1 ORIGINAL ARTICLE 두종의 external hex implant 의변연골흡수에관한연구 : 예비연구 (preliminary study) 이지은 * 허성주* 곽재영* 김성균* 한종현** * 서울대학교치과대학보철학교실, ** 연세대학교영동세브란스치과보철과 목적 : 치과임플랜트주위의변연골변화는임플랜트의기능적인유지뿐만아니라심미적성공을위해서도중요하다. 변연골의유지를위한임플랜트디자인이연구되고있다. 이번예비연구는두종류의 external hex implant 시스템에서임플랜트의상부미세나사산이변연골변화량에미치는영향을평가하고자한다. 재료및방법 : 총 24 명의환자를대상으로 Bra nemark 임플랜트식립군 (1 군, 거친표면임플랜트, 20 개 ) 과원플란트임플랜트식립군 (2 군, 미세나사산을가진거친표면임플랜트, 20 개 ) 으로분류하였다. 기준시점 ( 임플랜트부하 ) 과부하를가한 1 년뒤임상적, 방사선학적검사를시행하였다. UTHSCSA Image Tool 을이용하여임플랜트 - 보철경계부로부터변연골높이변화량으로계측하였다. 계측치는 SAS 프로그램을이용하여 three-level ANCOVA 로통계처리하였다. 결과 : 기준시점과비교하여 1 년기능후, 두그룹간의변연골소실변화량에있어서통계학적으로유의한차이가있었다 (P<0.05). 1 군의평균변연골높이변화량은 0.83±0.31mm, 2 군에있어서는 0.44±0.36mm 이었다. 미세나사산을가진거친표면임플랜트가미세나사산을가지지않은거친표면임플랜트보다더적은변연골소실변화량을보였다. 결론 : 미세나사산을가진거친표면임플랜트가기능적부하후변연골높이를유지하는데있어더유리한디자인으로보인다. ( 대한치과보철학회지 2008:46:169-74) 주요단어 : 변연골높이, 미세나사산, 거친표면 서론 골내임플랜트의장기간결과는주로골조직의보존에의존하므로골유착과변연골높이의유지는중요하다. 임플랜트주위의변연골상실에대해자주보고되었다. 임플랜트의생존과성공에대한확립된평가기준에따르면, 변연골높이변화는첫해에 1-1.5mm 미만이고그후매년골소실은 0.2mm 미만이어야한다고하였다. 1 Adell 등 2 은 Bra nemark 시스템을사용한 15 년연구에서기능첫해에첫번째나사산 (first thread) 으로부터평균 1.2mm 의골소실을보고하였다. Bra nemark 임플랜트주위의기능첫해의변연골상실에대해보고한, 13 개저널의리뷰에서 Goodacre 등 3 은평균 0.93mm 값을보였고, mm 의범위를가진다고하였다. 이러한변연골흡수에대해여러가지가능한요인들 이제시되고있다. 수술시외상, 교합의과부하, 임플랜트주위염, 미세간극 (microgap) 의존재, 생물학적폭경 (biologic width) 의재형성, 임플랜트치조골정부 (crest module), 그외기타요소들이초기임플랜트골소실 ( 임플랜트식립에서 1 년부하후 ) 의가능한요소들로제시되었다. 4 그외다양한임플랜트시스템에서, 12 개월후의골소실량과 machined surface 의길이간에상관관계가보였으며관련된골소실높이는 첫번째나사산 (first thread) 이라고하는보고가있다. 또한이연구에서연마된경부 (polished neck) 하방에발생하는변연골흡수 ( 연구된모든임플랜트종류에서약 0.4mm 의골흡수량 ) 는부하가관련된다고제안하였다. 5 어떤연구에서는특정임플랜트디자인이골소실에관련된다고하였고, 6 그런골소실은임플랜트칼라 (implant collar) 내에생역학적안정연결부 (stable joint) 와 교신저자 : 허성주 서울시종로구연건동 28-1 서울대학교치과보철학교실 : , 0504heo@hanmail.net 원고접수일 : 2008년 3월 13일 / 원고최종수정일 : 2008년 4월 14일 / 원고채택일 : 2008년 4월 17일 대한치과보철학회지 2008 년 46 권 2 호 169

환자및임플랜트 피실험자는 2004 년 10 월부터 2006 년 4 월까지서울대학교치과병원보철과에내원한환자중선택하였다. 본연구는서울대학교치과병원임상연구윤리위원회의심의후승인을받았다. 총 24 명의환자중남자 15 명, 여자 9 명이었고, 환자들의나이는평균 59.7 세였다.

2 미세나사산 (microthread) 같은유지요소 (retention element) 를부여함으로써방지할수있다고보고하였다. 7 또한 Norton 8 은임플랜트경부 (implant neck) 에거친표면 (rough surface) 과미세나사산 (microthread) 를가진임플랜트에서더적은양의변연골소실을나타냈다고하였다. 이번연구의목적은임플랜트의치관부 (coronal part) 에서로다른디자인을가진 2 종류의 external hex 임플랜트시스템에서관찰된변연골변화량을분석함으로써, 임플랜트의거시및미세구조 (macro- and microstructure) 가골소실에미치는영향을평가하기위함이다. 연구재료및방법 1. 환자및임플랜트 피실험자는 2004 년 10 월부터 2006 년 4 월까지서울대학교치과병원보철과에내원한환자중선택하였다. 본연구는서울대학교치과병원임상연구윤리위원회의심의후승인을받았다. 총 24 명의환자중남자 15 명, 여자 9 명이었고, 환자들의나이는평균 59.7 세였다. 환자들중한군은양극산화법의표면을갖는 Bra nemark 시스템 (TiUnite Bra nemark MK III, Nobel Biocare USA, Yorba Linda, CA) 임플랜트를시술받았고, 다른군은매식체의치관 (coronal) 부분에미세나사산 (microthreads) 을가진원플란트시스템 (Hexplant Oneplant; Warantec, Seoul, Korea) 임플랜트를시술받았다 (Fig. 1). Bra nemark TiUnite 임플랜트는양극산화표면 (anodic oxidation surface) 을가지며, 표면거칠기는치관부 (coronal part) 에서 1-2 μm (peak-tovalley distance), 치근단부 (apical part) 에서 7-10 μm로증가한다. 9 Oneplant Hexplant 임플랜트는 ABE (Advanced Blasting & Etching) surface 로, 1.44 μm의표면거칠기값을가지며, 임플랜트의치관부 (coronal part) 에미세나사산 (microthreads) 을가진다. 미리정해둔다음의기준에맞는환자들로구성되었다. - 제조사의가이드라인에따라임플랜트를식립할수있는충분한골높이를가져야한다. - 임플랜트식립동안천공혹은열개를방지할수있도록충분한골폭경을가져야한다. - 발치부위의완전한치유기간을거쳐야한다. ( 발치후적어도 3 개월후에식립 ) - 단일치혹은 2-3 본브릿지구치부수복물을가진다. 차폐막을사용하여골이식이필요한경우혹은이갈이가있는환자들은제외되었다. 동통, 감염, 임플랜트동요도등이관찰되면실패로간주하였다. 2. 임플랜트치료 모든임플랜트는제조사가추천한가이드라인에따른깊이로식립되었고, 하악은 3 개월, 상악은 6 개월의치유기간후단일치혹은 2-3 본보철물로수복하였다. 수술은치주과에서시행되었고보철치료는동일한임상의가수행하였다. 3 3. 정기관리 (follow-up) 기준시점 ( 보철물시적 ) 과부하를가한 1 년정기관리 (f/u) 후에임상적, 방사선학적검사를시행하였다. 감염, 농양, 비정상적혈종, 감각이상, 동요도등이관찰되는경우기록하였다. 보철적결과는임플랜트 - 유지수복물이기능하고있다면성공적인것으로기록되었다. 디지털구내방사선사진 (70 KVp, 7 ma, Instrumentarium Corp., Tuusula, Finland) 은디지털센서와 XCP 센서홀더 (CDR sensor and Schick CDR Holder, Schick technologies, Long Island City, NY, USA) 를사용하여평행촬영법으로획득하였다. 임플랜트 - 보철경계부 (implant-abutment interface) 를기준점으로사용하였고, 임플랜트주위변연골의최하방치근단까지의골높이를측정하여, 이둘사이의수직거리를변연골높이변화량으로계측하였다. UTHSCSA Image Tool (version 3.0 for windows, University of Texas Health Science Center in San Antonio, TX, USA) 소프트웨어를이용하여 0.01mm 단위까지측정하였다 (Fig. 2). 골소실량은임플랜트의근심면과원심면에서각각측정한뒤, 평균값을사용하였다. 4. 통계분석 1 년정기관리 (follow-up) 후, 기준시점의골높이와비교한변연골변화량이두임플랜트시스템간에차이가있는지분석하기위하여나이, 성별, 근원심면의측정등을고려해공분산분석 (three-level mixed-effect analysis of covariance, ANCOVA) 을시행하였다. 통계적유의성을결정하기위한유의수준 (significant level) 은 0.05 를채택하였다. 실험결과의통계처리는 SAS 프로그램 (SAS Institute, Cary, NC, USA) 를이용하여최종적으로 MIXED procedure 를통해분석하였다. 결과 연구결과는총 24 명의환자 ( 남자 15 명, 여자 9 명, 평균나이 57.4 세 ) 에서총 40 개의임플랜트로측정되었다. 20 개는 Bra nemark 임플랜트이고, 나머지 20 개는원플란트임플랜트였다 (Table I). 모든환자에서임플랜트초기안정성이확보되었고, 연구기간중동통, 동요도혹은보철적합병증등을포함한특별한문제점은관찰되지않았다. 4 변연골높이변화량 (marginal bone level changes) 각임플랜트의변연골소실량이 Table II 에나타나있다. 기능 1 년후, 임플랜트시스템종류에따른변연골소실량은 MIXED procedure 를통해분석되었고, 두군간에통계학적으로유의한차이점이관찰되었다 (p=.0006<.05). 기준시점과비교하여기능적부하를가한 1 년후, Bra nemark 임플랜트군은 0.83±0.31mm 의변연골높이변화량을보였고, 원플란트임플랜트군은 0.44±0.36mm 의변화량을나타내었다. 고찰 이연구에사용된두종류의 external-hex 임플랜트에있어서의기준시점과비교한변연골높이변화량은, 원플란트군은 0.44±0.36mm, Bra nemark 군은 0.83±0.31mm 를나타내었다. 이전 Adell 등 2 에의한연구는주로무치악환자에서 machined surface 를가진임플랜트를대상으로연구하였고, 최근에임상적으로사용되는대부분의임플랜트들은표면이거친표면 (rough surface) 이다. 거친표면 (rough surface) 을가진임플랜트는초기골유착동안에는임플랜트주위골과의긴밀한연결을제공하고, 장기적으로는기능적부하 (functional loads) 의분산과적응골개조 (adaptive bone remodeling) 에기여한다고하였다. 10 또한, 장기간의임플랜트주위골치유에유리하다고보고되고있다. 11,12 본실험에사용된두종류의임플랜트는매식체치관부분의거시와미시구조 (macro-, microstructure) 에서차이가난다. 원플란트임플랜트는매식체치관부 (coronal part) 에미세나사산을가진거친표면 (rough-surfaced microthreads) 임플랜트이고, Bra nemark 임플랜트는미세나사산이없는거친표면 (rough-surfaced) 임플랜트이다. 기능적부하일년후, 기준시점과비교하여변화된변연골소실량의차이는원플란트군은 0.44±0.36mm, Bra nemark 군은 0.83±0.31mm 값으로미세나사산 (microthreads) 을가진원플란트군에서통계학적으로유의하게더적은변연골소실변화량을보였다. 이것은골높이를유지하는데있어서의미세나사산 (microthreads) 의영향이크다고할수있으며, 이에대한많은임상적연구가시행되고있다. 13,14 Wilke 등 15 은임플랜트표면이어떤형태로든거칠어지면, 임플랜트와골사이의계면간전단강도 (interfacial shear strength) 에대한저항이증가한다고하였다. Norton 8 은 33 개의단일치임플랜트를 4 년간방사선학적으로연구한결과, 근심측으로 0.32mm 의변연골소실을보였고원심측에는 0.34mm 의골소실을 보였다고하였다. 또한 Palmer 등 16 은미세나사산 (microthreads) 을가진 Astra Tech Implants 를이용한관찰에서 1 년에서 2 년동안임플랜트주위변연골소실은 0.4mm 로통계학적으로유의한차이를나타내지않았다고하였다. 그외에도 Karlsson 등 17 은미세나사산 (microthreads) 디자인을가진 47 개임플랜트의 2 년관찰연구에서평균변연골소실은 0.31mm 라고보고하였다. 미세나사산 (microthreads) 을가진임플랜트를이용한이러한임상적연구결과들은유한요소분석 (finite element analysis) 을통해설명될수있다. Hansson 18 은임플랜트경부 (neck) 에있는미세나사산 (microthreads) 이피질골에서의정점계면간전단강도 (peak interfacial shear stress) 를감소시켰다고하였다. 또한그는표면거칠기혹은미세나사산 (microthreads) 같은저항형태 (retentive elements) 가임플랜트표면과변연골사이의연동력 (interlocking force) 에대항하는저항성을증가시킨다고가정하였다. 그외에도 Oh 등 4 에따르면, 임플랜트주위변연골소실에있어서첫번째나사산 (first thread) 이전단력 (shear force) 을압축력 (compressive force) 으로전환시켜변연골이더잘저항할수있다고하였다. 골의기계적물성 (mechanical property) 에대한 Guo 19 의연구에서도, 골은압축강도 (compressive strength) 에가장잘저항하고, 인장강도 (tensile strength) 에는 30% 더적게, 전단강도 (shear strength) 에는 65% 더적게저항한다고하였다. 그러므로변연골에서전단력 (shear force) 을감소시킬수있는임플랜트매식체치관부 (coronal part) 의디자인이골소실을최소화하는데중요하다. 임플랜트주위의변연골상실에대한원인이현재완벽히밝혀지지는않았다. 이번연구는생역학적관점 (biomechanical aspects), 특히기능적부하 (functional load) 후변연골소실변화량에대해초점을맞춘것이었다. 추후임플랜트매식체의치관부 (coronal part) 디자인과변연골상실에대한매커니즘을확실히하기위해서는다양한매식체디자인 (fixture design) 을가진미세나사산 (microthreads) 에대한연구와보다많은수의환자를대상으로한추가적인임상연구가필요하리라본다. 결론 이연구를바탕으로한결과내에서, 임플랜트디자인에있어서매식체치관부 (coronal part) 에미세나사산 (microthreads) 을가진임플랜트시스템이기능적부하후에변연골소실변화량을감소시키는데유리하다고할수있다. 172 대한치과보철학회지 2008 년 46 권 2 호

It was proposed that bone-retention elements such as microthreads at the coronal part of implant might help maintain the marginal bone level. Purpose: This study was designed to evaluate the effect of microthread configuration within the marginal coronal portion of the implant fixture at the marginal bone changes after loading around two different external hex implants. Material and methods: Twenty-four patients were included and randomly assigned to treatment with Bra nemark system implants (Group 1, rough-surfaced implants, n=20) and Oneplant system implants (Group 2, rough-surfaced neck with microthreads, n=20). Clinical and radiographic examinations were conducted at baseline (implant loading) and 1 year postloading. Data analysis was performed by the SAS statistical package version (SAS Institute, Cary, NC, USA) and the final model was calculated by the MIXED procedure (three-level ANCOVA) for marginal bone change of each test group at baseline and 1 year follow-up. Results: Comparing to baseline, significant differences were noted in marginal bone level changes for the 2 groups at 1 year follow-up (P<0.05). Group 1 had a mean crestal bone level changes of 0.83±0.31mm; Group 2 had a mean crestal bone level changes of 0.44±0.36mm. Rough-surfaced with microthreads implants showed significantly less marginal bone loss than rough surfaced neck without microthread implants. Conclusion: A rough surface with microthreads at the implant was beneficial design to maintain the marginal bone level against functional loading.
