He developed VRE bacteriuria at the hospital day 42. and genes were detected from 6 µg/ml vancomycin-containing BBL Enterococcosel broth (BD), of which color changed to black after overnight incubation, by both Seeplex VRE detection (Seegene, Seoul, Korea) and Anyplex VanR real-time PCR (Seegene). Subculture of an aliquot of the blackened broth on blood agar plate produced only VRE. All of the four subsequent consecutive surveillance cultures for 1 month until discharge at hospital day 75 resulted in PCR-positive but culture-negative for VRE from the enrichment broths. Therefore, the presence of a non-enterococcal intrinsic reservoir bearing is more likely than low burden of VRE. Considering the rare occurrence of VRE in Korea, -positive PCR results from the enrichment broth requires confirmation by microbiological studies. MicroScan PBC28 (Baxter Diagnostics, West Sacramento, CA, USA) 을이용하여항균제감수성검사를실시한결과 vancomycin 및 teicoplanin의최소억제농도 (minimum inhibitory concentration, MIC) 가 >16 µg/ml로 VanA 형이었다. 대변검체를 6 µg/ml vancomycin을포함한 Enterococcosel broth (Becton Dickinson, Sparks, MD, USA) 에서증균배양한후검게변한배지를 Seeplex VRE detection (Seegene, Seoul, Korea) 으로중합효소연쇄반응 (PCR) 검사를실시한결과 와 유전자가동시에검출되었다. 는동일 PCR을재검하였을때에도양성이었다 (Fig. 1). 하지만혈액한천배지에계대배양후시행한 PCR 검사에서는 VRE만양성이었고 VRE는관찰되지않았다. 이후 10월 13일까지 4회의대변감시배 eissn

Seeplex VRE detection kit의증폭산물로염기서열분석을시행하였고, BLAST (Genbank) 검색에서증폭산물은 와 유전자염기서열과 100% 일치하였는데, 유전자염기서열은 VRE에특이적이었지만, 유전자염기서열은내인성으로 를가진 Atopobium minutum, Clostridium clostridioforme, Clostridium sp., Eggerthella lenta 등의유전자와도 100% 일치하여구분할수없었다. 와 유전자양성인증균배지를혈액한천배지에계대배양후얻은장구균집락중다수를선택하여 PCR하였으나모두 만양성이었고 는음성이었다. Phenylethyl alcohol 한천배지로혐기성배양을시도한검체에서는집락이뭉친데를긁어서 PCR을했을때 와 모두양성이었지만, 만양성인집락을순수분리하는데는실패하였다. 검사의학적진단 유전자는양성이지만 VRE를배양검사에서검출할수없었다. 상호토론 원다해 ( 서울아산병원진단검사의학과 ): 한국에서 VanA 형 Enterococcus durans 1주가최초의 VRE로보고된이래 [1] VRE 빈도가급속히증가하였다. 1996년 4월부터 1997년 3월까지국내한대학병원의임상검체에서분리된장구균 790주를대상으로 VRE 검색을시행하여 36주 (4.6%) 의 VRE가분리되었다 [2]. Korean Nationwide Surveillance of Antimicrobial Resistance (KONSAR) 의전국진단검사의학과항균제감수성자료를모아서분석한자료에의하면 E. faecium 의 vancomycin 내성률이 1997년 2.9% 에서 2001년은 16% 로, 2003년도에는 20% 로증가하여 2000년도초반급증하였다 265

3 [3, 4]. 국내에서 VRE가증가하는데있어서 VRE는어떻게기여했는가? 홍기호 ( 서울의료원진단검사의학과 ): VRE가처음보고된것은앞서언급한국내대학병원으로 36주의 VRE에대해,, vanc 유전자 PCR을시행한결과 VRE가 13주, vanc 이 10주있었다 [2]. 그병원에서 VRE가집중적으로분리된것은 유전자의수평적전파에의한유행으로추정되었다 [5]. 같은시기에국내다른병원의연구중에는 VRE 장보균율이 4.1% 였다는보고가있는데 가 3주, vanc가 9주로 VRE는없었다 [6]. 1998년에 5개대학병원에서분리된 42주 (21주의 E. faecium, 6주의 Enterococcus faecalis, 2주의 Enterococcus avium, 13주의 Enterococcus casseliflavus) 의 VRE를대상으로분자역학조사를시행한연구에서는 12주, 9주, vanc 16주였다 [7]. 9주중 7주는모두처음 VRE를보고하였던대학병원에서분리된균주였다. 1999년에서 2000년도에 6개대학병원에서모은 VRE 중유전형분석이된 70주는모두 로 는없었다 [8]. 또한 2004년 8개대학병원에서 2개월간모은 245개의 E. faecium 을대상으로한연구에서는 vancomycin-resistant E. faecium (VREF) 이 59균주로 23.2% 였으며모두 를가지는균주였다 [3]. 2005년 12개병원에서혈액투석을받는외래환자를대상으로대변검체에서시행된연구에서는위장관에서 4.5% 의 VRE가분리되었으며, 1차또는 2차병원에서의보균율은 2% 이하였으나 3차병원에서는평균 13.3% 의보균율을보여대형병원의 VRE 보균율이훨씬높았다. 대부분이 형이었고 (15/18) 나머지는 VanC 형이었다 [9]. 결론적으로 VRE는 1997년에서 1998년사이국내한대학병원에서유행을일으킨이후국내에서는보고되지않고있다. 원 : 이환자의대변검체에서 VRE는양성이라고할수있는가? 확인하기위하여시행할수있는검사실적방법은? 홍 : 지난 10년간국내에서 VRE가보고되지않고있어증균 PCR에서 유전자양성인것만으로 VRE 양성이라고보기는어렵다. 특히이증례에서임상검체인소변배양에서는 VRE만분리되며, 감시배양검체인대변검체에서만 가지속적으로출현한것은위양성의가능성을시사한다. 감시배양검체에대하여증균 PCR은감시배양에서가장민감한검사법이다. 감시배양검체에서 VRE의선별을위한 PCR 성능평가에대한이전연구들을살펴보면 유전자의검출에대해서는민감도 %, 특이도 % 였으며양성예측도는 %, 음성예측도는 % 로비교적좋은결과를보였다. 반면 유전자의검출에대해서는민감도 %, 특이도 %, 양성예측도는 %, 음성예측도는 % 로특이도와양성예측도가낮았다 [10, 11]. 따라서대변이나직장도말로부터직접 PCR을 해서 가양성이라는것만으로 VRE가있다고할수는없다고생각한다. 서울의료원에서대변검체로, 에대해직접 PCR을한결과 3건의 PCR양성사례가있었으나 VRE 는아니었을것이다 ( 미발표자료 ). 원 : 싱가폴에서 2005년 2/B3 VRE 유행당시시행된연구에서도총 695개의대변검체를증균배양후 PCR을시행하여 53개검체에서 2/B3 양성결과를얻었으나계대배양결과이중 19 검체만 VRE 양성이어서양성예측도가 36% 였다 [12]. 이렇게 위양성률이높은이유는장구균뿐만아니라혐기성세균인 Clostridium spp., Eggerthella lenta, Ruminococcus spp. 등이 유전자를포함한 Tn5382 또는 Tn1549 transposon을가지고있기때문이다 [13-15]. 이증례의 PCR 산물을 Genbank BLAST 검색을한데서도이들균종의유전자와 100% 일치도를보여역시감별할수없었다. 이상을종합해볼때국내에서직접검체뿐아니라증균한액체배지에 PCR을해서 검출시위양성위험이높을것으로생각된다. 성흥섭교수 ( 서울아산병원진단검사의학과 ): 혈액에서검출된 Streptococcus mitis [16] 와 Streptococcus bovis [17] 에서도 가분리된바있다. PCR 위양성일가능성을생각하여액체증균배지로시행한 Anyplex VanR Real-Time PCR (Seegene, Seoul) 에서도 양성이었으며증폭산물로시행한염기서열분석에서도 염기서열과일치하여 를확진할수있었지만계대배양하여얻은 VRE 집락에서는 가검출되지않았다. 이때생각할수있는것은생존할수없는 VRE, 생존가능배양불능 (viable but nonculturable, VBNC) VRE, VRE가소수로존재할가능성, 장구균이아닌다른세균에의한 양성이다. VBNC VRE는척박한환경에서 VRE의생존전략으로배지에서집락을형성할수있는능력을잃었지만아직살아있으며대사활성과유전자발현도할수있고 [18], 다시분열하는증식형으로회복해서반코마이신내성을나타낼수있다 [19]. 반코마이신의존성장구균이통상적인배양조건에서자라지않는것은이와유사한상황이지만, 우리검사실은반코마이신의존성장구균을여러번분리한적이있어서 [20] 반코마이신의존성장구균의가능성은배제할수있다. 증균배지 PCR을해보면 PCR 산물은 PCR산물보다는훨씬밴드강도가약했고, real time PCR에서도 Ct 값이 2.7차이가나서 10배정도정량적차이가있는것을고려할때 VRE가소수로존재할가능성을생각해볼수있다. 하지만, 감시배양을반복적으로실시해도동일하게증균배지에서는 PCR 양성이고, 배양에실패하였고, 소변에서만 VRE만지속적으로출현하여 VRE 외의다른균종에의한 PCR 위양성의위험이높다. 혐기성배양에서집락들을모아서 PCR 한데서는 PCR 양성이었기때문에 를가진균종을순수분리하지는못했지만, 배양이잘 266

4 되지않는혐기성세균에의한 양성가능성이가장높다고판단할수있다. 이들혐기성세균이가진 의임상적의의는불분명하지만 transposon이장구균으로이동할수있기때문에 [21, 22] 를가진혐기성세균의감염관리적의의는더연구가필요하다. 원 : 이처럼 PCR 검사시 위양성가능성이문제가된다면 VRE 보균자검사를위해직접검체에적용하는 PCR 검사가국내에도입될때 에대한특이도를검증할필요가있다고생각된다. 최근국내에서도 LG AdvanSure, Seegene에서실시간 PCR 검사를출시하였고비급여검사로등재되었다. 앞으로임상검사실에이검사들이도입된다면어떤주의가필요할것인가? 김미나교수 ( 서울아산병원진단검사의학과 ): PCR을이용한반코마이신내성유전자의검출을임상검체에서직접뽑은 DNA에적용할경우 24시간이내로 VRE 검출시간을단축시킬수있는장점이있다. 그러나이때생각해야할점은이러한방식은 VRE의검출이아닌반코마이신내성유전자의검출이라는것이다. 50개의대변검체에서직접뽑은 DNA로 GeneOhm VanR Assay (BD Diagnostics, Erembodegem, Belgium) 와 Xpert / (Cepheid, Bouwel, Belgium) 를이용하여 VRE 검출을시도한연구의결과를보면 에대해서는매우특이적 (93.3% vs. 83.3%) 이었으나민감도가떨어졌고 (40% vs. 70%), 에대해서는민감도는다양했으나 (100% vs. 66.7%) 일관되게매우낮은특이도를보였다 (14.9% vs. 12.8%). PCR 음성결과의경우 VRE 보균가능성을배제하는데유용하며 양성이라면격리기준으로사용할수있을것이다. 하지만국내의 VRE는극히빈도가낮기때문에 양성은계대배양결과를확인하여위양성을감별할필요가있다. 국내실시간 PCR 시약들은직접검체에적용하는목적이아닌증균 PCR에적용하도록안정성과유효성이검증되었다. 따라서직접검체에적용할때보다는 위양성위험이적지만이증례에서보듯이증균 PCR에적용할때도 위양성의위험이있다. 국내에서증균 PCR에서 양성시반드시계대배양하여 VRE인지확인해야할것이다. 증례중요점 VRE 감시배양에서증균 PCR 또는직접 PCR 검사를이용하면 양성결과는특이성이떨어지므로배양결과를확인해야함. 요약 저자들은파킨슨병환자의대변으로반코마이신내성장구균 (vancomycin-resistant enterococci, VRE) 감시배양중대변증균 PCR에서는 VRE가검출되었으나배양음성이었던증례를보고한다. 환자는요양병원에입원중이던환자로흡인성폐렴때문에본원으로전원되었다. 재원 42일째 VRE 세균뇨가발생하였다. 대변검체를 6 µg/ml vancomycin 을포함한 Enterococcosel broth (Becton Dickinson, Sparks, MD, USA) 에서증균배양한후검게변한배지에서 Seeplex VRE detection (Seegene, Seoul, Korea) 와 Anyplex VanR Real-Time PCR (Seegene) 으로중합효소연쇄반응 (PCR) 검사를실시한결과 와 유전자가동시에검출되었다. 증균배지를혈액한천배지에계대배양후얻은장구균집락중다수를선택하여 PCR하였으나모두 만양성이었고 는음성이었다. 이후퇴원할때까지한달간시행된 4번의보균검사에서증균 PCR은 VRE 양성, 배양음성이었고재원 75일에타원으로전원되었다. 최종적으로 VRE가소수로존재할가능성보다는배양이되지않는타균종이 유전자를가지고있어서발생한위양성으로생각되었다. 국내에서 VRE의빈도가극히낮기때문에 양성은계대배양결과를확인하여위양성을감별할필요가있다.
