College of Medicine. Chonnam National University Percutaneous nephrostomy was performed in 82 patients under the f1uoroscopic guidance for recent 3 years. The causes of hydronephrosis were as follows : unknown origin of stricture (N =37). stone (N = 20). tumor (N = 14). tuberculosis (N=8). postoperative ur ete ral 띠 urγ (N = 1). postoperative anastomotic stricture (N = 1) and renal transplantation complication (N = 1). Successful n ephrostomy was achieved in 79 patients (96 %). Causes of failure were minimal dilatation of pelvocaliceal system (N = 2) and staghorn calculi (N = 1). Follow up laboratory test shows high BUN and creatinine level returned to normal limit within 1 or 3 weeks in 73 patient. Major complication was not found. but temporary hematuria (N = 4) or fever (N = 1) was noted. In conclusion. percutaneous nephrostomy is the safe and effective method fo r the temporary and permanent relief of urinary obstruction and the maintenance of ureteral patency. Index Words: Kidney. percutan eous n ephrostomy Ureter. stenosis Ureter. calculi 터보고되어왔으나국내에서는 80 년대부터보고되었 1. 서론경피신루설치술 (Perc ut a n eo u s n ep hrostomy ) 은요로폐쇄의일시적인배액뿐만아니라최근에는이를이용한 sten t를넣어서 ureteral patency을확보하는중재적방사선과학의분야로서외국에서는 50 년대부이논문은 1990년 8 월 3 1 일접수하여 년 11 월 26 일에채택되었음 다. 저자들은 년 l 월부터 1 990년 4 월까지전남대학교병원진단방사선과에서경피신루설치술을 82 명의환자에서시도하고그결과를문헌고찰과함께보고한다. 11. 대상및방법 년 1 월부터 년 4 월까지전남대학교병원진

2 류인훈외경피신루설치술 - 단방사선과에서방사선학적인검사로수신증이 발견 된환자중경피신루설치술을시도한 82 명을대상으로 하였고그중 1 2 명에서는양측을실시하였으며 9 명에 서는 s t e n t 를시도하였다. 시술방법으로는먼저초음파로신우, 신배의확장 을확인한다음엎드린자세에서투시하에호홉을멈 추고 21-gauze Chi ba nee dl e 로확장된신배를천자한 후조명제를주입하여선우, 신배의확장을확인하고 " hair-wire ( Meditech R) 를넣은후에천자침 을빼고천자부위를 5mm 정도절개한다. 그후 19G 300mm ye ll ow sheath(hanaco R) 를가이드와이어 를통해넣은후 Amplatz 가이드와이어 (Meditech R ) 로바꾼후 yellow s h eat h 를뺀다. 다음 에 6 F, 7F, 8F vessel dil ator (Cordis R ) 로확장시킨 후소아에서는주로 5F pigtail 배액관 (Meditech R ) 을성인에서는 8F pigtail 배액관 ( Meditech R ) 을삽 입하였으며혈중 BUN creatinine 수치가정상으로될 때까지추적김사하였다. 배액만을목적으로할경우 에는천자를중부또는하부신배에했고 stent 삽입을 위해서는상부신배를천자했다. 배액카테타를넣은 후에는카테타를통한하행신우조영술을시행하여폐 왜위치와정도를확인했다. Stent 삽입은조영제가 폐쇄하방으로내려가는것이관찰이되면 혹은 inch straight 혹은 3mm J 가이드와이어를폐 쇄나누공의하방부위까지통과시킨후 stent (6F 24 cm 혹은 26 cm double pigtail stent Cook Urological R 혹은 8F 26 cm Percuflex R stent Meditech R ) 를 삽입하였다. 다음에체외배액카테타를신우에위치 시켰다. 2-3 일후에 24 시간동안배뇨관을막아놓은 후배액관주위로요누출이없거나추적 nephrostomography 상에조영제가폐쇄하방으로잘내려가 면 stent 를통해서요로배설이잘되는것으로판단하 고체외배뇨관을제거하였으며 ste nt 가잘통하고있 는가는초음파및요로조영술로추적검사하였다. rn. 걸 시도한 82 명의환자의나이는고른분포를보였고 성별분포는남자 ( 남 : 여 =48 : 34 ) 에서많았다. 수신증의원인들은원인이밝혀지지않는협착이 37 명으로가장많았고결석 20 명, 종양 14 명, 결핵 8 명, 술후요로손상, 술후문합부협착, 신이식합병증이 Table 1. Causes of the Hydronephrosis Causes Stricture Ureteropelvic junction Ureterovesical junction Ureter No. of Patien ts 3 7 πωm Ston e 20 Tu berculosis Tumor Metastasis Bladder cancer Cervix cancer Rectal cancer Epidermoid cancer (Psoas musclel Renal cancer P 6 7I J nι鋼빼때m 뼈띠때Renal transplan t complication m 야디Total 82 각각 1 명이었다 (T ab l e 1 ). 원인이밝혀지지않는폐 쇄가많았는데 1 4 세이하소아에서는요관신우연결 부협착이 6 명으로 ( Fi g. 1 ) 성인에서는요관방광연 결부협착이 1 4 명으로많았다 ( Fig 2 ). 82 명중 79 명에서성공하였고실패한경우는 3 명으 로신우, 신배의확장정도가미약한경우가 2 명, 결석이 녹각 1 명으로하행신우조영술은시행할수있었지 만배액관의삽입은어려웠다 시술의합병증은혈뇨 4 명, 발열 1 명이있었지만특 별한치료없이수일후에호전되었고치료를요하는주 요합병증은없었다. 배액관이빠진예는 5 명으로대개환자의부주의로 빠졌는데누도가남아있는경우에는바로투시하에가 이드와이어를넣은후에다시배액관을설치했고, 도가없는경우에는초음파로신배가확장될때까지 기다린후에다시배액관을설치하였다. 배액관이막힌예는 4 명으로 1 명에서혈종때문이었 고이때는가이드와이어를이용해서뚫어주었으며 3 명에서는요산이뭉쳐서배액공을막았는데이경우에 는새로운배액관을설치했다 누

Right ureterovesical stricture in 45 years old female. 혈중 BUN 과 Creatinine은 79 명의환자에서경피신루설치술후 3 일에서 26 일사이에추적검사하여 6 명은감소후다시증가하였는데원인질환으로는만성협착이 5 명, 양측요관결석이 1 명이었다 (T abl e 2). Stent 삽입은 9 명에서시도하여 8 명에서는성공하였고 1 명에서는심한요관의폐쇄로실패하였다 (Table 3). Ste n t 의작동여부는추적초음파검사나요로조영술로하였으며수신증의재발이없고조영제가방광으로배설이잘되면 6 주후에방광경하에서제거하였다. 재발성자궁경부암이원인이었던 2 례에서도항암제투여와방사선치료로더이상의요로폐쇄는요도조영술검사상관찰되지않아서 ste nt를제거했다. 이중 1 명은자궁탈출증으로수술후좌측요관의손상을받아누공을통해서농양이형성되어배액관을통 해서배농시켜주고신장에는경피신루설치술후에요 관의협착을방지하기위해 internal ste n t 를삽입하였 다 ( Fi g.3 ). 다른 1 명은신이식후유증으로하부요로 Table 2. 5 - 大韓放射線훌훌學會픔 : 第 26 卷第 6 號 1990 신피질하농양이있는경우주의해야한다 9 ) 천공퉁이보고되고있다 23 ) 유도방법으로는초음파의투시하방법둥이있으나 가이드와이어조작과카테타위치결정이편리하기때문에투시하에서많이실시하고, 천자방법으로 Seldinger법, Tro ca r 법및 n eed l e - cathe ter볍뭉 3 가지기 V. 결론 저자들은 년 1 월부터 년 4 월까지전남대학 본방법이있으나모두환자체위와천자위치는차이가 교병원진단방사선과에서 82 명의 경피신루설치술을 없마. 저자들의경우에환자의체위는복와위로, 천자 시도하여 79 명 ( 96 % ) 에서성공하였고신우, 신배의 는신문혈관을피하기위해후측방천자를하였으나, 1 명에서는복부수술상흔으로복와위를취할수없어 확장이미약한 2 명과녹각결석 1 명은실패했다. 치료를요하는주요합병증은없었고일시적으로혈 서측와위에서후방천자를하였다. 또한하행신우조 뇨 4 명과발열 1 명이있었다. 영시감염이의심되는경우는조영제를빼낸요량보다적은양을주입하여감염된요의역류로인한균혈증의위험률을감소시켜야한다 9, 1 0) 성공율은대개 % 정도되지만신우, 신배의확장이안되었거나녹각결석인경우 % 정도된다 11, 12, 13 ) 저자들의경우는약 96 % 의성공율을보였고확장이미약한경우 2 명과녹각결석 1 명에서실패했다. 추적검사상 79 명중 73 명에서혈중 BUN 치와 Creatinine 가 1-3 주동안에정상수치로감소를보였 으나나머지 6 명에서는일시적인감소후다시증가하 였다. 9 명에서 ste nt 를시도하여 8 명에서 ste n t 를삽입하여 호전을보였다. 결론적으로경피신루설치술은요로폐쇄에의한나 빠진신장기능을호전시킬뿐아니라폐쇄된요로유 합병증은카테타의이탈, 폐쇄와일시적인혈뇨외 지에안전하고효과적인방법으로사료된다. 에특별한치료를요하는출혈의경우가 4-5 %, 감 염 2 %, 요낭종 % 정도보고되었으나 12, 14, 1 5, 16 ) 저자들의경우일시적인혈뇨와발열이외의특별한합병증은발견되지않았다. 대개요로의폐쇄기간이오래될수록경피신루설치술후신기능이회복되는정도가적으며, 최대한의신기능회복은신루설치후 2개월에이루어진다고한다 17) 저자들의경우 79 명중 73 명에서혈중 BUN 치와 creat1mne 치의지속적인감소를보였으나나머지 6 영에서는감소후다시증가하였다. Ureteral stenti n g 은요관폐쇄에대한비수술적치료방법으로최근에널리사용되어지고있는데방법은 external 이나 internal 이나 internal stent 두가지인데적웅증은치유기간중요로협착을방지하고후복막강으로뇨의누출방지, 신장에서방광에이르는요로의배액을호전시키기위해서또한드물게는수술혹은의인성누공의치료퉁에이용되고있다. 저자들의경우 8예에서성공하였는데 7 예에서는 inte rnal ste nt를 1 예에서는 external s tent를시술하였다 18, 1 9, 20, 2 1) Stent 의후유증으로 external sten t의주요합병증은감염이며특히이식받은환자에서는신우조영술하기전과후에반드시항생제를사용하여야한다 22) Internal stent 의합병증은폐쇄, 하방이탈, 파손, 신우 REFERENCES 1.
