Increased cortisol secretion is associated with low levels of growth hormone concentrations. Insulin-like growth factor-1 (IGF-1) production is stimulated by growth hormone. We hypothesize that stress may be associated with IGF-1 levels. Methods: We assessed circulating levels of IGF-1 in 2,371 individuals (Men 1,460, Women 911) who coming at health promotion center. Stress score was measure by Modified-Korean BEPSI self reported questionnaire. Results: In young men, IGF-1 level of high stress group was significantly decreased than lower stress group. This significance was remained after multivariable adjusted. But, young women and elderly groups had not significant difference of IGF-1 level according to the low stress or high stress. Conclusion: The IGF-1 level of high stress group was decreased than low stress group. But, this significant difference was showed in only young men. (Korean J Str Res 2008;16: ) Key Words: Stress, Modified-Korean BEPSI, Insulin-like growth factor-1 서 최근건강에대한관심이많아지면서건강에영향을주 는생활습관에대한연구도많이되고있다. 건강에영향 을주는생활습관으로는음주, 흡연, 활동량외에도스트레 스또한중요한인자로대두되었다. 스트레스는심리학적 장애뿐아니라심혈관질환, 소화기계질환및알레르기 질환등다양한신체적질환에영향을주는것으로알려져 책임저자 : 공미희, 제주도제주시삼도 2 동 , 제주대학병원가정의학과 Tel: , mdoc@nate.com 접수 : 2008 년 6 월 19 일, 게재승인 : 2008 년 8 월 19 일 론 있다 (Henry et al., 1993; Raiha et al., 1998; Buske-Kirschbaum et al., 2002; Bunker et al., 2003). 스트레스를받으면대뇌변연계를통해신경호르몬들이시상하부를자극하게되어시상하부-뇌하수체-부신축 (Hypothalamus-Pituitary-Adrenal gland axis, HPA axis) 을활성화하여코티졸 (cortisol) 의분비가증가하게된다. 이러한스트레스자극이만성적으로진행되면 HPA axis가계속적으로자극되면서증가된코티졸의영향으로혈중성장호르몬및성호르몬의분비가감소하게된다고하였다 (Chrousos & Gold, 1992). 인슐린양성장인자-1 (Insulin like growth factor-1, IGF-1) 은성장호르몬의자극으로분비되는것으로주로조직의 207

2 스트레스硏究 : 제 16 권제 3 호 2008 성장과분화에관여한다. 성장호르몬과비교하여일중변화가적어성장호르몬관련질환이나치료효과판정에도움을주는것으로알려져있다 (Thissen et al., 1996; Aimaretti et al., 2004). 본연구에서는스트레스를받으면성장호르몬이감소하여결국혈중 IGF-1 농도가감소하리라는가정하에국내한종합병원으로건강검진을위해내원한사람을대상으로나이에따른 IGF-1농도의변화를고려하여젊은군과노인군두군으로나누어스트레스정도에따른혈중 IGF-1 농도의차이여부를알아보고자하였다. 재료및방법 1. 연구대상자 2000년 3월부터 2004년 6월까지국내한종합병원으로건강검진을위해내원한 18세이상성인을대상으로성별과나이에따라 35세미만의젊은남자군 (705명), 젊은여자군 (755명) 그리고 60세이상의노인남자군 (499명), 노인여자군 (412명) 으로나누었다. 이중혈중 IGF-I 농도에영향을줄수있는만성질환자및약물복용자를제외하였다. 제외기준은다음과같다. (1) 간질환 (B형및 C 형간염보균자, 간경화, SGOT or SGPT >50 U/L), (2) 갑상선질환 ( 갑상선질환과거력, 갑상선호르몬관련약제복용, TSH>10.00 μu/ml or <0.15μU/mL, free T4>1.70 ng/dl or <0.6 ng/ dl), (3) 신장질환 (Cr>1.5 mg/dl ), (4) 성호르몬및스테로이드약제복용, (5) 인슐린이나혈당강하제복용, (6) 암과거력. 2. 혈액검사 12시간이상의금식을유지하고오전 8시에정맥혈을채혈하였다. 공복혈당측정은 colorimetric glucose oxidase method (Shin yang chemistry), 중성지방측정은 enzymatic colorimetric method (KYOWA MEDEX), 고밀도콜레스테롤측정은 modified enzymatic method (KYOWA MEDEX) 방법을이용하였다. IGF-1은 Immunoradiometric assay using a DSL-2800 ACTIVE R kit (Cherwell Innovation Centre) 방법을이용하여측정하였다. 3. 스트레스정도측정국내에서어느정도타당도가검증된한국어수정판 BEPSI (Brief Encounter Psychosocial Instrument) 를 (Yim et al., 1996) 이용하여자기-기입식으로측정한후총점수를합 산하여스트레스총점을측정하였다. 점수는각 5개의문항마다느끼는정도에따라 Likert s Scale로측정하여 1점에서 5점까지부여한후합산하여, 최저 5점부터최고 25점까지측정할수있도록하였다. 스트레스총점을 SPSS 프로그램을통하여높은군과낮은군 2개의군으로범주화하였다. 4. 통계학적분석통계학적분적은 SPSS Window version 11.0을이용하였다. 일반적특징에서성별비교는 t-검정 (Independent-Samples t-test) 을이용하였다. IGF-1 농도와의관련요인분석에서는편상관분석 (partial correlation) 을이용하여나이를보정하고관련요인을알아보았다. 각군에서의스트레스정도에따른혈중 IGF-1 농도의비교는공분산분석 (ANCOVA) 을이용하였다. 모든통계학적유의수준은 p value<0.05로하였다. 결과 1. 기본적특징젊은군에서남자가여자보다체질량지수, 허리둘레, 혈압, 공복혈당, 총콜레스테롤, 중성지방이유의하게높았으며, 고밀도콜레스테롤과 IGF-1 농도는유의하게낮았다. 노인군에서는남자가여자보다허리둘레와 IGF-1 농도가유의하게높았으며, 체질량지수, 총콜레스테롤, 고밀도콜레스테롤이유의하게낮았다. 스트레스정도를알아보는한국어수정판 BEPSI 총점은젊은군에서는남녀의차이가없었으나, 노인군에서는여자가남자보다스트레스총점이더높은것으로나타났다 (Table 1). 2. IGF-1과의관련인자들나이를보정한후혈중 IGF-1 농도와의연관성을살펴본결과남자의경우체질량지수, 허리둘레, 혈압, 공복혈당, 중성지방이통계학적으로유의한양의상관관계를보였으며, 여자의경우는체질량지수, 허리둘레, 혈압, 중성지방이유의한양의상관관계를보였다. 남자의경우혈중 IGF-1과고밀도콜레스테롤및스트레스총점은유의한음의상관관계를보였고, 여자의경우는고밀도콜레스테롤및스트레스총점이혈중 IGF-1농도와음의상관관계를보였으나통계학적으로유의하지않았다 (Table 2). 208

Adjusted for age, body mass index, waist circumference, systolic blood pressure, diastolic blood pressure, fasting glucose, total cholesterol, triglyceride and HDL cholesterol levels. *significant difference comparisons by stress score within same group (p=0.019). 점 (420명) 이였다. 노인남자군의경우스트레스가낮은경우는평균 6.8±1.48점 (340명), 높은군은 11.2±1.71점 (159명) 이고, 노인여자군은스트레스가낮은경우는평균 7.2 ±1.42점 (197명), 스트레스가높은경우는 11.9±2.39점 (215 명 ) 이였다. 각군에따라혈중 IGF-1농도를비교한결과젊은남자군에서스트레스가높은경우의 IGF-1 농도가 221.3±89.88 ng/ml로스트레스가낮은경우의 IGF-1농도 (235.7±

4 스트레스硏究 : 제 16 권제 3 호 2008 ng/ml) 보다유의하게감소되었으며, 이는나이, 체질량지수, 허리둘레, 혈압, 공복혈당, 총콜레스테롤, 중성지방그리고고밀도콜레스테롤을모두보정한이후에도지속되었다 (P=0.019). 젊은여자군과노인남자군및노인여자군에서는스트레스가높고낮음에따라혈중 IGF-1농도의차이가통계학적으로유의성이없었다 (Fig. 1). 고찰본연구결과젊은남자에서스트레스정도가높을수록혈중 IGF-1 농도가유의하게낮아진다는것을알수있었다. 이러한결과는나이및기타인자들을보정한후에도유의한것으로보아스트레스정도와혈중 IGF-1 농도는독립적인연관성이있다고할수있다. 이는본연구시작의가설과일치하는결과라고할수있다. 과거연구에서도스트레스상황인중환자실환자를대상으로혈중 IGF-1 농도를측정한결과감소되었다고보고하였다 (Kyle et al., 2005). 혈중 IGF-1 농도에영향을주는것으로알려진것은주로나이의영향으로나이가증가함에따라성장호르몬과평행하게 IGF-1 농도또한감소한다고하였다 (Corpas et al., 1993; Goodman-Gruen & Barrett-Connor, 1997). 그외에도폐경기이후여성의 IGF-1 농도가감소하게되고여성호르몬대체치료가혈중 IGF-1 농도를높인다는연구결과등 (Slowinska-Srzednicka et al., 1992; Poehlman et al., 1997) 여성호르몬이 IGF-1농도에영향을미치며, 간질환및신장질환등이동반되었을때 IGF-1농도가감소되는등기저질환이영향을준다고알려져있다 (Schalch et al., 1998; Hall et al., 1999). 본연구에서젊은남자에서만스트레스가높고낮음에따라혈중 IGF-1의농도의차이가유의하게나타났고, 젊은여성이나노인에서는스트레스정도에따라혈중 IGF-1 의농도에차이가유의하지않았다. 이러한차이를보이는이유에대해서는혈중 IGF-1 농도에미치는영향력의차이때문이라는가설로설명할수있겠다. 혈중 IGF-1 농도에나이와여성호르몬이미치는영향력이스트레스정도가 IGF-1 농도에미치는영향력보다크기때문에나이와여성호르몬영향이크게나타나는여성이나노인에서는스트레스정도가 IGF-1 농도에변화를주지못했으리라생각한다. 그러나이는가설일뿐보다정확한연구가추가적으로필요할것이다. 본연구의한계점으로는무엇보다 35세미만과 60세이상만을대상으로했다는것을들수있다. IGF-1의농도가 나이에큰영향을받으므로젊은군과노인군으로나누어분석하다보니그나이의경계를정하기가어려웠으며또한대상자들이상기나이로구분하였을때남녀의비율이가장균형적이여연구자가임의로설정하였다는큰단점을가지고있다. 또한본연구만으로는스트레스와 IGF-1 의관련성에성별및나이가어떠한영향을주었는지, 그리고단면연구로서인과관계를규명하지못하는단점들에도불구하고대상자의수가많고스트레스정도와혈중 IGF-1의농도를나이와성별군으로나누어살펴본첫연구로서그가치를가진다고할수있다. 결론적으로젊은남자의경우스트레스정도가높을수록혈중 IGF-1 농도가낮아졌다. 이는혈중 IGF-1과생활습관과의관련성을연구한다른연구들의 (Landin-Wilhelmsen et al., 1994; Taaffe et al., 1994; Lee et al., 2003) 연장선에서스트레스또한혈중 IGF-1 농도에영향을줄수있다는가능성을보인연구이다. 그러나젊은여자나노인에서같은결과가보이지않은원인과스트레스와 IGF-1 농도사이의인과관계여부등을규명하기위해서는추가적연구가필요할것이다. 참고문헌 Aimaretti G, Corneli G, Rovere S, et al. J Korean Acad Fam Med 17:42. = 국문초록 = 장기간스트레스를받으면성장호르몬분비가억제되어그에따라혈중 IGF-1 농도또한감소한다는실험실적연구들이보고되고있다. 본연구에서는스트레스정도가높은사람의혈중 IGF-1 농도가낮을것이라는가정하에스트레스정도에따라혈중 IGF-1 의농도를나이와성별군에따라알아보고자하였다. 건강검진을위해내원한 18 세이상의성인을대상으로혈중 IGF-1 농도와한국어 BEPSI ( 수정판 ) 을이용한스트레스총점을측정하였다. 나이에따른혈중 IGF-1 의변화를고려하여 35 세미만의젊은군 ( 남자 705 명, 여자 755 명 ) 과 60 세이상의노인군 ( 남자 499 명, 여자 412 명 ) 으로나누었다. 각군에서스트레스총점이높고낮음에따라혈중 IGF-1 농도를비교하였다. 젊은남자군의경우스트레스총점이높은경우혈중 IGF-1 의농도가 221.3±89.88 ng/ml 로스트레스총점이낮은경우의혈중 IGF-1 농도 (235.7±94.20 ng/ml) 보다유의하게감소하였다 (p=0.019). 그외노인남자군 (141.9±82.49 vs 139.3±82.33 ng/ml, p=0.578) 이나젊은여자군 (255.2± vs 263.6± ng/ml, p=0.256) 과노인여자군 (113.4±69.34 vs 117.1±68.89 ng/ml, p=0.660) 은모두스트레스총점이높고낮음에따라혈중 IGF-1 농도의차이는통계학적유의성이없었다. 본연구결과스트레스정도가높은경우혈중 IGF-1 농도가유의하게낮아있었다. 그러나이러한유의성은젊은남자에서만나타났다. 나이와성별이미치는영향과인과관계규명등의추가적인연구가필요할것이다. 중심단어 : 스트레스, 한국어 BEPSI, 인슐린양성장인자
