In patients with symptomatic cardiac involvement, prognosis is even worse with the reported median survival of 6 months. One patient, diagnosed as L amyloidosis with cardiac involvement, was treated with thalidomide and showed remarkable improvement in functional capacity, cardiac function, and various laboratory parameters without any significant adverse effect and, therefore, we report the case with the review of literatures.(korean J Med 69:S910-S914, 2005) Key Words : myloidosis, Thalidomide 서론 소견및심초음파소견이현저히호전된예를경험하였 기에문헌고찰과함께이를보고하는바이다. 원발성아밀로이드증 (primary amyloidosis) 은예후가매우불량하여평균생존율이 1~2년정도이며, 특히심장침범을동반할경우 6개월정도로감소된다고알려져있다 1). 최근까지 melphalan, prednisone, colchicine 이가장많이사용되는치료약제이나, 치료성적은좋지못하였다 2). 저자들은심장침범을동반하여실신이주된증상이었던원발성아밀로이드증환자에서 thalidomide 치료후특별한부작용없이임상적증상, 검사실 증례 52세남자환자가실신을주소로내원하였다. 8개월전부터전신쇠약감, 노작성호흡곤란, 하지부종이발생하였고, 최근 2개월전부터는계단을급히오르거나, 힘든일을한뒤수차례실신을하였다. 과거력과가족력에서특이사항은없었다. 내원당시측정한혈압은 130/70 mmhg, 맥박 98회 / 분, 접수 : 2004년 9월 30일 통과 : 2004년 12월 21일 교신저자 : 손대원, 서울시종로구연건동 28, 서울대학교병원내과 ( ) dwsohn@snu.ac.kr -S 910 -

The systolic functions of the left ventricles improved with the calculated ejection fraction from 35% (, at the diagnosis) to 57% (, 17 months after the treatment with thalidomide). 호흡수 18회 / 분, 체온 36.4 였고, 신체검사에서간과비장이우측늑골단아래로각각 2횡지, 1횡지만져지는간비대와비장비대가있었다. 혈액검사에서백혈구 3,620/ mm 3, 혈색소 10.6 g/dl, 혈소판 129,000/mm 3 이었다. 생화학검사에서 UN 14 mg/dl, creatinine 1.3 mg/dl, LDH 247 IU/L, cholesterol 143 mg/dl, total protein 5.8 g/dl, albumin 3.2 g/dl, total bilirubin 1.7 mg/dl, alkaline phosphatase 326 IU/L, ST 59 IU/L, LT 70 IU/L이었다. 소변의면역고정전기영동검사에서 ence Jones 단백뇨가 468mg/day 이었으며, 혈청의면역전기 -S 911 -

examination (, Congo red stain, 100) 영동검사에서단세포군감마글로불린병증 (monoclonal gammopathy, kappa type) 가관찰되었다.

심전도검사에서는사지유도에서 low voltage가관찰되었다 ( 그림 1). 심초음파검사에서는심근벽의비후와수축기기능장애 (EF: 35 %) 소견이관찰되었으며 ( 그림 2), 하대정맥의확장이관찰되었다 ( 그림 3).

Myocardial degeneration and atrophy (, H&E, 100) and typical apple-green birefringence under polarizing microscopic examination (, Congo red stain, 100) 영동검사에서단세포군감마글로불린병증 (monoclonal gammopathy, kappa type) 가관찰되었다. 골수조직검사에서정상골수소견과정상유핵세포의분포가관찰되었으며, 형질세포가증가되어있는형질세포질환 (plasma cell dyscrasia) 을시사하는소견이관찰되었다. 심전도검사에서는사지유도에서 low voltage가관찰되었다 ( 그림 1). 심초음파검사에서는심근벽의비후와수축기기능장애 (EF: 35 %) 소견이관찰되었으며 ( 그림 2), 하대정맥의확장이관찰되었다 ( 그림 3). 심내막조직검사에서심근의변성이관찰되었고, 편광현미경검사에서전형적인 apple-green 색의편광을보이는아밀로이 드가확인되었다 ( 그림 4). 흉부 X-ray에서심비대가관찰되었다. 상기소견으로심장을침범한원발성아밀로이드증으로진단되었다. 우선폐울혈을줄이기위해서소량의이뇨제를시작하였으나, 혈청 creatinine 상승소견이관찰되어중단하였다. 이후 thalidomide 300 mg/day 와 dexamethasone 40 mg/day 치료를시작하였다. 치료시작 3개월뒤환자의증상은호전되었고, 간비대, 비장비대, 빈혈이사라졌으며, alkaline phosphatase 가정상화되었다. 치료시작 17개월뒤에시행한심전도검사에서 low voltage 소견이이전에비해감소하였 -S 912 -

4 - 오일영외 6 인 : Thalidomide 치료후호전된심장침범을동반한원발성아밀로이드증 1 예 - 으며, 심초음파검사에서심근벽의비후는약간감소되었고, 수축기기능은완전히정상으로회복되었으며, 하대정맥의확장도관찰되지않았다. 환자는초기 9개월간 dexamethasone 을병행치료받았고, 하지의신경통으로 thalidomide를중단할때까지총 20개월간의치료를받았다. 치료시작후 33개월뒤인현재환자는아무런증상없이등산을할정도의정상적인생활을하고있다. 고찰원발성아밀로이드증은형질세포의자율증식과단일클론면역글로불린의과잉생산을특징으로하는질환이다 3). 아래일곱가지증후군중한가지이상이관찰될수있다 4). (1) 심근병증 (2) 신증후군범위의단백뇨 (3) alkaline phosphatase 상승을동반한간비대 (4) axonal peripheral neuropathy (5) 기립성저혈압, 방광, 장기능부전으로나타나는자율성신경병증 (6) 가성폐쇄, 변비, 설사등으로나타나는소장침범 (7) 관절병증, 혀나턱밑샘의비대를유발하는연조직침범. 최근까지 melphalan, prednisone, colchicine 이치료에가장많이사용되어왔으나치료성적은평균생존율이 17~18개월로저조하였다. 아밀로이드원섬유에친화력을갖는 4'-iodo-4'-deoxydoxorubicin 같은새로운약제나골수이식이대체치료로시도되고있으나역시치료성적은좋지못하다 5, 6). Thalidomide 는한때기형유발부작용때문에시장에서사라졌었으나, 나병결절홍반에대해효과가있는것이알려지면서다시사용되게되었다 7). 또한면역조절기능이있다는것이밝혀지면서여러분야에서재조명을받고있으며, 특히다발성골수종을포함한여러암질환의치료에서주목받고있다. 최근들어서는원발성아밀로이드증의치료에서도사용되고있다 8, 9). 지금까지보고된원발성아밀로이드증의치료에서 thalidomide는모두 2차치료에서사용하는경우였으며 8, 9), 예후에중요한영향을갖는심장침범에대한치료효과에대해서는보고된바가없었다. 본환자의경우에는혈청 creatinine이상승되어있고, 심장기능이저하되어있어서 melphalan, colchicine을사용하기어려웠고, thalidomide 가처음치료로이용되었다. Thalidomide는 16개월간 300 mg/day, 4개월간 200 mg/day을사용하였고, 처음 9개월간은 dexamethasone 40 mg/day을함께사용하였다. 본증례에서는 thalidomide 사용후임상적증 상과심장기능이현저히호전되었고, 혈색소, LDH, alkaline phosphatase의검사실소견이정상화되었고, ence Jones 단백뇨가소실되었다. 또한치료시작후 20개월뒤부터하지의신경증이생긴것이외에는다른특별한부작용이관찰되지않았다. 비록치료후의심내막조직검사를통해서호전여부를재확인하지는못했지만심초음파검사와검사실소견에서뚜렷한호전소견이관찰되었다는점은 thalidomide가 melphalan, colchicine 이사용하기어려운경우좋은대안이될수있음을시사한다고하겠다. 이부분에대해서는더많은원발성아밀로이드증환자에서임상연구가필요하겠다. 요약 저자들은심장침범을동반한원발성아밀로이드증이생긴 52세남자환자에서 thalidomide 치료후임상적증상, 심초음파소견, 검사실소견이큰부작용없이현저히좋아진 1예를경험하였기에문헌고찰과함께보고하는바이다. 색인단어 : 원발성아밀로이드증, Thalidomide REFERENCES 1) Falk RH, Comenzo RL, Skinner M.
