있다고볼수있다. Costen(1934) 이처음 TMD 를학계에발표할때그는 TMD 의원인을마모된치아로인하여낮아진 occlusion 이주요원인이라고하였다. 따라서그는 occlusion 을높여주는장치로 TMD 환자를치료할것을주장하였다. 1930, 40 년대는그의이론이지배적이어그에대응하는이론이나타나지않았다. 아직도그의이론을따라서낮아진교합과그로인하여 condylar head 가 joint 내후방조직에압박을가함으로서 TMD 증상이일어난다고믿고있는임상의들이많이있다. 그러나곧그의이론은맞지않음이밝혀지었다. 실질적으로 TMD 의원인은그와같은단순한이론으로설명하기엔훨씬복잡한양상을보이고있다. 그후 Schwartz(1955) 는잘못된구조의문제가아니고부조화로운운동이원인이라는 muscle dysfunction 이론을주장하였다. Ramjort(1961) 는 occlusal disharmony 즉 malocclusion 이 TMD 의원인이라고주장하였다. 특히 CO, CR 이일치하지않아서생기는 central sliding 이주된원인이라주장하였다. Laskin(1969) 은 mental stress 와연관된 oral habit(clenching, grinding) 이주된원인이라고주장하였다. 이밖에 central nerve system theory, hormonal theory, viral infection theory 와같은여러다른

8 이론이있으나어느한가지이론만으로는 TMD 의원인을정확히설명하지는못한다. TMD 의증상은한가지가아닌여러종류의다양한증상의복합체이다. 따라서각증상에따른다양한원인을갖고있다고할수있다. 그러나궁극적인 TMD 의원인은 trauma 라고할수있다. TMD 의원인이되는 trauma 는 2 종류로나누어볼수있다 : Acute macro-trauma, Chronic micro-trauma. Macro-trauma는 traffic accident나 fist blow와같이 mandible에갑작스럽게가해지는 trauma로이 macro-trauma가 TMJ에여러가지이상을일으키는원인임은논란의여지가없다. 다음은 macro-trauma로인하여 TMJ에나타날수있는여러증상들이다. 1. 갑작스러운 mandible에의충격은 joint의 synovial wall이나 capsular ligament에손상을주어 synovitis, capsulitis등 inflammation을일으킨다. 2. Condylar head가 retrodiscal tissue에충격을주어 Retrodiscitis를일으킬수있다. 3. 심한 trauma는 Joint ligament의 elongation을일으켜 disk의 internal derangement나 mandibular hypermobility의원인이된다. 4. Condyle의 fracture, muscle이나 tendon에 demage를주어 tendonitis를일으킨다. 5. Joint 내의 hemorrhage로인한 joint의 ankylosis를일으킬수도있다. 이와같은 macro-trauma에의한 TMJ 증상은치과치료과정중에발생할수도있으며 3 rd molar 발치와같이장시간무리한치과치료후환자가 joint pain이나 clicking을호소하는것이이러한예이다. Micro-trauma 로인한 TMJ disorder 는좀더복잡한양상을보인다. Micro-trauma 가 TMJ disorder 를일으키는 mechanism 은 2 가지경로로볼수있다. 하나는외부에서오는 mild 한 force 가지속적으로 mandible 에압박을가함으로서 joint ligament 나그밖에 TMJ 조직에변형을일으키는경우이다. 예를들어, 손으로턱을괴는습관이있다거나, sleep posture 에문제가있거나, 심지어는 violin 연주가에서와같이지속적으로 mandible 에압박을가하는경우에 joint ligament 가늘어나고 articular disk 가한편으로압박을받음으로서 internal derangement 를일으키게된다. 또하나 TMJ 에가해지는 micro-trauma 는내부적으로 masticatory muscle 의 hyperactivity 나 contraction 으로인한것이다. 어떤원인에의하여 masticatory muscle 이무의식상태에서 contraction 을지속적으로하는경우 muscle pain 과더불어 disk derangement 를비롯한여러 TMJ disorder 를일으킨다. Masticatory muscle disorder 가 Joint 내의 disk derangement 를일으키는 mechanism 에대해서는정확히알려져있지는않지만 superior lateral pterygoid muscle 이중요한역할을하는것으로추측되어지고있다. Anatomy of TMJ 에서설명한바와같이 superior lateral pterygoid muscle 은그일부분이 articular disk 에연결되어있으며다른저작근들과함께 mouth closing 시에수축을한다. 특히강한저작작용이나 clenching, grinding 과같은 power stroke 에강한수축을함으로서 articular disk 를지속적으로전방으로잡아당겨결국 joint ligament 의 elongation 과함께 disk derangement 의원인이된다. 결국 TMD 의원인에대한오랜논란의본질은 무엇이지속적인저작근수축의원인인가? 에있다. Mental stress 로인한 clenching 이나 grinding, malocclusion 에의한 muscle 의긴장, 혹은 central nerve system 에서오는 muscle contraction 등이주된원인으로거론되고있으며최근에는 estrogen 같은 hormone 의 TMD 에대한역할

9 등이연구되고있다. 그밖에유전적인요인도 TMD 발생에중요한역할을한다. 위에열거한여러요인을갖고있다하여도선천적으로강한 TMJ structure 를갖고있으면 TMJ disorder 를일으키지않는다. 그러나선천적으로느슨한 joint ligament 를갖고있거나 steep 한 articular eminence, flat 한 condylar head 나 articular disk 등은모두 disk displacement 를쉽게일어날수있도록하는 predisposing factor 가될수있다. Occlusion as an etiologic factor of TMD Occlusion과 TMD와의관계, 즉 malocclusion이 TMD의원인이라는 theory는가장오래동안보편적으로 dental practitioner들에게알려져있는이론이다. 이를바탕으로많은학자들이 occlusion을 change하여 TMD를치료하려는노력을해왔다. 최근에실시된여론조사에따르면아직도미국치과의사의 60% 이상이 malocclusion을 TMD의주요원인으로믿고있는걸로나타나있다. 그러나우리는매일매일의임상에서너무나많은 TMD 환자들이좋은 occlusion을갖고있고또한심한 malocclusion 환자가건강한 TMJ를갖고있는것을보고 과연 occlusion과 TMD가밀접한관계가있는가? 하는의문을갖게된다. Occlusion의형성과정을보면영구치가 mixed dentition에서 eruption을시작하여 permanent dentition에이르기까지수년에걸쳐서 temporomandibular joint나주변의 muscle들의작용에의하여가장안정된 equilibrium이이루어지는위치에서영구치가배열되면서서서히 occlusion이완성된다. 갑작스러운 trauma나 restoration 혹은교정치료와같은인위적인치과치료에의하지않고는 TMJ에영향을미칠만한갑작스러운 occlusion의변화는거의일어나지않는다. 이밖에도임상적으로 occlusion과 TMD와의관계를의심케하는증거의하나는여성이남성보다훨씬높은 TMD 발병률을보인다는것이며노인이나무치악환자에서는매우낮은발병률을보인다는것이다. 통계적으로여성이남성보다 occlusion에더많은문제가있다는증거가없으며 edentulous나 partial edentulous 환자는임상적으로매우 unstable한 occlusion과 mandible의운동을보이지만 TMD를호소하는경우는극히드물다. 여성이남성보다높은 TMD 발병률을보이는것은여성이보다많은 mental stress에노출되어있기때문으로추측되고있다. 그것은가정주부들이자녀교육과같은외적인 stress에더많이노출되어있기때문인지혹은여성 hormone 등에의하여내적으로 stress에대한 threshold가낮아져있기때문인지아직정확한원인은밝혀지지않고있다. 최근에는 occlusion과 TMD와는연관관계가많지않음을보여주는 research 결과들이많이나오고있다. 그럼에도불구하고 gnathologist를비롯한많은치과임상의들은 occlusal adjustment, restoration, 혹은교정치료등을통하여 occlusion을바꾸어줌으로서 TMD를성공적으로치료할수있음을주장하고있으며또실질적으로임상에서그러한결과들을보여주고있다. 이에대하여 TMJ 임상가들은 Occlusal therapy로 TMD를성공적으로치료할수있는것은 TMD의발생원인이 occlusion 자체에있는것이아니고 clenching이나 grinding과같은 parafunctional muscle hyperactivity가어느특정한 occlusion에 memory를갖고반복적인운동을하기때문이다 라고설명하고있다. 따라서 occlusion을 change 하여 TMD 증상을치료할수있었던것은 occlusion의 change가특정한 occlusion에익숙해져있는 muscle memory를분산시킴으로서 TMD 증상이완화되었다고보고있는것이다.

10 따라서새로운 occlusion 에익숙해진 muscle memory 가새로형성되고 parafunctional muscle hyperactivity 가계속되면 TMD 증상이다시재발될수있는것이다. 그렇지만 occlusion 을 TMD 의원인에서완전히제외시킬수있는것은아니다. Occlusion 이 TMD 의보편적인원인은아니지만 premature contact 와같은 occlusal disharmony 는 masticatory muscle 에지속적인 tension 과 hyperactivity 를유발하여 muscle pain 과 disk derangement 의원인을제공할수있다. 이와같이 TMD 의 etiologic factor 로서의 occlusion 의역할에대한 concept 의변화로인하여최근들어 dental practitioner 가 TMD 를치료하는데있어그접근방법이많이달라지고있다. Treatment of TMD TMD의원인과증상이다양한만큼그의치료방법또한다양하고복잡하여임상에서적절한치료방법의선택에많은혼동을준다. 좋은 TMD의치료를위해서는그의증상을정확히감별진단하고또그원인을정확히분석할수있어야한다. TMD 치료가어려운이유중의하나는일반적으로하나의 TMJ disorder는곧또다른 disorder를일으킴으로서 TMD 환자는여러개의 disorder를동시에갖고있는경우가많기때문에환자의증상이어느진행과정에있는지정확한감별진단이어렵기때문이다. 예를들어초기의 muscle hyperactivity는 muscle pain과 disk derangement를일으키고 disk derangement가있는환자는 joint 내의 inflammation(retrodiscitis) 을일으키기쉬우며반복된 inflammation은 joint내조직의파괴로결국 TMJ의 degenerative arthritis와같은더욱진행된형태의 disorder를일으킨다. 또한 emotional stress와같이많은 TMD의원인들은예방하거나완전히제거하는것이거의불가능하기때문에완전한치료또한어려운경우가많다. TMD에사용되는치료방법은상당이많다. 현재문헌에나와있는과학적으로 approved된 TMD 치료방법만도 28가지가넘는다. 일반적으로 TMD 치료방법은크게 2가지로나누어볼수있다 : definitive treatment( 결정적치료 ), supportive treatment( 보조적치료 ). Definitive Treatment는 TMD의원인을 control하거나제거하여근본적인치료를꾀하는것으로 muscle hyperactivity를방지하기위하여 emotional stress를제거하거나 premature contact를제거하기위한 occlusal therapy 등이이에속한다. SupportiveTreatment는환자의증상을변화시키거나완화시키는치료방법을말하며이에는 pharmacological therapy (antianxiety drug, anti-inflammatory drug, muscle relaxant, analgesic), physical therapy (thermotherapy, ultrasound therapy, electric stimulation therapy, acupuncture), manual technic (massage, muscle exercise, joint distraction) 등이있다. 아직도 TMD 치료에대한과학적인 guideline이정립되어있지않기때문에올바른치료방법의선택에많은혼란이있다. 지역에따라서치료방법이다르고임상가의종류에따라서치료의방법이다르다. 환자가 chiropractitioner나 physical therapist ( 물리치료사 ) 를찾아가면그들은 massage, electric stimulation, thermal therapy, 등근육을 relaxation하여 pain을제거하는방법으로치료할것이며, 한의사를찾아가면침, 경락 massage, 한약등을통한치료를시도할것이다. 같은 dental practitioner에도 orthodontist는교정을통한 occlusion change를, oral surgeon은수술을통한 joint structure의 readjustment를, 그리고 prosthodontist들은

11 restoration 을통한 occlusion 의 readjustment 를시도할것이다. 동일한증상과 disorder 를지역에따라서또는환자가찾아가는임상의의종류에따라서다른치료를받는다는것은어찌보면 irony 가아닐수없다. 일반적으로치료방법의특성상다른임상의들은 supportive treatment 를 dental practitioner 들은 definitive treatment 를시도하는경우가많으며이중 occlusal therapy 는 dental practice 에서가장보편적으로시도되어지는 treatment 이다. 일반적으로 dental practice 에서시행되는 occlusal treatment 는크게 2 가지로나누어질수있다 : conservative(reversible) treatment, non-conservative(irreversible) treatment. Conservative treatment 는 splint 등을이용하여 tooth structure 나 occlusion 에 permanent 한 change 없이하는치료를말하며, Non-conservative treatment 는 orthodontic, surgery, occlusal adjustment, prosthodontic reconstruction 과같이 dental structure 나 occlusion 을 permanent 하게 change 하는 aggressive 한치료방법을말한다. 아직도많은 dental practitioner 가 TMD 치료를위하여 occlusal adjustment 나 full mouth rehabilitation 같은 aggressive 한 treatment 를우선적으로선택하는경우가많다. 그러나매우흥미있는사실은치료성공률에있어서 conservative treatment 나 non-conservative treatment 나거의차이가없다는것이다. TMD 의 etiologic factor 로서의 occlusion 의역할은아직과학적으로정확히규명되어있지않으며최근의많은 TMD specialist 에의하면특별한 case 를제외하고는그의역할이과거에생각되었던만큼그리많은비중을차지하고있지않음이주장되고있는상황이다. 따라서 dental practitioner 가 TMD 치료를하는데있어서가장중요한치료원칙중의하나는특정한환자의 TMD 의원인이 malocclusion 에의한것이라는확실한증거를가지고있지않는한 irreversible 한 treatment 에앞서 conservative treatment 가항상우선적으로선택되어야한다는것이다. 치과에서행해지는가장대표적인 reversible treatment 는 splint therapy 이라고볼수있다. Splint 는모든 TMD 에거의만병통치약과같이사용되고있으나모든 TMD 증상에적용되는것은아니다. 적절한 splint 의사용을위해서는 TMD 에대한 splint 의작용 mechanism 을잘이해하여야된다. Splint 의작용 mechanism 은크게 2 가지로나누어볼수있다. 첫째 splint 는 muscle hyperactivity 를완화시킴으로서 muscle pain 을감소시킨다. Splint 가 muscle pain 을감소시키는 mechanism 은아직완벽히설명되고있지않지만 splint 가특정한 occlusion 에익숙해져있는 muscle memory 를분산시킴으로서 clenching 이나 bruxism 같은 masticatory muscle 의 parafunctional reflex 를제거해주기때문인것으로추정하고있다. 또하나의 TMD 에대한 splint 의작용은환자가 splint 를끼고있는동안 condylar head 가하방으로이동하여 joint 내의 space 를증가시켜주므로 condylar head 가손상된 tissue 에직접적으로압박을가하는것을방지하여 pain 을감소시키고조직의 healing 을도와주는것이다. 특히 disk derangement 가있는환자에서발생하는 retrodiscitis 에서는 splint 가염증상태에있는 retrodiscal tissue 가 healing 되면서 scar tissue 를형성토록하여도와주므로서이 scar tissue 가 pain 없이 articular disk 의기능을대신할수있도록도와준다. 이때 splint 의선택은 condylar head 가충분이하방으로이동할수있도록하는 anterior repositioning splint 나충분한 thickness 를가진 splint 가필요하다. 12 Internal derangement는가장보편적인 joint 내의 disorder로서 joint sound의원인이며다른 joint 질환의출발점이되기도한다. TMJ specialist에게있어 disk derangement의치료는가장어려운과제의하나로남아있다. 그것은 disk displacement가 acute trauma에의한경우를제외하고는일반적으로오랜시간에걸쳐 chronic한진행과정을거쳐 ligament가 elongation 됨으로서발생하기때문에일단이탈된 disk를제위치로회복시켜주기가거의불가능하기때문이다. 종종초기의 disk displacement는 splint therapy나 mandible의적절한 manipulation을통해서회복이가능하나만성적인 disk displacement는치료가거의불가능하며일시적으로회복을시킨다하여도쉽게 relapse되는경우가많다. Joint 내의 clicking이나 popping은 pain과더불어가장흔한 TMD 환자의 complaint 중의하나이다. 통계에의하면일반인의 30% 정도가 joint sound를갖고있는것으로나타나있다. 그러나 Disk displacement를치료하지않고는 joint sound의제거가불가능하다. 따라서 TMJ specialist들은 TMD의치료에있어 joint sound의제거까지를환자에약속하지말것이며 pain을동반하지않는 joint sound는치료의대상에포함시키지않을것을권하고있다. Inflammatory Joint disease 환자가임상의를찾는가장큰이유의하나는 TMJ의심한 pain과그로인한 mouth opening의제한일것이다. Joint내의 pain의주된원인은 synovitis, capsulitis, 또는 retrodiscitis 같은 joint 내부조직의 inflammation이다. Macro-trauma로인한 inflammation은원인이되는 trauma가계속되지않는한자연적으로치유가되므로통증을완화시키고 healing을돕는 supportive treatment를제외하고는특별한치료가필요하지않다. 특히손상된조직에더이상의 trauma를주지않도록 mandible의운동을제한하는것이중요하다. 그러나경우에따라서 hemorrhage로인한 joint의 ankylosis를방지하고 muscle의 stiffness를방지하기위하여 mild한 mandible의 exercise는필요하다. 같은 joint의 inflammation이라도 capsular ligament의손상으로인한경우는 splint therapy가도움이되지않지만 swelling을동반한 retrodiscitis의경우는 joint space를넓혀주고 condylar head로인한 retrodiscal tissue에의이차적 trauma의방지를위하여 splint therapy가필요하므로정확한감별진단이매우중요하다. Conclusion TMD는 dental practitioner 사이에오랜논란의대상이되어왔다. Arthrogram이나과학적인장비, 그리고최근의많은연구에의하여 TMD에대한많은의문점들이밝혀지고있으나아직도 pathogenesis나그발생원인에대하여논란이계속되고있다. 특히그간 dental practitioner에게정론처럼알려져왔던 TMD와 occlusion의관계에관한이론의변화로인하여 TMD의치료개념에많은변화가있었다. 그동안 TMD에대한정확한이해가없는상태에서여러가지입증되지않은추론과가설이임상가들사이에알려지기도하였다. 특히한국에서 TMJ disorder와전신질환과의연관관계에대하여연구가있었고이이론을임상에적용하려는시도가있었던걸로알고있다. 필자도이이론에대한과학적인근거를찾기위하여여러문헌들을조사해보았으나이를입증할만한근거를찾을수가없었다. 이와유사한이론으로미국의 chiropractitioner사이에서가설로남아있는이론이있다. 그이론에따르면 만곡된제 1, 2 경추 (C1,C2) 로인하여 spinal cord가

13 압박을받으므로서이신경조직의지배를받는여러장기들이영향을받아전신질환의원인이된다. 는것이다. 일견 make a sense 한면이있는 theory 이다. 그러나이에대한대부분의 chiropractics 임상가의견해는 그것이사실이었으면정말좋겠다.' 는것이다. 그렇다면 C1 C2 를교정하는것만으로많은전신질환들을간단이치료할수있으며더구나이렇게되면 chiropractitioner 가모든의료계의우위에서서모든질환들을 control 할수있기때문이다. TMJ 에는전신질환에영향을줄만한중요한 anatomy 를포함하고있지않을뿐더러이이론을뒷받침할만한입증된연구결과나임상증례를찾을수없다. 비약적인상상력은과학발전의원동력이되어왔다. 하지만의학적인가설을임상에적용하는것은충분한연구결과로과학적인입증과정을거치기전에는신중이선택되어져야할것이다.
