1 가정의학회지 2007;28: J Korean Acad Fam Med 원저 한국어판알쯔하이머병평가척도의임상적유용성 가톨릭대학교의과대학강남성모병원가정의학교실, * 정신과학교실, ** 의정부성모병원가정의학교실 김세홍ㆍ임현국 * ㆍ김경수ㆍ정주혜 ** 연구배경 : 알쯔하이머병평가척도 (ADAS) 는치매환자의조기진단과질병진행단계를민감하게평가해주는종합적인지기능평가도구이다. 이연구에서는한국어판알쯔하이머병평가척도 - 인지영역평가도구의타당도와치매진행단계에따른변화를분석하고교육정도, 성별, 발병연령등관련된요인들에대해알아보고자한다. 방법 : 70 명의알쯔하이머병환자를대상으로 ADAS-K-cog, MMSE-K, GDS, NPI-Q, SIADL 을측정하여이들평가도구간관련성및 GDS 군별 ADASK-K-cog 총점및세부항목별점수를비교하였다. 그리고 ADAS-K-cog 점수와발병연령, 성별, 교육수준, 치매진행단계사이의관련성을알아보았다. 결과 : Pearson 상관분석에서 ADAS-K-cog 총점은 MMSE-K 점수와강한음의상관관계 (r=-0.755, P<0.001) 를보였고 GDS (r=0.732, P<0.001) 및 SIADL(r=0.700, P<0.001) 점수와강한양의상관관계를보였다. ADAS-K-cog 와 GDSS, NPI-Q, 교육수준사이에는유의한관련성이없었다. 다중회귀분석결과에서교육정도 (P=0.150) 나성별 (P=0.077), 발병연령 (P=0.349) 이 ADAS-K-cog 점수에영향을주지않았다. ADAS-K-cog 총점은 GDS stage 가증가할수록유의하게증가하였고 ADAS-K-cog 검사항목중언어표현력과단어선택장애, 언어이해력, 지남력항목은모든 GDS stage 사이에통계적으로유의한차이를보였다. 결론 : ADAS-K-cog 는교육정도와성별및발병연령에영향을받지않으며알쯔하이머병의진행단계를평가하는데유용한도구이다. ADAS-K-cog 세부항목중에서는지남력척도, 언어표현력, 단어선택장애, 언어이해력항목이알쯔하이머병진행정도를민감하게감별해주는것으로생각한다. 중심단어 : 알쯔하이머병평가척도 (ADAS-K-cog), 알쯔하이머병, 전반적퇴화척도, 한국 서 인구의고령화가진행됨에따라노인성질환의빈도가급격하게증가하고있으며그중가장사회적으로문제가되는질환이치매이다. 국내역학조사에따르면 65 세이상노인에서의치매유병률은 12.8% 로 2020 년에이르면최소 60 만명의치매환자가발생할것으로추정되고있다. 1) 최근치매로인한기능적인퇴화를지연시킬수있는치료약물들이개발됨에따라임상적으로쉽게치매를조기진단하여치료에이용할수있는평가도구를개발하고기존에사용하고있는평가도구를정리하는것은매우중요하다. 치매란진행성질환이므로임상에 접수일 : 2007 년 7 월 17 일, 승인일 : 2007 년 10 월 23 일 교신저자 : 정주혜 Tel: , Fax: juana@catholic.ac.kr * 본연구는보건복지부보건의료기술진흥사업의지원에의해이루어진것임 (A060093). 론 서치매환자를평가할때는인지영역뿐만아니라행동영역이나운동영역등의다양한측면에서체계적으로접근하여야하고치매환자의증상은평가도구들을체계적으로사용하여검사하지않는다면여러영역에서의변화를객관적으로검증하기는어렵다. 그동안치매의진단과치료에이용할수있는평가도구들이많이소개되었으나실제임상에서사용하는데는여러면에서어려움이있었다. 우리나라에서는치매치료시설대부분에서신경심리분석가에의한도움을받기어려운실정이므로임상의사가직접치매환자의포괄적인평가를실시하고있다. 따라서표준화된신경심리학적평가도구의개발및활용이절실하게필요한실정이다. 알쯔하이머병평가척도 (Alzheimer s disease assessment scale, ADAS) 는원래경미한인지기능장애를보이는치매환자를정확하게감지해내고, 치매환자의시간에따른기능저하정도를측정하기위하여개발된평가도구이다. 2-4) 최근 ADAS 는치매치료약물의임상약물시험에서인지기능영역의임상효과를측정하는표준평가도구로서도사용되고있다. 5,6) 알쯔하이머병평가척도는기억, Vol. 28, No

2 Se Hong Kim, et al: The Cognitive Subscale of ADAS-Cog for Patients with Alzheimer's Disease in Korea 언어, 시공간구성능력, 행위능력등의인지영역을평가하는알쯔하이머병평가척도-인지영역 (Alzheimer's disease assessment scale-cognitive subscale, ADAS-Cog) 과기분상태와행동변화등을평가하는알쯔하이머병평가척도-비인지영역 (Alzheimer s disease assessment scale-noncognitive subscale) 으로구성되어있다. 알쯔하이머병평가척도-비인지영역 (ADAS-Noncog) 은관찰에의해평가를내리는항목이기때문에내적일치도, 신뢰도뿐만아니라검사자간신뢰도나검사-재검사신뢰도에도영향을미쳐 ADAS-cog보다낮은신뢰도를나타낸다. 6) 이로인해임상시험에서는 ADAS-Cog만을인지평가를위해사용하고, 행동평가에는신경정신행동검사 (neuropsychiatric inventory, NPI) 나행동적정신병리평가척도 (behavioral pathology in Alzheimer s disease rating scale, BEHAVE-AD) 를사용하고있다. ADAS는전세계여러나라의언어로번역, 제작되었고일본어판, 브라질판, 독일어판, 프랑스어판, 그리스어판, 스페인어판, 이태리어판, 대만어판등이있다. 7-14) 한국판알쯔하이머병평가척도-인지영역 (Korean version of Alzheimer s disease assessment scale-cognitive subscale, ADAS-K-cog) 의신뢰도및타당도는여러연구를통해서밝혀져있지만 15,16) 아직까지교육정도및성별, 나이등평가척도에영향을줄수있는요인들과의관련성및치매진행단계에따른변화를관찰한연구는찾아보기힘들다. 이에본연구에서는한국형알쯔하이머병평가척도- 인지영역평가도구 (ADAS-K-cog) 의타당도와치매진행단계에따른변화를분석하고교육정도, 성별, 연령등관련된요인들에대해알아보고자한다. 1. 연구대상 방 2006년 1월 1일부터 2006년 12월 31일까지 12개월간일개종합병원치매클리닉을방문하여알쯔하이머병으로진단된 70명을환자군으로선정하였다. 모든환자군을대상으로면담및이학적검사, 신경학적검사, 일반혈액검사, 간기능검사, 혈청 vitamin B12와 folate, VDRL, 갑상선기능검사를시행하였고뇌자기공명영상검사 (MRI) 를시행하였다. 알쯔하이머병의진단은미국정신의학협회의정신장애진단및통계편람 4판의치매진단기준 (DSM-Ⅳ) 및 National Institute of Neurological and Communicative Disorders Association (NINCDS-ADRDA) 진단기준상유력한알쯔하이머병 (probable Alzheimer s disease) 에해당되는경우로하였고 17) 이들은모두개정판 Hachinski 허혈척도가 4점미만이었으며뇌자기공명영 법 상검사결과에서두뇌피질위축이나경도의뇌실주위연화이외에는국소적병변이없었다. 또, 모든대상자는이전에치매진단을받거나치매치료를받은적이없었다. 알쯔하이머병진단기준에부합하지않거나과거병력이나검사결과에서알콜및약물남용의과거력, 치매와관련된신경과적또는정신과적질환의과거력이있는경우, 향정신성약물을복용하고있거나악성종양진단을받은경우환자군에서제외하였다. 모든대상자와대상자의보호자를대상으로연구에대한자발적인동의를구하였고본연구는소속병원임상시험윤리위원회 (Institutional Review Board) 의윤리및안전지침을준수하였다. 2. 신경심리학적평가 모든환자및환자의보호자를대상으로 ADAS-K-cog, 간이정신상태검사 - 한국판 (MMSE-K), 전반적퇴화척도 (global deterioration scale, GDS), 신경정신행동검사 - 간편형 (neuropsychiatric inventory-questionnaire, NPI-Q), 복합일상활동평가 (Seoul-instrumental activities of daily living, S- IADL), 노인우울척도 (geriatric depression screening scale, GDSS) 를측정하였다. ADAS-K-cog 에서평가하는인지영역은기억력 (50%), 계산능력 (28%), 행위능력 (14%), 명령이해능력 (8%) 으로기억력 4 항목 ( 단어즉각기억, 지남력, 단어재인기억, 검사지시기억 ), 언어능력 5 항목 ( 물건혹은손가락이름말하기, 단어선택장애, 지시따르기, 언어표현력, 언어이해력 ) 과수행능력 2 항목 ( 구성적수행, 관념적수행 ) 등총 11 개항목으로구성되어있다. 검사시간은 30 분에서 45 분가량이소요되고각항목의최고점수는 5 12 점으로총점은 0 70 점이고점수가높을수록인지기능저하가심함을의미한다. 전반적퇴화척도 (GDS) 18) 는치매환자의중증도를제시하는대표적인평가도구로치매임상평가척도 (clinical dementia rating scale, CDR) 에비해짧은시간안에초기인지장애를세밀하게분류할수있어본연구에서는 GDS 를사용하여치매단계를분류하였다. GDS 는인지장애를 GDS 1 ( 인지장애없음 ), GDS 2 ( 매우경미한인지장애 ), GDS 3 ( 경미한인지장애 ), GDS 4 ( 중등도의인지장애 ), GDS 5 ( 초기중증의인지장애 ), GDS 6 ( 중증의인지장애 ), GDS 7 ( 후기중증의인지장애 ) 의일곱단계로구분하고있다. 모든평가는신경심리검사에대해전문적으로교육을받아평가방법에대해숙지하고있는전담간호사또는의사가피검자와일대일로검사를실시하였다. 854 가정의학회지

ADAS-K-cog 점수와연령, 교육수준, 치매진행단계사이의관련성은 ADAS- K-cog 점수를종속변수로하고발병연령, 성별, 교육정도, GDS stage 를독립변수로하여다중회귀분석으로알아보았다. 결 과 1. 연구대상자의일반적특성및 GDS stage 에따른비교 대상자 70 명중여자가 55 명 (78.6%), 남자가 15 명 (21.4%) 이었고, 평균연령은 77.0±6.2 세 (65 93 세 ), 평균발병연령은 75.4 세였다. 교육수준은평균 4.6±4.6 년 (0 16 년 ) 이었고 MMSE-K 는평균 19.6±4.5 점, NPI-Q 는 3.2±3.9 점, GDSS 는 14.0±3.7 점, ADAS-K-cog 는평균 21.9±8.1 점이었다 ( 표 1). GDS stage 별로대상군의발병연령과교육수준, MMSE-K, GDSS, ADAS-K-cog 의평균을비교분석한결과를표 2 에제시하였다. 전체 70 명대상자중 GDS stage 3 에 40 명, GDS stage 4 에 20 명, GDS stage 5 에 10 명이속하 Table 2. ADAS-K-cog 총점및항목별평균점수는 GDS stage 가높아짐에따라일관되게증가하였고이런경향은그림 1 에서확인할수있다. GDS stage 에따른항목별평균점수의사후분석 (Tukey's multiple comparison test) 에서기억력평가항목인지남력척도와언어관련항목에속하는언어표현력, 단어선택장애, 언어이해력이인접한모든 GDS stage 사이에통계적으로유의한차이를보였다. 언어표현력과단어선택장애, 언어이해력, 지남력항목을제외한나머지 7 개소검사항목들은 GDS stage 4 와 GDS stage 5 사이에서는통계적으로유의하게평균점수가증가하였고 GDS stage 3 와 GDS stage 4 사이에서는평균점수가증가하기는하였지만통계적유의성을보이지않았다 ( 그림 1). 2. ADAS cog-k 의구성타당도및연령, 성별, 교육정도와의관련성 Pearson 상관분석에서 ADAS-K-cog 총점은 MMSE-K 점수와강한음의상관관계 (r= 0.755, P<0.001) 를보였고 Vol. 28, No

Coefficient of determination (R 2 ): MMSE-K) 가있고종합적인지기능평가를위한알쯔하이머병평가척도 - 인지영역 (ADAS-K-cog), 일상활동의기능성평가도구 (S-IADL, S-ADL), 신경정신행동검사 (NPI), 치매단계평가도구 (CDR, GDS) 등이있다. 치매환자의일차적선별에 MMSE-K 와 S-SDQ 가흔히사용되고있으나초기치매의감별진단이나진행정도의평가에는민감하지않은것으로알려져있다. 19) 이에비해 ADAS-Cog 는초기치매환자선별에유리하고위양성 (false positive) 이적으며주의집중력, 기억력, 언어능력, 시공간구성능 856 가정의학회지

5 김세홍외 : 한국어판알쯔하이머병평가척도의임상적유용성 력, 행위능력 (praxis), 전두엽 (frontal lobe) 기능등여러인지영역을종합적으로포함하고있어치매유형의판단에도움이되며무엇보다치료효과에대한반응을민감하게보여줄수있는유용한도구로알려져있다. 20,21) 한국어판알쯔하이머병평가척도 (ADAS-K-cog) 의신뢰도와타당도도이미입증된바있으나 15,16) 평가에영향을줄수있는사회, 문화적인요인에대한연구는부족한실정이다. 이에본연구에서는 GDS 단계에따른 ADAS-K-cog 총점및소검사항목점수의변화양상과 ADAS-K-cog 에영향을줄수있는요인들을알아보았다. 이연구에서 ADAS-K-cog 는 GDS 와강한양의상관관계에있었고 (r=0.720) MMSE-K 및 SIADL 과도높은음의상관관계를보였다. 또, ADAS-K-cog 총점및소검사평균점수는일관되게 GDS stage 가높아질수록증가하였고 ADAS-K-cog 11 개측정항목중에서지남력척도와언어표현력, 단어선택장애, 언어이해력항목이 GDS stage 3 와 GDS stage 4 사이에서도유의한차이를보여치매진행정도를초기에가장민감하게반영하는척도로나타났다 ( 그림 1). 이는 ADAS-K-cog 가치매의진단및진행단계를평가하는데타당한도구임을보여주는결과이다. 이전의여러국외연구에서도본연구에서와같이 ADAS- Cog 가알쯔하이머병진행정도를효과적으로감별해주는것으로나타났다 ) 하지만, ADAS-K-cog 와 GDS 를비교한한국내연구에서는 GDS stage 3 에속하는경증치매환자에서지남력항목을제외한나머지 ADAS- K-cog 항목에서 GDS stage 증가에따른유의한변화가관찰되지않았다. 15) 본연구에서는 GDS 3 기와 4 기사이에서 ADAS-K-cog 총점및지남력, 언어표현력, 단어선택장애, 언어이해력항목에서유의한차이가관찰되었고이러한차이는 ADAS-Cog 가치매초기나말기에비해서중등도이상의중증치매환자에서급격하게변하므로 25) GDS stage 3 에속하는환자가적었던 (12 명 ) 이전의연구에서초기치매환자군을대상으로통계적유의성을검증하지못한것으로생각한다. 따라서초기치매환자가대부분이었던본연구의결과가더욱의미가있는것으로생각되며짧은시간안에많은수의환자를진료해야하는한국의의료현실에서치매환자의단기간인지기능변화를추적관찰하거나조기진단을위해위의네항목을선별적으로사용하는것이도움이되리라생각한다. 이연구에서 ADAS-K-cog 는한국인알쯔하이머병환자에서발병연령이나성별, 교육수준에영향을받지않는것으로나타났다. 한국노인인구의평균교육수준은낮은편으로 1995 년 Korean census (Korean National Statistical Office, 2001) 에서 60 세이상 4,135,287 명중 3,243,424 (78.4%) 명이초등학교이하의학력을가지고있었다. 본연구의대상자들도교육수준이낮은편에속하였고 ( 평균교육년수 4.5 년, 초등학교이하 81.4%, 대학교육이상 5.7%) 이는한국노인의평균적인교육정도를반영하는것으로생각한다. 치매평가도구로사용되는신경심리학적검사가교육정도에영향을받는다면대상자의평균교육수준이낮을경우치매진단을과도하게내릴수있다. 26) 한국노인인구의교육수준은높지않기때문에교육정도와 ADAS-K-cog 와의관련성을알아내는것은중요하며본연구의결과에서보듯이 ADAS-K-cog 는한국인치매환자에게교육정도에상관없이적용가능할것으로생각한다. 교육정도와 ADAS-Cog 의관련성에대한이전연구들에서본연구와같이교육수준에영향을받지않는다는결과가보고된바있지만 13,23,27,28) 반대로학력이낮을수록 ADAS-cog 점수가높게나온다는연구들도있었다. 8,10-12,29,30) Liu 등은교육기간이짧은치매환자군 ( 교육년수 6 년이하 ) 에서교육정도가 ADAS-cog 점수에영향을준다고하였고 10) Doraiswamy 등도대규모다기관연구에서교육정도가낮을수록 ADAS-cog 점수의상승이빠르다고하였다. 8) 이런상반된결과의원인은연구대상의수와교육정도, 분석방법의차이에부분적으로기인하지만가장큰이유는무엇보다치매의진행정도가다른연구대상을분석하였기때문으로생각한다. 본연구에서와같이경증및중등도치매환자에해당하는 GDS stage 3, 4, 5 환자만을대상으로한이전의연구들에서는교육정도가 ADAS-Cog 점수에영향을주지않는것으로나타났다. 28,31) 이연구의제한점으로심각한인지장애를보이는환자는포함하지않았다는점을들수있다. 이연구는 KT & G 복지재단이후원하는 초기치매환자지원사업 등록환자를대상으로이루어졌고복지사업대상이초기치매환자이었기때문에 GDS stage 6 이상의중증치매환자를대상으로한연구는이루어지지않았다. 향후다양한인지장애정도를보이는치매환자를대상으로한후속연구가필요할것으로생각한다. 또다른제한점으로는전체대상환자중남성대상자가여성에비해적었다는것이다. 이는알쯔하이머병의유병률이남성보다여성에서높기때문인것으로생각한다. 하지만이전연구들에서남성대상자수의부족이연구결과에영향을주지않는다는것이관찰되었고본연구에서도남녀간분석결과에차이를보이지않아전체적인연구결과에영향을주지않았을것으로생각한다. 결론적으로 ADAS-K-Cog 는교육정도와성별및발병연령에영향을받지않으며알쯔하이머병의진행단계를 Vol. 28, No

ADAS-K-cog 세부항목중에서는지남력, 언어표현력, 단어선택장애, 언어이해력항목이알쯔하이머병진행정도를민감하게감별해주는것으로생각한다. 향후보다많은수의치매환자를대상으로한전향적관찰연구가필요할것으로생각한다. ABSTRACTS monitoring the progression of dementia. We also studied the possible influence of age, sex, and education on a ADAS-K-cog. Methods: A total of 70 patients with AD meeting the NINCDS-ADRDA criteria of probable AD were recruited. The ADAS-K-cog was administered to all AD subjects. Each patient was also rated on the Global Deterioration Scale (GDS). The probable AD patients were divided into three groups based on their GDS stage to evaluate the sensitivity of ADAS-K-cog to disease severity and their mean scores on ADAS-K-cog were compared. Results: The subjects' age range was years and their education range was 0 16 years. The total score of ADAS-K-cog and each item score differed significantly between each groups. The item score of orientation, expressive language, word finding difficulty, comprehension of spoken language and the ADAS-K-cog total score could classify severity of early AD patients efficiently. ADAS-K-cog had high correlations with scores on the GDS (Pearson's r=0.732) and the Mini-Mental State Examination (Pearson's r=-0.755). Multiple regression analysis revealed that performance on the Korean ADAS- Cog was uninfluenced by age or gender, and neither by education level. It's language items were best for 참고문헌 1.
