Objectives To evaluate effectiveness of one hundred thirty kinds of pharmacopuncture made of herbs used in Sasang Constitutional Medicine. 2. Methods We searched at fifteen kinds of Korean oriental medical journals websites and four kinds of Korean oriental medicine databases. ' 약침 ', ' 수침 ', 'pharmacopuncture', 'aqua acupuncture' and 'herbal acupuncture' were keywords. Searching period was March to April Results One hundred eighty three articles are searched. And these are the results. 1) A lot of Soeum herb pharmacopunctures are effective in anti-tumor, brain damage, rheumatoid arthritis. 2) A lot of Soyang herb pharmacopunctures are effective in kidney injury, asthma, rheumatoid arthritis. 3) A lot of Taeeum herb pharmacopunctures are effective in obesity, asthma, anti-tumor, anti-oxidation, rheumatoid arthritis. 4) Taeyang herb pharmacopunctures are effective in rheumatoid arthritis, amyotrophia.. 4. Conclusions There was similar effectiveness between oral administration and pharmacopuncture in Sasang Constitutional herbs. It suggest that pharmacopuncture could be used for Sasang constitutional disease and symptoms. Well-designed clinical studies for Sasang constitutional pharmacopuncture are needed. 2 김구외사상체질의학회지 2010;22(4): Ⅰ. 序論 약침은 1960년대국내에처음소개된이래임상에서높은빈도로사용되고있으며, 한의학의보편적치료법중하나로자리잡아가고있다. 이에따라약침에대한연구도다양하게진행되어많은연구결과가약침치료를적용하는데근거가되고있으며 1, 2, 한방진료영역의확대가능성을높이고있다. 사상체질의학적관점의건강관리및치료는질병의예방및관리, 건강증진의필요성이대두되면서더욱주목받고있으며, 체질건강관리에대한욕구또한증가하고있다 3. 이에사상체질의학과약침치료의응용가능성또한제시되고있는데, 기존의연구로환자의발진, 변비, 현훈, 무력감, 흉부불편감등의주소를태음인체질병증으로인식하여우황을약침으로활용한예가있었고 4, 사상체질별保命之主강화를치료원칙으로체질과체질병증에따라활용할수있는경혈및약침이제시되기도하였다 5. 약침치료의유효성에대한연구로는현재무작위대조군연구에대한체계적고찰을통해약침치료의유효성을검증하고자하는연구가있었으나 6, 7, 약침을사상체질약재별로분류하여유효성을분석한연구는없었다. 본연구는 東醫壽世保元 新定處方의구성약재로추출한약침에대하여국내한의학학술지검색을통해유효성을조사하여보고하는바이다. Ⅱ. 方法 1. 분석항목약재별로연구주제와약침후의효과를조사하였다. 연구주제는논문제목과내용을종합하여한가지주제로요약하였으며, 각논문의결론에서유의한효과가있다고기술한질환을기록하였다. 2. 문헌검색가. 검색약재선정 東醫壽世保元辛丑本 에서新定少陰人病應用要藥二十四方, 新定少陽人病應用要藥 十七方, 新定太陰人病應用要藥二十四方, 新定太陽人病應用設方藥二種 ( 이하신정처방 ) 의구성약재의약침을검색하였다. 소음인약재는감초, 건강, 계, 계지, 계피, 곽향, 당귀, 대산, 대조, 반하, 백두구, 백작약, 백출, 백하수오, 보골지, 부자, 사인, 산사, 생강, 소엽, 양강, 앵속각, 오수유, 익모초, 익지인, 인삼, 인진, 적석지, 적하수오, 진피, 창출, 천궁, 청밀, 청피, 총백, 파두, 향부자, 황기로 38종이었다. 소양인약재는감수, 강활, 경분말, 고삼, 과루인, 구기자, 금은화, 독활, 목단피, 목통, 박하, 방풍, 백복령, 복분자, 산수유, 석고, 생지황, 숙지황, 시호, 연교, 우방자, 인동등, 저령, 적복령, 전호, 주사, 지골피, 지모, 차전자, 치자, 택사, 황련, 현삼, 형개, 황백, 활석으로 36종이었다. 태음인약재는갈근, 감국, 건율, 고본, 과체, 관동화, 금박, 길경, 나복자, 녹용, 대두황권, 대황, 마황, 맥문동, 백과, 백렴, 백자인, 백지, 사군자, 사향, 산약, 산조인, 상백피, 서각, 석창포, 승마, 연자육, 영양각, 오매육, 오미자, 용뇌, 용안육, 우황, 웅담, 원지, 의이인, 부평, 저근백피, 제조, 조각, 천문동, 포황, 행인, 황금으로 44종이었다. 태양인약재는교맥미, 노근, 모과, 미후도, 미후등, 송엽, 송화, 앵도육, 오가피, 저두강, 청송절, 포도근으로 12종이었다. 이중산삼, 홍삼은인삼에, 가시오가피는오가피에포함시켰다. 나. 대상학회지및검색조건대상학회지는국내학술연구재단에등재되었거나등재후보인 15개의한의학학술지인경락경혈학회지, 대한한의학회지, 동의생리병리학회지, 대한약침학회지, 대한침구학회지, 대한한방내과학회지, 대한본초학회지, 동의신경정신과학회지, 한방안이비인후피부과학회지, 대한한방부인과학회지, 대한한방소아과학회지, 대한한의학원전학회지, 사상체질의학회지, 한방재활의학회지, 대한한약학회지를대상으로 2010년 3월부터 4월까지 1개월간각학회홈페이지를통해검색하였으며, 검색논문의누락을방지하기위하여논문검색포탈사이트인 한국전통지

Flow chart of the retrieved literature 식포탈, 과학기술학회마을, 한국과학기술정보통합서비스, 한국학술정보학술데이터베이스 에서병행하여검색하였다. 검색어는제목및키워드에 약침, 수침, pharmac- opuncture, herbal acupuncture, aqua acupuncture 로검색하였다. 검색결과중시험관내연구및동물실험, 임상증례및임상시험등의원저만을선택하였으며문헌고찰논문, 체계적고찰등은조사대상에서제외하였다. 단미약침만을대상으로선택하였으나기원이같은약재들을함께시험한경우에는포함하였다. 논문발행연도에대한제한은없었으며, 조사대상연구논문들의출판기간은 1985년 1월이후부터 2009년 12월까지였다 (Figure 1). Ⅲ. 結果 1. 전체약재분석검색대상인 130종약재의단미약침논문은총 198 편이었다. 그중소음인약재 18종에대하여 72편, 소 양인약재 20종에대하여 38편, 태음인약재 21종에대하여 76편, 태양인약재 3종에대하여 12편의논문이조사되었다. 유효성연구는 62종의약재가조사되었다. 소음인약재중계지, 당귀, 반하, 백작약, 백하수오, 보골지, 부자, 산사, 인삼, 생강, 오수유, 익지인, 인진, 천궁, 청피, 파두, 향부자, 황기로 18종이었으며, 소양인약재중강활, 과루인, 구기자, 금은화, 독활, 목단피, 방풍, 백복령, 복분자, 시호, 연교, 우방자, 저령, 전호, 생지황, 숙지황, 차전자, 치자, 택사, 황련, 황백으로 20종이었다. 태음인약재중갈근, 감국, 관동화, 길경, 나복자, 녹용, 마황, 맥문동, 백지, 부평, 사향, 산조인, 상백피, 오미자, 우황, 웅담, 의이인, 제조, 천문동, 행인, 황금으로 21종이었으며, 태양인약재중모과, 오가피, 포도근으로 3종이었다. 유효성이연구된약재의분포는소음인약재가 18종으로 47.4%, 소양인약재가 20종으로 55.6%, 태음인약재가 21종으로 47.7%, 태양인약재가 3종으로 25% 였으며, 총 62종으로 47.7% 가조사되었다 (Table 1). 7 16 사상체질의학회지 2010;22(4):10-19 東醫壽世保元 新定處方藥材의藥鍼有效性文獻調査 2. 약침의유효성분석가. 소음인약재소음인약재 18종에대한약침논문 72편중계지, 당귀, 반하, 백하수오, 보골지, 부자, 산사, 인삼또는산삼또는홍삼, 생강, 오수유, 익지인, 인진, 작약, 천궁, 청피, 파두, 향부자, 황기의 18종약재에서유효성과관련하여 67편의논문이검색되었다. 그중오수유, 익지인, 인삼, 천궁, 황기가항암효과가있는것으로나타났다. 파두는대조군에비하여유의한항암작용이없었다. 계지, 부자, 천궁, 황기는류마티스관절염에효과가있었으며, 당귀, 인삼, 인진, 천궁은뇌신경세포손상의회복효과가있었다. 부자, 산사, 인삼은비만에효과가있었으며, 인삼, 인진, 청피는간손상에회복효과가있었다. 백하수오, 작약, 황기는항산화작용이있었다. 계지, 인삼은당뇨에, 인삼, 황기는면역기능활성, 인삼, 황기는자율신경계기능변화에유의한효과가있었다. 이밖에당귀는난소기능부전, 갑상선기능저하증에효과가있었으며, 반하는천식, 보골지는부신피질기능부전증, 부자는척수손상및신경병증성동통질환, 생강은중풍후유증으로인한견비통에효과가있었다. 인삼은자궁내막증, 대장염, 성장등에도효과가있었다. 향부자는골다공증에유의한효과가있었다 (Table 2). 나. 소양인약재소양인약재 20종에대한약침논문 38편중강활, 과루인, 구기자, 금은화, 독활, 목단피, 방풍, 복령, 복분자, 시호, 연교, 우방자, 저령, 전호, 지황, 차전자, 치자, 택사, 황련, 황백의 20종약재에서유효성과관련하여 38편의논문이검색되었다. 그중백복령, 저령, 숙지황, 차전자는신장기능변화와신장손상의회복효과가있었다. 강활, 과루인, 연교, 전호는천식에효과가있었다. 강활, 독활, 황백은류마티스관절염에효과가있었으며, 금은화, 복분자, 황백은항암작용이있었다. 시호, 차전자, 치자는간손상에회복효과가있었다. 시호, 택사는항산화작용이있었다. 이밖에구기자는골다공증, 목단피는고지혈증에효과가있었으며, 방풍은소염진통효과와기관지상 피세포손상의회복효과가있었으며, 백복령은당뇨에효과가있었다. 시호는관절염과부종및알러지질환에효과가있었다. 우방자는항알러지효과가있었으며, 생지황은자율신경계기능변화에효과가있었다. 치자는염좌에, 황련은대장염에효과가있었다 (Table 3). 다. 태음인약재태음인약재 21종에대한약침논문 76편중갈근, 감국, 관동화, 길경, 나복자, 녹용, 마황, 맥문동, 백지, 부평, 사향, 산조인, 상백피, 오미자, 우황, 웅담, 의이인, 제조, 천문동, 행인, 황금의 21종약재에서유효성과관련하여 66편의논문이검색되었다. 그중나복자, 마황, 부평, 황금은비만에효과가있었으며, 관동화, 길경, 마황, 맥문동은천식에효과가있었다. 녹용, 마황, 백지, 의이인은류마티스관절염에효과가있었으며, 녹용, 제조, 천문동, 행인은항암작용이있었다. 감국, 녹용, 오미자, 황금은항산화작용이있었다. 갈근, 녹용은뇌신경세포손상, 녹용, 황금은신장손상에회복효과가있었다. 갈근, 사향은고지혈증에효과가있었다. 이밖에녹용은관절염, 골다공증, 두통, 이명, 성장에효과가있었다. 마황은부신피질기능부전증에효과가있었으며, 맥문동은당뇨에효과가있었다. 산조인은불면, 상백피는간손상, 알러지질환에효과가있었다. 오미자는용혈성빈혈에효과가있었다. 우황은항균작용에대한연구가있었으나대조군에비하여유의한효과가없었다. 웅담은아토피피부염에효과가있었으며, 황금은항염증효과가있었다 (Table 4). 라. 태양인약재태양인약재 3종에대한약침논문 12편중모과, 오가피, 포도근의 3종약재에서유효성과관련하여 12편의논문이검색되었다. 모과는류마티스관절염, 근위축증에효과가있었다. 오가피는류마티스관절염, 골다공증, 뇌신경세포손상, 당뇨, 대사증후군, 알러지질환에효과가있었다. 포도근은항산화작용과동맥경화에효과가있었다 (Table 5).

Lim SC, Study type : a in vitro, b animal experiment, c case study, d clinical study Extraction method : 1 water extract-alcohol precipitate, 2 water extract, 3 distillation, 4 squeeze, 5 immersed in cold water, 6 alcohol extract, 7 other, 8 do not write * JKAMS = The Journal of Korean Acupuncture and Moxibustion Society, KJMA = The Korean Journal of Meridian and Acupoint, JKOM = The Journal of Korean Oriental Medicine, JKIP = Journal of Korean Institute of Phamacopuncture, KJH = Korea Journal of Herbology, KJOIM = Journal of Korean Oriental Internal Medicine, # JON = Journal of Oriental Neuropsychiatry Ⅳ. 考察 약침의유효성을살펴본결과, 소음인약재는항암작용, 뇌손상, 류마티스관절염등에대한연구가많았으며, 국내항암약침의연구를살펴봐도소음인약재를이용한연구가많은수를차지하고있었다 8, 9. 소양인약재는신장기능변화와신장손상, 류마티스관절염등에대한연구가많았고, 태음인약재는비만과천식, 항암작용, 항산화작용, 류마티스관절염등에대한연구가많았으며, 태양인약재는류마티스관절염, 근위축등의질환에대한연구가있어체질별로차이가있었다. 이것을사상체질의학적병리관으로본다면, 소양인은脾大腎小하기때문에신장기능에이상이생길가능성이높고, 따라서신장을치료하는약재가많았으며, 태음인은吸聚之氣가강하여에너지가과잉축적되어발생하는비만또는고지혈증과같은질환이생길가능성이높으므로이에유효한약재가많고, 폐의기능과呼散之氣가약하여발생하는천식이생길가능성이또한높으므로이에유효한약재가많았다고해석할수있다. 소음인약재중인삼, 인진, 청피는간손상에대한회복효과가있었다. 소음인의황달은소음인태음증으로분류되며인삼, 인진, 청피는태음증처방에주로사용된다. 청피가구성약재로포함된처방인십이미 관중탕과곽향정기산은소음인의황달을치료한예가있으며 10, 11, 인삼이구성약재인오수유부자이중탕과십이미관중탕은간세포에대한항산화작용과간보호효과가있는것으로나타나 12, 인삼, 인진, 청피와소음인처방의활용에서유사점이있었다. 소양인약재중백복령, 저령, 숙지황, 차전자는신장기능변화와신장손상의회복효과가있었다. 복령, 저령, 차전자의사상체질의학적약리는소양인에서利水를통하여降陰하며, 숙지황은滋腎을통하여淸陽上升의목적을달성하는것이다. 천식에효과가있었던강활, 과루인, 연교, 전호중과루인과전호는胸膈의熱痰을淸化시켜陰氣가하강하도록함으로써咳嗽, 胸痞, 寒熱往來등을치료한다. 강활은解表하면서降陰시키며, 연교는淸熱解毒한다 13. 백복령, 숙지황, 차전자등의약재로구성된처방인형방지황탕은신장및방광세포등에항산화작용이있고 14, 강활, 과루인, 전호등의약재로구성된처방인형방도적산은결흉증의범주에해당하는咳嗽와喘息에활용하므로, 위의약침과소양인처방의활용에서유사점을찾을수있었다. 태음인약재중나복자, 마황, 부평, 황금은비만에효과가있었고, 관동화, 길경, 마황, 맥문동은천식에효과가있었다. 사상체질의학적약리는나복자는消食작용을통해小腸濕熱을배출하고, 마황은發散
9 18 사상체질의학회지 2010;22(4):10-19 東醫壽世保元 新定處方藥材의藥鍼有效性文獻調査 風寒, 부평은發散風熱, 황금은간과소장의濕熱을풀어냄으로서肺熱을다스려, 肝大肺小한태음인의肺局을강화한다. 맥문동은補肺, 潤肺, 淸肺하며길경과관동화는肺氣管支의痰을제거하여開肺解表한다. 나복자, 마황등으로구성된처방인태음조위탕은경구투여에서비만에효과가있었으며 15, 태음인처방중청폐사간탕, 갈근부평탕, 청심연자탕, 태음조위탕은지방세포의증식을억제하여비만에효과가있었다 16. 관동화, 길경, 마황, 맥문동등으로구성된마황정천탕은천식에효과가있어 17 경구투여와약침에서동일한효과가있었다. 또한마황은부신피질기능부전에효과가있었는데, 이는태음인의寒厥證의범주에해당되며, 寒多熱少湯은부신기능부전증환자에활용되어유사한효과가있었다 18. 태양인약재중모과는근위축에효과가있었는데, 태양인해역증의양하지무력증에대하여미후등식장탕을활용하여호전된보고가있어 19 약침과태양인처방의활용에유사한치료효과가있었다. 이와같이본연구를통하여사상체질처방의경구투여와약침의효과가유사하거나, 약침치료의사상체질의학적해석이가능한부분을확인할수있었다. 한편, 본연구의한계점으로, 조사한논문중약 90% 이상이실험설계가용이한시험관내연구와동물실험이었으며, 임상시험과증례논문은 16편으로비교적적은편이었다. 또한, 각논문마다실험중대상개체에대한주입량및농도가달라서실험간상호비교가어려웠으며, 유효량에대한기준이나근거가없이시행된경우가많아유효량평가가어려웠다. 약침투여기간및횟수에서도기간및횟수가질환또는약재에따라달라지는경우가많아, 이에대한기준마련이필요하다. 본연구에서는자입경혈및깊이에대하여는조사하지않았으나, 약침요법에서는약물의선정뿐만아니라경혈의선택이매우중요하기때문에, 이후에는각체질별유효한경락과경혈선택에대한추가적인연구가이루어져야할것으로생각된다. Ⅴ. 結論 동의수세보원신정처방의구성약재가약침으로 사용된논문들에대한문헌검색을통해 183편의논문에서유효성에대한내용을조사하여결과는다음과같았다. 1. 소음인약재약침은항암작용, 뇌손상, 류마티스관절염등에유효성이확인된연구가많았다. 2. 소양인약재약침은신장손상, 천식, 류마티스관절염등에유효성이확인된연구가많았다. 3. 태음인약재약침은비만, 천식, 항암작용, 항산화작용, 류마티스관절염등에유효성이확인된연구가많았다. 4. 태양인약재약침은류마티스관절염, 근위축등에유효성이확인된연구가있었다. 이상의결과에서체질별약재들의경구투여와약침처치간의효능에유사점을확인할수있었으며, 체질병증에약침의활용가능성을확인하였다. 향후보다객관적이고높은질을갖춘임상연구를통해체질별약침의응용가능성을확인해야한다.
