3 대한영상의학회지 2006;54: 수 있었다. 본 연구는 정량적 분석과 이상적인 천자 지표 및 위치 결정 을 위한 분석으로 나누어 실시하였다. 정량적 분석은 서혜부 피부 주름선과 대퇴 동맥 분기와의 거 리(대퇴 동맥 분기가 서혜부 피부 주름선 보다 위에 있는 경 우는 +, 아래에 있는 경우는 -), 대퇴 골두 아래 경계와 동맥 분기와의 거리(대퇴 동맥 분기가 대퇴 골두 아래 경계의 위에 있는 경우는 +, 아래에 있는 경우는 -), 총 대퇴 동맥의 길이, 그리고 서혜 인대와 상 전 장골 극(anterior superior iliac spine)과 치골 결절(pubic tubercle)을 잇는 선의 거리(서혜 인 대가 상 전 장골 극과 치골 결절을 잇는 선의 위에 있는 경우 는 +, 아래에 있는 경우는 -)를 측정하였다. 이상적인 천자 지표 및 위치 결정을 위한 분석은 골격 지표 및 서혜부 피부 주름선으로 나누어 서혜 인대와 대퇴 동맥 분 기와의 관계를 조사하였다. 골격 지표는 투시 하에 상 전 장골 극 및 치골 결절을 이은 선을 기준으로 이에 평행한 선을 그어 각 구역을 나누었다. 상 전 장골 극 및 치골 결절을 그은 선의 윗부분을 zone 1, 이 선 과 비구와의 사이를 zone 2, 비구와 대퇴 골두 사이, 즉 고관 절 간격(hip joint space)을 zone 3, 대퇴 골두를 3개의 선으 로 4등분 하여 zone 4, 5, 6, 7로 나누고, 대퇴 골두 아랫부분 을 zone 8로 하였다(Fig. 2). 서혜부 피부 주름선을 기준으로 하는 방법은 1 cm 간격으로 서혜부 피부 주름선과 평행하게 선을 그어 윗부분을 가까운 순서대로 zone 1, 2, 3, 4, 5, 6, 7, 8로 하였고, 아래로는 zone -1, -2, -3, -4로 하였다(Fig. 3). 각 지표에서 가장 좋은 천자 위치는 모든 서혜 인대의 위치 가 그 구역의 위에 있으면서, 대퇴 동맥 분기의 윗부분이 천자 될 확률이 가장 높은 곳으로 하였다. 결 Fig. 3. Twelve zone divided according to inguinal crease landmark a: Deep circumflex iliac artery b: Inferior epigastric artery c: Common femoral artery d: Inguinal crease 과 서혜부 피부 주름선과 대퇴 동맥 분기의 평균 거리는 11.8 mm±13.3( )이고, 대퇴 동맥 분기가 서혜부 피부 주름선보다 위에 있는 경우는 74예(80%), 아래에 있는 경우 는 18예(20%)이었다(Fig. 4). 대퇴 골두 아래 경계와 대퇴 동맥 분기의 평균 거리는 10.8 mm±11.2( )이고, 대퇴 동맥 분기가 위에 있는 경 우가 74예(82.2%), 밑에 있는 경우가 16예(17.8%)이었다 (Fig. 5). 골격 지표와 서혜 피부 주름선을 기준으로 정한 구역에 따 른 서혜 인대와 대퇴 동맥 분기의 위치를 분석한 결과를 보면 골격 지표에서는 대퇴 동맥 분기는 zone 1, 2에서는 없었고, zone 3에서 1예(1.1%), zone 4에서 2예(2.2%), zone 5에서 3예(3.3%), zone 6에서 24예(26.7%), zone 7에서 44예 (48.

4 정환훈 외: 대퇴 동맥 천자에 관한 연구 총 대퇴 동맥의 평균길이는 43.3 mm±12.6( )이었 다. 상 전 장골 극과 치골 결절을 이은 직선과 심 장골 회선 동 맥 및 하 복벽 동맥이 분지하는 서혜 인대와의 평균거리는 19.3 mm±9.4( )이고, 이 두 동맥의 분지점이 상 전 장골 극과 치골 결절을 이은 선의 상부에 경우는 2예(2/67; 3.0%)이었고, 아래에 있는 경우는 65예(65/67; 97.0%)이었다 (Fig. 6). 골격 지표와 서혜 피부 주름선을 기준으로 정한 구역에 따 른 서혜 인대와 대퇴 동맥 분기의 위치를 분석한 결과를 보면 골격 지표에서는 대퇴 동맥 분기는 zone 1, 2에서는 없었고, zone 3에서 1예(1.1%), zone 4에서 2예(2.2%), zone 5에서 3예(3.3%), zone 6에서 24예(26.7%), zone 7에서 44예 (48.9%), zone 8에서 16예(17.8%)여서 대퇴 동맥 분기는 zone 7에 44예(48.9%)로 가장 많이 위치하는 것을 알 수 있 었고, 서혜 인대는 zone 1에서 2예(3.0%), zone 2에서 34예 (50.7%), zone 3에서 25예(37.3%), zone 4에서 6예(8.9%) 였고, 그 이하에서는 없었다(Table 1). 서혜부 피부 주름선을 기준으로 할 때, 대퇴 동맥 분기는 zone 4는 4예(4.4%), zone 3은 19예(21.1%), zone 2는 30예 (33.3%), zone 1은 19예(21.1%), zone -1은 13예(14.4%), zone -2는 3예(3.3%) 및 zone -4는 2예(2.2%)가 위치하였 다. 한편 서혜 인대는 zone 8에서 7예(10.4%), zone 7은 11 예 (16.4%), zone 6에서 19예 (28.4%), zone 5는 20예 (29.9%), zone 4에서 7예(10.4%), zone 3은 3예(4.5%)였고, 그 이하의 zone에서는 없어 실제로 많은 경우에서 서혜 인대 가 서혜부 피부 주름선보다 높게 위치한다는 것을 알 수 있었 다(Table 2). 그러므로 서혜 인대 아랫부분이면서 대퇴 동맥 분기점 상부 에 있는 가장 좋은 천자 지표는 골격 지표에서는 zone 5이고, 서혜부 피부 주름선 에서는 zone 2였다. 골격 지표의 zone 5 는 모든 대상 환자의 서혜 인대의 아래에 있으며 zone 3, 4, 5 Fig. 5. Materials and Methods: We studied ninety consecutive patients who underwent femoral arterial puncture for performing angiogram. For the evaluation of bony landmarks, the pelvis and inguinal areas were divided into 8 zones according to 7 lines that were drawn parallel to the line drawn between the anterior superior iliac spine and the pubic tubercle. For evaluation of the inguinal crease as a landmark, the 8 zones above and 4 zones below the inguinal crease were determined. The zones were divided by 11 lines drawn parallel to the inguinal crease, and the interval between each line was 1cm. Locations of the inguinal ligament and femoral bifurcation were recorded for every patient according to the above zones, and an ideal zone for the femoral arterial puncture was decided upon. The ideal zone was considered if the locations of all of inguinal ligaments were above the zone and the least possibility to puncture was below the femoral bifurcation. Results: On the bony landmark, the femoral bifurcations were located at zone 3 in 1 patient (1.1%), at zone 4 in 2 patients (2.2%), at zone 5 in 3 patients (3.3%), at zone 6 in 24 patients (26.7%), and at zone7 in 44 patients (48.9%). Inguinal ligaments were at zone 1 in 2 patients (3.0%), at zone 2 in 34 patients (50.7%), at zone 3 in 25 patients (37.3%), and at zone 4 in 6 patients (8.9%). When the inguinal creases were used as a landmark, the femoral bifurcations were located at zone 4 in 4 patients (4.4%), at zone 3 in 19 patients (21.1%), at zone 2 in 30 patients (33.3%), at zone 1 in 19 patients (21.1%), at zone -1 in 13 patients (14.4%), at zone -2 in 3 patients (3.3%) and at zone-4 in 2 patients (2.2%). Inguinal ligaments were at zone 8 in 7 patients (10.4%), at zone 7 in 11 patients (16.4%), at zone 6 in 19 patients (28.4%), at zone 5 in 20 patients (29.9%), at zone 4 in 7 patients (10.4%), and at zone 3 in 3 patients (4.5%). Therefore, the best zone for femoral arterial puncture was zone 5 with using bony landmarks and zone 2 with using inguinal crease landmarks. In terms of zone 5 on the bony landmark, every locations of inguinal ligaments was above it and 84 patients (93.4%) had their femoral bifurcation below it, excluding the 6 patients who had their femoral bifurcations at zones 3, 4, and 5. Therefore, zone 5 with using the bony landmarks was a good indicator for femoral arterial puncture. In case of zone 2 on the inguinal crease landmark, although every location of the inguinal ligament was above it, 53 patients (58.8%) had their femoral bifurcation above it at zones 4, 3, and 2. So, it was not a good indicator for femoral arterial puncture. Conclusion: Bony landmarks are more accurate indicators for performing femoral arterial puncture than the inguinal crease landmark. Zone 5 on the bony landmark is an ideal location for femoral arterial puncture.
