But it does not involve the skull, meninges and cerebell um. CT proved to be unique tool in diagnosis of hydranencephaly We analysed CT findin gs of 6 pati ents w ith hyd ranencephaly who visited Cat h 이 i c University Medical Col lege Hospitals, during last 6 years Th e results were as foll ows: 1. The male to female ratio was 1:2 and the age ranged 29 to 121 days 2. Skull, brain stem, cerebellum and thalami were intact in all the cases 3. alx cerebri was fully intact in 2 cases and partially defective in 4 cases 4. In 2 cases, th e basal ganglia were seen bilaterally and symmetrical ly 5. Remnant cerebral hemispheres were as follows; Partial and bilateral occipital lobes in al l the cases Partial and unilateral frontopari eta l lobe in 1 case Partial and bilateral temporal lobes in 1 case. 6. AII resulting cavities were filled with fluid 7. In all the cases, 3rd and 4th ventri cles were present. Among the cases, 3 cases showed choroid pl ex us and latera l ventricles, communicating w ith cavity bilaterally 8. CT 가개발된이후마릎질환고벼감벨이용이하며정확한 3 예에서는좌측으로. 2 예에서는우측으로천위되어있 었다 ( i g. 2- C, 3- B). 뇌실은모든예에서제 3 뇌살과제 4 뇌실이요출되거 진단이가능하게되었으며 CT 는바침습적검사방법이 냐추적이가능하였고측뇌살은그중 3 예에서묘출되었 묘로추적검사에도유용하게쓰이게되었다 1, 7) 저자들은가폴럭의대산하 4 개부속영원에서 C T 을 는데 ( Ta b le 1) 이들은모두양측성으호공동파개방되 어있었다 (ig. 1-B, 3- A). 시행하여진단된수무뇌증 6 예를내상으로 C T 소견을 대뇌반구는모든예에서소량의후두엽이 % 택에 냐 분석하고운현고찰과함께보고하는바이마. 다냐있었 ιk 펀측에전두엽과두정엽이확인되었던예 ll. 대상및방법 와양측에측두엽이냐타난예도각각 l 예씩있었다 (ig. 1, 2 & 3). 공동은모두액체음영으로충만되어있었마 ( i g.3 ) 년이후 년까지가툴럭의대산하 4 개부 속영원에서 CT 상수우뇌증으로확진된 29 일부터 121 일사이의남아 2 명, 여아 4 영등 6 명을대상으로하 한예에서는두꺼워진수악구조물이두캐골 o.f 측의 연으로부터분리되어판찰되었다 ( i g.4 ). 내 였다 ( T ab l e 1). 이들의 CT 소견을두개골, 간뇌, 대뇌겸, 뇌살, 소뇌, 대뇌, 시상, 기저신경절의형태빛크기에따라분석하였마. Bilateral thickening of meningeal structures, separated from the inner table of the skull, are accompanied by lateral ventricles and choroid plexus which is communicating with fluid filled cranial cavity. C. Bilateral meningeal structures, appearing with partial and bilateral occipitallobes, are visible continuously. 이질환은태생 3 개월로부터생후 2 년사이에서파 괴적혹은발생학적이 % 벼 l 의해생긴다 6 ) 발생원인에대해서는여러가지설이있으나아직 확하지않마. Ya k ov lev 등은발생학적 A 로신경벽 (neura l wall) 의일차적무형성에의해발생하며, 이는태 생기에해연동상부내경동액의페색에의한대뇌쿠조 물의경색에기인하는것으로설명하였다 11 ) 영 많은저자 들이이와비슷한혈관의폐색을일차적인원인으로주 장하고있마 10, : 또한동물실험에서경동맥결찰이 냐파라펀주업등으로수무뇌증파유사한뱅변을유말 시킬수있었마는보고도있마 1 4, 1 힌 이와는달려 Marbu rg 등은심부대뇌정벡의혈천파혈류정체를원인으로설영하였으여 16) 그밖에고충증, 세균, 바이러스등의감염증 1 2, 1 7) 외상 3, 1 낀다혈쿠혈증18 ) 등도원인으호판여하는것 로얄혀져있마. 임상석드로는말육미숙, 두부종대, 과도한감응성, 경련둥이냐타냐냐득이소견응아니다 2, 8, 10 ). 두개공타잔상공명음, 뇌파검사상전기활동의완전소실, 광투파법상의파투파첸상응을또한볼수있다자그외방사선학적인검사로는단순총맹상두게골의크기가다양하게나타나며, 뇌실촬영울에서는뇌실의윤곽이냐대뇌딴구가보이지않으연서두개강이조영제로 - 732

Resulting cranial cavity, filled with fluid, shows homogenous low density 채워지고저부에서소뇌천막상부위종괴와함께전후 사진상대뇌겸을따라중심부조직이보이는수가있다 낀 또한산전초음파검사에서두개강이뇌척수액 A 로 채워진소견을볼수있어서산천진단의 시한보고도있었마 19, 20) 혈관조영슐에서는내경동맥이해연동상부셰서 가능성을제 형성 부전과또는펴 l 색이냐타냐고외경동맥응정싱적으로보 이며전두엽피질의일부를공급하는전대놔동액의 영킴이나시상구조물에혈뷰를공급하는후대뇌동액이 보일수있으며 소뇌천악혈관등과추골기저동맥이정상 뒤 적으로냐타날수있마 11, 21, 22) 한펀 CT는수무뇌증의좋은진단방법우로안정받게 되었고객 CT 소견상소뇌에원인이확실치않는저음영부위를냐타낸경우도있었으냐둔개골, 간뇌, 소뇌는정상적으로냐타나고, 대뇌겸은정상혹은일부결손을보이며중심선 /b벼 l 있거냐약간천위되어보인다. 둥근시상쿠조물파기저신경절이대뇌혹은소뇌반우에연결되어나타나기도하며대뇌반구는완천허소실되거나측두엽, 후두엽, 전두엽등의일부가냥을뿐냐머지부분은액체읍영으로충만된공동으로나타난마 1, 3. 7, 19) 이

Remnant occipitallobes are seen partially and bilaterally 와같은소견들은저자들의중례와일치하였는헤 6 예 모해 1 서작은후두엽을볼수있었고, 천두엽과측우엽 의알부을보인경우가한예, 두정엽이보인경우가한 예있었다. 부검상두정엽이남아있었던경우가있었 으냐 6) CT 상발견된경우는운헌상보고된바없막이 는침습된대뇌혈관의분포에짜라마양한정도의우형 성또는딸육부전의결과이며문 - 현에 보고된바와같이 후대뇌통액의분포부위는정도에따라다르지만남아 있기때문이라생각된다. 전하였고, 4 예는두개저근처의중간부위가 결손되어 있었마. 4. 기저신경결응 2 예에서에뇌겸에 연결되아관찰 되었다. 5 후우엽은말육부전의정도차이는있으냐선예에 있어서양측에서판찰되었고, 전두엽과두정염의 일부, 그리고측두엽의일부가각각한예에서보였우며 나머 지대놔반구는묘출되지않았다. 6 나머지두개내강은액체음영으로충만되어있었마. 7. 전예에서제 3, 4 뇌실이요출되거나추적이 가능 하였고측뇌실은그중 3 예에서 맥락막총파함께냐타났 으며공동으로개방되어있었마 예에서두꺼워진양측의수박구조물이 내칭석 우호두개골내띤으로부터분리되어나다나있았마
