Anemia is commonly found in rheumatoid arthritis (RA); however, PA is rarely found in RA. There are few reports describing patients with both conditions; none in Korea to date. We report on a case of a 46-year-old female who presented with hypesthesia and general weakness. She was previously diagnosed as seropositive RA with myelodysplastic syndrome. She had severely impaired sensation, especially for vibration and proprioception in all limbs. Subacute combined degeneration was observed on her magnetic resonance imaging and serum vitamin B12 level was very low. Further exam results were consistent with PA and her symptoms improved with cobalamin injection. This case demonstrates that PA should be considered in RA patients presenting with both central nervous system manifestations and anemia. (J Rheum Dis 2015;22: ) Key Words. Pernicious anemia, Rheumatoid arthritis, Vitamin B12 서론 악성빈혈 (pernicious anemia) 은거대적아구성빈혈 (megaloblastic anemia) 의일종으로위장점막위축소견을보이며벽세포 (parietal cell) 에서생성되는내인자 (intrinsic factor) 에대한자가항체생성으로비타민 B12가체내에제대로흡수되지못하여발생한다. 현기증이나숨참, 식욕부진등의일반적인빈혈증상외에심하면신경병증이동반되어저림증, 기억력감퇴, 근무력증, 인지기능장애등이동반될수있다 [1]. 저자들은빈혈과함께심한저림증과사지위약감을주소로내원한류마티스관절염환자에 서아급성연합변성 (subacute combined degeneration) 을발견하여그원인으로악성빈혈으로인한비타민 B12 결핍을진단하고, 코발라민 (cobalamin) 주사치료이후증상호전사례를경험하여이에보고하는바이다. 증례 46세여자로사지위약감과감각저하를주소로내원하였다. 환자는 3년전손가락, 발가락의다발성관절통과조조강직을주소로본원정형외과를방문하였다. 신체검사에서다발성관절염이있었고혈액검사에서백혈구 Received:September 15, 2014, Revised:(1st) December 1, 2014, (2nd) December 15, 2014, Accepted:December 16, 2014 Corresponding to:kichul Shin, Department of Internal Medicine, Seoul National University College of Medicine, 103 Daehak-ro, Jongno-gu, Seoul 03080, Korea. kideb1@gmail.com pissn: X, eissn: Copyright c 2015 by The Korean College of Rheumatology. 2 Jae Myung Lee et al. 4,940/mm 3, 절대호중구수 3,621/mm 3, 혈색소 9.6 g/dl, 적혈구침강속도 (erythrocyte sedimentation rate) 는 37 mm/h ( 참고치 : 0 20 mm/h) 로상승되어있었으며, 류마티스인자 (rheumatoid factor) 는음성이었으나, 항CCP 항체 (anti-cyclic citrullinated peptide antibody) 는 600 U/mL 이상으로측정되었다. 수부 X-선사진에서우측손세번째, 좌측세번째, 네번째근위지기저부위에심한골미란소견이관찰되었다. 이에혈청양성류마티스관절염으로진단하여 methotrexate 7.5 mg/ 주, prednisolone 5 mg/ 일복용을시작하였고임상상이조금호전되는듯하였으나, 오심과구토등의약물부작용이심하여결국치료 6개월째자의로치료를중단하였다. 환자는식욕이점차감소하며하루반그릇정도밖에식사를하지못하였고, 1년에 7 kg 정도 ( 전체체중의 14%) 의체중저하및사지위약감이심해져치료를중단한지 7개월뒤외래를재방문하였다. 혈액검사에서백혈구 1,810/mm 3, 혈색소 3.4 g/dl, 혈소판 43,000/mm 3, 절대호중구수 859/mm 3, 망상적혈구 2.19%, 평균적혈구용적 fl, 평균적혈구혈색소량 43.6 pg, 평균적혈구혈색소농도 33.7 g/dl로범혈구감소증이관찰되어, 골수생검을시행한결과골수형성이상증후군 (myelodysplastic syndrome, refractory cytopenia with multilineage dysplasia) 으로진단되었다. 형광동소교잡반응 (fluorescence in situ hybridization) 검사에서 1번, 5번, 7번, 8번, 20번염색체의결실등의이상소견은관찰되지않았다. 또한추가혈청검사에서는비타민 B pg/ml ( 참고치 : pg/ml) 으로정상이었으나, 엽산<1.0 ng/ml ( 참고치 : 3 17 ng/ml) 으로감소되어있어적혈구수혈뒤엽산염을하루 5 mg 경구투여하며외래추적관찰과지지적치료를유지하였다. 류마티스관절염에대해서는 prednisolone 10 mg/ 일, hydroxychloroquine 300 mg/ 일, leflunomide 20 mg/ 일그리고비스테로이드성항염제를추가하였다. 이후 5개월뒤외래경과관찰에서식욕부진과왼쪽팔, 다리의저림증이동반된사지위약감은크게호전을보이지않아, 류마티스종말증 (rheumatoid cachexia) 의가능성및그외질환을감별하기위해입원을권유하였다. 그러나환자는요양원에서엽산염만복용하고지내며이후재방문하지않았다. 그로부터 20개월뒤양쪽다리에타이즈를두세겹끼어신은것같은감각저하, 보행장애및심한전신위약감주소로외래를방문하였다. 환자는특별한수술력이없었으며, 가족력으로는어머니가퇴행성관절염으로진단받았다. 내원당시활력징후는혈압 126/81 mmhg, 맥박 70회 / 분, 호흡수 20회 / 분, 체온 36.9 o C였다. 경부림프절종대는없었으며흉부와복부검진에서이상소견은없었다. 관절평가에서는양쪽상완관절, 양쪽손목관절, 우측첫째중수지관절에압통이있었고, 우측첫째근위지관절의종창과압통, 우측셋째와넷째, 다섯째근위지관절압통, 좌측 첫째중수지관절압통, 좌측셋째근위지관절의종창과압통, 좌측무릎관절종창, 우측첫째부터다섯째까지중족지관절압통, 좌측첫째와넷째중족지관절압통소견을보였다. 신경학적검진에서근력은상지근위부 IV/IV, 상지원위부 IV/IV, 하지근위부 IV /IV, 하지원위부 V/V로비교적양호하였다. 그러나사지감각이저하되어있었고, 특히진동감각과위치감각이통각이나온도감각보다현저히감소되었다. 심부건반사는모두정상이었고, 오른쪽발목간대반사 (ankle clonus) 는양성소견을보였다. 혈액검사는백혈구 6,350/mm 3, 혈색소 12.8 g/dl, 혈소판 272,000/mm 3, 평균적혈구용적 fl, 평균적혈구혈색소량 34.8 pg, 평균적혈구혈색소농도 34.0 g/dl, 적혈구침강속도 37 mm/h, C-반응단백 0.42 mg/dl였다. 류마티스인자양성 (14.3 IU/mL, 참고치 : IU/mL), 항핵항체양성 (1:40, speckled pattern) 이었고, 항Smith항체, 항SS-A/Ro 및항SS-B/La항체는음성이었으며, 항ds-DNA 항체 3.5 IU/mL, C3 90 mg/dl, C mg/dl였다. 말초혈액도말검사에서는대적혈구정상색소성적혈구와과다분엽핵중성구 (macrocytic, normochromic red blood cell and hypersegmented neutrophil) 가관찰되었다. 신경전도검사 (nerve conduction test) 는정상이었으나, 감각유발전위검사 (sensory evoked potential test) 에서중추전도장애를시사하는전기생리학적이상소견이관찰되었다 (Figure Figure 1. 병력과 신체검사 및 혈액검사 소견 등을 고려 찰 하여 비타민 B12 결핍으로 인한 아급성연합변성으로 판 악성빈혈은 혈액, 위 및 면역학적 변화로 구성된 복합성 단하였으며, 추가적으로 시행한 검사에서 혈중 엽산 수치 질환으로, 1849년 Thomas Addison에 의해 특발빈혈(idio- 는 19.

3 Pernicious Anemia in Rheumatoid Arthritis 1). 이에 시행한 전척추 자기공명영상(whole spine mag- 는 289 pg/ml로 정상 범위 내였다. netic resonance imaging)에서 척수의 뒤가쪽기둥(poste- 고 rolateral column) 부위에 국한된 선상 병변이 관찰되었다 (Figure 2). 병력과 신체검사 및 혈액검사 소견 등을 고려 찰 하여 비타민 B12 결핍으로 인한 아급성연합변성으로 판 악성빈혈은 혈액, 위 및 면역학적 변화로 구성된 복합성 단하였으며, 추가적으로 시행한 검사에서 혈중 엽산 수치 질환으로, 1849년 Thomas Addison에 의해 특발빈혈(idio- 는 ng/ml이었고, 비타민 B12가 80 pg/ml로 감소 pathic anemia)로 최초 기술되었고, 1872년 Anton Biermer 되었다. 이은 검사에서 항내인자항체(anti-intrinsic factor 는 이를 악성빈혈(pernicious anemia)로 명명하였다[1]. antibody) 양성, 항벽세포항체(anti-parietal cell antibody) 악성빈혈으로 진단하려면 남성의 경우 혈색소가 13 g/dl 양성, 가스트린은 pg/ml (참고치: pg/ml) 미만, 여성의 경우 12 g/dl 미만, 평균적혈구 용적 120 fl 로 증가 소견을 보였고, 위내시경검사에서는 만성 위축성 이상, 혈중 비타민 B12 농도가 낮고, 위체부 점막의 위축 위염소견이 관찰되었다(Figure 3). 위의 소견들을 근거로 성 소견을 보이며 내인자 혹은 벽세포에 대한 항체가 있어 악성빈혈이 동반되어 있다고 진단하였다. 야 한다[1]. 역학적으로는 북유럽 가계와 미국 내 흑인에 7일 동안 매일 코발라민 1 mg을 근주하였으며 이후 1개 게서 10만 명당 150명의 유병률을 보이지만, 남유럽과 아 월 동안 주 1회 근주, 이후 매월 1회 근주를 계획하였다. 시아에서는 드물고, 우리나라에서는 10만 명당 0.3명의 또한 자의로 중단하였던 류마티스관절염 치료를 위해 hy- 연 발병률이 보고되었다[2]. 비타민 B12 결핍이 동반되면 droxychloroquine 300 mg/일, prednisolone 2.5 mg/일, 골수나 위장관계에 영향을 미쳐 조혈과정 장애와 설사 흡 leflunomide 10 mg/일, 비스테로이드성 항염제를 재시작 수장애, 체중감소, 식욕부진 등이 나타날 수 있는데, 특징 하였다. 코발라민 근주와 경구 약물을 투여한 이후 2주째 적으로 설염 및 빌리루빈의 상승으로 황달 등의 증상이 종 시행한 관절평가에서 상당부분 호전을 보이기 시작하였 종 보고되며, 초기 혈액검사상으로는 과분엽 중성구 및 다 다. 신경학적 검사에서도 사지에서 통증, 온도, 진동 및 위 형핵 거대세포 소견과 빈혈, 백혈구 감소증, 혈소판 감소 치감각 모두 조금씩 호전을 보였으나, 양하지 원위부의 저 증의 소견을 보일 수 있다[1]. 특히 손발저림, 감각저하 및 린감은 지속되었다. 발목간대반사도 양측 모두 음성이었 보행실조 등의 신경계 증상이 나타날 수 있으며, 운동장애 다. 혈액검사에서 백혈구 10,170/mm3, 혈색소 12.6 g/dl, 보다 우선하여 감각장애가 나타나는 것이 흔하고, 대개 대 3 혈소판 407,000/mm, 평균적혈구 용적 fl, 평균적 칭적으로 관찰된다. 특히 위치감각과 진동감각 전달을 주 혈구 혈색소량 34.8 pg, 평균적혈구 혈색소농도 33.7 g/dl 로 담당하는 척수의 뒤기둥(posterior column) 아급성연 였으며, 혈중 비타민 B12는 11,930 pg/ml로 호전되었다. 합변성(subacute combined degeneration)으로 근력저하 외래 추적 결과 사지의 감각저하와 위약감은 점차 호전되 에 비해 실조증(ataxia)이 먼저 나타나 보행이 불안정해지 었으며, 치료 6개월 뒤 시행한 혈액 검사에서 비타민 B12 는 것이 특징이다[3]. 본 증례의 경우, 초기에 골수검사 결과 골수이형성증후 Figure 2. 4 Jae Myung Lee et al. 군로진단되었고이에대한치료후범혈구감소증은일부호전되었으나, 3년뒤만성빈혈이외에도과거에는정상이었던비타민 B12의저하소견이발견되면서악성빈혈이진단되었다. 악성빈혈과골수이형성증후군이동시에존재하는경우는매우드물며 [4], 상기환자에서처음내원당시정상비타민 B12 결과를보인것은악성빈혈의전단계였거나검사기법의민감도및특이도에기인한것으로생각된다 [5]. 두질환이서로오진될수있는가능성에관해언급된연구들에서는비타민 B12의검출방법에서과거방사선동위원소희석분석 (radioisotope dilution assay) 방법에비교하여최근의경쟁적결합발광분석 (competitive binding luminescence assay) 방법이위음성률이높아악성빈혈진단을놓칠수있는경우를보고하였다 [6]. 그러나상기환자의경우 2011년 6월초기검사에서부터현재까지동일한장비 (the Roche Elecsys Systems Modular Analytics E170; Roche Diagnostics, Mannheim, Germany) 로시행한전기화학발광면역측정법 (electrochemiluminescence immunoassay) 의결과였다. 결국드물지만골수이형성증후군의검사소견과악성빈혈이공존한증례로생각된다. 골수이형성증후군과류마티스관절염의병발은이미보고된바가있으며국내에서도발표되었다 [7]. 류마티스관절염과악성빈혈이동반된외국의증례보고에서는자가면역질환으로서의공통기전을언급하였으나, 두질환의연관성에대해서는아직명확하게밝혀진바가없고, 류마티스인자와항내인자의관련성도현재로서는불분명하다 (Table 1). Partridge와 Duthie [8] 는 2,544명의류마티스관절염환자를대상으로 27명의악성빈혈환자를보고하였으나, 대적혈구성빈혈의빈도와발병률을 관찰하기위한것으로류마티스인자와항내인자항체유무의연관성에대해살펴보지않았다. Bieder와 Wigley [9] 은 20명의류마티스관절염환자들중 2명에서악성빈혈을진단하였다. 두환자각각 22년, 8년의류마티스관절염병력이있었으며, 이중하지의감각이상과정신착란이관찰된 1명은악성빈혈로인한아급성연합변성으로진단되었고, 비타민 B12 투여후모든증상이호전되었다. Carter 등 [10] 은 92명의류마티스관절염환자를대상으로거대적아구성빈혈, 비타민 B12 결핍그리고엽산결핍의발생빈도를관찰하였으나, 이중 1명에서만악성빈혈로밝혀졌고, 87명의류마티스관절염환자중항벽세포항체가있는환자는 7명 (8%) 이었다. Pitcher 등 [11] 은 100명의류마티스관절염환자를대상으로악성빈혈로진단된 3건을보고하였으며, Ghazi [12] 의연구에서는 99명의악성빈혈환자중 5명이류마티스관절염이동반되었다. 흥미롭게도악성빈혈환자에서류마티스인자양성결과는 99명중 11 명 (11.1%) 으로대조군 (3.8%) 에비해높게나타났다. 추가연구에서 50세이상의악성빈혈환자를별도로분류하여류마티스인자양성률을확인하였는데, 그결과 94명중 11명 (11.7%) 으로대조군 115명중 5명 (4.3%) 에비해통계적으로유의하게높았다. 그외 1993년류마티스관절염과악성빈혈이있는환자에게서백반증이동반된사례가보고된바있으며, 아시아에서는대만에서류마티스관절염과악성빈혈이동반된 62세환자가보고하였다 [13,14]. 현재까지류마티스관절염과악성빈혈의관계를규명하는데있어서류마티스인자와자가항체간의상관관계는불분명하나, 자가면역질환이라는범주안에서두질환의연관성을완전히배제하기는어렵다. 이에실조증, 대적혈 Table 1.
