1 14 CO2, Er:YAG, Er,Cr:YSGG 레이저를임프란트표면에조사시표면성상의변화및열변화에관한연구 수원리빙웰치과의원 1), 리빙웰치과병원 2),3) 강만석 1), 김현철 2), 이장렬 3) Ⅰ. 서론임프란트실패의중요한원인인임프란트주위염은크게교합력, 치태세균이나치석의축적등과같은국소적요인과전신질환이나신체방어력의약화등과같은전신적요인에의해단독적또는복합적으로발생된다. 임프란트주위염은임프란트주위치은의염증을동반한임프란트주위골의임상적, 방사선적인상실을가지는임상적인증상의표현이다 1). 이러한증상의치료는교합조정이나플라스틱이나카본스케일러를사용한치석제거및치은연하소파술을시행하거나중탄산나트륨과증류수를섞어서임프란트표면을기계적으로삭제하거나, 초음파공기연마기를사용하여서기계적삭제후표면을연마한후 Citric acid로 1-2분간 detoxification시키는방법들이많이사용되어지고있다. 결국이러한임프란트주위염의치료는만성염증상태로영향을받은임프란트표면과주위조직을제거함으로임프란트표면을깨끗하게만들어서임프란트주위에골의재생을유도함으로써그목적을달성하는것이다 2),3)4),5). 이들술식들은다양한연구및임상적적용의결과임프란트주위염의치료에있어매우유의성있는결과를얻을수있는것으로알려져있다. 그러나이러한치료술식들은치과마취가필요하고, 대부분관혈적인술식이므로출혈성질환이나소모성질환등의전신질환이있는환자나면역기능이감소된환자의경우금기증이될수있으며, 시술후동통과같은문제점이빈번하게존재한다. 따라서이와같은통상적인임프란 트주위염치료방법의단점을극복하고금기증을가진환자에게대체할수있는방법을찾기위하여최근들어치과진료의다양한분야에서유용하게사용되고있는레이저 (LASER, light amplification by stimulated emission of radiation) 를임프란트주위염치료에적용하기위한연구가시행되고있다. 이와같은여러연구에의해레이저가치주질환의치료에상당한효과가있음이밝혀졌고, 임프란트표면의 decontamination의목적으로적절한출력으로사용시co2, Er:YAG, Er,Cr:YSGG 레이저는임프란트표면의변화가없으며또한낮은열상승효과를가지며출력의강도에따라 99.4% 까지의 bacteriocidal effect가있다는보고 6) 도있지만 Nd:YAG 레이저나 Er:YAG 레이저, CO 2, Er,Cr:YSGG 레이저등은모두적외선 (infrared) 영역의파장을방출하는고출력레이저로치질이나치수, 임프란트에영향을끼칠수있는위험이다분히있다 7). 현재많이사용되어지는 CO2, Er:YAG, Er,Cr: YSGG 레이저를 Blasting 처리된표면에임프란트주위염의처치용도로사용되어질때그표면의온도변화와표면성상의변화가일어나지않는적절한사용조건을같이알아본예가아직은없는실정이다. 따라서본연구에서는 CO 2, Er:YAG, Er,Cr:YSGG 레이저를사용하여저출력에서부터고출력까지여러조건으로 Blasting 처리된임프란트표면에조사하여서그임프란트표면의형태학적변화, 조성의변화, 열변화등을관찰하여표면의성상의변화가없는적절한출력을 in vitro에서알아보고자한다.

Er:YAG (λ=2,940nm, SDL-3300E,B&B system,korea) 레이저를사용하여서 Blasting 처리된임프란트 ( Platon Type I,

이조사한표면을주사전자현미경 (FE-SEM, S- 4300, Hitachi, Tokyo) 을이용하여서표면의변화를관찰하고, EDAX로표면성분의분석을하였다.

또한각각의출력으로 5초간표면조사후온도상승을 Thermometer (LR8100 recorder, Yokogawa, Japan) 를사용하여측정하였다.

2 15 Ⅱ. 실험재료및방법 1. 실험재료및기기각기다른종류의파장을가지는 CO2 (λ=10.6μm, UM-L25, Unionmeical, Korea) 레이저, Er,Cr: YSGG (λ=2,780nm, Waterlase, Biolase, USA) 레이저, Er:YAG (λ=2,940nm, SDL-3300E,B&B system,korea) 레이저를사용하여서 Blasting 처리된임프란트 ( Platon Type I, platon co.ltd) 표면에 0.5W, 1W, 2W, 3W의조건으로 5초간조사하였다. 이조사한표면을주사전자현미경 (FE-SEM, S- 4300, Hitachi, Tokyo) 을이용하여서표면의변화를관찰하고, EDAX로표면성분의분석을하였다. 그리고 Optical 3D profiling system (Wyko NT8000 (Veeco)) 을사용하여서표면조도를관찰하였다. 또한각각의출력으로 5초간표면조사후온도상승을 Thermometer (LR8100 recorder, Yokogawa, Japan) 를사용하여측정하였다. Surface treatment procedure 임프란트표면에레이저를조사하기시작하고 5초뒤금속스트립이레이저팁과임프란트표면사이로내려와서차단된다 (Fig.4, Fig.5). CO 2 레이저, Er,Cr:YSGG 레이저, Er:YAG 레이저를 Blasting 처리된임프란트 ( Platon Type I) 표면에 0.5W, 1W, 2W, 3W의조건으로위의방법으로조사하여표면의변화를관찰하였다. Fig.4 2. 실험방법 Standardization of testing environment 각각의레이저를이용하여서측정하고자하는출력으로정확히 5초간조사하기위해서릴레이를이용한전자셔터 (Fig.1) 를제작하였다. 그리고바이스를이용하여서레이저핸들부분을고정 (Fig.2) 하였으며레이저출력측정기 (Fig.3) 로매번조사하기전에출력을측정하여표준화하였다. 표면의변화가약간보이기시작하는 1W의출력조건에서표면조도를관찰한결과 CO 2 레이저는전혀변화가보이지않았으며, Er,Cr:YSGG

3 16 Thermal change testing CO2 레이저는무주수하에레이저를조사하였으며, Er,Cr:YSGG 레이저, Er:YAG 레이저는주수하에조사하였다. 36.5도의워터배스내에레이저가조사되는부위의 1mm하방까지물에잠기도록설치하고 Thermocoupling은레이저조사부위 1mm하방에연결하였으며정해진각각의출력조건으로 5초간레이저를조사하였다 (Fig.6). 모두가 0.5W에서는표면의큰변화를보이지는않았다. 그렇지만 Er,Cr:YSGG 레이저, Er:YAG 레이저는 1W부터표면에약간의변화을보이기시작했다 (Fig.7). Thermal change(thermometer) Fig.7 Fig.9 CO 2 레이저로조사한군은 0.2도에서 2.2도사이에서출력의세기에따라서비례하는온도상승을보였고, Er,Cr:YSGG 레이저로조사한시편과 Er:YAG 레이저로조사한시편의경우는출력의세기와상관없이 1.8에서 3.7도의온도상승을보였다 (Fig.10).

4 17 Ⅳ. 총괄및고찰 Fig.10 CO2 레이저의 decontamination효과와온도변화에대해서는 1992년Walsh등 8) 은CO2 레이저의에너지는금속표면에크게흡수가되지않으며또한그다지큰열변화가보이지않는다고보고한이래, Oyster DK 9) 가 Pig Mandible을사용해서 saliva와 blood에오염된임프란트표면을레이저를사용한결과오염물들이완전히제거가되지않는다고했으며, Kato T 등 10) 은 Ti 디스크상에서 CO 2 레이저의조사는 Streptococcus sanguis, Porphyromonas gingivalis 의제거에효과가있음을입증했다. 2005년 Stubin ger S등 11) 은 air-powder abrasive와 laser irradiation 그리고 air-powder abrasive와 laser irradiation을같이조사한경우새로운골조성이후자의두개 group에서유의성있게더많이생성된다고보고했다. Deppe H등 12) 은 TPS, SLA표면의경우 30 J/cm 2 이상의에너지조사시 melting, glazing된표면을보이며, HA표면의경우 15.2 J/cm 2 에서 melting 된표면이보이기시작했다고하였다. 또한 Ganz CH 등 13) 은 2-4W의출력으로 4초간조사시 7 이내의온도변화를보인다고했으며, Swift JQ등 14) 은출력과조사시간에따라 CW mode에서는직선적으로임프란트표면의온도가증가하며 pulsed mode에서는온도상승이덜하다고했다. Oyster DK등 9) 은CO2 레이 저조사시출력과시간에따라임프란트상부는 1.2에서 11.7, 임프란트하부는 0.0에서 5.0 의온도상승이있다고했다. Barak SD등 15) 은CO2 레이저조사시 CW mode는 4W pulsed mode에서는 8W이하로사용하는것이임프란트주위조직에영향을미치지않는범위에서의온도상승을보인다고했다. Mouhyi J 등 16) 은 pulsed 8W 20Hz 5초에서 3 이하의변화를관찰하였고, Wooten CA등 17) 은 CW, pulsed mode에서는출력과조사시간에비례해서직선적으로온도상승이일어남다고했으며 superpulsed mode에서는온도상승효과가감소된다고보고했다. Kreisler M등 18) 도출력과조사시간에따른직선적인온도변화양상을관찰하였다. 1994년 Aoki등 19) 이 Er:YAG 레이저가치석제거에효과적이라는것을보고한이래 Er:YAG 레이저조사가 implant 표면에미치는영향에대해서도많은보고가있어왔다. Kreisler M등 20) 이 smooth surface, SLA, TPS, HA 표면에레이저를조사한결과 TPS 표면은 8.9 J/cm 2 SLA표면은 11.2 J/cm 2 HA는 17.8 J/cm 2 smooth표면은 28.0 J/cm 2 에서표면의 Melting과 Grazing이나타나기시작한다고했다. Matsuyama T등 21) 은 50 mj/pulse이하에서는표면온도변화가크지않으며, 주수하에서적절한출력을사용하면임프란트표면의치석과치태의제거가비교적효과적이라고말했다. Schwarz F등 22) 은 100 mj/pulse, 10 Hz를사용해서효과적으로치은연하치석제거를할수있다고했다. Kreisler M등 20) 은60 mj,10 pps이상에서 99.51% 이상의세균감소가보인다고하였다. 2005년 Kreisler M등 23) 은오염된임프란트표면을 Er:YAG laser 와 air powder system 으로처리한상태를비교하여레이저로처리하는표면의효과가우수함을보고했다. Er,Cr:YSGG 레이저의 Decontamination의효과는 Miller RJ등 24) 이임프란트고유의성상에변화가없이표면의오염물의제거에효과가있음을보고하였다. 이러한고찰들과본실험결과를고려하면 CO 2, Er:YAG 레이저, Er,Cr:YSGG 레이저가 Periim

5 18 plantitis 처치를위한임프란트표면의 deconta mination에응용될수있으나 CO 2, Er:YAG, Er,Cr:YSGG레이저는표면변형을야기시킬수있고주변조직열손상을일으킬수있으므로조사조건에주의가된다고생각한다. CO 2, Diode, Nd:YAG, Er,Cr:YSGG, Er:YAG 레이저모두세균감소효과가있고특히 CO 2 레이저의치유촉진가능성에대한보고와, Er:YAG 레이저는우수한표면오염물제거효과가있다는보고는주목할만하다. Er,Cr:YSGG 레이저는 Periimplantitis 처치에유용하다는보고가있었으나앞으로보다많은연구가필요할것으로생각된다. Ⅴ. 결론 1. CO2, Er:YAG and Er,Cr:YSGG레이저가임프란트표면에사용되어질경우약간의표면변화와비교적적은온도변화를보인다. 2. 그러나임프란트주위조직의손상을방지하기위해서는적절한사용방법과출력조절이필요하다. 3. 적절한출력으로조사한경우위 3가지레이저는모두임프란트표면에안전하게사용되어질수있다. 4. 타액이나혈액그리고임프란트주위조직에레이저가미치는영향은앞으로더많은연구가필요할것으로생각한다. 7 20 Abstract Effects of CO2, Er:YAG, Er,Cr:YSGG laser irradiation on surface properties and thermal change of dental implants Kang, Man-Seok 1), Kim, Hyoun-Chull 2), Lee, Jang-Yeol 3) Livingwell Dental Clinic 1) 2), 3) Livingwell Dental Hospital Dental laser has been increasingly used in implant surgery and the use of different lasers proposed. It has been suggested that laser may be beneficial in reducing surgical trauma and discomfort during the second phase of implant surgery and peri-implantitis treatment. Purpose The aim of the study was to investigate the effect of dental lasers (CO 2, Er:YAG, Er,Cr:YSGG) on the surface of implant and temperature change during laser irradiation for determining optical dose of laser irradiation in vitro model. Materials and Methods CO 2 (λ=10.6μm, UM-L25, Unionmeical, Korea), Er:YAG (λ=2,940nm, SDL-3300EN, B&B system, Korea), Er,Cr:YSGG (λ=2,780nm, Waterlase, Biolase, USA) laser irradiated on the surface of implant with different energy and frequency. A scanning electron microscope study was presented for observation of surface change. And thermocouples prepared to estimate thermal increasement of implant body during laser irradiation of implant surface. Results All of three dental laser systems can induce implant surface alterations with high energy irradiation. Melting and glazing were observed on the implant surface. But for selective range of power output, there was no significant morphologic change on the implant surface in three dental laser systems. CO2 laser produced minimal temperature changes with forming carbonization layer on the implant surface. But in irradiation dose for clinical application, there was no temperature change on the implant body. Er:YAG and Er,Cr:YSGG laser did not excessively heat implant body, especially under a water spary. Conclusion When using the CO2, Er:YAG and Er,Cr:YSGG laser seems to be safe as far as possible surface alterations and thermal damage. But the power output must be limited so as to avoid implant and periimplant tissue damage. This experiment was carried out in vitro to standardize test conditions.
