Methods: We developed a protocol on positive/negative control materials for five special stains (iron, myeloperoxidase [MPO], periodic acid-schiff [PAS], Sudan black B [SBB], and alpha-naphthyl acetate esterase [ANAE]) using a hematological malignant cell line. First, we compared stainability of seven cell lines (HL-60, THP-, K56, Kasumi-, KG-, KO5, and NKM-), then confirmed duration of stable stainability. A proficiency test using external quality control materials was conducted at eleven institutions, which participated voluntarily. Results: HL-60 and THP- cell lines, which showed good stainability among the seven cancer cell lines, were selected as external quality control materials. The stainability of a prepared cell line fixed on control slides was stable for 3 4 weeks (MPO, SBB, and PAS) or 9 0 weeks (ANAE). The stainability of paraffin-embedded control material for iron stain was stable for 3 months. The results from institutions were the same on iron, MPO, SBB, and ANAE. Nevertheless, two of 0 institutes showed discrepant results on PAS. Conclusions: In this study, we demonstrated that cell lines could serve as a standard quality control material for special stains. Most institutions showed representative results on special stains except for PAS. This protocol for special stain may be useful as an external or internal quality control in a haematology laboratory. 2 검사자의숙련도를동일수준으로유지하기어렵다. 또한보편화된외부양성 / 음성대조절차가없으며, 검체에따라내부대조를찾기어려운경우검사결과에신뢰성이떨어지고판독자의주관이개입될우려가있다 [6]. 이는분석전및분석시오류를유발하게되므로정도관리의개선이필요하다 [4]. 본연구의목적은특수염색 5종 (iron, PAS, MPO, SBB, ANAE) 의양성 / 음성대조슬라이드를제작한뒤시범적외부정도관리를시행하여검사신뢰도를높이는데기여하는것이다. 재료및방법. 특수염색항목의정도관리용슬라이드제작슬라이드제작법에따른염색상을비교하기위해백혈병기원암세포주인 HL-60세포의농도를 /μl 로맞추고슬라이드에피펫으로 30 μl 분주하여 PAS 염색을시행하였고. μl 당 400개의세포부유액 00 μl 로 cytospin (,000 rpm, 6분 ) 을이용하여염색용슬라이드를제작하였다. 7종의암세포주 (THP-, K56, Kasumi-, HL-60, KO5, NKM-, KG-) 를대상으로 MPO, PAS, SBB, ANAE, sodium fluoride (NaF) 염색을시행한뒤염색상이우수하게나타나는세포주를선정하기로하였다. 4종의암세포주 (THP-, K56, Kasumi-, HL-60) 중에서 HL-60 및 THP-의암세포주들을현탁배양 (suspension culture) 한후슬라이드를제작하였다. Iron 염색용검체슬라이드는동물 ( 쥐 ) 의간조직을이용하여파라핀블록으로절편을만들어제작하였다. 검사실간비교에서는염색방법에서고정단계를비교해야하므로고정이되지않은슬라이드로비교평가하였다.. 염색성유지기간검증 MPO, SBB, PAS는 formalin과 ethanol을 :9 비율로혼합한용액으로 0분간고정, ANAE는 formalinacetone mixture (NaHPO4+KH PO 4 +acetone+ 증류수 +formalin) 로 분간고정을시행한슬라이드를가지고 6주간매주염색을시행한뒤결과를비교함으로써염색성이유지되는기간을검증하였다. 3 외부정도관리시범사업앞에서확립한프로토콜에따라제작한슬라이드를이용하여시범적외부정도관리사업을계획하였다. 참여기관을모집하기위해대한진단혈액학회산하표준화위원회소속위원과학술위원이소속된 5개기관에참여의사를묻는안내문을발송하였고, 이외에 개기관에서추가로참여를희망하였다 (Fig. ). 연구를주관하는국립암센터와참여를결정한 0 개기관 ( 삼성서울병원, 서울아산병원, 연세대학교세브란스병원, 양산부산대학교병원, 동국대학교일산병원, 인하대학교병원, 이화여자대학교의료원, 건강보험공단일산병원, 인제대학교부산백병원, 원자력병원 ) 검사실에안내문, 특수염색결과보고서, 특수염색설문지등과함께 가지의세포주를도말한슬라이드를 6장씩발송하였다. 6장의슬라이드마다시행해야하는특수염색항목 (iron, PAS, MPO, SBB, ANAE, NaF) 을지정하였다. 각기관에서염색및판독한슬라이드는다시발송처로보내도록하여주관기관에서도재판독하였다. 결과. 정도관리용슬라이드제작프로토콜수립슬라이드제작은 HL-60 세포주의농도를 /μl 로맞 Fig.. 추고 슬라이드에 피펫으로 30 μl 분주하여 PAS 염색을 시행 하였으나 세포질의 염색상을 판독할 수 없었고, μl당 400개 의 세포 부유액 00 μl로 cytospin을 이용하여 슬라이드를 제 작함으로써 세포핵과 세포질을 관찰할 수 있는 시야를 충분 히 확보할 수 있었다(Fig. ).

3). Human promyelocytic leukaemia cell line인 HL-60은 MPO 및 SBB 염색에서 양성이고[7], human acute monocytic leukaemia cell line인 THP-은 PAS 및 ANAE 염색에서 양성이었다[8].

3 추고 슬라이드에 피펫으로 30 μl 분주하여 PAS 염색을 시행 하였으나 세포질의 염색상을 판독할 수 없었고, μl당 400개 의 세포 부유액 00 μl로 cytospin을 이용하여 슬라이드를 제 작함으로써 세포핵과 세포질을 관찰할 수 있는 시야를 충분 히 확보할 수 있었다(Fig. ). 7종의 암세포주(THP-, K56, Kasumi-, HL-60, KO5, NKM-, KG-) 중 PAS 염색상 이 좋지 않은 KO5 및 NKM-과 세 가지 염색에서 모두 음 성인 KG-은 제외한 후 실험을 진행하였다. 나머지 4종의 암 세포주(THP-, K56, Kasumi-, HL-60) 중에서는 HL60 및 THP-의 염색상이 가장 우수하였다(Fig. 3). Human promyelocytic leukaemia cell line인 HL-60은 MPO 및 SBB 염색에서 양성이고[7], human acute monocytic leukaemia cell line인 THP-은 PAS 및 ANAE 염색에서 양성이었다[8]. ANAE 염색 후 NaF에 의한 억제 여부를 확인하는 기관은 3개였고, NaF에 의한 ANAE의 억제 여부를 확인하는 대신 ANAE와 alpha-naphthyl butyrate esterase 염색을 함께 시행하는 기 관은 개였다. 참여기관에서 특수염색 시 사용하는 염색시약 의 종류와 소요시간은 Table 에 나타내었다. 각 항목마다 고정/염색/대조염색 단계별 시약종류 및 소 요시간은 기관마다 차이가 있었다. Iron 염색은 고정 시 methanol, formalin, 또는 formalin vapour를 사용하거나 고 정을 거치지 않고 바로 염색을 하는 기관도 있었다. 염색액은 HCl+potassium ferrocyanide를 혼합하여 사용하였다. 대조 염색은 nuclear fast red, safranin O, neutral red solution, Harris hematoxylin, 또는 basic fuchsin solution+neutral red solution을 사용하였다. PAS는 고정 시 PAS 고정액, formalin+ethanol, 또는 methanol을 사용하였고 염색액은 periodic acid 후 Schiff solution을 혼합하여 사용하였다. 대조염색은 Harris hematoxylin, Mayer hematoxylin, Gill s hematoxylin 또는 Carrazi s hematoxylin을 사용하였다. x,000 x00 A x,000 x00 C B J Lab Med Qual Assur 07;39:-8 D Fig.. NaF에의한억제여부를판정할경우에는위의염색시약에 NaF를첨가하였다. 대조염색에는 Harris hematoxylin, Mayer hematoxylin 또는 esterase kit 내 Carrazi hematoxylin이사용되었다. ) 특수염색판독결과비교 Iron은 개기관모두에서 increased/decreased 여부판정결과가일치하였고, grade는평균 4.6, 표준편차 ±0.7로고른결과를나타내었다. MPO는 0개, SBB는 3개기관에서 번 /번검체에대해각각 negative/positive로보고하였고이는예상결과와일치하였다. ANAE 염색을행하는 0개기관중 NaF 처리를시행하지않는 7개기관에서는 번 /번검체에대해각각 ANAE negative/positive로보고하였고, NaF 처리를시행하는 3개기관에서는 ANAE negative/positive and inhibited by NaF로예상과일치하는결과를보였다. PAS 염색의경우 0개기관중 8곳은 번 /번검체에대해각각 positive/negative로보고하여예상결과와일치한반면, 곳에서 negative/negative, negative/positive로보고하였으며, 예상결과와일치한 8개기관중 3곳에서는 a few positive 로판독하였다 (Table, Fig. 5). 또한각기관에서판독한결과와국립암센터에서판독한결과가대부분일치하였으나, PAS에서판독차이가있었던경우가 건있었다 ( 그기관에서는 positive로판독하였으나본원에서는 negative로판독됨 ). Iron 염색용파라핀절편은 3개월이상염색성이유지되었다. 개기관을대상으로시범적외부정도관리를시행한결과, 4가지 (iron, MPO, SBB, ANAE) 항목에서는참여기관모두같은결과를보고하였다. PAS 염색용 번검체의경우일부기관에서는뚜렷한양성소견이관찰되었으나다수의기관에서염색성이약하거나음성소견이었다. 따라서 PAS 염색의경우양성염색성을보이는다른암세포주를추가로확보하는한편, 염색성이좋지않았던기관에서는염색법의개선을고려하는것이추천된다. 지금까지국내에는진단혈액분야특수염색에대한외부정 J Lab Med Qual Assur 07;39:-8 5

PAS 를제외하고대부분의검사실에서특수염색의결과가일치했다. 특수염색을위한프로토콜은혈액학검사실에서외부및내부정도관리를위해유용하게사용될것으로생각된다.
