Hematogeneous pulmonary metastasis was induced by injecting suspension of VX-2 carcinoma through the vein of the ear. Five rabbits in each group were sacrificed after 3, 7, 14, 21 and 30 days of injection. Sequential follow-up chest radiography in vivo state and soft tissue radiography, high resolution CT, microradiography of the inf1ated and fixed lungs were done. Gross pathological and radiological evidence of pulmonary metastasis were seen in all rabbits sacrificed after 21 days. Chest radiographic evidence of pulmonary metastasis began to appear after 14 days of injection and showed well defined nodules about 4 mm in diameter. High resolution CT of the inf1ated and fixed lung showed metastatic nodules at 7th day of injection as tiny nodules about 1 mm in diameter. Besides the nodules, HRCT and microradiography showed clearly peribronchovascular and perinodular lymphangitic tumor spread in all five rabbits sacrificed after 30 days. ln conclusion, even with a guinuine hemotogeneous pulmonary metastasis, lymphangitic metastasis is associated in late stage of the disease and HRCT provides early detection of the nodules. 2 - 大韓放射線훌훌學會誌 : 第 26 卷第 2 號 서론 폐장은풍부한모세혈관의결집체로서전신으로부 터유입되는정맥혈내의이물질이일차적으로걸러지 는곳이며, 관이기도하다 1) 이로인해혈행성암전이의주된표적기 폐로전이된암의방사선학적소견 으로혈행성전이의경우다발성의폐결절로, 링프행 성의경우망상결절형의병변으로나타나는것으로알 려져있다 그러나실제임상환자의경우양자의 소견이병발되어나타나는경우가많으며이러한경우 선행요인을파악하기는실제적으로불가능하다. 라서순수한혈행성경로를통한폐전이의모댈은이 론적으로실험적동물을이용할수밖에없으나, 따 이와 같은실험적전이폐암에관한연구는저자가아는한 발표된바가없다. 저자등은인체의암과병리생태학적특성이유사한 VX-2 ca r c inom a 를현탁액상태로가토의귀정맥 에주입하여폐에혈행성전이암이발생함을예비검사 로확인하였으며, 이를바탕으로순수한혈행성전이 폐암의시기에따른방사선학적소견의변화와, 특히 병발되는림프행성전이여부, 전이폐암의방사선학 적검사방법에따른검출능을구명하기위하여본연 구를시행하였다. II. 대상및방법 3-4 k g 의 New Zealand 숫토끼를정상대조군, 종 양세포주입후 3 일 ( 제 1 군 ), 7 일 ( 제 2 군 ), 14 일 ( 제 3 군 ), 21 일 ( 제 4 군 ) 및 30 일 ( 제 5 군 ) 에희생시킨군 으로각군에 5 마리씩총 30 마리를사용하였다. Chl orpromazine (Sepamin) mg 을대퇴부근육 주사로진정한후 0.5 mg penthotal so dium 을 40ml 생리식염수에희석하여 kg 당 3 ml 를천천히정맥주사 하여마취하였다. VX-2 carcinoma 종양은냉동상태로일본국립암센 터에서제공받았다. 토끼의양측대퇴부외측에털을 제거소독한후종양조각을배양액 RMPI-1640 (Rosewell Park Memorial Institute, Roswell Park, New York) 에섞어근육에주사하였다. 주사후종양이 2-4cm 에도달하면 (1 4 일내지 21 일 ) 종양을분리하여 새로운토끼에근육주사하여보존하였다. 종양세포 현탁액의준비는외과적으로제거된종양을 calcium and magnesium-free Hank s balanced salt solution ( 이하 HBSS 로약함 Grand Island Biological Co., Grand Island, New York) 에넣은후, 종양조직을죽상태가될때까지가위로잘라배양액에넣어침전시킨후상층액체만을덜어내어원심분리하였으며, 이의침전물만을분리하여다시배양액에넣은후세찬 p lp e t tmg 으로종양세포가잘섞이도록하였다. 이용액 0.5 ml 를 trypan blue 0.5 cc 와혼합하여 hemocy tometer를사용종양세포농도가 20-30X10 7 / mm 3 가되도록 RPMI-1640 배양액으로희석하였다. 이와같은현탁액 1 ml 를토끼의귀정맥에서서히주사하였다. 종양세포주입직전과주입후 2 일간격으로각군별로희생시키기직전까지흉부 X 선촬영을하였다. 각군별로정해진시기에희생시킨후기관 (trac h ea ) 과함께양폐를적출한후 Heit z man 의방법을 이용폐를생존시의용적과유사하도록공기에의하여펴진상태에서고정시켰다. 이때과팽창에의한폐포파열을방지하기위하여폐내로유입되는공기압력은 cm H 2 0 를넘지않도록하였다. 적출및고정된폐를연조직 X 선촬영, 고행상 CT 및 CT 영상해당부를 1. 5mm 두께로횡절단한절편의미세방사선촬영 (m i crora di og raph y) 을시행하였다. 고해상 CT 는 GE CT/T 9800 스캐너 (General E lectric Medical System, Mi lw a uk ee ) 를사용 1 cm 간격으로스캔하였다 (5 1 2 X 512 matrix, 1.5 mm Collimation, 140 k Vp, 170 ma, 3 sec, bone algorithm, Field of View, 12.5 cm)1l 이때스캔부위를펜으로표시하여후에시행될미세방사선촬영시통일부위절편을얻을수있도록하였다. 연조직 X 선촬영은 Mammorex-XR 촬영기 (Picker International Inc., Ohio) 를사용하였다 _ CT 스캔시표시된부위를 1. 5 mm 두께의절편으로자른후 Faxi tron N (Hew l e tt-packard) 을사용미세촬영을하였다 (20-30 kvp, 30 ma, 30 minute). High resolution glass plate 2A (K o dak) 를필름으로사용하였다 1 3 ) 병리조직검사는미세방사선촬영이끝난각절편을일상적방법으로병리표본슬라이드를제작하였으며, 전이된결절의양상, 수효및크기를측정하였다

3 - 엄정기외 : 혈챙성션이폐앙 - m. 결과 흉부 X 선소견 종양현탁액주입후추적검사상 16 일째에 10 마리중 1 마리에서결절이발견되었으며, 그이후 18 일 (3 /9), 20 일 (3/8) 및 22 일 (2/2 ) 째에는급속히출현빈도가증가하였다 (Fi g. 1, 2). 사진상발견되는결절의직경은 4 mm 이상의것이었고그이하의결절은정상구조물이나인공산물 (artifact) 과감별이어려웠다. 병변의양상은대부분경계가분명한결절로나타났으며, 주입 21 일이후의군인제 4 및제 5 군에서는일부경색에의한경결및늑막삼출, 국소적늑골파괴가있는경우도있었다. 적출된폐표면의육안적소견 주입후 7 일째인제 2군부터평균 1. 4 mm 크기의회백색결절이관찰되었으며결절의평균직경은제 3 군 mm, 제 4 군 mm, 및제 5 군 mm 로증가하였으며, 결절의수효도제 4군으로갈수록증가하였다 (Fig. 3, 4). 제 3 군에서 1 마리, 제 4 군에서 l 마리가폐경색의소견을보였으며, 제 5 군중 1 마리는종격동종괴외에도심낭및횡격막침습소견이있었다. 고해상 CT 소견 적출된폐의고해상 CT에서제 2 군의 3 마리, 그리고제 3군부터는전예에서결절이관찰되었다. 결절의크기및수효는육안적소견과같이주입후오래된군 일수록증가하였다 (Fig. 5A, C). 결절주위또는기관 지나혈관주위로선상의침윤으로간질올통한종괴의확산소견이제 2군에서 1 마리, 제 3군에서 3 마리, 그리고제 4 군이후에는전예에서나타났다 (Fig. 2). 연조직 X선촬영소견제 2군의 1 마리에서 1.5 mm, 제 3 군의 3 마리에서 2-3m m 의결절을관찰할수있었으며, 선상의간질성침윤소견은제 3군에서 1 마리, 제 4군에서 2 마리, 제 5 군에서 3마리에서만관찰되었다 (Fig. 2, 3). At 30th day of injection, there are rnultlple well rnarginated pulrnonary nodules (white arrowheads) as well as band like atelectasis (black arrows) and rnediastinal rnass (black arrowheads). 미세방사선촬영술소견 높은해상력으로 1 mm 이하의결절을뚜렷하게영 상화였으며 (Fig.5B), 특히제 4 및제 5 군의표본에서 는결절주위의기관지나혈관을따라서선상혹은관 상으로확산되고있는종양을뚜렷하게보여, 선행된 혈행성전이에 2 차적립프행성전이가있음을강력히 시사하였다 (Fig. 5D)

6 - 大韓放射線훌훌學會誌, 第 26 卷第 2 號 광학현미경적소견혈행성전이결절이외에국소적으로파급된기관지및혈관주위결체조직에종양세포침윤이있었으며, 기관지점막, 폐포및기관지강내에서도유두상의암세포성장을관찰할수있었다 (Fig.6). 괴는폐동맥으로부터자양혈관을받아성장하고 1 5) 원발생폐암과달리주위조직으로의침윤이적기때문에흉부 X 선혹은 CT 상의소견은경계가분명한원형의결절로나타난다. 본연구에서도압도적다수의전이성결절이흉부 X 선, 고해상 CT 및절편의미세촬영에서경계가분명한결절로나타났으나, 종양세포주업 21 일이후의폐에서는결절주위로국소적파급은물론기관지및폐혈관속동의간질을통한파급이뚜렷하여 (Fig.5), 전이폐암이순수한혈행성인경우에도경과에따라간질및림프행성파급이병발됨을밝히고있다. 이러한사실은폐의림프행성암전이통로로알려진폐문부립프절로부터의역행성파급, 횡격막을통한폐장내렴프관으로의직접침습, 선행된혈행성전이로부터의 2 차적림프관침습풍의통로중 16 ) 가장후자의경로에해당된다. 폐간질을통한암의전이로고해상 CT 상뚜렷한 소견은기관지 - 혈관속올따르는파급으로이주위의 Fig. 6. X200) N. 고잘 실험적동물에서사용되는대표적종양으로백서의 Leydig cell tumor, 마우스의악성섬유종, 백서의 Walker carcinoma, 토끼의 VX-2 ca rcinoma 가있는데 이중 VX-2 ca r ci n o ma 는 V lt US 에의해서유발된 Shoppe 유두종 (papilloma) 의 악성변성의결과로생성된 것으로, 쉽게이식시킬수있고국소적침윤이강하며 급속히성장하는종양으로형태적, 생화학적, 생물학 적양상이인체의종양과유사한특정이있다 4 ) 이와 같은 VX-2 carcinoma 를이용한실험적연구는간동맥 색전 5) 뇌종양유발 6 ) 골격계에대한전신적효과 7) 실험적고칼습중 8.9) 나, 진다. 동의분야로연구발표된바있으 폐전이유발의모댈은본연구가첫보고로믿어 폐의혈행성전이암은혈류중미세암입자가폐소 동맥및모세혈관에걸림으로써유발되며 14 ) 이암종 종괴에의한두께증가와, 제 2소엽간중격 (interlobular septa) 의비후인데 ) 가토의폐를대상으로한본연구에서는기관지-혈관속을따른종양의파급은뚜렷한반면제 2소엽간중격의비후소견은뚜렷하지않는데, 이는가토폐의해부학적구조상소엽간중격의발달이인체에비하여미약하기때문으로판단된다. 전이성폐결절의검출에있어서본연구의흉부 X 선상 16 일이후에 4mm 이상의크기일때비로소주위혈관둥의구조물과구분되어나타났는데, 이는인체의경우 2-3mm 크기의결절을흉부 X선상발견가능한점과비교할때인체보다왕성한호홉운동에의한 artifact로공간분해능의저하에의한것으로보인다. 적출후고정된폐의고해상 CT에서는종양주입후 7 일부터 lmm 내외의결절을발견할수있어서적출된폐에서육안적으로관찰가능한결절은모두영상화가가능함을보여주고었다. 실제본연구에서사용된 CT 의 pixel 횡단면크기는 O. 24XO. 24 mm 이므로 1 mm 이하의결절올영상화하는데무리가없다. 이에비하여적출된폐의연조직 X 선촬영에서는이의공간분해능은고해상 CT 보다높으나, 고해상 CT 에비하여투과된중첩영상이라는점과대조해상력이상대적으로날은점으로인하여소결절의검출능은고해상 CT보다낮은점이 (Fig.2) 설명된다. 본연구의의의는실험동물올사용하여혈행성전 -228-

7 - 엄정기외 : 혈행성천이폐암 - 이를인위적으로유발시키는방법을개발하였다는점과이방법을이용혈행성전이에서도병변이진행함에따라, 림프행성전이가병발되어나타남을밝힌점이라하겠다. V. 결론혈행성전이폐암병변의진행에따른방사선학적소견의변화와흉부 X 선, 고해상 CT 동의방사선학적검사법의전이폐암검출능을구명하고자, 가토 V. X.2 c arcinoma를귀정맥에종양세포현탁액을주입전이폐암을발생시키는방법을개발하였으며, 이와같은방법을사용하여유발시킨전이폐암의시기에따른방사선학적검사및적출된폐장의병리조직학적소견을분석한결과, 초기의소결절상태에서는전형적인혈행성전이의방사선학적소견을보이나진행되어결절이커진상태에서는기관지-혈관속혹은결절주위의간질로의종양의파급소견을보여림프행성전이가병발됨을시사하였다. 소결절의검출능은고해상 C T가흉부 X 선이나, 적출된폐의연조직 X 선보다우월하였다.
