Ra, M.D. 5chool of Medicine Kyung-pook National University Departent of Tadiology Taegu, Korea Percutaneous transhepatic cholangiography (P.T.C.) is a ost valuable procedure in differentiation of obstructive jaundice, wheter benigh (chiefly stone) or alignant condition. Authers present the clinical and P.T.C. findings of 30 cases of obstructive jaundice which were experienced fro Deceber 1975 to May 1977, at departent of Radiology, Kyung-pook National University Hospital. The suerized results were as follows. 1. The incidence of alignancy beyond the age of 40 was high copared with stone, which revealed relatively equal age distribution. 2. The stone showed as concave cupping in all cases (11 cases-), but the alignancy showed as nipple foration (10 cases) and irregular narrowings (9 cases). 3. The dilation of the intrahepatic bile duct appeared 10 cases on alignancy aong 19 cases of alignancy (52.6%) and 4 cases on stone which were ultiple intrahepatic or coon hepatic duct stones. 4. The dilation of the justproxial bile duct was severe on alignancy copared with stone. 5. The ean bilirubin level was high on alignancy (12.8 g-%) copared with stone (10.3 g-%). 1. 서론해서영상증폭기의도업과 X 선촬영술의발달로일반 화하게되었다. 경피경간담판조영술 ( 이하 PTC 라청함 ) 은 1937 년 1970 년대초일본에서종래의앙법을개량하여 22,,-,23 Huard 와 Do. Xuan - Hop 에의해서처음시도되었으며 G 의유연한 Chiba 칭을사용한새로운 PTC 를시행함 1942 년 Le e 와 Royer'6 l 는복강경을이용해서당냥에조으로서항병층이감소되고성공율이애우높아졌다고 영제를주엽하였고, 년 Carter 와 Sa y pol " 은경복부보고하였다 1, 3, 5, 9, 11, 2 1, 22, 28, 29, 31, 35, 36} 37, 38J 39 ) 당판조영숭을시행했으며, 년 Kidd'" 는 PTC 를성 년 Housset 와 Vantis 7l 가처음시도한측방천자공적으로시행했으며, 년 Arn e r 와 Gl e nn 1 6 l 등에의법에의한보고서들은 8, 17, 18, 3 2) 오두 1952 연 Cart e r 와 * 본논운의요치는 1978 년 1 0 월 14 일대한방사선의학회제 34 차학울대회에서발표되었옴

2 Saypol ) 에의해서시도된전방천자법에의한보고서들 에 2, 3, 6, 1, 12, 13, 16, 20, 2, 30}33 ) 비해서성공율이높고위험율과함영증이낮은것으로보고하고있다. 최근에는 PTC릎치료에도이용하고있으며, 이에매한많은연구보고서들이나오고있다 13, 23, 27 ) ll. 대상및검사방법 1) 대상 197 5년 12월부터 연 5월까지경북의래부속병원방사선과에서폐쇄성황달 30 례에대해서 PTC를시행하였다. PTC시행 2얼전부터항생제를투여하여시행후 3 일에중단하였으며 PTC시행전에내부분의환자에서상.DI. 성 적 총 3 0 례중남자가 17 례그러고여자가 1 3 례로나타났 으며 종양의 경우는 40세이후가 1 9 례중 16 례로서 % 를차지했고당도결석의경우는 비교척고른연령분 포를나타내었다. 평균연령도결석의경우는 47세였으 나종양의 경우는 58세로서종양이 결석에비해서 비교 적 높은연령에서 나타났다 (Table 1). Age and sex Male Feale 복부위장검사나저장성십이지장촬영술을시행함을원 칙으로하였으며출혈의합 1 청을예방하기위하여 Proth. robin 시간을측정하여건상치보다 낮으연정액으로 바다민 K 를주어서교갱후 PTC 를시행하였다. 2) 방법 환자는술전최소한 8 시간이상공복시키고전처치로 시행전 30 분에 50g 의 De e rol 파 1 0g 의 Valiu- 을 근주하였으며출혈로인한항병증에대처하기위하여 ~ Total % 포도당액이나 Hartan 용액의건주를계속유지하내원당시환자의주소률보연철석의경우는상북부 였다. 사용한천은내경 0.5c. 외경 0.7c 길이 15 통증이 11 례중 6 례로가창많고종양의경우는 19 례중 c 의유연한 22G 의 Chiba 칭을사용하였으며조영제는 7 례에셔황달을주소로하였다 (Table 2). 60% Urograf in 을사용하였다. 술식은환자를촬영대 에늄히고 X 선투시로간의 크기빚형태, 그러고우측 횡경악의위치응을확인하여우측중액와선제 7-8 늑간 을천자부위로설경하였다. 섣정된천자부위는소독후 1 % procaine 으로국소마취후세침윤간의하연에형행 하게쩔러서환자의호홉윤중단한상태에서제 번째흉추우연에 2-3c 거리까지도달하도록 며앨단칭을넣은후는조용히호홈을 하였으 하게하였고웅 직이거나기칭을금지시켰다. 성칭을뽑은후조영제 가든주사기를끼우고투시하연서서서히청윤뽑으면 서조영제를주업하였으며첨단이당관내에들어가연 조영제가담판을채우는것을볼수있었다. 칭의내갱 이적어서대부분의예에서담즙의유출은없었으며 l 의조영제를넣은뒤첨응뽑고환자의체위를 여러부위로바꾸어서당관에조영제가골고루침투되게 한후여러부위로 X 선을촬영하여페쇄부위와그모양 을규명하였다. 담판의폐쇄가확인되연바러움을먹여 서십이지장파담판과의상관관계를비교관찰하였다. 술후 4 시간은공복시켰으며 24 시간동안합병증발생에 대비하였다 Table 2. 3 이학척소견상결석파종양에서모두황달성공막이가장많았으며종양의경우간촉지도 19 례중 12례로나타났다 (Table 3). 검사실소견을보면평균당척소치도결석의경우는낮으나종양의경우는비교걱높은평균땀척소치를블수있었다 (Table 4). Table 4. Nipple Concave Irregular foration cupping narrowing 간내당판의확장여부를비교해올애결석의경우는 11 례중 4 례에서간내당판의확장이있었으며이들은오우다말성의총수맘판혹은간담판의결석의경우였으며 (Fig. 5), 종양의경우는 19 례중 10 례에서간내당관의확장을올수있었다 (Fig. 2, 3, 4), (Table 7)

N. 고안 1970 년이전의 polyethylene 도판을사용한경우의 갱공율은 Hines 와 Evan 등 7, 빈 27 ) 의 보고서에서지척하 듯이당관의확장이있는경우는 93% 였으며맘판확장이없는경우는 % 의성공율밖에얻지못했으냐유연한 Chiba 첨을사용함으로서담판확장이있는경우는거의 100% 의성공율을나타냈으며담관확장 l 없는경우도 60-80% 의성공율을얻을수있었다고보고 하고있으며 8, 2 년 9, 31, 35, 36, 3 낌8., 39 ) 본저자들은 30 례모두에서성공할수있었다. 천자횟수변에서도내부분의저자들은 5, 8, 10, 13, 15, 34, 35, 36 ) 37, 38, 39) 3-6회천자로담관에 Fig.4. Peripheral dilatation(ihd) 4 10 들어가지않으연당관확장이없마고보고하였으며, 본 증례에서는 모두 3 회이내에협게담판을천자할수있 었다. 함병증의발생빈도를보연종래의 방법을사용한저 자을은 6, 19) 최고 25% 까지보고하고 있으나 Chiba칭을 사용한 Okuda 29 ) 의 보고서에서는냐 8% 를그러고국내 의많은저자들은 35, 36, 37, 38, 39) 매개 6-7% 의 합병증율 보고하였으며, 본증례에서는 30 례중 10 례에서 일시적안 고열, 복통, 오심, 구토등의 증세를나타냈으며 Koch 23) 나 Isl ey2 0 ) 등이 보고한심한항병증은없었다. 낮은 항병증딸생율은시행천 2얼전부터 항생제를 투여하였 으므로술전향생제투여로합병증을낮출수있었다는 Ferrucci 15 ) 의보고와도유관성이있음을알수있었다

Mosby Co., Saint Louis, 년 12월부터 1977년 5월까지경북의에 부속벙원 방사선과에서폐쇄성황달 30 례에 배해서 PTC를 시행 하여다음파같은성적을얻었다. 1. 총 30 례중낭자가 1 7 례, 여자가 13 례였으며 종양 의경우 40세이후가 19 례중 16 례로서 % 를 차지했 으며결석의경우는비교석 고은연령분포를볼수있 었다. 2. 혜쇄부위의 오양은결석의 경우 11 례 모두에서 Concave cupping 음 나타냈으며 총양의경우 19 례중 10 례에서는 nipple foration 을 9 례에서는 irregular narrowing응나타냈다. 3. 간내담관의확장여부를 초사한 결과결식에서는 11 례중 4 례. 종양의 경우는 19 례뚱 10 례에서간내맘관 의확장을올수있었다. 4. 폐쇄상부의 당관확장은결식에 비해서종양에서 더욱심하게나타났다. 5. 평균당적소치는 결석에서는 10.3g-% 였으나 종양에서는 12.8g-% 를 나타내서 종양에서 더욱높 게 나타났다.
